연골모세포종은 골단에서 기원하는 연골형성 양성 종양이다. 중족골의 기저부는 골단 또는 이차 골화중심을 갖고 있지 않기 때문에 연골모세포종의 발생이 드물다. 저자들은 34세 남자의 다섯번째 중족골의 기저에서 발생한 연골모세포종 1예를 경험하였기에 보고한다. 조직학적 검사에서 파골세포와 닮은 거대세포가 거대세포종과 유사할 정도로 풍부하였으며 연골모양 세포사이 기질이 섞여 관찰되었으나 그 양이 적었다. 그러나 배경의 단핵세포는 연골모세포로의 분화의 증거로서 불규칙하며 만입된 핵을 보였고 면역조직화학 검사에서 S-100 단백에 양성 반응을 보였다. Chondroblastoma is a benign chondroid-forming tumor usually originating in the epiphysis. The base of metatarsal bone contains neither an epiphysis nor a secondary ossification center and so is the rare site of chondroblastoma. Here, we present a case of chondroblastoma of the base of fifth metatarsal bone in 34-year-old man. Histologically, the osteoclast-like giant cells were abundant enough to simulate a giant cell tumor. And the chondroid intercellular matrix was intermixed but scanty. However, the background mononuclear cells showed irregular and indented nuclei with longitudinal clefts and positive immunoreactivity for S-100 protein, as the evidence of chondroblasts.

Since inflammatory myofibroblastic turner was initially recognized in the lung, this tumor has been described in other extrapulmonary sites. In spite of relatively uniform histologic findings in various organs, a rarity in extrapulmonary sites and highly vascular characteristics frequently lead to a misdiagnosis in preoperative radiology and fine needle aspiration cytology. We present a case of inflammatory myofibroblastic tumor occurring in the mesentery of a 4-month-old girl. Fine needle aspiration cytology smear disclosed characteristic spindle cells intermixed with prominent mature plasma cells and lymphocytes. According to the immunohistochemical staining, we recognized that the intervening spindle cells are myofibroblasts which have reactivity for the both actin and vimentin.

소아에서 신체활동과 대사이상 군집의 관계

Objectives: This study was performed to assess the association between physical activity and the clustering of metabolic abnormalities among Korean children. The effect of substituting moderate to vigorous physical activity for the time spent in inactivity was examined as well. Methods: The study subjects were comprised of 692 (354 boys, 338 girls) 4th grade elementary school students. We used a modified form of the physical activity questionnaire that was developed in the Five-City Project. The subjects with clustering of metabolic abnormalities were defined as having two or more of the following five characteristics: waist circumference ${\geq}90\;%$, systolic or diastolic blood pressure ${\geq}90\;%$, fasting glucose ${\geq}110\;mg/dl$, triglycerides ${\geq}110\;mg/dl$ and HDL cholesterol ${\geq}40\;mg/dl$. We calculated the odds ratios to assess the effect of substituting moderate to vigorous physical activity for time spent in inactivity. Results: The risk of clustered metabolic abnormalities was inversely correlated with the increased time spent on moderate to vigorous physical activity, but the correlation was not significant. The odds ratio for clustering of metabolic abnormalities that represented the effect of substituting moderate to vigorous physical activity for 30minutes of sedentary activity was 0.87(95% Cl=0.76-1.01). Conclusions: These findings suggest that substituting moderate to vigorous physical activity for sedentary activity could decrease the risk of clustered metabolic abnormalities.

일본의 수산용의약품 관리의 현황과 법제

Along with the rapid growth of the aquaculture industry in the world, the production of aquatic products has been rapidly increasing. In addition, the use and frequency of use of Aquatic medicines are increasing every year. Aquaculture is highly susceptible to mass death due to the surrounding environment, such as red tide and water temperature rise, and under human control, the production rate is greatly affected by aquaculture management. Therefore, appropriate response to disease management in the aquaculture industry is most important. In Japan, to maintain such a safe and healthy aquaculture environment, 「Sustainable Aquaculture Production Assurance Act」 implements a fishery improvement plan to prevent the fishery environment from deteriorating. Aquatic medicines have been used to prevent disease in the aquaculture field for many years. In spite of many advantages, overdose and abuse may cause environmental pollution and antibiotic resistance. Therefore, many countries try to protect the environment, biospecies and food security. Especially in countries surrounded by the sea such as Korea and Japan, aquaculture is economically very important. Proper use and management of aquatic medicines not only impacts the production of aquatic products, but also has a direct impact on human health. Marine aquaculture in Japan has developed steadily since the 1960s, and it occupies an important position in the Japanese marine industry at this stage. Marine aquaculture is one of the most important issues when considering Japanese fisheries policy. In addition, in the event of fish disease, the government restricts the use of drugs other than those approved by it under the 「Pharmaceutical Affairs Law」 so that there is no residual drug in the food due to overdose of fishery medicines. At the same time, antimicrobial medicines and insecticides for fisheries are obliged to prescribe the kinds of fish species, usage and capacity, as well as prohibition period. Japan has established a system to appropriately guide the National Fisheries Test Center(NFTC) and Guidance Agency for the appropriate use of medicines for fisheries and vaccines. Each aquaculture farmer is always paying attention to aquaculture management by visiting NFTC or through its website. This paper examines the current states and management of Aquatic medicine in Japan’s aquaculture industry and the legislation related to Aquatic medicine which has comparatively similar conditions to Korea, I would like to identify implications for the management of the Korean aquaculture industry and Aquatic medicine by analyzing the Japanese legal system for fisheries medicine.

자유선택활동의 놀이 계획, 수행, 평가의 일치정도와 유아의 자율행동과의 관계 연구

본 연구는 놀이 계획, 수행, 평가의 일치정도와 유아의 자율행동이 어떠한 관계가 있는지 알아보는데 목적이 있다. 연구 대상은 D광역시에 소재한 공립유치원에 재원중인 만5세 유아 54명이고, 연구 도구는 이학선(2007)에 의해 개발된 유아 자율행동 검사를 사용하였다. 놀이 계획, 수행, 평가의 일치는 유아가 놀이를 계획하고 평가한 것과 관찰자가 유아의 놀이 수행 여부를 기록한 관찰 일지를 확인하여 점수화하였다. 본 연구결과, 첫째, 유아의 놀이 계획, 수행, 평가의 일치정도가 흥미영역과 성별에 따라 다르게 나타났으며, 둘째, 유아기는 자율적으로 자신을 조절하기 위한 근간이 이루어지는 주요한 시기임을 알 수 있었고, 셋째, 유아의 자유선택활동의 놀이계획, 수행, 평가의 일치정도가 일과활동의 자율행동에 긍정적인 영향을 주었다. 이러한 결과는 유아의 자율행동을 증진시키기 위한 계획적인 놀이 계획 및 평가 활동의 중요성을 재인식히는 기회가 될 것이다. The purpose of this study is to examine the relationship between the congruence of play plan, practice, and evaluation and the children"s autonomous behavior. In a public kindergarten located in D city, 54 children participated in the study and took the Children Autonomous Behaviour Test, developed by Harkseon Lee(2007). Researchers checked and scored the following: that the congruence of the play plan, practice, and evaluation was used; that the children"s planning and their practice were evaluated; and that the observation diary which recorded the children"s practices was maintained. The study produced the following research results; First, the congruence of play plan, practice, and evaluation differed according to interest areas and gender. Second, early childhood was the important phase to build up autonomous control. Third, the congruence of play plan, practice, and evaluation of free choice activities had a positive effect on the children"s autonomous behaviour in daily activities. These results underscore the importance, in promoting children"s autonomous behavior, of predetermined play plans and evaluation activities.

Objective : The cyclin-dependent kinase inhibitor $p27^{kip1}$ protein is a negative regulator of the cell cycle, and its degradation is required for entry into the S phase. Loss of $p27^{kip1}$ expression has been reported to be associated with aggressive behavior in a variety of tumors of epithelial and lymphoid origin. However, its association with various astrocytic tumors has not been clearly demonstrated. We studied to investigate the relationship of $p27^{kip1}$ expression with the biological behavior of astrocytic tumors in addition to study on the role of $p27^{kip1}$ in the tumorigenesis of these tumors. Patients and Methods : From 1990 to 1998, a total of 29 astrocytic tumor of all grades obtained by operative resection were included for evaluation. We studied the expression of $p27^{kip1}$ protein immunohistochemical assay in astrocytic tumors and compared the findings with the clinicopathologic parameters. Immunohistochemical staining was performed on formalin-fixed paraffin-embedded sections by the avidin-biotin-peroxidase complex method. According to WHO classification, all cases were divided into astrocytomas(4 cases), anaplastic astrocytomas(9 cases), and glioblastomas(16 cases) by 3 pathologists. Clinical information was obtained from medical records, and others such as location and size of tumors from imaging studies. Results : Mean $p27^{kip1}$ protein labeling indexes(LI, mean${\pm}$standard deviation) of astrocytomas, anaplastic astrocytomas, and glioblastomas were $80.6{\pm}9.1$, $63.6{\pm}21.0$, and $28.9{\pm}18.7$, respectively, and were inversely correlated with grade of glial tumors(p<0.0001). Mean $p27^{kip1}$ protein LI in the recurrent group was lower than that in the nonrecurrent group, but there was no significant difference statistically(p=0.464). Additionally, $p27^{kip1}$ protein expression did not show any significant relationship to other prognostic factors such as age(p=0.1643), tumor size(p=0.8), or location(p=0.8). Conclusion : These results suggested that reduced expression of $p27^{kip1}$ protein may play a important role in the malignant transformation process of astrocytic tumor cells.

수막의 고립성 섬유성 종양 - 1예 보고 -

Solitary fibrous tumor(SFT) is a spindle cell neoplasm that usually arises in the pleura. Its involvement of the meninges is rare but increasingly being reported. Our case was a 33-year-old man who presented with clonic seizure around the right mouth angle and sensory disturbance at the right hand. Radiological studies showed a large extra-axial, dural-based mass in the left fronto-parietal region, typically isointense with gray matter and heterogeneous strong enhancement on T1-weighted images. Histologically, the tumor was composed of spindle-shaped cells growing in fascicles within the collagenous matrix. Immunohistochemical staining demonstrated diffuse CD34 positivity. Awareness of this neoplasm is necessary to accurate diagnosis.

하악골 후방부위에서 다양한 방사선학적 소견을 보이는 골화성섬유종의 증례보고

Common radiographic appearances of ossifying fibroma (OF) are well demarcated margin, radiolucent or mixed lesion. Lesions for the radiographic differential diagnosis with OF include fibrous dysplasia, focal cemento-osseous dysplasia. Other confusing lesions might be the mixed lesions such as calcifying odontogenic cyst, adenomatoid odontogenic tumor, calcifying epithelial odontogenic tumor, and benign cementoblastoma. We reported three cases of OF in posterior mandible. These cases showed a little distinguished radiographic features of OF and diagnosed from a combination of clinical, radiographic, and histopathologic information. We need to further refine radiographic and histopathological features of OF and other confusing lesions with literatures review because some cases of these lesions are not easily differentiated radiographically and histopathologically.
