        구순구개열 환자 양악교정술 후 회귀 증례

        It is well known that the main factor which contributes to the relapse of orthognathic surgery for Cleft Lip and Palate (CLP) patients is post-operative scar on hard and soft palate of maxilla. Therefore, to compensate the amount of relapse, though it cannot be the perfect way to prevent orthodontic, orthopedic relapse, the Le-fort I osteotomy of maxilla and set-back osteotomy of mandible are generally carried-out simultaneously. We are to review the factors contribute to the relapse of CLP patients after orthognathic surgery through this clinical case : The relapse of Skeletal Class III tendency immediately after orthognathic surgery for grown up CLP patients.

        핀수영 선수의 영양관련 상태와 영양교육 효과

        본 연구는 실업팀에 소속되어 핀수영선수 11명을 대상으로 영양인식 및 영양섭취 상태를 조사하고 영양교육 전후 식이섭취량을 분석함으로써 우리나라 핀수영선수에 대한 영양인식과 영양섭취 실태 및 영양교육을 통한 중재효과를 알아보고자 실시되었다. 영양인식은 설문지로, 영양섭취 상태는 24시간 회상법을 이용하여 식사일기를 통해 섭취한 음식의 종류와 양을 기록한 후 식이섭취량을 분석하였다. 영양교육 실시 후 식이섭취량 분석을 다시 조사하였으며, 영양교육 전후 영양교육의 중재 효과를 보기 위해 비모수검정을 이용하여 차이를 비교하였다. 남성선수의 경우, 에너지, 비타민 C, 마그네슘 섭취가 한국인 에너지 권장량의 60%정도로 나타났으며, 여성선수의 경우, 에너지, 비타민 C, 엽산, 칼슘, 칼륨, 마그네슘, 철분의 섭취가 한국인 에너지 권장량의 60%이하로 나타났다. 영양교육 후, 비타민 E, 비타민 C, 니아신, 비타민 B<sub>6</sub>, 엽산, 인, 나트륨, 아연의 섭취량은 유의하게 증가하였지만 에너지, 칼슘, 칼륨, 마그네슘, 철분의 경우 영양교육에도 불구하고 섭취량을 유의하게 증가시키지 못했는데 이러한 결과는 단기교육만으로는 선수들에게 충분히 이해시키지 못하였기 때문으로 사료된다. 나트륨의 경우 남녀선수 모두 한국인에너지권장량의 200%이상을 섭취하는 것으로 나타났으며, 영양교육 후에도 나트륨 섭취비율을 감소시키지 못한 것으로 나타났다. 앞으로 핀수영 선수를 위한 다양한 영양관련 연구와 함께 영양교육에서는 에너지, 칼슘, 마그네슘, 칼륨, 마그네슘, 철분에 대한 교육이 강화되어야 할 것으로 생각된다. 또한, 중장기적이고 지속적인 영양교육을 통한 효과적인 영양중재가 이루어지길 기대한다. This study was to investigate the nutritional recongnition and nutritional intake status of finswimming athletes, and to analyze the nutritional awareness and nutritional intake status before and after nutrition education. Nutritional recongnition was analyzed using a questionnaire, and nutritional intake data using 24-hour recall method with diet diary were obtained and analyzed. Energy, vitamin C, and magnesium intake were found to account for about 60% of the Korean dietary standard intake (KDRIs) in male athletes. Energy, vitamin C, folic acid, calcium, potassium, magnesium, and iron intake were less than 60% of the KDRI in female athletes. After nutrition education, vitamin E, vitamin C, niacin, vitamin B<sub>6</sub>, folic acid, phosphorus, sodium, and zinc intake increased significantly, but energy, calcium, potassium, magnesium, and iron intake did not increase significantly despite nutrition education. Thess result is due to the fact that short-term education alone was not sufficiently understood by athletes. Sodium, both male and female athletes, were found to consume more than 200% of KDRIs, and it was found that the sodium intake rate could not be reduced even after nutrition education. After that, the nutrition education and research for finswimming athletes focus on energy, calcium, magnesium, potassium, magnesium, and iron should be conducted. In addition, it is expected that effective nutritional intervention will take place through mid- to long-term and continuous nutrition education.

        저항운동이 폐경 전,후 비만여성의 신체구성, 골밀도 및 아디포사이토카인에 미치는 영향

        이 연구는 폐경 전·후 신체의 변화가 급격히 일어나는 비만 중년 여성들에 있어 16주간의 고강도 저항운동이 아디포사이토카인과 골밀도의 변화에 어떠한 영향을 미치는지를 알고자하였다. 이를 위하여 폐경 전 10명, 폐경 후 10명의 비만여성을 대상으로 16주간 일일 60분, 주 3회의 저항운동을 75%1RM에 해당되는 운동강도로 실시하였다. 그 결과 트레이닝 후 체중, BMI, 체지방량 및 허리둘레의 유의한 감소와 제지방량의 유의한 증가가 나타났으며, 에스트로겐은 집단 간 유의한 차이를 나타내었다. 트레이닝 후 렙틴과 TNF-α는 유의하게 감소되었으며, 대퇴경부 BMD는 집단 간, 시기 간 유의한 차이를 나타내었으며, 근위고관절 BMD는 집단 간 유의한 차이가 나타났다. 또한 트레이닝 후 아디포넥틴의 변화는 대퇴 전자부(r=.657, p=.002)와 근위 고관절(r=.476, p=.034) BMD의 변화와 유의한 상관관계를 보였으며, 골밀도의 변화를 위한 예측 변수로서 각각 58%와 28%의 설명력을 가지고 있는 것으로 나타났다. 이상의 결과를 종합해 볼 때, 비만 여성들의 16주 저항운동은 체중 및 체지방량의 감소, 제지방량의 증가, 렙틴과 TNF-α의 감소 및 대퇴경부 골밀도의 증가에 영향을 미치는 것으로 나타났으나, 이러한 운동의 효과는 폐경 상태에 의해 영향을 받지 않는 것으로 나타났다. 또한, 운동에 의한 아디포넥틴의 변화는 BMD의 변화를 예측하는 중요한 결정요인으로서 고려된다. The purpose of this study was to investigate the effect of 16 weeks high intensity resistance training on the relation of body composition, adipocytokines, and bone mineral density(BMD) in obese perimenopausal women. Middle aged obese pre(n=10) and post(n=10) menopausal women volunteered for this study. Resistance training consisted of 3sets with 75% 1RM intensity for 16 week (3days per week). Body weight, BMI, body fat mass, and waist circumference were decreased whereas lean body mass was increased significantly. Leptin and TNF-α were significantly decreased after exercise training. Femoral neck BMD was significantly different in group and time. Proximal total hip BMD was significantly different between groups. After training the changes of trochanter(r=.657, p=.002) and proximal total hip BMD(r=.476, p=.034) were significantly correlated to the changes of adiponectin. In addition, the changed adiponectin was an important predict variation to femoral neck BMD(p<.001) and proximal total hip BMD(p<.05). In summary, 16 weeks high intensity resistance training had a moderately favorable effect on BMD with improved body composition and decreased leptin level. However, it does not seem that the effect of exercise training on these adipocytokines and BMD is influenced by menopausal status in obese women. Furthermore, it suggested that exercise-induced changes of adiponectin is an independent predictor of changed BMD.

        Mediating Effect of School Nurses' Self Efficacy between Multicultural Attitude and Cultural Sensitivity in Korean Elementary Schools

        Purpose: This study examined the mediating effect of school nurses' self efficacy, which is one of the significant cognitive factors influencing cultural sensitivity, on the mutual relationships between multicultural attitude and cultural sensitivity in Korean elementary schools. Methods: A cross-sectional descriptive survey design was used. Participants were 157 school nurses in elementary schools located in Gyeonggi-do, South Korea. The survey instruments included Teacher Multicultural Attitude Survey, Teacher Efficacy Scale, and Multicultural Sensitivity Scale. Data were analyzed using three regression equations to test the mediation model. Results: The mean score of the school nurses' cultural sensitivity was relatively low. A positive correlation among multicultural attitude, self efficacy, and cultural sensitivity was noted. Self efficacy of school nurses showed a significant mediating effect on the relationships between multicultural attitude and cultural sensitivity. Conclusions: Given the meaningful influence of positive multicultural attitude on cultural sensitivity and significant mediator effect of self efficacy as a school nurse between the two variables, the strategies to cultivate a positive multicultural attitude and enhance school nurses' self efficacy in their unique role should be considered in a training program. School nurses' health care services will benefit from the improvement of cultural sensitivity toward young children from multicultural families.

        초등학교 물리적 환경에 대한 교사의 인식 고찰

        In general, teachers' perceptions on the physical environment of elementary school are related on the quality of education and school life. But, we don't have study what teachers think of their enviroment of school. This study analyzed elementary school teachers' satisfaction with overall physical environment and their assessment of spaces with specific functions in order to understand teachers' perception of the physical environment of elementary schools. A survey questionnaire was administered to 982 homeroom teachers working at 67 elementary schools located in Seoul and Busan. Teachers' satisfaction with physical environment of school and their assessment were analyzed comparatively according to the teachers' personal attributes and school conditions. 1) Most of the surveyed teachers were satisfied with overall physical environment of the school, and assessed high the substantiality of functional spaces indoor and outdoor. By sub-factor, however, their satisfaction with work environment was lower than that with education and living environment. 2) The teachers perceived that the optimal number of classes was 30~36, and this is probably because they were relatively accustomed to large scale schools. 3) As might be expected, negative assessments were more frequent among teachers working at a large-scale school, an old school, or a school with small outdoor spaces. The results of this study show that, though partially different according to region, gender, teaching experience and school condition, teachers' satisfaction with school environment was relatively high. This suggests that elementary school teachers perceive their current school environment positively. Because they are accustomed to traditional education methods that have been maintained.
