        비소세포폐암 환자의 종양 축소에 대한 SB 주사 효과 증례

        Objectives : To study the effect of SB injection on tumor size in an advanced non-small cell lung cancer patient. Methods : A patient was clinically diagnosed as advanced non-small cell lung cancer (Stage IIIa). Four cycles of intravenous SB injection were conducted. Each cycle lasted 4 days. The content of 7vials SB was injected every day. To compare the tumor size before treatment and after four cycles of SB injection, chest computed tomography (CT) was performed. Results : Follow-up CT images showed that the tumor size was reduced. In admission, size of the tumor $6.7{\times}8.5{\times}9.5cm$ on the left lower lobe of lung. After SB injection, size of the tumor $5.6{\times}6.8{\times}8.4cm$ by Chest CT. The patient's symptoms such as cough, sputum were improving until four cycles of SB injection. Numerical rating scale (NRS) showed improvement of Chest pain from point 3 to point 0. Conclusions : This case study suggests that intravenous SB injection may have significant effects of anti-tumor for non-small cell lung cancer.

        교원능력개발평가 사서교사 평가지표와 문항의 중요도 및 수행도에 대한 사서교사들의 인식

        이 연구는 2009년에 개발되어 현재 선도학교를 통해 활용되고 있는 교원능력개발평가 사서교사 평가지표 및 문항에 대한 설문조사를 통해 그 개선점을 제안하였다. 연구결과 현행 평가지표 및 문항의 중요도에 대한 사서교사의 인식이 대체로 긍정적인 것으로 나타났고, 각 문항의 중요도와 수행도에 대한 사서교사의 인식에는 통계적으로 유의미한 차이가 있었다. 평가지표 및 문항에서 개선해야 할 점으로는 현재의 평가문항에 다양한 독서교육활동 실시여부에 대한 항목 추가, 자료조직 및 관리 관련 평가문항 수정의 필요성, 학생지도 평가영역 명칭에 대한 제고 등이 제기되었다. 이러한 연구결과는 현재 활용되고 있는 사서교사 평가지표와 문항이 대체로 타당하기는 하지만 일부분에서 수정 보완이 필요함을 제시하였다. This study aimed to analyze teacher librarians' perceptions of the importance and performance frequency of their roles inherent in the indicators and items for teacher evaluation and to determine their appropriateness. The results revealed that most indicators and items are appropriate for evaluating teacher librarians' job performance, but some items need to be revised to properly reflect teacher librarians' job tasks. Specific recommendations for their improvement were discussed.

        폴리카보실란계 바인더를 이용한 다공성 SiC 세라믹스의 제조

        Porous SiC ceramics were prepared by using recycled SiC sludge, which is an industrial waste generated from solar cell industry. Polycarbosilane derivatives, such as polycarbosilane (PCS), polyphenylcarbosilane (PPCS) and hydridopolycarbosilane (HPCS) were used as binding agents for the fabrication of porous SiC ceramics at $1800^{\circ}C$ under Ar atmosphere. The effects of the various binding agents having different C/Si ratios were discussed on the sintering and porosity of the SiC ceramics. The prepared porous SiC ceramics were characterized by X-ray Diffraction (XRD) and Field-Emission Scanning Electron Microscope (FE-SEM). Thermal conductivity and porosity of SiC ceramics were measured at room temperature, and they were 56.7W/mK and 29.8%, respectively.

        항암제 유발 말초신경병증 변증평가도구의 신뢰도 및 타당도 평가

        Objectives: The purpose of this study was to evaluate the reliability and validity of an instrument for pattern identification and evaluation in chemotherapy-induced peripheral neuropathy (CIPN).Methods: The study consisted of 66 patients with CIPN (visual analog scale ≥30 mm, Eastern Cooperative Oncology Group ≤2). The test-retest reliability of the instrument for pattern identification and evaluation in CIPN was tested twice with the same patients in one week.Results: Pearson’s coefficient test-retest reliability of the instrument was 0.601-0.777. The internal reliability of each part was 0.619-0.811. To confirm the validity of the instrument for pattern identification and evaluation for CIPN, the correlation between the score for each identification pattern and that of other CIPN instruments was analyzed. The results revealed a positive correlation between the Functional Assessment of Cancer Therapy-General and CIPN-20.Conclusion: The reliability of the instrument for pattern identification and evaluation in CIPN was moderate, and the validity analysis revealed a positive correlation.

        원전 방사성 콘크리트 기계적 절단의 방사성 에어로졸에 대한 작업자 내부피폭선량 평가

        원전 해체 공정 중 다량의 콘크리트 방사성 폐기물의 절단 과정에서 불가피하게 방사성 에어로졸이 생성된다. 방사성 에어로졸은 인체 호흡기 흡착에 의한 내부피폭을 유발하기 때문에 작업자의 방사선 방호를 위한 내부피폭평가가 필수적으로 시행되어야 한다. 그러나 실제 작업환경의 에어로졸 특성값을 사용하기에는 선행 연구가 미비하며 콘크리트에 포함된 방사성 핵종의 수가 많기 때문에 정확한 작업자 내부피폭평가를 위해서는 상당한 시간과 인력이 필요하다. 따라서, 본 연구에서는 사전 연구된 콘크리트 에어로졸 특성값을 활용하여 원전 해체 전 절단 작업자의 내부 피폭량을 빠르게 예측할 수 있는 새로운 방법론을 제시하고자 한다. 본 연구팀은 콘크리트 절단 시 발생하는 사전 연구에서 발표된 에어로졸의 수농도 크기 분포데이터를 뉴턴-랩슨법을 이용하여 피폭평가 계산에 필요한 방사능중앙 공기중역학직경(Activity Median Aerodynamic Diameter)값으로 변환하였다. 또한 원전 정지 10년 후 비방사능 값을 ORIGEN code로 계산하였으며, 최종적으로 핵종별 예탁유효선량을 IMBA 프로그램을 이용하여 계산하였다. 핵종별 예탁유효선량값을 비교한 결과 152Eu에 의한 최대 예탁유효선량은 전체 선량값의 83.09%를 차지하고, <sup>152</sup>Eu를 포함한 상위 5개 원소(<sup>152</sup>Eu, <sup>154</sup>Eu, <sup>60</sup>Co, <sup>239</sup>Pu, <sup>55</sup>Fe)의 경우 최대 99.63%를 차지함을 확인하였다. 따라서 원전 해체 전 콘크리트의 구성 원소 중 상위 5개 주요 원소 측정을 먼저 시행한다면 더 빠르고 원활한 방사능 피폭관리 및 해체 작업 안전성 평가가 가능할 것으로 판단된다.

      • 설문조사를 바탕으로 부서 간 만족도 향상을 위한 개선 안

        Purpose Recently, there has been increasing demands for clinical departments at Asan Medical center in Seoul. They want to see the results of lab tests about blood samples as soon as possible and to be reported to them on the day. As the main contents of the survey, we explained the goodness of Asan medical center's in vitro and the points to be improved. Based on this, it became an opportunity to create a positive image of In vitro laboratory. In addition, we could identify their specific requirements through the surveys. Materials and Methods In September 2016, a total of 14 questionnaire surveys were conducted for 49 clinical departments and outpatient nurses at Asan Medical center in Seoul. The survey consists of description questions to be able to express the intention of the individual and the questions made on the Likert 4 point scale. The main contents of 14 questions are composed of goodness of In Vitro laboratory and points to be improved. Results 62% answered that the best service in In Vitro laboratory was "good accuracy and reproducibility". On the other hand, as an inconvenience when requesting blood tests, 73% pointed out that "the result report time was long", which was recognized as a part to be improved. There are many contents that "The result of all tests is reported within 2 hours" on the day of the examination. In the question - 'Are there some examination results which do not coincide well with clinical observations?', 55 of 56 people answered "no". Above all, the majority answered that waiting for re-examination results is too long. This problem must be causing discomfort to the patients. Conclusion In order to improve these problems, the first thing is to increase the number of blood tests by using the current personnel, equipments and reagents to the fullest by item in Asan Medical Center's Nuclear medicine in vitro part. Secondly, in case of re-examinations, we use the "AMIS message" to show other clinical departments the reporting time. This methode improves the efficiency of work with nurses and increases satisfaction of custom service. Thirdly, the correlation was evaluated by selecting the test species that can be carried out by the shortening method. Currently, C-peptide and insulin are implemented in the reaction process to shorten 2 hours into 1 hour. Finally, we are considering purchasing new equipments for quick test results.

        악안면부 충격시 치아와 악골의 응력 분포에 대한 구강보호장치의 역할에 관한 유한요소분석 - 상악 중절치에 가해진 충격에 대하여

        Statement of problem : The use of mouthguard is important as the leisure life is popular today. Purpose: The purpose of this study is to investigate the effect of a mouthguard on stress distribution in teeth, maxilla and mandible for maxilla impact. Material and methods: The 3-dimensional finite element model was based on a CT scan film of an average korean adult when the subject is using a customized mouthguard which was made with the Signature Mouthguard system of Dreve. The load was applied to the upper central incisor cervical area parellel impact force for 0.1sec(L1). The Von-mises stress analysis with a mouthguard and without a mouthguard was compared. Results: The results of this study were as follows: 1. Without the mouthguard, stress was concentrated on teeth and alveolar bone in all load conditions. 2. With the mouthguard. maximum stress value was decreased and stress was dispersed in all load conditions. 3. Stress extinction with the mouthguard was faster than without the mouthguard in all load conditions. Conclusion: We acknowledged that the mouthguard has a stress buffer effect as the maximum stress value was decreased and stress was dispersed when impact force was applied.

        매체순환연소를 위한 Ni계열 산소전달입자의 반응 특성 및 반응 모델

        Reaction characteristics and kinetics of a oxygen carrier (OCN717-R1SU) for chemical looping combustion (CLC) have been investigated using TGA by changing gas concentration (10-30 vol.% $CH_4$) and reaction temperature ($825-900^{\circ}C$). Reaction rate of OCN717-R1SU increased as temperature increased and it was found that reaction is delayed at the initial reaction regime. Johnson-Mehl-Avrami (JMA) model was adopted to explain the reaction phenomenon. The activation energy (E) determined by JMA model in reduction reaction of OCN717-R1SU is $151.7{\pm}2.03kJ/mol$ and pre-exponential factor and JMA exponent were also obtained. The parameters calculated in this study will be applied in design of the reactor and operation conditions for CLC process.
