        소아에서 대장 림프결절증식의 임상적 및 병리조직학적 소견

        목 적: 소아에서 대장의 림프 결절 증식은 비교적 드물다고 알려져 있으며 몇 예의 증례 또는 소규모 그룹에 대한 연구만 보고 되어 왔고 임상적인 의의가 아직 확립되어 있지 않다. 본 연구에서는 대장에 림프 결절증식 소견을 보였던 환아들의 임상적 특징, 검사 소견 및 조직학적 소견 등을 분석하여 이 소견에 대한 임상적 의의에 대하여 알아보고자 하였다. 방 법: 2003년 5월부터 2008년 10월까지 가천의대 길병원 소아청소년과에서 하부 소화관 내시경 검사를 시행 받은 환아들 중 검사 당시 육안적으로 대장의 림프결절 증식 소견을 보였던 38명을 대상으로 하였다. 내시경 검사를 받을 당시 성별, 연령, 출생력, 과거력 및 가족력을 조사하였으며 주 호소와 동반 증상, 식이 방법과 주요 신체 진찰 소견을 확인하였다. 혈액 검사, 대변 검사, 영상학적 검사를 시행하였으며 내시경 검사시 모든 환아에서 조직검사를 시행하여 이를 분석하였다. 내시경 검사 후 단기 및 장기 추적을 통해 증상의 경과를 관찰하였다. 결 과: 총 38명의 환아 중 남아가 24명(63.1%), 여아가 14명(36.9%)이었고 내원 당시 나이는 평균 12.5${\pm}$14.4개월이었으며 12개월 이하의 영아가 28명(73.6%), 13개월 이상의 소아는 10명(26.4%)이었다. 38명 모두 혈변을 주소로 내원하였다. 전체 환아 중 설사가 17명 (44.3%)으로 가장 많이 동반되었으며 점액성 변, 구토, 복통, 경구 섭취 부족, 발열, 변비 등의 동반 증상을 보였다. 모든 환아는 신체 진찰 상 비교적 건강해 보였다. 혈액 검사 상 헤모글로빈, 혈청 알부민, 혈액 응고 검사수치 등은 1예를 제외하고 모두 정상 범위였다. 대변세균 배양 검사, 영상학적 검사상 다른 출혈의 원인은 찾을 수 없었다. 조직검사 상 36예(94.7%)에서 림프 소절이 확인되었으며 34예(84.5%)에서 알레르기성 대장염의 조직학적 기준을 만족하였다. 단기 경과 추적이 가능했던 25명 중 20명에서 증상이 소실되었으며 장기경과 추적이 가능했던 33명 모두에서 증상이 호전되고 재발하지 않았다. 결 론: 대장의 림프 결절 증식 소견은 알레르기성 대장염과 관련하여 영아에서 더 흔하지만 소아 전 연령대에서 나타날 수 있다. 특히 비교적 건강한 소아에서 다른 하부 위장관 출혈의 원인을 찾을 수 없고 내시경상 LNHC를 보이면 이를 하나의 진단으로 간주할 수 있을 것이다. 증상은 식이 변경과 상관 없이 소실되었고 장기간 추적 관찰에서 재발하지 않아 양성 질환임을 확인할 수 있었다. Purpose: Lymphonodular hyperplasia of the colon (LNHC) is a rare finding in children and its significance as a pathologic finding is unclear. The aim of this study was to investigate the clinical significance of LNHC by analyzing clinical and histopathologic findings in children with LNHC. Methods: We analyzed data from 38 patients who were confirmed to have LNHC by colonoscopy. We checked age, birth history, past history, family history, and clinical symptoms. A hematologic exam, stool exam, and image studies were performed and biopsy specimens were examined by a pathologist. All patients were asked to have short- and long-term follow-up. Results: The mean age of the patients was 12.5${\pm}$14.4 months. All patients presented with complaints of bloody stool. They appeared healthy and the hematologic findings were within a normal range, with the exception of one case. There was no other identified source of bleeding. On histologic exam, 36 patients (94.7%) had lymphoid follicles and 34 patients (84.5%) fulfilled the criteria of allergic colitis. Regardless of diet modification and presence of residual symptom, there was no recurrence of bloody stool through long-term follow-up in all patients. Conclusion: LNHC is more common in infants who are affected by allergic colitis, but it can appear even after infancy. LNHC should be regarded as the etiology when there are any other causes of rectal bleeding, especially in healthy children. We suggest that LNHC has a benign course regardless of diet modification and it might not require excessive concerns.

        This study was done to put forward the direction of the hotel convention , through an empirical study on how the physical environment of hotel convention, an important place to host a convention, affects the customer satisfaction by emotional and cognitive responses of hotel customers. The results are as follows. First, the physical environment of hotel convention was confirmed by five things of ambient factor, convenience, attractiveness, complexity and functionality of equipment as follows. Second, of the physical environment of hotel convention, the complexity and the functionality of equipment had an effect on the positive emotions of hotel customers, and the attractiveness and the complexity had an effect on the negative responses of them. Third, of the physical environment of hotel convention, the convenience of use and the complexity had an effect on the comprehensive assessment of cognitive response, and the attractiveness and the functionality of equipment had an effect on the general image. Based on the findings stated above, hotel convention officials should be dedicated to result in the positive relationships such as customer satisfaction, which affects emotional and cognitive responses of hotel customers directly.

        호텔컨벤션 서비스스케이프와 행동의도와의 관계에서 감정반응의 조절효과

        In this study, a survey was carried out on customers who participate in hotel convention in the Daejeon region from January 5 through February 5, 2012 for an empirical analysis based on hypothesis generation, with the object of making investigations on the moderating effect of emotional responses on the relationship between hotel convention servicescape and behavioral intention. It was carried out by self-reported questionnaires. Total 250 copies were distributed to collect 238 copies. The factor analysis was carried out to figure out the level of hotel convention servicescape, emotional responses and behavioral intention using SPSS Win Ver. 17.0 about 203 copies except 30 copies, which are unsuitable for analysis. The level was explained. And the multiple regression analysis was carried out for hypothesis testing. Analysis showed that the hotel convention servicescape had a significant effect on the behavioral intention, and emotional responses had the moderating effect on the relationship between hotel convention servicescape and behavioral intention. Based on these findings, useful implications were brought forward.

        고객의 언어폭력이 컨택센타 상담사의 분노, 소진, 이직의도에 미치는 영향

        본 연구는 고객의 언어폭력이 컨택센타 상담사의 분노와 소진이 이직의도에 어떻게 영향을 미치는 규명하고자 하였다. 조사기간은 2013년 5월13일~5월31일까지 실시하였고 설문지의 배부와 수집은 대전지역에 본부를 두고 있는 H해상, L통신사, L보험사 컨택센타를 직접 방문하여 각 업체별로 총 300부를 배포하여 289부를 회수하였고 이 중에서 유효한 278부를 통계프로그램 SPSS 19.0을 사용하여 분석하였다. 실증분석결과를 요약하면 다음과 같다. 첫째, 요인분석 결과 언어폭력은 자아손상개념, 모욕감의 두 가지 요인이 도출되었으며, 분노는 분노억압, 분노표출, 분노통제의 3가지 요인으로 도출되었다. 그리고 소진은 정서적 고갈, 비인간화, 개인적 성취감 결여 3가지 요인으로 도출되었다. 둘째, 언어폭력이 분노에 어떠한 영향을 주는지에 대한 분석결과, 언어폭력의 2개 하위 요인 중 모욕감은 분노억압, 분노표출, 분노통제에 모두 유의한 영향을 미쳤으며, 자아손상개념은 분노에 영향을 미치지 않는 것으로 나타났다. 셋째, 분노와 소진의 영향관계를 검증한 결과 분노표출, 분노통제가 소진의 정서적 탈진과 비인간화에 유의한 영향을 미치는 것으로 나타났으며 분노억압은 유의한 영향 관계가 입증되지 않았다. 넷째, 소진과 이직의도의 영향관계를 검증한 결과 소진의 비인간화가 이직의도에 유의한 영향을 미치는 것으로 나타났다. 컨택센타 상담사가 경험하는 언어폭력은 분노를 통한 정서적 탈진과 비인간화, 개인적 성취감 결여의 소진으로 의사소통단절 업무능력 저하로 귀결될 수 있다. 따라서 이직의도를 낮추기 위하여 상담사가 접하는 언어폭력 수준의 지속적인 체크가 필요하며, 고객에 대한 적극적인 계도를 통해 보다 효율적인 직무환경을 조성해야 할 것이다. This research was to investigate how verbal abuses of the customer influence on turn over intention, anger and exhaustion of the counsellors of contact center. Surveys were conducted in contact centers of H Shipping, L News Agency, L Insurance headquarters located in Daejeon city from May 13, 2013 through May 31, 2013. Of 300 questionnaires distributed, 289 questionnaires were collected and 278 valid questionnaires were finally analysed using statistical program SPSS 19.0. The results provided three main findings. First, a factor analysis revealed that verbal abuse was derived into two factors, namely damage on self concept and insult, and anger was derived into three factors, namely repression of anger, expression of anger, and control of anger. And burn out was derived into three factors, namely emotional burn out, deviated personification, and lower accomplishment. Second, a regression analysis showed that among two sub-factors of verbal abuses, insult significantly affected all factors including repression of anger, expression of anger, and control of anger, while damage on self concept insult did not significantly affect anger. Third, it was also found that expression of anger and control of anger significantly affect on emotional burn out and deviated personification of burn out, while repression of anger did not significantly influence any dependent variables. Fourth, it was also indicated that deviated personification of burn out significantly affected turn over intention.

        채용박람회장에서의 지각된 혼잡이 회피행동에 미치는 영향 - 감정적 반응의 매개역할을 중심으로 -

        The purpose of this research is to study about the effect of the crowding degree which the visitors recognize in Job fairs on the emotional response and avoiding behavior. For accomplishing the research purpose, the objects as the viewers of the first Hotel Industry Career Fair in November 27th of 2012 for one day in COEX were actual proof analyzed using SPSS ver.19.0 statistic program. Crowding perception was divided in to human and spatial crowding degrees. Emotional response, also, was divided in to two dimensions, positive and negative responses. The main results were as followings. First, human crowding was indicated to have positive effect on positive emotion only, and spatial crowding was indicated to have negative effect on positive emotion and positive effect on negative emotion. Second, positive emotional response was indicated to have negative effect on avoiding behavior, and negative emotional response was indicated to have positive effect on avoiding behavior. Third, as a verification result of mediation effect on emotional response, it was known that the negative emotion effected on the enhancement of effect relation between human crowding and avoiding behavior significantly.

      • 방사선 손상 Balb/C 마우스 모델에서 누에 체액(Silkworm Hemolymph)의 간조직 보호 효과

        This study was designed to evaluate the protective effect of silkworm hemolymph againstγ-irradiation induced damage in the liver of mice. Female Balb/C mice (6 weeks old) were exposedto γ-irradiation (6 Gy) and administered orally to silkworm hemolymph (5 ml kg-1 BW) for 7 dayspost-irradiation. The body weight, spleen index, plasma aspartate transaminase (AST), plasmaalanine transaminase (ALT), and liver malondialdehyde (MDA) levels were determined. Comparedwith irradiated control mice, the activity of plasma AST and the level of MDA were significantlydecreased in mice treated silkworm hemolymph. These results show that silkworm hemolymph isfound to have a protective effect against γ-irradiation induced damage in mice.
