A case of described where omentum impacted and fenestrated through the anteior uterine wall after induced abortion at the private clinic. The patient had been aborted six times in the past, involving the two recently. And it is postulated that the uterus had been perforated on one of these occasions. We experienced one case of omental impaction in the uterine myometrium after induced abortion and so we present it with brief review of literature.
상피성 난소암 환자의 항암제 내성에 대한 통계학적 분석 : 콕스 모델을 이용한 다변량 분석

Background. Several molecular-genetic alterations in epithelial ovarian cancer, including expression of glutathione S-transferase-π(GST-π), p-glycoprotein(p-gly), and metallothionein(MT), have been associated with drug resistance and poor prognosis. To deterrmine the importance of molecular-genetic factors relative to more traditional surgical-pathologic prognostic factors, a multivariable analysis was performed. Method. Immunohistochemical staining for GST-π, p-gly, and MT was performed in relation to the response to chemotherapy with cisplatin in 87 patients with primary epithelial ovarian cancer who had not received any chemotherapy before surgery. In addition, information regarding surgical-pathologic features of the cancers was obtained. Univariabe analysis was performed followed by multivariable analysis using Cox's proportional hazards model to identify variables predictive of poor prognosis. Results. With univariable analysis, age, FIGO stage, grade, MT, ascites, residual tumor, and initial value oA 125 were predictive of patient's survival. In the multivariable analysis, only age, grade, stage, histologic type and residual tumor were significant. Cox's regression model fitting stratified by stage, MT(p=0.0362) was significant on early stage of cancer, however, histologic type(p=0.0001) was significant in advanced ovarian cancer. Conclusion. Among factors for drug resistance to chemotherapy, MT was the most strongly predictive of survival in early ovarian cancer.
원인불명 습관성유산 환자에서 자용량 아스피린 투여후 혈중 Lipid-Peroxides의 변화

Lipid peroxides(LPO) are toxic compounds that damage cells and inhibit prostacyclin synthase. It is responsible for endothelial cell damage and decreased prostacyclin(PC) levels. Preeclampsia is associated with an imbalance of increased thromboxane(TX) and decreased prostacyclin. Low-dose aspirin therapy reduces the incidence of preeclampsia, presumbly by selective inhibition of TX and LPO, however, there were limited studies in the field of recurrent abortion. This study was done to invesgate the effect of low-dose aspirin on serum LPO levels in patients of unknown recurrent abortion. The followings are summarized results. 1. Of 14 patients of recurrent abortion who were receiving low-dose aspirin(100 mg/day) therapy between 5-12th gestational weeks, 38 tests of LPO were done with TBA method. 2. There were no significant differences of mean serum LPO levels as increasing gestational seeks (21 serial tests in 7 patients). 3. As increasing gestational weeks in each patient, six patients among 7 patients(85.7%) revealed decreasing trends of serum LPO levels during low-dose aspirin therapy. the serum LPO levels were decreased after 48 hours of low-dose aspirin therapy in 4 cases among 10 paired-tests. From these results, we could confirm the low-dose aspirin therapy inhibit serum LPO levels during the first trimester in patients of recurrent abortion. And it is supposed that at least 1 week interval in needed for reducing the level of serum LPO during low-dose aspirin therapy.

The protein encoded by the Bcl-2 proto-oncogene has been shown to prolong cell su-rvival by preventing cell death(Apoptosis) induced by many insults including cancer therap-eutic drugs. Recently many researches have elucidated the bcl-2 expression in several hu-man solid cancers. However, there is still a few avaiable data to determine the role of Bcl-2 expression in the ovarian carcinogenesis and its prognostic significance in ovarian can-cers. Hence, we examined the expression of Bcl-2 in 68 ovarian epithelial cancers using immunohistochemistry and determined whether Bcl-2 expression has prognostic significance in the ovarian epithelial cancers. We found Bcl-2 expression(>5% positive cell) in 31 patients(40%). Bcl-2 expression were exclusively negative in the mucinous type of the ovarian epithelial cancer. Bcl-2 exp- ression was not correlated with tumor stage(stage Ⅰ & Ⅱ vs Ⅲ & Ⅳ, p=0.63). The patients with Bcl-2 positivity had lower recurrence rate than the patients with negativity at the se- cond look operation(p<0.01). Although there was a trend that the patients with Bcl-2 posi- tivity had better acturial survival than the patient with negativity, the stastical significance was not present(3 years acturial survival;+vs-;63%, 29%;p>0.05). These results sug- gest the Bcl-2 expression appears an early event in the ovarian carcinogenesis and has an inhibiting role in progression of ovarian tumor.
