Three species of Chenopodiaceae, i.e. Suaeda japonica, Salicomia herbacea, Beta vulgaris var. cicla, were investigated to compare the physiological characteristics through ionic balances and osmoregulations under different environmental salt gradients. Plants were harvested in two weeks from treatments with salt gradients(0, 50, 100, 200 and 400 mM NaCl) and mineral nutrition gradients(1/1, l/5, 1/10 dilutions of Hoagland solution). Plants were analyzed for growth responses, ionic balances, osmolalities, conductivities, glycinebetaine and proline contents quantitatively. Three plants of Chenopodiaceae accumulated salts into tissues unlike some salt sensitive species, and showed unique adaptation patterns to overcome saline environments, i.e. strong growth stimulation for Salicomia herbacea, growth negative tolerance for Suaeda japonica, and growth positive tolerance for Beta vulgaris var cicla. The absorption of inorganic $Ca^{2+}$ ions was inhibited remarkably due to the excess uptake of $Na^+$ with increasing salinity. The $K^+$ content in plants was significantly reduced with increasing salinity. Total nitrogen content was reduced as mineral nutritions and salinity increases. Conductivity and osmolality increased with increasing salinity regardless of mineral nutritions. The ranges of glycinebetaine and proline contents were $0.2{\sim}2.5{\mu}M/g$ plant water and $0.1{\sim}0.6{\mu}M/g$ plant water, respectively. 건조 및 염 환경에 우점종으로 분포하고 있는 명아주과 식물의 염 및 무기 영양 농도에 따른 생리적 적응특성을 비교하기 위해 염습지에 널리 분포하는 칠면초(Suaeda japonica), 퉁퉁마디(Salicornia herbacea) 및 채소로 재배되는 근대(Beta vulgaris var. cicla)를 재료로 하여 염 농도(0, 50, 100, 200, 400 mM NaCl) 및 무기영양염 농도(Hoagland 용액의 1/1, 1/5, 1/10배액)를 달리하여 식물의 엽생장과, 질소, 무기 이온, 전기 전도도 및 삼투조절물질의 변화를 정량적으로 분석하였다. 3종 명아주과 식물은 무기영양 및 염의 농도변화에 대해 식물체에 상당량의 염을 축적하였으며, 높은 내염성 기구를 통해 400 mM NaCl 처리에서도 현저히 생장을 촉진하는 호염성의 퉁퉁마디와 200 mM NaCl 처리까지 약간의 생장 촉진을 보이는 근대 및 높은 내염성을 가지면서 50 mM NaCl 이상 처리구에서 약간의 생장 저해를 보이는 칠면초의 세가지 염내성의 종 특성을 보였다. 무기이온의 동태에 있어서 3종 식물은 무기 영양 및 염 농도와 무관하게 체내에 소량의 수용성 Ca을 함유하였고($<10{\mu}M/g$ plant water), K이온은 염 농도가 증가함에 따라 다소 감소하였으며, $Na^+$과 $Cl^-$은 배지의 무기 영양과는 무관하게 염 농도가 증가함에 따라 현저히 증가하였다. 총 질소 함량은 무기 영양 및 염의 농도가 증가함에 따라 감소하는 경향을 보였으며, 전기 전도도와 삼투몰농도는 무기영양보다 염의 농도가 증가함에 따라 증가하는 양상을 보였다. Glycinebetaine과 proline의 함량은 각각 $0.2{\sim}2.5{\mu}M/g$ plant water와 $0.1{\sim}0.6{\mu}M/g$ plant water였고, 50 mM NaCl 처리구에서 최대치를 보였다.
서울시내 수계시설에서 분리된 Legionella spp.의 병원성에 대한 분자역학적 연관성

Legionella spp. is the causative agent of Legionellosis, which induces a potentially fatal form of pneumonia. With a concentrated performance during the summer of 2008, we secured 73 isolates from the water systems of 25 wards in Seoul. We analysed serotypes, pathogenic genes (Dot/Icm), and patterns of pulsed-field gel electrophoresis (PFGE) in an attempt to confirm relationships among them. Different from the previous year's pattern (2007), among the isolates, 69 were Legionella pneumophila and 4 were Legionella spp. The serotype distribution of Legionella pneumophila was sg1 43, sg6 9, sg5 8, sg3 8, and sg2 1. The serotype for the 4 Legionella spp. was Legionella nautarum. Most of the Legionella pneumophila had several pathogenic genes. On the other hand, the 4 Legionella spp. were defective in pathogenicity in genomic terms. The PFGE patterns of the serotypes showed a tendency for diversity of Legionella pneumophila and a close correlation with genetic pathogenicity. 레지오넬라균은 치명적인 폐렴을 일으킬 수 있는 균으로서, 기후 온난화와 함께 노출의 수위가 높아지고 있는 병원성균이다. 본 연구는 2008년 6월부터 8월까지 서울시내 25개구 소재 수계시설에서 분리한 레지오넬라균 73개 주에 대해 혈청형, Dot/Icm라 일컬어지는 병원성 유전자 분석 그리고 Pulsed field gel electrophoresis (PFGE)를 실시하여 유전자형과 그들 사이의 상관관계를 파악하였다. 73개주 중 69주는 Legionella pneumophila로 혈청형의 분포는 sg1 43주, sg6 9주, sg5 8주, sg3 8주, sg2 1주로 파악되었으며, Legionella spp. 4개주는 Legionella nautarum였다. 분리된 Legionella pneumophila 대부분이 여러 개의 병원성 유전자를 보유하고 있었으며, 반면에 Legionella nautarum은 병원성 유전자가 많이 결핍되어있었다. PFGE pattern을 분석해 볼 때, Legionella pneumophila가 동일한 혈청형안에서 다양하게 분화되어감을 볼 수 있었으며, 병원성 유전자의 분포와 깊은 상관관계를 보였다.

개화는 배추종 작물의 생산성과 연관된 중요 발달 특성 중 하나이다. 이식 후, 갑작스러운 저온에 노출되어 때이른 개화를 하게 되면 수확되는 생산물의 양과 질이 떨어지게 된다. 따라서, 개화조절 메커니즘을 이해하는 것은 배추 종 작물의 농업적 생산성을 향상시키는데 도움을 줄 것이다. 춘화는 배추과 작물에서 일반적으로 알려져 있는 개화를 유도하는 중요한 요소이다. 그러나 옐로우 사순이나 코마수나와 같은 배추 아종은 춘화처리 없이도 개화한다. 1일을 주기로 하여 생물의 생리기작을 조절하는 생체시계 유전자는 일장감응형의 개화 조절에 중요한 역할을 하지만 춘화처리를 통해 개화를 유도하는 기작과도 연관되어 있다. 본 논문에서는 22개의 배추 아종을 개화에 춘화처리가 필요한 춘화형과 춘화처리 없이도 개화하는 비춘화형으로 나누어 보존된 생체시계 유전자, BrPRR1 군의 염기서열 분석을 수행하였다. 그 중 BrPRR1b 유전자의 결손 영역으로 춘화형과 비춘화형 두 그룹을 구분할 수 있었다. 이 서열변이를 증폭할 수 있는 PCR 프라이머를 디자인하여 비춘화형 배추 아종에서는 451 bp의 긴밴드를, 춘화형 배추에서는 422 bp의 작은 크기의 밴드를 증폭할 수 있었다. 이 프라이머 세트는 43개 배추 아종과 4개의 배추속 작물, 브로콜리, 양배추, 갓, 그리고 유채의 개화형을 구분하는데 적용되었다. 각 작물의 PCR 결과와 개화시기에 대한 정보를 통하여 프라이머 세트가 개화형을 판별할 수 있는 마커로 이용될 수 있음이 확인되었다. 이 마커시스템은 배추 종 작물 육종에 유묘 단계에서 개화형을 판단하는데 이용할 수 을 것이다. 또한 이 결과들은 생체시계 유전자가 배추 종 작물의 개화를 조절하는 좋은 전략이 될 수 있음을 보여주었다. Flowering is one of the most important development traits related to the production of Brassica rapa crops. After planting, a sudden low temperature triggers premature flowering, which leads to a reduction in the yield and quality of harvested production. Therefore, understanding the mechanism of flowering control is important in the agricultural productivity for preventing Brassica rapa crops. Vernalization is generally known as the main factor of flowering in the Brassica plant. However, in the subspecies of Brassica rapa, some accession such as Yellow sarson and Komatsuna display the flowering phenotype without vernalization. Circadian genes, which diurnally regulate plant physiology, have a role for photoperiodic flowering but are related to the regulation of the vernalizarion mechanism. In this report, the 22 B. rapa accession were divided into two groups, vernalization and non-vernalization, and the sequenced circadian gene, BrPRR1s. Among them, the BrPRR1b gene was found to have deletion regions, which could classify the two groups. The PCR primer was designed to amplify a short band of 422bp in the vernalization type and a long band of 451bp in the non-vernalization type. This primer set was applied to distinguish the flowering types in the 43 B. rapa accession and 4 Brassica genus crop, Broccoli, cabbage, mustard, and rape. The PCR analysis results and flowering time information of each crop demonstrated that the primer set can be used as marker to discern the flowering type in Brassica crops. This marker system can be applied to the B. rapa breeding when selecting the flowering character of new progenies or introducing varieties at an early stage. In addition, these results displayed that the circadian clock genes can be a good strategy for the flowering control of B. rapa crops.
반복적인 구토 및 간헐적 의식 변화를 주소로 진단된 Ornithine Transcarbamylase Deficiency 여아 1례

저자들은 출생 직후에서 생후 7개월까지는 특별한 과거력 없이 건강하게 지내던 여아에서 생후 11개월에 반복적인 구토 및 간헐적인 의식변화와 경련으로 시행한 검사 결과 고암모니아 혈증 및 소변 orotic acid의 증가를 통해 OTC 결핍증을 진단하고, 분자 유전자 검사로 418번째 염기인 G가 C로 치환되어 140번 아미노산이 alanine에서 proline으로 바뀌는 돌연변이(p. Ala140Pro)를 확인한 1례를 경험하였기에 보고하는 바이다. 본 증례의 돌연변이는 환아의 부모 유전자 분석 결과 정상 소견으로 de novo mutation으로 사료된다. 환아는 출생 후 수 개월간 큰 이상 없이 성장 및 발달을 보였으나 생후 7개월 경부터 주기적인 구토 증세를 보였던 지발형 OTC 결핍증에 해당한다. 진단 후 arginine 및 phenylbutylate를 지속적으로 복용하며 저단백 식이를 유지하는 보존적 치료를 지속하여 현재 발달 및 성장 속도 양호하며 일상생활에 문제없이 지내고 있다. 따라서 출생 후 수개월간 특이 질환력이 없는 환아, 특히 여아에서 주기적인 구토, 기면, 경련 및 혼수 등의 증상을 보일 때 지발형 OTC 결핍증과 같은 대사질환을 의심할 수 있어야 한다. 본 증례는 반복적인 구토 및 의식변화를 보인 기저질환 없는 11개월 여아에서 OTC 결핍증을 진단하고 즉각적인 대처를 통해 7세까지 심각한 신경학적 장애 없이 정상적인 성장과 발달을 보인 환자를 보고하는 바이다. Ornithine transcarbamylase (OTC) deficiency is a rare X-linked genetic disorder of urea synthesis in newborns. It is the most common urea cycle disorder and leads to elevated levels of ammonia in the blood. Excessive ammonia can cause various symptoms, including vomiting, lethargy, and coma. Boys have a more serious form of OTC deficiency than girls. If not treated immediately, severe OTC deficiency can lead to neurologic abnormalities, hyperammonemic coma, and death. Because late-onset OTC deficiency, which is more common in girls, presents mild symptoms, it is easy to miss diagnosis and prompt treatment. We describe an 11-month-old girl who presented periodic vomiting, intermittent lethargy, and seizure. She was diagnosed with OTC deficiency by elevated serum ammonia and urine orotic acid levels. Genetic analysis of the OTC gene revealed a missense mutation in exon 5 (c.418G>C). We reported an experience of exact diagnosis and successful treatment of late-onset OTC deficiency in our patient.
브린졸라미드 복합제의 개방각 녹내장 또는 고안압증에 대한 효과 및 안전성 검증: 체계적 문헌 고찰 및 메타분석 연구

Background: The treatment of primary open-angle glaucoma (POAG) or ocular hypertension (OHT) for intraocular pressure (IOP) reduction is recommended to proceed with the use of the compound. Therefore, this study aimed to evaluate the efficacy and safety of brinzolamide combination therapy on POAG and OHT following the subgroup analysis among types of brinzolamide combined medications. Methods: By June 2019, PubMed, EMBASE and Cochrane Library were searched to find a study that met our inclusion criteria. Based on randomized control trials (RCTs), we collected studies that tested the brinzolamide combination therapy in POAG and OHT patients, and analyzed the literature identified by the results of the study on IOP reduction and adverse reactions. Results: A total of 13 literature was collected to conduct an analysis including 2,197 patients. The intervention included brinzolamide combination therapies, combined with timolol, brimonidine, PGA or combined with both brimonidine and PGA. The analysis showed significant decreasing tendency for values at morning and end treatment per day in the use of brinzolamide combination therapy in the absolute IOP change (mean difference (MD) -1.41; 95% CI -1.92, -0.90; p<0.001 vs. MD -1.46; 95% CI -2.03, -0.89; p<0.00001, respectively). We could see higher adverse reactions in the brinzolamide combination group using intervention (odds ratio 1.43; 95% CI 1.20, 1.71; p<0.0001). Conclusion: Regarding IOP reduction in POAG and OHT patients, brinzolamide combination therapy is more effective but less safe than control treatment, which diverse among types of combined medications. Thus, more individualized therapy should be applied in real-world practice.

This study was peformed to evaluate the served menu and investigate the management of food service in Korean Buddhist temples. Among the available temples in the nation, 34 temples were carefully selected considering practice type, location and the gender characteristics. A survey about meal preparation and management was conducted to the cooking staff in each the temple using questionnaire by interview between Jan 2004 and Aug 2004. The menus over A five consecutive days menu was were also collected for analysis to analyse. Civilian cooking staffs were preparing meals in 23(67.6%) of the temples and the proportion 18(52.9%) of the temples were planning their own the menus was 52.9%. Most kitchens in the temples were equipped with modernized kitchen appliances. The major food supply was the conventional market Even though the majority of the temples were using processed food, Korean fermented sauces as the most traditional temple food products were prepared by themselves. There were 114 menu patterns and the most frequently served meal pattern was ${\ulcorner}Rice+Soup+Kimchi+3{\sim}4\;side\;dishes{\lrcorner}$. Analyzing from the cultural characteristics point of view, the most frequently served dish type was 'Korean'(90%), while other types were 'modified Korean'(3.7%) and 'Western'(2.7%). The varieties of Various 438 different dishes provided were 438 provided and with Kimchies (19.8%), Seasoned vegetables(16.8%) and Rice(11.0%) being the highest were high in frequency when dishes were categorized into dish classes. Among the main dishes, the frequency of gruel(13.7%) was relatively high. These results suggest the need to conduct Based upon the results found, it was required to continue further research about preserving and inheriting the originality and uniqueness of the temple food.
사례분석을 통한 『상한론(傷寒論)』 변병진단체계(辨病診斷體系)의 서사의학적 가치의 탐색

Objective : The purpose of this study is exploring value of Shanghanlun six diagnostic system as Narrative Medicine by analyzing cases. Methods : In the article, we examined the theory of narrative medicine and current studies. And then key elements of narrative medicine was defined. On the basis of these, two medical cases were analyzed by reordering in accordance with time sequences and causality for confirming narrative factors. Results : The narrative approach to analyzing cases shows us that different diseases could be the result of same pathological patterns based on Shanghanlun. This tells us following four aspects. [1] Shanghanlun is the causes-oriented records and the text includes narrative factors. [2] Shanghanlun six diagnostic system is a process of constructing plot of medical case history through the clinical interpretation of contextual meaning of patient's life. [3] In the process of diagnosis, Schemata and script are engaged in assuming and confirming six patterns diseases. [4] The subjective factors of an individual can be applied universally through the provision of Shanghanlun. Conclusions : Narrative factors of Shanghanlun six diagnostic system suggest the possibility and value of Shanghan Medicine as Narrative Medicine in terms of Medical Humanities, essence of medical diagnosis and therapeutic action.
조기 혈관 재개통으로 뇌졸중 초기에 회복을 보인 환자 1예

Objective: The purpose of this case study is to show a case of early neurological recovery from stroke by recannalization. Methods: The patient was admitted from 12 July, 2004 to 26 July, 2004. The patient received Herb-Medication and Acupuncture Therapy. Brain MRI and Tc-99m HM-PAO brain SPECT was used to diagnose and prognose. Results: The patient fully recovered within a few hours. She was diagnosed as acute infarction in Lt. BG by using brain MRI. Tc-99m HM-PAO brain SPECT showed focal increased perfusion at Lt. BG. Conclusions: We suggest that the important thing to recovery from stroke is recannalization of cerebral arteries in early stage.
