Railway bridge contact of vehicle needs to design considering the running safety about the running train load of the railway bridge ride comfort and dynamic safety. Also upper structure of the railway bridge has to satisfy design standard about moving load(train). So the railway bridge has to satisfy the requirement for vertical acceleration of the bridge deck vertical displacement of the bridge and face distortion which is suggested railway design standard in Korea(2011.5.). In this study it was investigated and evaluated to the running safety about the running train load of the railway bridge ride comfort and dynamic safety with railway design standard for steel composite(Steel Box Girder) railway bridge considering KTX freight train and standard train load.
열차하중이 장대레일궤도의 도상 횡방향 저항력에 미치는 영향

장대레일 궤도의 비선형 해석은 주로 온도하중만을 고려하여 분석이 되었으나, 실제로 열차하중이 레일에 가해졌을 시 축하중에 의하여 부반력들이 발생하고, 이러한 부반력은 레일의 들림현상이 발생시킨다. 도상 횡방향 저항력의 저하는 장대레일 궤도의 좌굴에 큰 영향을 미친다. 열차하중을 고려한 장대레일 궤도의 좌굴거동을 분석하기 위하여 첫 단계로서 선형해석을 통하여 열차하중 재하 시 침목에 발생하는 연직방향 반력을 산출하고, 두 번째 각각의 침목에 발생하는 반력에 따른 도상 횡방향 저항력의 변화량은 산정하여, 마지막으로 재료 및 기하비선형해석 시 각각의 침목위치에 재 산정된 도상 횡방향 저항력의 값을 적용하여 해석을 수행 하였다. High thermal axial force and vehicle loads cause the track to shift in a lateral direction and the formation of track geometry imperfections (track irregularity). When the thermal stress level and track irregularity with vehicle load reach a critical value, the track loses stability. In many studies, the stability of CWR tracks is analyzed. However these studies are only considered in temperature load. The ballast resistance is changed by wheel load. When the wheel load is applied, rails and ties are moved upward or downward. In this case the friction between ties and ballasts is decreased or increased. In this study the change of the ballast resistance of each tie was applied to the nonlinear analysis of CWR tracks.

Actinomycosis is a chronic granulomatous and suppurative disease caused by Actinomyces species. As a pathogen, it can cause fistulas or sinuses, and may form a mass. Sulfur granules in the lesion, sinu walls or discharge are characteristics of actinomycosis. The most common location of abdominal actinomycosis is the appendix or ileocecal region of the intestine. In addition, actinomycosis has been found in the rectum, the sigmoid and the transverse colon as well as the liver, the pancreas and th pelvis. However, primary rectus abdominis actinomycosis is a rare form of actinomycosis. We repor a case of primary rectus abdominis actinomycosis presenting an abdominal mass in a 60-year-old man whose chief complaint is fever.

Angiodysplasia is probably responsible for 2.6-6.2% of cases involving of lower gastrointestinal bleeding and 1.2-8.0% of cases involving hemorrhages from the upper GI tract. Small bowel neoplasia is rare, accounting for about 5% of gastrointestinal tumors overall and 2-3% of all malignacies. The third most common malignany of the small bowel is the sarcoma, of which the leiomyosarcoma is the most frequent. A 54-year-old male patient was admitted with the chief complaints of dizziness and headache during 2 months. Laboratory findings revealed iron deficiency anemia. A superior mesenteric arteriography found an intensive vascular stained mass in the hepatic flexure. A celiac artery angiography discovered a irregulary vascular stained lesion in the Ll vertebral level. A colonoscopy located a 10 mm sized angiodysplasia in the right colon. According to these findings, we presumed that these lesions are a colonic angiodysplasia and a suspicious duodenal lesion. The operation was perfomed. The final diagnosis was a colonic angiodysplasia combined with leiomyosarcoma of the duodenum. The rarity of this case is emphasized and the literative reviewed.
지속성 신대체요법(Continuous Renal Replacement Therapy)으로 간기능이 회복된 전격성 간부전 1예

The likelihood of survival from folminant hepatic failure is extremely poor despite various therapeutic approaches. The liver transplantation has had the single test impact on the management of fulminant hepatic failure, but liver transplantation is risky and expensive. Moreover, a shortage of liver donors makes it difficult to procure an organ within a useful time frame. For these reasons, many efforts have been made to provide meticulous support to permit regeneration of liver parenchyme or to bridge patients to liver transplantation. One of them is the continuous renal replacement therapy (CRRT). Since the CRRT provides the greatest cardiac and intracranial stability, it becomes a preferred treatment for patients with hepatic failure which develop acute renal failure. We report a case of fulmlinant hepatic failure accompanying with acute renal failure. The patient in this case recovered during CRRT.

Background: A Stellate ganglion block (SGB) is a sympathetic nerve block method which has been used most frequently in pain clinics due to its wide range of indications. However, SGB leads to regional sympathetic denervation of the heart and to changes in the hemodynamics. The aim of this study was to compare hemodynamic effects as well as echocardiographic changes after a left SGB (LSGB). Methods: Fourteen healthy male volunteers were studied. The LSGB was performed with 1% mepicacaine 6 ml. Arterial blood pressure, electrocardiographic and echocardiographic variables were measured before the LSGB, 15 and 30 minutes after the LSGB. Results: Arterial blood pressure, ejection fraction and transmitral inflow velocity variables showed no significant changes compared to pre-LSGB values. P-P interval increased significantly 15 minutes after the LSGB, and the Q-T interval increased significantly 30 minutes after the LSGB. The diastolic pulmonary venous flow velocity decreased significantly 15 minutes after the LSGB. Conclusions: These results showed that a LSGB decreased the heart rate without detrimental changes of left ventricular relaxation in healthy male volunteers. (Korean J Anesthesiol 2000; 39: 798-803)
