        만성 요통근로자의 능동적 운동프로그램 효과

        목적: 본 연구는 만성 요통근로자의 세 가지 능동적 운동프로그램 효과에 관한 연구로서 능동적 운동이 요추의 안정성 향상에 미치는 영향을 알아보고자 시도되었다. 방법: 3개월 이상 만성요통을 호소하는 64명의 근로자대상으로 무작위로 일반적인 물리치료그룹, 요추 운동그룹, 흉추 운동그룹, 세 그룹으로 나누었다. 능동적 운동 프로그램은 주 3회, 6개월 동안 실시하였으며, 통증강도, Oswestry 장애지수, 양손을 머리위로 최대한 올려 높이 측정, 요추부 경사각도를 평가하였다. 모든 대상자들은 연구 전, 연구 후 2주, 3개월, 6개월을 같은 측정방법으로 재평가하였다. 결과: 통증강도는 세 그룹 간에 통계적으로 유의한 차이가 없었다. Oswestry 장애지수는 연구 후 6개월에서 세 그룹 간의 유의한 차이가 있었으며, 요추운동그룹과 흉추운동그룹이 일반적인 물리치료그룹에 비하여 유의하게 감소하였다(p<0.05). 양손을 머리위로 최대한 올려 높이측정은 연구 후 3개월과 6개월에서 세 그룹 간에 유의한 차이가 있었으며, 연구 후 6개월에서는 흉추운동그룹이 일반적인 물리치료그룹과 요추운동그룹에 비하여 유의하게 증가하였다(p<0.05). 요추부 경사각도는 연구 후 2주와 3개월, 6개월에서 세 그룹 간에 유의한 차이가 있었으며, 연구 후 6개월에서는 흉추운동그룹이 일반적인 물리치료그룹과 요추운동그룹에 비하여 유의하게 감소하였다(p<0.05). 결론: 3개월 이상 만성 요통을 호소하는 근로자 대상으로 흉추의 운동성 증가를 위한 운동프로그램을 적용하여 흉추의 운동성을 증가시킨 결과에서 보면, 2주 시행하였을 때 요추의 신전 운동프로그램 보다 요추의 안정성 향상에 효과적이며, 6개월 시행하였을 때만, 요추의 심부근 근력강화 운동프로그램보다 요추의 안정성 향상에 효과적이었다. Oswestry 장애지수는 요추의 심부근 근력강화 운동프로그램과 동일하게 감소하였고, 통증강도는 세 그룹 모두 2주 시행하였을 때 감소하는 것으로 나타났다. Objective: To examine the relative efficacy of three active exercise programs for work-related, chronic low back pain, and to observe to what extent the programs affected the mechanical stability of the lumbar region. Methods: The subjects were 64 employees who were randomly divided into three groups to match the three active exercise programs which were performed 3 times a week for 6 months. All subjects were assessed with the same measurements at a pre-study examination, and then were reassessed at 2 weeks, 3 months and 6 months after the study. Results: The pain intensity didn' t show any significant difference among the three groups. However, the Oswestry Disability Index showed significant differences among the three groups at 6 months and the lumbar and thoracic exercise groups showed significant decreases compared to the general physiotherapy group (p<0.05). Maximal stretching with both hands in the overhead direction showed a significant difference among the three groups at 3 months and 6 months, and the thoracic exercise group at 6 months showed a significant increase in overhead stretching compared to the lumbar exercise and general physiotherapy groups (p<0.05). The lumbar region angle of inclination showed significant differences among the three groups at 2 weeks, 3 months and 6 months, with the thoracic exercise group being decreased significantly more than the lumbar exercise and general physiotherapy groups at 6 months (p<0.05). Conclusions: Exercise aimed at increasing thoracic mobility has an effect on lumbar stability. Furthermore, it is far more effective for lumbar stabilization than general physiotherapy and deep muscle strengthening lumbar exercise.

      • 걷기 운동시 덤벨 부하가 비만중년여성의 신체구성 및 혈중지질성분에 미치는 영향

        본 연구는 체중의 4%중량 덤벨을 부하한 걷기 운동이 비만 중년여성의 신체구성과 혈중지질성분에 어떠한 영향을 미치는지를 알아보기 위하여 체지방율이 30% 이상인 비만 중년여성을 대상으로 무부하집단, 부하집단으로 나누어 8주간 운동을 실시한 후 신체구성 변인(체중, 체지방량, 근육량, WHR)과 혈중지질 변인(TC, TG, HDL-C, LDL-C)을 측정 및 분석한 결과 다음과 같은 결론을 얻었다. 1. 신체구성 변인 1) 체중, 체지방량, WHR은 집단별로 유의한 차이가 없었지만, 사전에 비해 사후에 유의하게 감소하였다. 2) 근육량은 집단과 시점별로 유의한 차이가 없었고, 사전에 비해 사후에도 유의한 차이가 없었다. 2. 혈중지질 변인 1) TC와 TG는 집단별로 유의한 차이가 없었지만, 사전에 비해 사후에 유의하게 감소하였다. 2) HDL-C는 집단별로는 유의한 차이가 없었지만, 사전에 비해 사후에 유의하게 증가하였다. 집단과 시점의 상호작용효과도 나타났다. 3) LDL-C는 집단별로는 유의한 차이가 없었지만, 사전에 비해 사후에 유의하게 감소하였ㄷ. 집단과 시점의 상호작용효과도 나타났다. 결론적으로 무부하 걷기운동과 체중의 4%중량의 덤벨을 부하한 걷기운동이 비만 중년여성의 신체구성과 혈중지질성분에 긍정적인 변화를 미친 것으로 볼 수 있다. 그러나 본 연구에서 무부하 걷기운동과 체중의 4%중량의 덤벨을 부하한 걷기 운동간의 뚜렷한 차이를 규명해 내지 못하였다. 이 부분에 대해서는 앞으로 지속적인 연구가 필요할 것으로 생각된다.

        The Effect of Imiquimod on Matrix Metalloproteinases and Tissue Inhibitors of Metalloproteinases in Malignant Melanoma Cell Invasion

        Background: A number of reports have been published regarding the use of imiquimod for the treatment of melanoma in situ and metastatic melanoma. Essential steps in the process of melanoma invasion and metastasis include degradation of basement membranes and remodeling of the extracellular matrix by proteolytic enzymes, including matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMPs). Objective: To evaluate the antiinvasive effect of imiquimod in human malignant melanoma cell lines, SK-MEL-2 and SK-MEL-24, in vitro, and to investigate imiquimod-induced changes in the expression of MMPs and TIMPs. Methods: Invasiveness of melanoma cell lines following imiquimod treatment was evaluated by invasion assays. In order to investigate the mechanism of the anti-invasive effect of imiquimod, mRNA and protein levels of MMP-2, -9, membrane type 1 (MT1)-MMP, TIMP-1, and -2 were assessed by real-time reverse transcription-polymerase chain reaction, gelatin zymography, and western blotting. Results: Imiquimod treatment decreased in vitro viability of melanoma cells in a concentration-dependent manner. Imiquimod also elicited a concentration-dependent suppression of invasion in both melanoma cell lines. A concentration- dependent decrease in MMP-2 and MT1-MMP protein levels and a concentration-dependent increase in TIMP-1 and -2 protein levels by imiquimod was observed in both melanoma cell lines. However, expression of MMP-9 protein was increased in SK-MEL-2 but decreased in SK-MEL-24 with increasing imiquimod concentrations. Imiquimod elicited alterations in MMPs and TIMPs mRNA levels that parallel the observed changes in protein levels.         Baseline Clinical and Biomarker Characteristics of Biobank Innovations for Chronic Cerebrovascular Disease With Alzheimer’s Disease Study: BICWALZS

        Objective We aimed to present the study design and baseline cross-sectional participant characteristics of biobank innovations for chronic cerebrovascular disease with Alzheimer’s disease study (BICWALZS) participants.Methods A total of 1,013 participants were enrolled in BICWALZS from October 2016 to December 2020. All participants underwent clinical assessments, basic blood tests, and standardized neuropsychological tests (n=1,013). We performed brain magnetic resonance imaging (MRI, n=817), brain amyloid positron emission tomography (PET, n=713), single nucleotide polymorphism microarray chip (K-Chip, n=949), locomotor activity assessment (actigraphy, n=200), and patient-derived dermal fibroblast sampling (n=175) on a subset of participants.Results The mean age was 72.8 years, and 658 (65.0%) were females. Based on clinical assessments, total of 168, 534, 211, 80, and 20 had subjective cognitive decline, mild cognitive impairment (MCI), Alzheimer’s dementia, vascular dementia, and other types of dementia or not otherwise specified, respectively. Based on neuroimaging biomarkers and cognition, 199, 159, 78, and 204 were cognitively normal (CN), Alzheimer’s disease (AD)-related cognitive impairment, vascular cognitive impairment, and not otherwise specified due to mixed pathology (NOS). Each group exhibited many differences in various clinical, neuropsychological, and neuroimaging results at baseline. Baseline characteristics of BICWALZS participants in the MCI, AD, and vascular dementia groups were generally acceptable and consistent with 26 worldwide dementia cohorts and another independent AD cohort in Korea.Conclusion The BICWALZS is a prospective and longitudinal study assessing various clinical and biomarker characteristics in older adults with cognitive complaints. Details of the recruitment process, methodology, and baseline assessment results are described in this paper.

        Ⅰ. 서 론 Ⅱ. 이론적 배경 Ⅲ. 연구모형 및 가설설정 Ⅳ. 실증분석 Ⅴ. 결 론 참고문헌 본 연구에서는 자유무역지역, 외국인투자지역, 경제자유구역과 같은 특수 경제구역 중 자유 무역지역을 대상으로 자유무역지역 내 존재하는 투자요인과 여러 속성을 고찰한 후, 기업측면 의 자유무역지역 투자 고려요인이 자유무역 지역 참여의사와 기대편익에는 어떠한 영향을 미치는 지에 대해 설문조사를 통한 실증분석을 실시하였다. 연구결과, 자유무역지역 투자고려 요인은 탄력성, 지원성, 가용성, 연계성, 활용성 등의 5개 요인으로 도출되었으며, 이들 요인 중 탄력성, 가용성, 연계성, 활용성 등은 자유무역지역 참여 의사에 영향을 미치는 것으로 나타났으며, 기대편익에 영향을 미치는 요인으로서는 탄력성과 활용성인 것으로 분석되었다. 마지막으로 자유무역지역의 기대편익은 자유무역지역 참여의사에 영향을 미치는 것으로 분석되어져 자유무역의 활성화를 위해서는 해당 지역 투자 시 기업이 공유하게 될 이점을 강조하는 것이 필요 할 것이다.

      • Tumor Mesenchymal Stem-Like Cell as a Prognostic Marker in Primary Glioblastoma

        <P>The isolation from brain tumors of tumor mesenchymal stem-like cells (tMSLCs) suggests that these cells play a role in creating a microenvironment for tumor initiation and progression. The clinical characteristics of patients with primary glioblastoma (pGBM) positive for tMSLCs have not been determined. This study analyzed samples from 82 patients with pGBM who had undergone tumor removal, pathological diagnosis, and isolation of tMSLC from April 2009 to October 2014. Survival, extent of resection, molecular markers, and tMSLC culture results were statistically evaluated. Median overall survival was 18.6 months, 15.0 months in tMSLC-positive patients and 29.5 months in tMSLC-negative patients (<I>P</I> = 0.014). Multivariate cox regression model showed isolation of tMSLC (OR = 2.5, 95% CI = 1.1~5.6, <I>P</I> = 0.021) showed poor outcome while larger extent of resection (OR = 0.5, 95% CI = 0.2~0.8, <I>P</I> = 0.011) has association with better outcome. The presence of tMSLCs isolated from the specimen of pGBM is associated with the survival of patient.</P>

        Development of In Vitro Co-Culture Model to Mimic the Cell to Cell Communication in Response to Urban PM<sub>2.5</sub>

        Background: Airborne particulate matter (PM), a widespread air contaminant, is a complex mixture of solids and aerosols composed of particles suspended in the air. PM is associated with inflammatory responses and may worsen inflammatory skin diseases. However, the mechanisms through which PM affects atopic dermatitis (AD) remain unclear. Objective: To establish an in vitro model that more accurately mimics AD using human keratinocyte (HaCaT), dermal fibroblast (HDF), and mast cell (HMC-1) and using this model to investigate the mechanism through which PMs affect AD. Methods: An AD-like in vitro model was established by seeding HaCaT, HDF, and HMC-1 cells with recombinant human interleukin (IL)-1α and polyinosinic:polycytidylic acid. We confirmed the effect of PM on the inflammatory cytokine expression of a triple-cell culture model. SRM 1649b Urban Dust, which is mainly composed of polycyclic aromatic hydrocarbons, was used as the reference PM. The effects of PM on the expression levels of proinflammatory cytokines and skin barrier markers were assessed using quantitative real-time polymerase chain reaction and western blotting. Inflammatory cytokine levels were measured using an enzyme-linked immunosorbent assay. Results: Interactions between various skin cell types were evaluated using a co-culture system. PM treatment increased mRNA and protein levels of the inflammatory cytokines IL-6, IL-1α, tumor necrosis factor-α, IL-4, and IL-1β and decreased the expression of the skin barrier markers filaggrin and loricrin. Conclusion: Our results suggest that an in vitro triple-cell culture model using HaCaT, HDF, and HMC-1 cells may be reliable for obtaining more physiological, functional, and reproducible data on AD and skin barriers.

        Photodynamic Therapy Using Photosensitizer-Encapsulated Polymeric Nanoparticle to Overcome ATP-Binding Cassette Transporter Subfamily G2 Function in Pancreatic Cancer

        <P>Chlorin-based photosensitizers are commonly used in photodynamic therapy (PDT). These drugs are effluxed by cell membrane transporters, such as the ATP-binding cassette subfamily G member 2 (ABCG2). PDT efficacy is limited in tumor cells expressing high levels of these proteins. Pancreatic cancer cell lines AsPC-1 and MIA PaCa-2, which have high and low ABCG2 expression, respectively, were used, and ABCG2-over-expressing MIA PaCa-2 cells were generated. We compared PDT efficacy between chlorin e6 (Ce6) and cationic photosensitizer-encapsulated polymeric nanoparticle (PS-pNP), which is comprised with Ce6, polyethylene glycol, and polyethylenimine. The intracellular concentration of Ce6 was significantly higher in MIA PaCa-2 cells than in AsPC-1 or ABCG2-overexpressing MIA PaCa-2 cells. PS-pNP increased intracellular levels of the photosensitizer in all cell lines. The cell viability experiments indicated increased Ce6 resistance in ABCG2-overexpressing cells. In contrast, PS-pNP produced similar levels of cytotoxicity in each of the cancer cell lines tested. Singlet oxygen production was higher in cells treated with PS-pNP than in those treated with Ce6. Furthermore, in heterotopic and orthotopic AsPC-1 xenograft mouse models, PDT using PS-pNP significantly reduced tumor volume in comparison with that of Ce6 treatment. PS-pNP could increase intracellular Ce6 concentration, which was related with reduced ABCG2-mediated efflux of Ce6, thereby enhancing the effects of PDT in pancreatic cancer cells. (C)2017 AACR.</P>

      • RGS1 expression is associated with poor prognosis in multiple myeloma

        <P>Aims Multiple myeloma (MM) is an invariably fatal disease with highly heterogeneous outcome. Because of this heterogeneity of MM, risk stratification is crucial for therapeutic decision-making. However, no immunohistochemical prognostic or predictive markers have been established yet. The expression of regulator of G-protein signalling (RGS) proteins, which desensitise G-protein-coupled receptor signalling, has been reported to be associated with the prognosis of various malignancies. Recently, our group demonstrated the importance of RGS1 in chemokine signalling in a human MM cell line and normal plasmablasts. In the present study, we explored the prognostic value of RGS1 expression in patients with MM using immunohistochemistry. Methods We evaluated RGS1 protein expression in 79 bone marrow biopsies obtained from patients with MM between 2008 and 2010 at Asan Medical Center. Correlations between RGS1 expression and clinicopathological factors were analysed. Results High RGS1 protein expression was significantly associated with poor overall survival (p=0.005). After an adjusted multivariable analysis, high RGS1 protein expression (p=0.010), high International Myeloma Working Group risk (p=0.003) and high serum lactate dehydrogenase levels (p=0.040) were significantly associated with poor outcomes. Conclusions RGS1 expression may be a prognostic marker for risk stratification and a promising target for the development of a new MM therapy.</P>
