        연쇄알구균 감염에 의한 급성 인후염 후 반응성 관절염 1예

        연쇄알구균에 의한 급성 인후염 후 발생한 반응성 관절염은 급성 류마티스열의 개정된 진단기준을 충족하지 못하면서, 지속적 혹은 재발성의 비이동성 관절염과 발열, 홍반성 결절, 일시적 간기능 이상 등을 보이면서 심장염과 무도증을 드물게 동반하는 질환이다. 따라서, 발열을 동반한 급성 관절염 환자에서 감별해야 할 질환 중의 하나이며, 심근염 예방을 위한 페니실린 요법의 적응증 및 기간에 대해서는 아직 이론이 많은 상황으로, 보다 광범위한 연구가 필요하다. 저자들은 연쇄알구균에 의한 급성 인후염 1주 후에 발생한 하지의 다발성 비이동성 관절염과 함께 결절홍반 및 일시적 간기능 이상을 보인 환자에서 PSRA로 진단하고 비스테로이드성 항염증제 및 스테로이드로 성공적인 치료를 하였기에 이를 문헌 고찰과 함께 보고하는 바이다. Poststreptococcal reactive arthritis (PSRA) is associated with recent streptococcal infections. However, PSRA is distinct from acute rheumatic fever by its clinical manifestations: non-migrating arthritis, erythema nodosum or erythema multiforme, and transient elevation of serum transaminases. We experienced a 33-year-old man who presented with fever, arthritis of both knees and ankles, and erythema nodosums on extensor surfaces of lower legs which developed 6 days after the onset of pharyngitis symptoms. Blood and urine cultures were negative. Throat culture was negative for group A β-hemolytic streptococcus. The ASO titers increased up to 2080 IU/mL in sequential monitoring. The result of bone scan was compatible to arthritis of both knees and ankles. There were no signs or symptom of carditis. He showed clinical improvement with anti-inflammatory drugs (naproxen 1,000 mg/day and prednisolone 7.5 mg/d). PSRA should be included in the differential diagnosis of patients presenting with arthritis combined with fever.

        cDNA array 방법을 이용한 망간에 노출된 흰쥐 뇌기저핵의 유전자발현 분석

        Objectives: This study investigated the gene expression profile in basal ganglia of manganese-exposed rats based on cDNA array analysis. Methods: For cDNA array, 25 male Sprague-Dawley rats (250±25 g) were intraperitoneally injected with 25 ㎎/㎏ B. W./day of MnCl2 (0.3 ㎖) for 10 days. For dose-related gene expression analysis, rats were intraperitoneally injected with 0.2, 1.0, and 5.0 ㎎/㎏ B. W/day of MnCl2 for 10 days. Control rats were injected with an equal volume of saline. RNA samples were extracted from brain tissue and reverse-transcribed in the presence of [α^(32)P]-dATP. Membrane sets of the Atlas Rat 1.2 array Ⅱ and Toxicology array 1.2 kit (Clontech, Palo Alto, CA) were hybridized with cDNA probe sets. Northern blot hybridization method was employed to assess the dose-related gene expression. Results: Fifty-two genes showed significant changes in expression of more than two-fold. Twenty-eight were up-regulated and 24 were down-regulated in the manganese-exposed group compared to the control. Among the 52 genes, 28 genes including nuclear factor I-X1 (NF1-X1), neuroligin 2 and 3, mitochondrial stress-70 protein (MTHSP70), neurodegeneration-associated protein 1 (Neurodap 1), multidrug resistance protein (MDR), and endoplasmic reticulum stress protein 72 (ERP72), were reported for the first time related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia. According to the dose-related gene expression analyses, MTHSP70, Neurodap 1 and ERP72 genes were up-regulated compared to the control even in the group exposed to low manganese dose ( 0.2 ㎎/㎏ B.W./day). Conclusions: Twenty-eight genes detected for the first time in this study were closely related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia and further study of these genes can give some more useful information about the manganese metabolism.

      • 미용사들의 이직욕구에 관한 연구 : 직무스트레스, 근골격계 증상을 중심으로 Focusing on Musculoskeletal Symptoms and Occupational Stress

        Objective : This study was conducted to prepare the fundamental data on desire of job transfer in hairdresser and prevent the health impairment of hairdresser due to occupational stress and musculosketetal symptoms. Methods and Material : The author surveyed the desire of job transfer and muscurlosketetal symptoms and occupational stress with self-reported questionnaire to 105 hairdresser who were working in beautyshop of Busan area and also 121 student in department of cosmestology as control group. Results : The results were as follows: 1. The rates of self reported symptom were 81.0% for shoulder, 72.4% for back, 67.6% for knee, 58.1% for hand in hairdresser, and 79.3% for shoulder, 71.9% for back, 66.9% for neck, 64.5% for hand and knee in student. 2. The rate of musculoskeletal symptom by NIOSH criteria was 36.2% in hairdresser, it was higher than student group(24.8%)(p<0.1). Occupational stress of the groups by occupational stress category was high as decision latitude, skill discretion, psycological job demand, decision authority in order. 3. The results of logistic regression of related varibles with desire of quitting job by logistic regression were statistical bordline significance between desire of quilting and social support(p<0.1). Conclusion : As above results, the author suggest to prepare the preventive program an musculoskeletal symptoms in hairdresser, especially shoulder, and also to control the occupational stress to hairdresser in beauty shop because occupational stress can be a factor of desire of quitting job.

        부산지역 소방공무원의 폐기능과 호흡기증상

        목적: 본 연구에서는 소방관 집단의 호흡기 증상과 폐기능을 조사하여 호흡기 질환의 유병 정도를 비교 분석함으로써 소방관의 호흡기 질환과 화재 노출과의 관련성을 규명하고자 하였다. 방법: 2004년 11월 13일부터 동년 12월 15일까지 부산 소재 4개 소방서와 부속 파출소의 남성 소방공무원 699명을 대상으로 폐기능 검사와 호흡기 설문조사를 실시하였다. 호흡기 증상 설문은 BMRC에서 개발한 설문조사표를 이용하였다. FVC, FEV_(1), MMEF 예측치는 Morris의 폐기능 예측식을 사용하여 계산하였고, 폐기능 검사 이상을 판정하기 위하여 미국 NHNES 기준에 따라 정하였다. 폐기능 측정치와 호흡기 증상은 노출유무와 흡연여부로 집단을 나누어 각각 비교분석하였다. 유의수준은 p<0.05로 정하였다. 결과: 폐기능 비교에서 화재노출군이 FVC, FVC%, FEV_(1), FEV_(1)%, FEF_(25%)는 비노출군에 비해 통계학적으로 유의하게 저하되어 있었고, 이를 흡연여부에 따라 층화하였을 때, FVC 와 FEV_(1)은 흡연여부와 관계없이 노출 여부에 따른 차이를 보이나 FVC%, FEV_(1)%, PEFR은 흡연군에서만 차이를 보였다. 폐기능 검사 이상을 비교하면 전체적으로 화재노출군이 비노출군보다 폐기능 이상자의 수가 많았고 흡연군에서 FEV_(1) 이상자의 차이는 통계적으로 유의하였다. FVC, FEV_(1), MMEF를 종속변수로 하여 다중회귀분석을 시행하였을 때, FVC와 FEV_(1)은 부분적으로 화재노출유무와 관련성이 있었다. 호흡기 증상 조사에서 화재노출군은 비노출군에 비해 코막힘, 목아픔, 흉부압박감의 증상의 호소율이 통계학적으로 유의하게 높았으나, 이를 흡연여부에 따라 층화하면 이들 증상들은 경계역의 차이를 보였다. 결론: 이러한 결과들을 종합하면 소방관은 화재진압과정에서 노출되는 많은 호흡기 유독물질로 인해 급성 폐기능 저하와 높은 호흡기 증상 호소율을 보이며, 이는 화재노출 외에 흡연의 부가적 효과가 영향을 미친다고 생각된다. 따라서 폐기능이 저하된 경방군에서 주기적인 폐기능 검사가 실시되어야 하며, 금연과 호흡기 보호구의 철저한 착용이 권장되어야 하겠다. Objective: The purpose of this study was to evaluate the relationship between respiratory disorders and fire exposure by investigating and analyzing the lung function and respiratory symptoms of firefighters. Methods: The health effects of firefighting on respiratory function were investigated in 699 male fire officers in 4 fire departments in Busan by recording respiratory symptoms and measuring lung function. The data were analysed according to fire exposure and smoking habits. Results: Mean spirometric data showed significantly decreased levels in the fire-exposed group (FVC, -0.20 L; FVC%, -4.2%; FEV_(1) -0.21 L; FEV_(1)%, -5.1%; FEF25%, -0.13 L/SEE p<0.05). After stratification by smoking habits, FVC and FEV1 showed a significant difference between groups. The prevalence of spirometric abnormality was greater for the fire-exposed group than for the non-exposed group. Significantly higher prevalences of nasal stiffness (16.7% vs. 10.9%), sore throat (17.7% vs. 14.2%) and chest tightness (6.5% vs. 2.5%) were recorded in fire fighters compared to controls (p<0.05). However, after stratification by smoking habits, the 3 symptoms were showed only marginal differences in the smoking group. Conclusions: This study demonstrated that fire-exposed firefighters generally suffer a decline of lung function and a higher prevalence of respiratory symptoms. These results suggest that the exposure to routine firefighting activity is associated with adverse health effects to the respiratory system.

      • 수입 외국산 모발세정용 화장품의 중금속 농도에 관한 연구

        Objective : For the purpose of preparing the fundamental data and preventing the health impairment due yo heavy metals in hair cleaner. Methods and Material : The author determined the concentration of heavy metals(Fb, fn, Cr, Ni and Cu) in hair cleaner with flameless atomic absorption spectrophotometer. Results : The results were as follows : 1. The mean concentration of heavy metals in total hair cleaner were 0.046㎍/g for Pt, 0.O67㎍/g for Mn, 0.069㎍/g for Cr, 0.046㎍/g for Ni, and 0.022㎍/g for Cu, respectively. 2. There was not statistically significant difference to mean concentrations of each heavy metal in hair cleaner by producing nation. 3. The highest mean concentration of each heavy metal in hair cleaner by type of use were 0.140㎍/g for Mn in sensitive hair cleaner(p<0.05), 0.053㎍/g for Cu in others products(p<0.01). 4. The mean concentration of heavy metals in hair cleaner by type of hair cleaner were high at 0.O54㎍/g for Pb in shampoo products(p<0.O5), 0.120㎍/g for Cr and 0.06㎍/g for Cu in combination products(p<0.01). 5. The mean concentration of heavy metals in hair cleaner by acidity were high at 0.056㎍/g for Pb, 0.124㎍/g for Cr, and 0.032㎍/g for Cu in alkali products (p<0.05). 6. Correlation coefficient of heavy metal concentration in hair cleaner were 0.175 for Pb and Cr, 0.165 for Pb and Mn, 0.824 for Cr, and Mn, 0.189 for Cr, and Ni, 0.406 for Cr, and Cu, 0.197 for Mn and Ni, and 0.375 for Mn and Cu(p<0.05). 7. Exposure amount of heavy metals per when we used lOg of hair cleaner (onetime using amount) were 0.46㎍ for Pb, 0.67㎍ for Mn, 0.69㎍ for Cr, 0.46㎍ for Ni, and 0.22㎍ for Cu, respectively. Conclusion : As above results, there was very low level concentration of heavy metal in hair cleaner but we are using the hair cleaner everyday, therefore we can exposed to heavy metals as cronic exposure so the author suggest to prepare the program of preventing the health impairment due to heavy metals.

        Characterization of Poly(ethylene-co-vinyl acetate) (EVA) Using Thermal Analytical Techniques

        Poly(ethylene-co-vinyl acetate) (EVA) is a copolymer of ethylene and vinyl acetate (VA). It is important to determine the VA content of EVA, since the properties of EVA depend highly on the VA content. EVA copolymers have been used in a wide range of applications appropriate for the different VA contents. IR, NMR, and TGA are generally used for determination of the VA content of EVA copolymers. Of these, TGA is the most reliable method and can be applied to cured EVAs. Analytical methods for determination of the VA content and properties of EVA copolymers via TGA were herein reviewed. Thermal behaviors of EVA copolymers (glass transition temperature (T<sub>g</sub>), melting point (T<sub>m</sub>), and crystallization temperature (T<sub>c</sub>)) determined by DSC were also reviewed. Analysis of the related literature revealed that the T<sub>g</sub>, T<sub>m</sub>, and T<sub>c</sub> decrease by about 0.46, 1.36, and 2.08°C, respectively, for every 1 wt% in VA content. A method for determining the degree of crosslinking of cured EVA copolymers was also reviewed, and the degree of crosslinking tends to increase with the decrease in the VA content.

        Abrasion Behaviors of NR/BR Compounds Using Laboratory Abrasion Tester

        The abrasion behaviors of NR/BR blend vulcanizates were investigated using NR/BR = 100/0, 80/20, and 60/40 compounds. The abrasion test was performed using a laboratory abrasion tester (LAT) at slip angles of 1° and 7°. The size distributions of the wear particles and the abrasion rates were examined according to the rubber compositions and slip angles. The most abundant wear particles at the slip angle of 1° were sizes above 1,000 μm, irrespective of the rubber composition. The most abundant wear particles at 7° slip angle had sizes in the range of 212-500 μm, except for the NR = 100 sample. The wear particle size distribution shifted to a smaller size as the slip angle and BR content increased. The abrasion rate at 7° was much larger than that at 1° slip angle. Furthermore, the abrasion rate was notably increased by adding BR to NR.

        Analytical Techniques for Measurement of Crosslink Densities of Rubber Vulcanizates

        It is important to analyze crosslink densities of rubber articles because the physical properties are dependent on the crosslink densities. In this paper, analytical techniques for the measurement of crosslink densities of rubber vulcanizates are described. The most widely used method to measure the crosslink density is a swelling method combined with the Flory-Rehner equation. Application of the interaction parameter (χ) of rubber and swelling solvent is critical because the crosslink density is absolutely dependent on the χ value. Methods for obtaining χ employ not only solubility parameters of the polymer and swelling solvent but also inverse gas chromatography (IGC). The solubilities of rubbers can be obtained using micro differential scanning calorimetry (μDSC), intrinsic viscosity measurement, and UV-visible spectroscopy. Nuclear magnetic resonance (NMR) spectroscopy has been also used for the measurement of the crosslink density using the T<sub>2</sub> relaxation time, which is determined by spin-spin relaxation in solid-state NMR. For sulfur-cured rubber vulcanizates, crosslink densities according to the crosslink types of mono-, di-, and polysulfides are measured by treating the rubber samples with a chemical probe composed of thiol and amine compounds. Measurement methods of physical crosslinking by filler, crystallization, and ionic bonding have also been introduced.

        Analytical Method for Determination of the Content of Tire Wear Particle in Tire and Road Wear Particles

        Tire and road wear particles (TRWPs) were collected from road dust and thermogravimetric analysis (TGA) was performed to measure the content of tire wear particles (TWPs) in the TRWPs. The TGA thermograms of TRWPs showed two weight loss steps associated with polymer decomposition including weight loss after 480℃ which may be due to road wear particles. Different samples gave different TGA thermograms because the types and contents of the road wear particles attached to the TWPs should be different from each other, and each TWP might have different composition. The TGA results of the model asphalt pavement wear particles, with (volatile organics + polymers + carbon black) : ash = 33.5 : 66.5, was applied to the TRWP results, and the TWP contents of TRWPs were found to be 50-65%. The zinc oxide content in the rubber compound was negligible.

        Organocatalytic enantioselective synthesis of phthalans via Wittig/oxa-Michael cascade reaction

        An enantioselective synthetic method for 1-substituted phthalans has been developed. The organocatalytic reaction between 1,3-dihydro-2-benzofuran1-ols and Wittig reagents using cinchona squaramide-based organocatalyst proceeded with sequential Wittig reaction followed by an enantioselective intramolecular oxa-Michael reaction, yielding enantioenriched phthalans with moderate to good enantioselectivities.
