        방사선사진의 인덱스-순서 분석을 통한 치아경조직의 탈염화 검출

        Purpose : The purpose of this study was to develop the radiographic technique for detecting the demineralization which is known as indication of dental caries. Materials and Methods : This technique was based on the comparing of multiple radiographs which was irradiated by multiple X-ray spectra. For the meaningful comparing, the multiple radiographs were reconstructed to the dosimetrically consistent images using a standard material. The difference of resulting images of same target with multiple spectra represents the difference of response of material as regards the spectra. Results : We have found about 10% of demineralization of dental hard tissues particularly in the proximal region through the analyzing of differences. Conclusion : Most intriguing thing in this investigation was that the method to analyze difference shows us to an anatomic structure of dental hard tissues even if absolute values of optical density were excluded during the procedures.

        인공 치아우식 발생 모델에서 디지털 방사선 공제술을 이용한 인접면 치아우식증의 진단

        Purpose : The purpose of the experiment was to evaluating the diagnostic ability of dental caries detection using digital subtraction in the artificial caries activity model. Materials and Methods : Digital radiographies of five teeth with 8 proximal surfaces were obtained by CCD sensor (Kodak RVG 6100 using a size #2). The digital radiographic images and subtraction images from artificial proximal caries were examined and interpreted. In this study, we proposed novel caries detection method which could diagnose the dental proximal caries from single digital radiographic image. Results : In artificial caries activity model, the range of lesional depth was 572-1,374 μm and the range of lesional area was 36.95-138.52mm². The lesional depth and the area were significantly increased with demineralization time (p<0.001). Furthermore, the proximal caries detection using digital subtraction radiography showed high detection rate compared to the proximal caries examination using simple digital radiograph. Conclusion : The results demonstrated that the digital subtraction radiography from single radiographic image of artificial caries was highly efficient in the detection of dental caries compared to the data from simple digital radiograph.

        백서 정중구개봉합 확대후의 CGRP 면역반응 신경섬유의 변화

        측방확대장치에 의한 정중구개봉합부의 확대시 고정력은 구개부의 신경요소들에 변화를 일으키면서 동통을 유발하게 되고 구개봉합부가 확대되고 그것이 유지되는 동안에 기계적 자극에 의한 구개부신경섬유의 반응성에 변화를 일으킬 것으로 생각된다. 본 연구에서는 백서 정중구개봉합부의 CGRP 면역반응 신경섬유의 교정력 적용시간에 따른 형태학적인 반응성, 밀도, 분포변화등을 관찰하고 그와 연관된 CGRP의 기능을 알아보고 자 250gm내외의 Sprague-Dawley 웅성백서 상악전치에 200gm의 치아이개력이 가해지도록 활성화시킨 helical spring을 삽입하여 정상대조군과 장치 장착후 경과시간에 따라 1일, 4일, 7일, 14일 경과군으로 나누어 정중구개봉합부를 확대후 희생시켜 면역조직화학법으로 염색하여 관찰하였다. · 대조군에서 정중구개 봉합붑 결합조직의 CGRP 면역양성 신경섬유는 거의 관찰되지 않았다. · 1일군에서는 대조군에 비해 CGRP 면역반응 신경섬유가 현저한 증가를 보였으며 주로 혈관 주위에 염주알 모양의 가는 신경섬유가 관찰되었다. · 4일군에서는 CGRP 면역반응 신경섬유가 대조군에 비해서는 현저한 증가를 보였으나 1일군에 비해서는 크게 증가하지 않았으며 혈관의 크기가 훨씬 확장되어 관찰되었다. · 7일군에서는 특징적으로 조골양세포가 새로이 형성된 골변연을 따라 줄지어 배열되는 독특한 양상이 관찰되었고 신경섬유의 수는 4일군에 비하여 감소하고 혈관의 직경도 감소되었다. · 14일군에서는 CGRP 면역반응 신경섬유가 7일군과 유사한 분포를 보였으며 확대에 의한 골변연의 불규칙성도 대체로 감소되었다. CGRP 면역반응 신경섬유는 정중구개봉합의 확대시 초기에 일어나는 신경원성 염증반응에 주로 관련되어 증식되는 것으로 생각된다. Midplatal suture expansion is often used for patients having narrow maxillary arch, cleft palate, respiratory handicap with narrow nasal cavity. CGRP has been known as a modulator of pain transmission in central nervous system and a local effector to peripheral tissue causing vasodilation, increase of blood flow, modulation of immune system, regulation of macrophagic function and stimulation of bone formation. To investigate changes of CGRP-immunoreactive nerve fibers in midpalatal suture during the the expansion, immunohistochemical study was performed by using rats. Experimental rats(10weeks, 250gm) were divided into five groups(control, 1, 4, 7, 14 days group (each n=4) and applied orthodontic force (approximately 200gm) to upper anterior incisors. Frozen sections of midpalatal suture area were immunostained by using rabbit antisera. The results were as follows. · The CGRP-immunoreactive nerve fibers were hardly observed in control group. · In 1 day group, the CGRP-immunoreactive nerve fibers were more increased around the vessels than control group. · In 4 days group, the CGRP-immunoreactive nerve fibers were more increased than control group, but not more increased than 1 day group. Vascular diameter was more enlarged. · In 7 days group, expecially, hematoxilin affinity of cells was remarkable and cells were arranged along the bone margin. The CGRP-immunoreactive nerve fibers were more reduced than 4 days group and vascular diameter was also reduced. · In 14 dayds group, the CGRP-immunoreactive nerve fibers were similar to those of 7 days group and the irregularity of bone margin was almost recoverd. In Conclusion, the CGRP-immunoreactive nerve fibers may be related to initial neurogenic inflammatory reaction in expanding mid-palatal suture.

      • Helicobacter pylori가 만성위염과 위종양에 미치는 영향에 관한 면역조직화학적 밑 초미세구조적 연구

        Background and Objectives : Helicobacter pylori (H. pylori) is accepted as a principal cause of chronic gastritis and an important factor in the pathogenesis of peptic ulcer and gastric malignancy. The aim of our studies is to investigate relationships between H. pylori and gastric epithelial cells. Materials ðods : One hundred and eighty one H. pylori positive cases of gastric biopsy specimens were studied immunohistochemically and ultrastructurally. Results : p53 positive staining exhibited foveolar epithelial hyperplasia in 1 out of 32 cases, regeneration in 2 out of 12 cases, incomplete intestinal metaplasia in 5 out of 41 cases, complete intestinal metaplasia in 3 out of 18 cases, chronic ulcer in 1 out of 9 cases, tubular adenoma in 2 out of 14 cases and intestinal type of adenocarcinoma in 16 out of 21 cases. Ultrastructurally, H. pylori was observed to be in direct contact with the microvilli of the target cells and was demolishing the surface microvilli. The organisms were in close contact with the uncoated cell membrance. Penetration into the gastric cells by a few organisms was associated with marked cell damage and ultimately to cell disintegration. Conclusion : Our results suggest that H. pylori plays a role in the pathogenesis of gastric cancer, leading a relatively slow, long lasting process that induces chronic active gastritis, intestinal metaplasia, dysplasia, and adenocarcinoma of intestinal type.

      • Cd(Ⅱ) 폴라로그람의 異常波에 關한 硏究

        pH 6.0∼12.0 範圍에서 0.15M 타르타르 酸鹽支持溶液 속에서 Cd(Ⅱ) Polarogram을 25℃에서 二段還元波를 調査한 결과 pH8.2部分 까지는 限界電流 및 半波電位값이 一定하며 한편 같은 濃度의 窒酸鹽 支持溶液을 사용하였을 때에 比하여 一段還元波의 半波電位값은 0.03volt 陰의 電位값으로 移動하고, 限界電流 값은 21% 減少되었다. pH8.2 部分 以上부터 限界電流 값이 急激하게 增加하다가 pH8.9 以上부터는 다시 急激하게 減少하는 現象을 보여 주었다. 한편 pH8.2 以上부터는 半波電位가 pH11.0에서는 -1.37volt로 되었다. 可能한 電極反應을 假定하여, pH11.0에 이르기까지의 還元波移動의 原因을 檢討하였다. The second reduction waves of the polarogram of Cd(Ⅱ) in 0.15M sodium tartrate solution were investigated in the range of pH from 6.0 to 12.0 at 25 C. Up to pH 8.2, the limiting current and the half wave potential were found to be constant, and when compared with the nitrate solution equal to the concentration of sodium tartrate, the limiting current of the first reduction wave has been decrease 21% and the half wave potential of the first reduction wave shifted by -0.03 volt. The values of limiting current were sharply increased above pH 8.2, and maximum value was obtained at pH 9.8. The half wave potentials were gradually proceeded in negative direction with increasing pH and the half wave potential of -1.37 volt was obtained at the pH 11.0. Suggesting possible mechanisms of electrode reactions, the anomalous behavior of reduction waves of Cd(Ⅱ) in sodium tartrate solution up to pH 11.0 was discussed.

        삼차신경주감각핵에서 치수유래 들신경섬유 종말의 정량적 분석

        To analyze the synaptic characteristics of axon terminals originated from tooth pulp in the trigeminal principal sensory nucleus, and find morphological evidence that p-endings on these terminals use GABA as neurotransmitter, labeling of tooth pulp afferents with wheat-germ agglutinin conjugated horseradish peroxidase, postembedding immunocytochemistry and morphometric analysis were performed. The results obtained were as follows. The labeled boutons contained clear and round synaptic vesicles, and the mean number of synaptic contacts was 4.21±2.58. The postsynaptic profiles were usually middle or distal dendrites and dendritic spines rather than soma or proximal dendrites. The mean number of synaptic contacts with p-endings was 2.62±2.01. The volume of labeled bouton was 4.39±3.08 μm3 and highly correlated (P?.01) with surface area (r = 0.96), total apposed surface area (r = 0.84), total active zone area (r = 0.84), mitochondrial volume (r = 0.95), the number of synaptic vesicles (r = 0.92), the number of synaptic profiles (r = 0.76) and the number of synaptic p-endings (r = 0.67). The p-endings contained pleomorphic synaptic vesicles. The volume of p-ending was 0.91±0.47 μm3 and highly correlated (P?.01) with surface area (r = 0.94), total apposed surface area (r = 0.76), total active zone area (r = 0.62), mitochondrial volume (r = 0.70) and the number of synaptic vesicles (r = 0.94). P-endings were GABA immunoreactive. These results suggest that tooth pulp afferent terminals in trigeminal principal sensory nucleus may be related to complex processing of sensory information and presynaptic p-endings using GABA as neurotransmitter, are important in this sensory processing. The “size principle” of Pierce and Mendel (1993) is also applicable to tooth pulp afferent terminals and presynaptic p-endings. 흰쥐에서 삼차신경주감각핵으로 투사하는 치수유래 신경종말의 연접특성을 이해하기 위하여 WGA-HRP로 표식된 신 경종말 및 이와 연접하는 p-ending에 대하여 GABA 면역반응을 실시한 후 전자현미경을 이용하여 관찰하고 형태측정 법 (morphometry)을 이용하여 계측 분석함으로써 다음과 같은 결과를 얻었다. 표식종말에서 연접소포의 형태는 투명하고 둥글었으며, 연접의 개수는 4.21±2.58개로 다소 복잡한 양상을 보였다. 표식 종말의 연접부위는 신경세포체의 몸체나 몸쪽 가지돌기 보다는 중간 및 먼쪽 가지돌기 및 가지돌기가시에 집중되어 나타 났다. 또한 표식종말 하나에 대하여 2.62±2.01개의 p-ending이 연접하여 비교적 많은 수의 p-ending으로부터 입력을 받 는 것으로 나타났다. 표식종말의 체적은 4.39±3.08 μm3이었으며 신경종말의 표면적 (r = 0.96), 전체 인접부위의 면적 (r =0.84), 전체 활성부위의 면적 (r = 0.84), 신경종말내 사립체의 체적 (r = 0.95), 연접소포의 수 (r = 0.92), 신경종말과 연접하는 구조물의 수 (r = 0.76), 연접하는 p-ending의 수(r = 0.67) 등과 강한 양의 상관관계를 나타내었다 (P?.01). P-ending에서 연접소포는 둥근모양, 납작한 모양과 타원형 등 다양한 형태를 나타내었다. P-ending의 체적은 0.91±0.47 μm3로서 표식종말에 비하여 작았으며 p-ending의 표면적 (r = 0.94), 전체 인접부위의 면적 (r = 0.76), 전체 활성부위의 면적 (r = 0.62), 신경종말내 사립체의 체적 (r = 0.70), 연접소포의 수 (r = 0.94) 등과 강한 양의 상관관계를 나타내었다 (P?.01).P-ending은 GABA에 양성반응을 보였다. 이상의 결과로 미루어 보아 치수에서 삼차신경주감각핵으로 투사하는 신경종말은 GABA를 사용하여 억제적으로 작용하는 여러 개의 p-ending과 연접하여 복잡한 감각의 처리에 관여하는 것으로 생각되며, 치수유래 신경종말 및 이와 연접하는 p-ending 모두에서 Pierce와 Mendell (1993)의 “크기의 원리 (size principle)”가 적용된다는 것을 알 수 있었다.

      • Cisplatin 병용화학요법시 발생한 오심, 구토에 대한 Ondansetron과 Metoclopramide의 효과에 대한 비교

        Inspite of possible effects for emesis following chemotherapy including cisplatin, nausea and vomiting are the most unpleasant side effects of cancer chemotherapy. None of the currently available antiemetic agents is entirely effective preventing emesis. Ondansetron, a 5-HT(5-hydroxytryptamine)_(3) receptor antagonist, has been shown to be effective antiemetic agent in the control of cisplatin induced emesis. Twenty solid tumor patients who are scheduled to receive cisplatin containing combination chemotherapy participated in a prospectively randomized study to compare the antiemetic efficacy and safety of ondansetron and metoclopramide. Ondansetron was given in dose of 8mg intravenous 30min before cisplatin and then 4hours and 8hours after cisplatin on day 1, and Ⅰ.Ⅴ. every 8hours from day 2 to 5. In the metoclopramide group, metoclopramide was given in dose of 2 ㎎/㎏ intravenous every 8hours from day 1 to 5. The control of emesis was graded in the following way : complete response, no vomiting : major response, 1-2 emetic episodes : minor response, 3-5 emetic episodes : failure, above 5 emetic episodes. Complete or major response of emesis was achieved 10 of 12(83.2%) patients receiving ondansetron and in 5 of 8(62.5%) patients receiving metoclopramide, but effectiveness was not significantly different. Ondansetron was at least as effective as metoclopramide therapy in controlling cisplatin induced emesis. Side effects in ondansetron was less than in Metoclopramide.
