        정신보건법의 주요 쟁점 조항에 대한 선진국과의 사례 비교

        Objectives The Mental Health Act is an initiative aimed at changing and shaping mental health services and protecting human rights of persons with mental disorders. Since the Mental Health Act was legislated in 1995, four amendments have been made according to the issues that arose from public concerns. However, there are still many debates about the human rights protection of the mentally iII. This study aimed to provide information regarding major aspects of the Mental Health Act by comparing them among several developed countries. Methods Current Mental Health Acts of the state of Michigan in the United States, Scotland in England, the state of Victoria in Australia, and Japan were reviewed. Issues regarding the Korean Mental Health Act were collected from seminar materials, news media contents, and mental health professionals Results The definition of subjects in Korean Mental Health Act was more inclusive than other countries and was derived from a medical classification of mental illness. Family members or guardians were granted important responsibilities for deciding the involuntary admission of mentally ill patients in Korea and Japan. In Western countries, Mental Health Review Tribunals or courts have the primary responsibility for important decisions about mentally ill patients. The regulation of immediate dis- charge after request by voluntarily admitted patients was not enacted in all countries except Korea. The mandatory procedure for involuntary admission in Western countries includes an individual case review with personal interview by a Mental Health Review Tribunal or court. Conclusion The Korean Mental Health Act appears to meet the basic standards of Guidelines from international organizations. Our traditional culture and inherent health systems seem to influence the legal regulation of mental health service and might be related to the problems of human rights protection of mentally ill patients in Korea.

        전자현미경 In Situ Hybridization에 의한 Viral RNA의 진단에 관한 연구

        토끼 바이러스성 출혈증의 원인체를 실험 토끼에 접종하여 증식을 유도하고 간장에서 hematoxylin & eosin 염색에서 조직학적 진단과 세포내 viral RNA의 소재를 결정하기 위해 post-unicryl 포매한 block의 절편을 사용하여 단 염색과 전자현미경적 in situ hybridization을 시도하였다. 토끼 출혈증 viral RNA의 보합 결합에 이용하는 probe는 4717에서 4800(84bases)까지 oligonucleotide를 5'말단에 biotin-CE phosphoramidite로 표지하여 사용하였다. 보합결합물의 증명은 신호 표지로서 antibiotin antibody-10mm gold를 사용하였으며, hubridization이나 증명은 기존 protocol에서 약간의 변법을 사용하였다. 0.02% glutaraldehyde에서 고정하고 unicryl resin 포매한 표본, biotinylated oligonucleotide probe, antibiotin antibody-10mm gold로 실험한 결과 증강된 신호를 얻을 수 있었다. 특히 전처리를 생략하므로써 실험 과정을 간단하게 하여 신속한 결과를 얻을 수가 있었다. 전자현미경 in situ hybridization을 통하여 토끼 출혈증 바이러스의 주요 표적은 간세포로 간염 세포의 세포질 내 미토콘드리아와 핵사이에서 immuno gold입자가 뚜렷하게 표지됨으로서 viral RNA를 증명할 수 있었다. Simple stain and electron microscopic in situ hybridization is studied and applied for the identification of rabbit haemorrhagic disease viral RNA in a unicrylated preparation of the liver after innoculation of rabbit haemorrhagic disease virus. Hybridization for detection of viral RNA in unicryl embedded tissues using complementary 84 bases oligonucleotide probe labelled by biotin CE-phosphoramidite compared with 4717∼4800 sequences of rabbit haemorrhagic disease virus, modified hybridization protocol and antibiotin antibody-l0nm gold as signal marker. The best results were obtained in 0.02% glutaraldehyde, Unicryl resin cell block, biotinylated oligonucleotide probes, antibiotin-l0nm gold. In this report, RHD viral RNA was distributed widely within the mitochondria and nucleus of liver cell by electron microscopic in situ hybridization. In situ hybridization has become a standard method for localizing DNA or RNA sequences in tissue or cell preparation. In situ hybridization is detected the virus genome in the cells and tissue as specifically compared with others nucleic acid hybridization method.

      • 食鹽水 또는 蒸溜水 投與가 副腎皮質에 미치는 影響에 關한 電子顯微鏡的 硏究

        It is well known that various degrees and combinations of regressive changes and hemorrhagic disorders of different organs may be observed in sodium chloride intoxication. Reports on the ultrastructural changes of adrenal cortex in sodium chloride intoxication are rare, however. An attempt was made to study alterations of fine structures of the adrenal cortex by light and electron microscopy. Twenty albino rates, maintained on a stock diet, weighing from 160 to 310 gm, were divided into 3 groups. Group 1. Control group. 2 rats: tap water was given as drinking water. Total daily NaCl intake, 2.2mEq. Group 2. 9 rats: a 2% NaCl solution was substituted for drinking water. Total daily NaCl intake, 25.0 mEq. Group 3. 9 rats: distilled water was substituted for drinking water. Total daily NaCl intake, 1.0mEq. Three rats were sacrificed at the end of the lst, 2nd and 3rd experimental weeks from groups 2 and 3. The control group was sacrificed similarly. Adrenal glands were extirpated and studied by both light and electron microscopic examinations. The results were as follows: 1. Considerable weight loss was observed in the group of rats given the 2% NaCl solution (group 2), whereas retarded weight increase was noted in the experimental group of rats given distilled water (group 3). 2. In the zona glomerulosa of the adrenal cortex, a marked atrophic change for the group of rats on the 2% NaCl solution (group 2) was noted and a mild hydropic swelling of the cytoplasms of the cells of the zona glomerulosa on the group of rats on the distilled water (group 3) was observed. 3. Electron microscopic study revealed that a degenerative change occurred on the group of rats on the 2% NaCl solution (group 2), characterized by and atrophy of sER and the Golgicomplex; and increased number of lipid droplets, lysosome and glycogen particles; also a swelling of the mitochondrial and a decreased number of mitochondrial cristae; plus a chondriosphere formation. On the other hand, the group of rats on the distilled water (group 3), had a proliferative change such as an increased number of sER, with the dilatation of Golgi complex and both a decreased number of lipid droplets and glycogen particles. It can be concluded, therefore: The functional activity of the intracellular organells of the zona glomerulosa of the adrenal glands responsible for synthesis and secretion of mineralocorticoid was depressed when the experimental rats were given 2% NaCl solution as a substitute for drinking water after a considerable length of time. This functional activity was accelerated when the rats were given distilled water in lieu of drinking water over long duration.

        Left ventricular outflow tract obstruction and systolic anterior motion of the mitral valve after myxomatous mitral valve repair is due to excess tissue of mitral valve among numerous other factors. A patient who had myxomatous mitral valve and acute mitral regurgitation due to chordae rupture was admitted for mitral valve repair. The hospital course of the patient was not significantly different compared to other patients. On the 40th postoperative day, the patient experienced syncope and came to ER. The patient showed severe left venthcular outflow tract obstruction and systolic anterior motion of mitral valve. The patient was treated medically and was discharged with normal echo findings. We report this case with brief review of related literatures.

        The College of Engineering was specialized to have a sole function for supplying technical high school teachers in 1977 and referred to as the College of Industrial Education according to the Government's policy. The economic impact mainly due to the oil-crises caused a severe problem to this college, namely discrepancy of supply to demand scale. Accordingly the number of student enrollment was to be fluctuated from 479 in 1979 through 800 in 1979 and 900 in 1980 to 430 in 1981. And the engineering departments have been attached to this Engineering Education College since 1981. These circumstanses met singnificant inefficiency in facility operation. This program aims at providing an overall relocation of the college facilities and preparing the practical system to make the joint-operation possible taking into account circulation, academic stream, budget, maintenance, etc. However this study could not get optimal spacing but some trade-off, considering the current situation.

        광학현미경 In Situ Hybridization에 의한 Viral RNA 증명

        토끼 출혈증 바이러스에 감염된 조직을 10% 포르말린 고정, 파라핀 블록으로 보관했던 것으로 표본을 만들고 biotin 표지된 올리고뉴클레오티드 probe를 사용하는 in situ hybridization 기법으로 viral RNA를 조사하였다. in situ hybridization 기법은 핵산을 규명하는 다른 방법들에 비하여 신속하고 특이성이 높은 기법으로 모든 과정이 Microtrobe(Tm) capillary action system에서 1-2시간 이내에 완료된다. Viral RNA는 간세포의 세포질과 신장의 피질에서 주로 관찰되었으나, 폐조직과 신장의 수질에서는 부분적으로 적색신호가 보였다. 비록 기술적인 한계를 가지고 있지만 다른 핵산 진단방법 보다 많은 장점을 가지고 있어 조직 병리학적으로 바이러스 진단하는데 하나의 독특한 기법으로 채용되리라 기대된다. In this paper, a in situ hybridization(ISH) has been used to investigate the yield of viral RNA expression from each organ tissues. It is studied to establish a rapidly, specific diagnostic method detecting rabbit haemorrhagic disease virus(RHDV) RNA in 10% formalin-fixed, paraffin-embedded tissues of naturally RHDV-infected rabbits using oligonucleotide probe to be made by RHDV total sequences. Biotin was used as the oligonucleotide probe marker. In situ hybridization is detected the virus genome in the cells and tissue as specifically compared with others nucleic acid hybridization method. All ISH procedure of RHDV were completed to Microbrobe(Tm) capillary action system within 1-2 hours. In this report, RHDV was distributed widely in the cytoplasm of liver cell and the cortex of kidney but lung tissue and medulla of kidney were showed to positive reaction at locally. Although not entirely free of technical limitations, nucleic acid identification holds advantages over other diagnostic tests, including exquisite sensitivity, specificity, interchangeability and speed. It is expected that, in the immediate future viral nucleic acid detection will be a prominent part of the methods used in histopathology.

        생약복합제제의 병용투여에 관한 연구 (제1보) : 가미청심연자탕과 항고혈압약물과의 병용투여 Combined usage of Kamichungsimyunja-Tang and Anti-hypertensive Drugs

        Effects of Combined usage of combined preparation of crude drugs(Kamichungsimyunja-Tang) and anti-hypertensive drugs were examined in anti-hypertensive effects in rats and diuretic effects in mice. Kamichungsimyunja-Tang is an added and subtracted prescription of Chungsimyunja-Tang which it has become one of the favorate prescriptions at the Oriental Medical Hospital, Kyung-Hee University. Kamichungsimyunja-Tang has been used widely in cardiovascular disorder in Oriental medicines. Kamichungsimyunja-Tang was orally given and anti-hypertensive drugs such as hydralazine, verapamil and atenolol were intravenously given to rats. Effects of combined usage of Kamichungsimyunja-Tang and antihypertensive drugs for the anti-hypertensive actions significantly increased the anti-hypertensive effect of anti-hypertensive drugs. Kamichungsimyunja-Tang and diuretic drugs such as furosemide and hydrochlorothiazide was orally given to mice. Effects of combined usage of Kamichungsimyunja-Tang and diuretic drugs for the diuretic effects significantly increased the effect of diuretic drugs. These result suggested that combined usage of Kamichungsimyunja-Tang and anti-hypertensive drugs and diuretic drugs for the anti-hypertensive and diuretic actions significantly increased the effect of anti-hypertensive and diuretic drugs.

      • 유압관로내의 동특성을 이용한 비정상 유량계측에 관한 연구

        This paper describes an approach for estimating the unsteady flowrate through a hydraulic pipe lines and a components in real time. Concerning the dynamic characteristics of a hydraulic pipe lines, experimental research from the view point of focusing on the unsteady flowrate through a pipe is insufficient because of the lack of adequate flowmeters. In this experiment, a new instantaneous flowrate measurement system making use of a sharp-edged cylindrical choke and LDV has been proposed and developed by the authors. The unsteady flowrate is obtained from the measurements center-line velocity of a circular pipe using LDV and the pressure difference using a sharp-edged cylindrical choke, and experiments on the frequency characteristics are carried out. The results show a good agreement between the estimated and directly measured flowrate waveforms illustrates the validity of the method proposed here.
