        양극 산화 임플란트 표면 구조에 대한 염산 테트라싸이클린의 영향

        The present study was performed to evaluate the effect of Tetracycline-HCI on the change of implant surface microstructure according to application time. Anodic oxidation surface were utilized. Implant surface was rubbed with 50mg/ml Tetracycline-HCI solution for ${\frac{1}{2}}min.$, 1min., $1{\frac{1}{2}}min.$, 2min., $2{\frac{1}{2}}min.$, and 3min. respectively in the test group. Then, specimens were processed for Ra Value test and scanning electron microscopic observation. The results of this study were as follows. 1. The anodic oxidation surface roughness tests don't show significant difference on conditioning with saline and Tetracycline-HCI. 2. The anodic oxidized surfaces showed the craterous structures. The surface conditioning with Tetracycline-HCI didn't influence on its micro-morphology. In conclusion, Anodic oxidation implant surface is stable to detoxificate with 50mg/ml Tetracycline-HCI of implant surface.

        상피하 결합조직 이식편을 이용한 치근 피개술

        Purpose: One of the main objectives of periodontal reconstructive surgery is the coverage of exposed roots that occur due to gingival recession. and Aestheic concerns are usually the reason to perform root coverage procedure. This case report was performed to evaluate the effect of root coverage using subepithelial connective tissue graft(SCTG) on Miller's Class I marginal tissue recession. Materials and Methods: One patient, with two Miller's class I marginal tissue recession on both maxiallay canines, was treated with root coverage using SCTG (modified Nelson's technique). At baseline, the following measurements were recorded: 1) recession depth; 2) width of keratinized giniga. At 9, 10 months post-surgery, all clinical measurements were repeated. Result: 1) The mean root coverage from baseline to 9, 10 months post-surgery was 92.3%. 2) The mean recession depth decreased from 6.5 mm to 0.5 mm. 3) The mean width of keratinized gingiva increased from 1.25 mm to 3.5 mm. Conclusion: Within the above results, root coverage using SCTG is an effective procedure to cover Miller's class I marginal tissue recession defect. Also, patient with aesthetic concern could be satisfied with this result.

        염산 테트라싸이클린이 이중 산부식 임플란트 표면 구조에 미치는 영향

        The present study was performed to evaluate the effect of Tetracycline-HCI on the change of implant surface microstructure according to application time. Implants with thermal dual acid etched surface were utilized. Implant surface was rubbed with $50mg/m{\ell}$ Tetracycline-HCI solution and sterilized saline for O.5min, 1min, 1.5min, 2min, 2.5min and 3min. respectively in the test group. Then, specimens were processed for scanning electron microscopic observation and measured surface roughness by optical interferometer. The results of this study were as follows. 1. The thermal dual acid etched surfaces showed many small peaks and valleys distributed overall surface. 2. The surface conditioning with Tetracycline-HCI and saline didn't influence on its micromorphology. In conclusion, the implant with thermal dual acid etched surface has a protective micromorphology from the detoxification with $50mg/m{\ell}$ Tetracycline-HCI and a scrubbing with cotton pellet. Therefore, the detoxification with $50mg/m{\ell}$ Tetracycline-HCI is an effective method for peri-implantitis in case implants with thermal dual acid etched surface.

        염산티트라싸이클린의 적용시간에 따른 다공성 임프란트 표면 미세구조의 변화

        Mechanical and chemical methods are the two ways to treat the implant surfaces. By using mechanical method, it is difficult to eliminate bacteria and by-products from the rough implant surface and it can also cause the structural change to the implant surface. Therefore, chemical method is widely used in order to preserve and detoxicate the implant surface more effectively. The purpose of this study is to evaluate the effect of tetracylcline-hydrochloride(TC-HCI) on the change of implant surface microstructure according to application time. Implants with pure titanium machined surface, SLA surface and porous surface were used in this study. Implant surface was rubbed with sponge soaked in 50mg/ml TC-HCI solution for $\frac{1}{2}$ min., 1 min., $1\frac{1}{2}$ min., 2 min., and $2\frac{1}{2}$ min. respectively in the test group and with no treatment in the control group. Then, specimens were processed for scanning electron microscopic observation. 1. Both test and control group showed a few shallow grooves and ridges in pure titanium machined surface implants. There were not significant differences between two groups. 2. In the SLA surfaces, the control specimen showed that the macro roughness was achieved by large-grit sandblasting. Subsequently, the acid-etching process created the micro roughness, which thus was superimposed on the macro roughness. Irrespective of the application time of 50mg/ml TC-HCI solution, in general, test specimens were similar to control. 3. In the porous surfaces, the control specimen showed spherical particles of titanium alloy and its surface have a few shallow ridges. The roughness of surfaces conditioned with tetracycline-HCI was lessened and seen crater-like irregular surfaces relative to the application time. In conclusion, pure titanium machined surfaces and SLA surfaces weren't changed irrespective of the application time of tetracycline-HCI solution. But the porous surfaces conditioned with tetracycline-HCI solution began to be slightly changed from 2 min. This results are expected to be applied to the regenerative procedures for peri-implantitis treatment.

        860 MHz 대역 고출력 Cartesian 피드백 송신기 설계

        This paper presents the design of 860 MHz band transmitter for improving power amplifier's linearity using Cartesian feedback method. For eliminating the effects of gain, phase mis-match, and DC offset, we estimate the property variations using ADS software. The implemented Cartesian feedback transmitter exhibits IMD3 of -54 dBc at an output power of 43 dBm and this result shows that the linearity is improved for 22.4 dB, compared with the test of the power amplifier without Cartesian feedback system. Thus, we verify that the proposed Cartesian feedback transmitter can be applied to narrow-band transmitter systems. 본 논문은 Cartesian 피드백 방법을 이용하여 전력 증폭기의 선형성을 개선한 860 MHz 대역의 송신기의 설계에 관한 것이다. 피드백 루프를 통하여 발생하는 이득 및 위상의 불일치와 DC offset에 의한 영향을 제거하기 위하여 ADS simulation을 이용하여 특성 변화를 예측하였다. 제작된 Cartesian 피드백 송신기는 출력 전력 43 dBm에서 -54 dBc의 IMD3 특성을 나타내었으며, 이는 선형화 전과 비교하여 22.4 dB의 선형성 개선 효과가 있음을 의미한다. 따라서 본 논문에서 제작한 Cartesian 피드백 송신기를 이용하여 협-대역 전송시스템에 적용할 수 있음을 확인하였다.

        위의 저등급과 고등급 이형성에서 면역조직화학염색을 이용한 telomerase 발현의 비교

        목적: Telomere는 세포 염색체의 말단에 존재하며 염색체의 안정을 유지하는데 필수적인 역할을 수행한다. 이러한 telomere의 합성을 촉매하는 telomerase 활성이 전암 병변에서도 관찰되면서 이 효소의 활성화는 발암과정의 초기 현상으로 암화과정에 기여하는 것으로 생각되고 있다. 전암성 병변으로 알려진 이형성의 단계에 따른 telomerase의 발현 정도를 측정하고 연관관계를 밝히고자 하였다. 방법: 현재까지 본원에서 위내시경을 시행한 환자들 중 병리조직검사에서 위의 저등급 이형성과 고등급 이형성으로 확진된 환자 43명을 대상으로 하여 후향적으로 연구하였다. 조직을 포르말린에 고정한 후 제작된 환자의 파라핀 조직 절편을 4 ㎛ 두께로 자른 다음 슬라이드에 부착하여 충분히 건조시켰고, 탈파라핀과 함수과정을 거친 조직 절편은 과산화수소에 반응시켰다. 슬라이드를 버퍼(buffer)에 넣고 압력기구에서 끓인 후 실온에서 방치하였으며, PBS로 세척한 다음 blocking solution으로 전처치 하였다. 일차 항체를 희석하여 4℃에서 하룻밤 동안 반응시켰고, 이차항체를 각각 반응시킨 후 세척하고 발색하여 대조 염색하였다. 광학현미경에서 면역조직화학염색에 양성반응을 보이는 이형성 세포의 비율을 25% 미만, 25~50%, 51~75%, 76% 이상으로 나누어 다섯 등급(0, 1+, 2+, 3+, 4+)으로 나타내었다. 염색은 세포의 핵에 국한되어야 하며, 두 군 간의 구분 기준은 세포의 25% 이하가 염색된 경우와 26% 이상이 염색된 경우로 하였다. 결과: 저등급 이형성 22명 중에는 남자 13명, 여자 9명이었으며 고등급 이형성 21명 중에는 남자 14명, 여자 7명이었다. 연령은 저등급 이형성이 64.4±9.0세였고, 고등급 이형성이 69.2±9.6세였다. 두 군 간의 성별과 연령은 무관하였다. 면역조직화학염색에서 25% 이하로 염색된 조직은 저등급 21명, 고등급 5명이었으며 26%이상 염색된 조직은 저등급 1명, 고등급 16명으로 저등급과 고등급 간에 telomerase 발현 정도의 유의한 차이가 있었다. 결론: 위의 이형성에서 저등급에서 고등급으로 갈수록 telomerase 발현 정도가 높아짐을 알 수 있었다. Background: Telomeres are simple repeats elements located at each end of the chromosomes of eukaryotic cells. The main function of telomeres is to cap the chromosome end and protect it from enzymatic attack. Telomerase that facilitates the synthesis of telomere has been detected in not only cancer, but also in precancerous lesion. In this study, we compared the telomerase expression between low-grade and high-grade gastric dysplasia. Methods: The telomerase expression of 43 patients with gastric dysplasia (22 low-grade and 21 high-grade) was evaluated by immunohistochemical staining in tissues. Results: The telomerase expression was much higher in the tissues from the patients with high-grade gastric dysplasia than in those tissues of the patients with low-grade gastric dysplasia. Conclusions: Activation of telomerase may be related with the malignant potentiality in gastric cells. Further studies are needed to define the role of telomerase in gastric tumorigenesis.         산발적으로 발생한 급성 E형 간염 3예

        Acute hepatitis E is an endemic disease, commonly reported in Indian subcontinent, China, Africa, Central America, and so forth. It is a self-limiting disease like other acute hepatitis except in pregnant patient. Although sporadic hepatitis E is noted all over the world, most of them are associated with travel history to HEV-endemic area. In Korea, Hepatitis E is rarely reported. Moreover, sporadic acute hepatitis E without travel history to HEV-endemic area is very rare. We experienced three sporadic cases of acute hepatitis E, without travel history. All of them presented acute hepatitis symptoms, elevated aminotransferase, and positive IgM HEV Ab. Symptoms and aminotransferase levels were normalized during hospitalization and IgM HEV Ab converted negative after 4-8 months. We report three sporadic cases of onset-acute hepatitis E without travel history to HEV-endemic area.(Korean J Gastroenterol 2007;50:121-125)

        약물방출 스텐트 시술 후 항 혈소판 삼제 복합치료의 효과

        목적: Cilostazol은 phosphodiesterase (PDE) III를 선택적으로 차단하는 항혈소판 제제로 혈관 내막증식 억제 작용을 함께 가지고 있다. 최근의 임상연구에서 관상동맥 내 BMS 삽입 후 cilostazol을 포함한 삼제 병합치료(aspirin+clopido-grel+cilostazol)를 하였을 경우 표준치료(aspirin+clopidogrel+placebo)에 비하여 유의한 재협착률 감소를 나타내었다. 본 연구는 DES 삽입 후 저용량의 cilostazol을 포함한 6개월 간의 항혈소판 삼제 병합요법의 효과를 알아보고자 하였다. 방법: 2004년 6월부터 2006년 1월까지 순천 성가롤로 병원에서 관상동맥 내 DES 삽입술을 시행 받은 환자를 대상으로 6개월간 저용량의 cilostazol (50 mg/BID) 복용 여부에 따라 I군(aspirin+clopidogrel+cilostazol)과 II군(aspirin+clopidogrel)으로 나누어 추적 관상동맥 조영술 및 임상결과를 전향적으로 비교하였다. 결과: 대상 환자는 109명이었으며 I군은 56명(67.3±10.8세, 남자 37명), II군은 53명(67.6±10.5세, 남자 26명)이었다. 대상 환자 중 80명(74%)에서 6개월 후 추적 관상동맥 조영술을 시행하였고, 임상 추적관찰은 모든 환자에서 실시되었다. 추적 관상동맥 조영술상 양 군 간에 최소 혈관 내경(I군: 2.25±0.63 mm, II군: 2.30±0.56 mm, p=0.742), 후기 손실(I군: 0.47±0.47 mm, II군: 0.52±0.53 mm, p=0.747)과 재협착률 (I군: 7.2%; II군: 5.6%, p=0.611)의 유의한 차이는 없었다. 또한 임상 추적 결과상 스텐트 혈전증은 일어나지 않았고, 양군간에 심장관련 사고나 출혈성 합병증 발생률의 유의한 차이는 없었다. 결론: DES 삽입 후 저용량의 cilostazol을 포함한 6개월 간의 항혈소판 삼제 병합치료는 출혈성 합병증을 증가시키지 않고 비교적 안전하게 사용할 수 있지만 보다 큰 최소혈관 내경을 얻고 재협착률을 감소시키는데 효과가 없었다. Background/Aims: A recent clinical trial demonstrated that triple anti-platelet therapy resulted in significantly larger minimal luminal diameter and lower restenosis rate compared with conventional therapy after bare-metal stent (BMS) implantation. However, it is uncertain that this result will be repeated after drug-eluting stent (DES) implantation, especially with low-dose cilostazol therapy. Thus, we performed a prospective, randomized study to evaluate the effectiveness of long-term triple therapy with low-dose cilostazol after DES implantation. Methods: We analyzed 109 patients (132 lesion) prospectively, who underwent successful coronary DES implantation. The patients were divided into two groups according to combined anti-platelet regimen: triple combination of aspirin, clopidogrel, and low-dose cilostazol (50 mg/bid) (Group I, n=56) or dual combination of aspirin and clopidogrel (Group II, n=53) for 6 months. The minimal luminal diameter and binary restenosis rate were compared at 6-month follow-up by coronary angiogram. The rates of stent thrombosis, major adverse cardiac events (MACE), and bleeding complication were also analyzed. Results: The baseline clinical and angiographic characteristics were not different between the two groups. Angiographic follow-up was performed in 80 patients (109 lesions, 74%). The minimal luminal diameter at 6 month was 2.25±0.63 mm in group I and 2.30±0.56 mm in group II (p=0.742). Restenosis occurred in 4 patients (7.2%) in group I and 3 patients (5.6%) in group II (p=0.611). There were no differences in the rates of stent thrombosis, MACE, or bleeding complications between the two groups. Conclusions: Long-term triple anti-platelet therapy with low-dose cilostazol after DES implantation was not effective in obtaining larger minimal luminal diameter or reducing restenosis rate, but it was used safely without increasing bleeding complication.(Korean J Med 74:368-375, 2008)

      • Rifampicin에 의한 것으로 추정되는 위막성 대장염1예

        Pseudomembranous colitis, caused by altering the normal colonic flora and allowing the multiplication of Clostridium difficile, is an deleterious adverse effect of antibiotics. But it is rarely reported by rifampicin. Rifampicin is one of the first line drug in the treatment of tuberculosis and many patients are exposed to its potential adverse effects. We experienced a patient that had abdominal discomfort and hematochezia due to pseudomembranous colitis after receiving antituberculous medication, and which was probably caused by rifampicin. A 82 years old man was admitted with abdominal discomfort and hematochezia for one week. On the past history he had been diagnosed as endobronchial tuberculosis about 4 months ago. Colonoscopy revealed multiple discrete whitish mucosal lesion on rectosigmoid colon, and histologic findings were consistent with pseudomembranous colitis. The antituberculous agents were discontinued and vancomycin was administered. The patient's symptoms were resolved within several days. There was no recurrence after reinstitution of the antituberculous agents excluding rifampicin. We report here on a case of pseudomembranous colitis probably due to rifampicin.
