        편도림프관종성 폴립 - 2예 보고 -

        Tonsillar lymphangiomatous polyp is an uncommon hamartomatous lesion that generally arises from the tonsillar surface, and it has rarely been reported in the medical literature. Because of the uncommon clinical and pathological features of these polyps, pathologists and clinicians may experience difficulty in correctly classifying them. We report herein two cases of lymphangiomatous polyp of the tonsil in a 49 year-old man and a 30 year-old man who both presented with a tonsillar mass of the palatine tonsil. Microscopically, there were protruding polypoid lesions that had a core of fibroadipose tissue with dilated multiple lymphatics and aggregated lymphoid tissue. Both patients have been stable since resection.

        ERCC1 Predicts a Poorer Platinum-based Chemotherapy Outcome but a Better Outcome for Uracil-Tegafur in the Resected Stage I-II NSCLC

        Background: The role of excision repair cross-complementation group 1 (ERCC1) has been controversial in non-small cell lung cancer (NSCLC) patients who received adjuvant chemotherapy with a platinum agent. We investigated ERCC1 expression in stage I-II NSCLC to clarify its significance for adjuvant chemotherapy. Methods: The ERCC1 expression profile was evaluated by immunohistochemistry and compared according to adjuvant chemotherapeutic agents in 146 patients who underwent surgical resection for stage I-II NSCLC. The patients were divided into 3 groups; adjuvant chemotherapy with a platinum based agent (18.5%, 27/146); adjuvant chemotherapy with uracil-tegafur (UFT) (40.4%, 59/146); surgery-alone (41.1%, 60/146). Results: Nuclear ERCC1 expression was detected in 71.9% (105/146) of NSCLC and was significantly associated with a shortened survival period in the group 1 patients who received the platinum based regimen after surgery. The group 2 patients who received UFT showed the longest survival period, followed by the surgery-alone group (overall survival, p=0.049; disease-free survival [DFS], p<0.001). Conclusions: These results suggest that stage I-II NSCLC patients with ERCC1 expression experience a shorter DFS period with adjuvant chemotherapy with a platinum based regimen and may benefit from adjuvant chemotherapy with UFT, instead of platinum after surgery.

        It is important to recognize the pathognomonic cytologic findings of chordoma, because of overlapping cytologic features between chordoma, chondrshdosarcoma, myxofilbrillary ependymoma, and metastatic adenocarcinoma. We reviewed the cytomorphologic features of 5 cases of chordoma diagnosed by fine needle aspiration cytology at Korean Cancer Center Hospital from 1987 to 2003. Clinical and radiographic findings of each case were reviewed. Four males and one female (29-54 years) had tumors involving the sacrum. Pain was the presenting symptom in 4 cases. The three cases showed moderate to high cellularity. In all cases, typical physaliferous cells with or without cytoplasmic processes were noted. In two cases, the background was myxoid with single scattered cells. Cell clusters showing cord-like arrangement were occasionally seen. The single or clustered cells showed mild cellular pleomorphism with slightly increased nuclear/cytoplasmic ratio. Mitotic figures were not seen. In our review, the recognition of physaliferous cells is the most important feature to diagnose chordoma and to differentiate it from other lesions mimicking chordoma.

        Effects of UV Radiation on Oxidative Stress in Yellowtail Clownfish Amphiprion clarkii

        We investigated oxidative stress under UV radiation (380 nm) at two intensities (0.2 and 0.4 W/m2) for 14 days (0, 1, 3, 7 and 14) in the yellowtail clownfish Amphiprion clarkii. We analyzed mRNA expression and the activity of antioxidant enzymes [superoxide dismutase (SOD) and catalase (CAT)], levels of plasma hydrogen peroxide (H2O2), lipid peroxide (LPO), alanine aminotransferase (AlaAT) and aspartate aminotransferase (AspAT). Furthermore, DNA damage was measured using comet assays and terminal transferase dUTP nick end labeling (TUNEL) assays were performed. As a result, the mRNA expressions and activities of antioxidant enzymes, the levels of plasma H2O2, LPO, AlaAT and AspAT were significantly higher in the group exposed to UV radiation than the control group. Finally, the comet and TUNEL assay showed that apoptosis was markedly higher at high UV intensity compared to the control group. These results indicate that UV radiation induces oxidative stress and decreases the antioxidant response in yellowtail clownfish.

        그레이브스병의 수술에서 적절한 절제 범위

        Background and Objectives Thyroidectomy for Graves’ disease is a rapid, safe, highly successful treatment modality. Also thyroidectomy allows pathological examination for diagnosis of suspected cancer. However, the extent of thyroidectomy in Graves’ disease is still controversial. The aim of this study is to evaluate the appropriate surgical extent by comparing the postoperative outcomes of two groups of patients with Graves’ disease who underwent total or less than total thyroidectomy. Subjects and Method We carried out a retrospective review of 33 patients who underwent thyroidectomy for Graves’ disease from January 2001 to December 2010. We investigated the postoperative thyroid hormone function and complication rate according to the extent of thyroidectomy. Results Among the 33 patients with Graves’ disease, 19 patients underwent total thyroidectomy, six patients bilateral subtotal thyroidectomy (BST) and eight patients lobectomy and contralateral subtotal lobectomy (LCSL). Postoperative hypothyroidism, euthyroidism, and hyperthyroidism occurred in 27 (81.7%), 2 (6.1%) and 4 (12.2%) patients, respectively. Postoperative recurrent hyperthyroidism was observed in 2 (33.3%) patients of BST and in 2 (25.0%) of LCSL. One patient was found with permanent hypoparathyroidism, and two with transient vocal cord paralysis. According to histopathologic report, nine patients were accompanied with thyroid cancer. Conclusion Thyroidectomy for Graves’ disease has high success rate, low recurrence rate and low complication rate. In terms of surgical extent, total thyroidectomy is a more appropriate procedure because of its low recurrence of hyperthyroidism.

        Characteristics of HfO₂ and Mo are compared using ICP and E-ICP. E-ICP that used axis magnetic field to ICP to improve plasma density is improved etch selectivity of Mo and HfO₂ than ICP. We analyzed plasma etching damage causing in etch process to manufacture MIS(Mo/HfO₂/Si) Capacitor - comb pattern with the same area but different periphery length. As a result, leakage current of E-ICP was larger than ICP. Through these result, E-ICP was improved selectivity of Mo and HfO₂ because axis magnetic field increased plasma density, but E-ICP had characteristics decreasing of uniformity, electron shading effect and selective increase of ion charge.

        연산 오골계 물 추출물이 조골세포와 파골세포의 활성에 미치는 영향

        본 연구는 연산 오골계의 물 추출물을 이용하여 MG-63 조골세포와 RAW 264.7 파골세포의 분화에 미치는 영향을 분석하였다. 조골세포의 분화에 미치는 영향을 관찰하기 위해 ALP 활성 분석과 alizarin red 염색을 통해 골 석회화를 측정하였다. 그리고 RAW 264.7 파골세포의 분화 억제 활성을 확인하기 위해 TRAP 활성과 염색을 분석 측정하였다. 연산오골계 물 추출물을 농도별(250-1,000 μg/ml)로 처리한 결과 조골세포와 파골세포에서 세포독성을 나타내지 않았다. ALP 활성은 3년산 암탉 육질이 133.8%, 3년산 수탉 육질은 129.6%로 육질이 껍질보다 조골세포 분화력이 높았다. 그리고 육질에서는 3년산이 1년산보다 ALP 활성이 높으나 껍질에서는 1년산이 3년산보다 조골세포 분화능력이 높았다. 성별 간 ALP 활성은 전체적으로 암탉이 수탉보다 높은 ALP 활성을 보였다. 또한, 골 석회화 능력은 3년산 암탉 육질이 연령과 성별을 통틀어 124.3%로 가장 뛰어났으며 TRAP 활성은 3년산 수탉 육질이 31.8%로 연령과 성별을 통틀어 억제 활성이 가장 뛰어났다. 연산 오골계 물 추출물은 조골세포 분화능력이 뛰어나 골의 석회화를 촉진하는 능력이 뛰어났으며 파골세포의 분화를 억제하여 골 흡수를 억제하는 능력이 뛰어났다. 이에 연산 오골계는 골 기능 강화와 골 관련질환에 대한 예방과 치료에 효과가 있을 것으로 보인다. The effects of water extracts of Gallus gallus var. domesticus (Yeonsan ogolgye, GD) on the activities of osteoblast differentiation and the restraint of osteoclast formation were investigated. The water extract of GD in the human osteoblast “MG-63” cell, was examined in relation to alkaline phosphatase (ALP) activity and alizarin red stains. In order to observe the effects of osteoclasts formation, we analyzed RAW 264.7 cell tartrate-resistant acid phosphatase (TRAP) activity and TRAP stains. The ALP activity of the water extract of hen and cock flesh (3 years) were 133.8% and 129.6%, respectively. The ALP activity of flesh extracts was also higher than that of the skin extracts. Concerning the effects of age, the 3 years old flesh extracts had a higher activity than that of the one year old extracts. However the activity of the 3 years old skin extracts was lower than that of the one year old extracts. For gender conditions, the ALP activity of the hen extract was higher than that of the cock. The degree bone mineralization in the three years old hen flesh exhibited the highest rate, at 124.3%, amongst all the groups. The TRAP activity of the flesh extracts of the three years old cock revealed the lowest rate, at 31.8%, compared to the control. Our results demonstrate that the water extract of GD increases bone mineralization and osteoblast differentiation activity in MG-63 cells and enhances the inhibitory activity of bone-resorption in RAW 264.7 cells. In conclusion, the water extracts of GD seem to be effective in the prevention and treatment of bone related disorders.

      • 동물실험을 통한 감압병 발생의 원인 인자 규명

        Objective: Decompression sickness (DCS) is one of the most important diseases in underwater medicine. However, we do not know about the pathogenesis and related factors of DCS, therefore prevention or treatment are difficult. Nowadays, inflammatory process has been known as the major pathogenesis in DCS, though, only few proteins are concerned with provocation factors. We tried to evaluate the expression of some inflammatory factors, especially Cyclooxygenase-2(COX-2) and Heat shock protein-70 (HSP-70), in DCS animal model. Methods: SD rats (n=15) were randomized into two groups. Control group (n=5) was safely decompressed after pressurization (175ft. 6.2ATA) for 10 hours. Experimental group (n=10) was abruptly decompressed to atmospheric pressure. All animals were euthanized, and blood and lung were collected for analysis of CRP and histologic evaluation such as degree of inflammation with immunohistochemical analysis for COX-2 and HSP-70 proteins. Results: Dehydrated group showed tendency of increasing level of CRP. The number of animals showing high serum level was observed in 7 out of 10 experimental group (70%). In the other hand, increased CRP level was found in two out of 5 control group (40%). Immunohistochemical analysis has been resulted that the status of COX-2 and HSP-70 expressions were higher in lung of experimental group than of control group as 40% and 50%, respectively. Conclusion: The two proteins showed elevated expression levels in experimental group than in control ones. Then we may suggest the relationship between inflammatory factors and DCS.

        In this work, the prior indentation theory for a bulk material is extended to an indentation theory for evaluation of thin-film material properties. We first select the optimal data acquisition location, where the strain gradient is the least and the effect of friction is negligible, A new numerical approach to the thin-film indentation technique is then proposed by examining the finite element solutions at the optimal point. With this new approach, from the load-depth curve, we obtain the values of Young's modulus, yield strength, strain-hardening exponent, with average errors less than 3, 5, 8% respectively.
