      • 입원중인 암환자에 대한 통증관리의 적절성평가 - 한 3차 의료기관 내과 전공의를 대상으로 -

        목적 : 암환자에서 가장 흔히 겪게 되고, 심각한 증세는 통증이다. 진행성 암인 경우 $60{\sim}90%$의 환자들이 통증으로 고상하게 되며, WHO통계에 따르면 전세계적으로 4백만 정도의 암환자들이 통증으로 고생하고 있다. 통증관리원칙에 따르면 환자 $70{\sim}90%$이상에서 적절한 통증조절이 이루어질 수 있음에도 불구하고 이들 중 $60{\sim}70%$의 암환자들이 적절한 통증치료를 받지 못하고 있다. 본 연구는 1) 암환자의 통증 빈도 2) 환자와 담당전공의에서 통증인지의 일치 여부 3) 암환자의 통증치료에서 진통제 투약의 적절성 등을 조사함으로써, 암환자 통증에 대한 이해와 통증관리에 대한 지침을 마련하는데 기초자료를 제공하고자 하였다. 대상 및 방법 : 서울대학교병원 내과병동에 입원한 암환자들의 통증 빈도, 담당전공의와 환자의 통증인지 일치도, 통증 관리의 적절성 등을 평가하기 위해서 환자와 담당전공의를 대상으로 설문조사를 시행하고, 진료기록을 평가하였다. 결과 : 7일 이상 내과병동에 입원해 있는 암환자 59명중 43명(62.8%)이 통증을 호소하였다. 전공의들은 담당 암환자들의 통증을 실제보다 과소평가하고 있었으며, 진통제 투약 원칙이 지켜지지 않은 경우도 상당수 있었고, 환자의 통증이 심해질수록 통증에 대한 치료가 더 불충분한 경향이 있었다. 결론 : 암환자에 있어서 통증은 아직까지 낮게 평가되고 있으며 통증 치료는 여전히 불충분하다, 이와 같은 점을 고려할 때, 통증의 평가 및 치료에 대한 더 많은 교육이 필요할 것으로 생각된다. Purpose : Pain is the most serious symptom that cancer patients experience. About $60{\sim}90%$ of advanced cancer patients and four million patients worldwide, according to the WHO statistics, are reported to suffer from cancer pain. Although about $70{\sim}80%$ of the pain could be controlled according to the pain control principles, to our regret, only $30{\sim}40%$ are managed appropriately. This research was aimed to (1) investigates the prevalence of pain among cancer patients, (2) compare patients' perception of pain with physician's recognition and (3) evaluate appropriateness of the doctor's prescription of analgesic. Materials and Methods : Patients with advanced or terminal cancers admitted at department of internal medicine of Seoul National University Hospital for at least 7 days were enrolled. A questionnaire for the patients and the physicians in charge were given and the answers were compared for each other. We also examined their medical records and the physician's orders. Results : Total 59 patients were enrolled. Among them, 43 patients answered the questionnaire, and 27 patients (62.8%) suffered hem cancer pain. The survey also showed that physicians underestimated the severity of pain, overlooked frequently analgesic prescription principle, and that as the patients' pain became severe, the less adequate was pain managements. Conclusion : For cancer patients, pain was frequently overlooked, and treatment still inadequate. Based on this evidence, it seemed that more active practice and education about evaluation and management of cancer pain are needed.

        마우스에서 방사선 영향에 대한 녹차와 Diethyldithiocarbamate의 조절효과

        본 연구에서는 방사선 손상 경감 효과를 나타내는 기호식품을 발굴하고자 국내에서 주로 음용되고 있는 녹차의 방사선 손상 경감효과를 확인하기 위하여 고선량 및 저선량 방사선을 마우스에 조사하고 소장움 생존, 내재성 비장 조혈세포집락 형성 및 apoptosis 유발 등을 관찰하였으며, 그 효과를 방사선 방호효과가 알려져 있는 diethyldithiocarbamate(DDC)와 비교ㆍ관찰하였다. 생존 소장움의 수는 방사선 조사 전 녹차 투여군(경구투여군 : p<0.01, 복강내 투여군 : p<0.01) 및 DDC 투여군(p<0.005)에서 유의성있게 증가되었다. 내재성 조혈세포집락 형성은 방사선 조사 대조군에 비하여 조사 전 복강내 투여한 군(p<0.05)에서만 유의성 있는 증가를 나타냈으며, DDC 투여군에서는 효과를 관찰할 수 없었다. 방사선에 의한 apoptotic cell의 형성은 방사선 조사 전 녹차 투여군(경구 투여군 : p<0.005, 복강내 투여군 : p<0.05) 및 DDC 투여군(p<0.005)에서 통계적으로 유의성 있게 감소되었다. 이상의 결과에서 녹차의 방사선 손상 경감효과를 조혈세포의 생존과 회복, 소장움 세포 생존 및 방사선에 의한 apoptosis 유발억제를 통하여 확인하였으며, 이는 독성이 적은 천연물이라는 관점에서 방사선 손상 경감 음료로서 적용이 가능할 것으로 사료되며 추후 유효 분획 및 성분에 관한 연구가 요구된다. We performed this study to determine the effect of green tea on jejunal crypt survival, endogenous spleen colony formation, and apoptosis in jejunal crypt cells of mice irradiated with high and low dose of gamma-irradiation. The radioprotective effect of green tea was compared with the effect of diethyldithiocarbamate (DDC). Jejunal crypts were protected by pretreatment of green tea (p<0.01). Green tea administration before irradiation resulted in an increase of the formation of endogenous spleen colony (p<0.05). The frequency of radiation-induced apoptosis in intestinal crypt cells was also reduced by pretreatment of green tea (p<0.05). The radioprotective effect on jejunal crypts and apoptosis in the DDC treated group appeared similar to those in the green tea treated groups. Treatment with DDC showed no significant modifying effects on the formation of endogenous spleen colony. These results indicated that green tea might be a useful radioprotector, especially since it is a relatively nontoxic natural product. Further studies are needed to characterize better the promotion nature of green tea and its components.

        ICR 마우스에서 방사선 조사 용량에 따른 혈액변화의 관찰

        The effects of ionizing radiation on the peripheral blood elements of ICR mouse were examined after varying doses of whole-body gamma-irradiation. ICR mice (n=50) were exposed to 0, 2, 4, 6 and 8 Gy gamma-ray ($^{60}Co$) at 10 Gy/min. The animals were studied for their hematological response on days, 3, 7, 14, 21, 42 and 56 post irradiation. No significant change was noted in erythrocyte, hemoglobin and hematocrit values after irradiation with dose of 2 Gy. Decreasing erythrocyte, hemglobin and hematocrit values occured after irradiation with doses of more than 4 Gy on day 7 after irradiation followed by a sharp fall on day 14. A recovery in these values was noted after 3 weeks of irradiation. Thrombocyte counts decreased on day 3, reaching minimal values on day 7. The total number of leukocytes was reduced on day 3, mainly because of a decrease in the lymphocyte population. An evident lymphopenia and neutropenia occur almost on the day 3 and last up to the day 28 after irradiation. All of the hematological values decreased in the blood in a dose-dependent manner at the same time.

        태아시기의 방사선 노출에 의한 마우스 뇌 발달에 미치는 영향에 관한 형태학적 연구

        The present study analyze the morphological aspects of the cerebrum of mice with prenatal exposure to high and low dose (0.5, 1, 2 Gy) of $\gamma$-radiation on gestation day 12 or 16. The animal were allowed to give birth and the offspring were sacrificed at postnatal days 28 for gross and microscopic examination of cerebrum. Their body weight, brain weight, brain length, brain width, cortical thickness and area of cingulum bundle were examined. The histological and planimetric analysis were performed observing coronal sections. The gross malfomation (microcephaly) and abnormality of cortical architecture were prominent after exposure to 2 Gy on day 12 of gestation. significant dose-related reductions in body weight, brain weight, brain size were found in all irradiated groups. A significant change was found in thickness of the cerebral cortex and area of the cingulum bundle in the groups exposed to 0.5 Gy or more. There was no difference a lamina patter of six layers in cerebral cortex between the control and irradiated groups, but cell packing density increased significantly in the group exposed to 1 Gy or more. These results suggested that dose as low as 0.5 Gy could cause a morphologically reduce change in developing mouse cerebrum and exposure on day 12 of gestation to $\gamma$-irradiation is a particularly sensitive phase in causing malformation and abnormality of central nerve system.

        신경독성 물질에 폭로되지 않은 제조업체 여성 근로자의 신경행동검사 수행능력범위

        유기용제와 같은 신경독성 물질에 폭로되지 않은 제조업체 여성 근로자를 대상으로 세계보건기구의 Neurobehavioral core Test Battery(NCTB) 7가지 검사 항목 중 사업장 여건을 고려하여 산타아나 민첩성검사와 목적점 찍기, 숫자 기호화, 단순반응시간, 시각기억 검사 등 5가지 항목을 세계보건기구 표준방법에 따라 실시하여, 신경독성 물질폭로로 인한 신경행동학적인 영향을 평가하기 위한 대조군 자료를 마련하고자 본 연구를 시행하였다. 잘 쓰는 손과 반대편 손의 산타아나 민첩성검사 수행능력은 각각 $45.7{\pm}7.1$ 및 $41.9{\pm}6.4$개, 목적점 찍기는 $191.9{\pm}38.6$개였다. 숫자 기호화와 시각기억검사는 각각 $57.7{\pm}16.0$ 및 $7.8{\pm}1.7$개, 단순반응시간의 평균은 $274.8{\pm}44.6msec$였고 평균 표준편차는 $70.5{\pm}69.0msec$였다. 대부분의 신경행동검사 항목들은 피검자의 연령, 교육수준의 영향을 받는 것으로 나타났다. 산타아나 민첩성검사는 양손 모두에서 피검자의 교육년한에 영향을 받는 것으로 나타났다. 목적점 찍기 검사에서 올바르게 한 것은 피검자의 연령과 교육년한에 영향을 받는 것으로 나타났다. 숫자 기호화는 연령과 교육년한에, 단순반응시간은 교육년한에, 그리고 시각기억검사도 교육년한에 영향을 받는 것으로 나타났다. 전체적으로 볼 때 피검자의 교육수준은 모든 검사항목에서 주요한 영향 변수로 작용하고 있었고 연령은 일부 검사의 영향 변수로 작용하였다. With increased use of neurotoxic agents in manufacturing industries, hazardous effects of neurotoxic agents pose significant problems in protecting health of workers who work in these industries. A normal range of neurobehavioral performance is required to study hazardous effects of neurotoxic agents among workers. However, such reference for Korean population is not available yet. The objective of this study is to estimate a normal range of neurobehavioral test performance of female workers in Korea. Data for neurobehavioral test performance developed by the World Health Organization were obtained from 165 female workers. Study subjects, 140 production workers and 25 clerks, who voluntarily participated in this study were not occupationally exposed to neurotoxic agents. The mean age and the mean education years of them were 32 years and 10.9 years, respectively Santa Ana dexterity, pursuit aiming, digit symbol, simple reaction time, and Benton visual retention tests among the Neurobehavioral Core Test Battery(NCTB) were included in the evaluation. Subjects were interviewed by a trained interviewer for their detailed occupational history $Mean({\pm}SD)$ performance of the participants were: $45.7{\pm}7.1\;and\;41.9{\pm}6.4$ in the Santa Ana dexterity test of the preferred and non-preferred hands; $45.7{\pm}7.1\;and\;41.9{\pm}6.4$ in correct dot of the pursuit aiming test; $57.7{\pm}16.0$ in the digit symbol test; $274.8{\pm}44.6$ msec in the mean simple reaction time and $70.5{\pm}69.0$ msec in the mean standard deviation of it; and $7.8{\pm}1.7$ in the Benton visual retention test. Most neurobehavioral performance scores were correlated significantly with age and educational level. Educational level was found to be a significant independent variable which was associated with all test scores. Age was significantly associated with scores of pursuit aiming and digit symbol tests.

        다문화 공감교육 실천 사례 연구-비폭력대화와 회복적 정의를 중심으로

        이 글은 `다문화 공감교육`실천 사례에 관한 세 연구자의 협력적 실행연구다. 구체적인 실천 모델은 `비폭력대화`와 `회복적 정의`의 패러다임과 기술이며,내러티브 교육과정으로 구성하였다.연구 과정은 `이야기에 들어가기(“모두 달라요”),이야기 살기(“달라도 괜찮아”),다시 이야기하기(“달라서 아름다워”)`로 진행되었다.연구 결과는 첫째,다양한 학습 주체들이 상호작용하는 학교 현장에서의 현실을 생생하게 비춰봄으로써 다문화교육의 과제를 되짚어 볼 수 있었다.둘째,현장 교사 연구자와 각 분야 전문가인 두 연구자가 함께 교육과정을 모색하고 실행함으로써 이론과 실제가 조화를 이루었다.셋째,교사와 아이들의 내러티브를 통해 다문화 공감교육에 대한 실천적 지식이 구성되는 계기와 내용을 확인할 수 있었다.향후 다문화 교수자 및 학습자를 위한 교육과정 개발의 기초자료로 활용될 것으로 기대된다. This study is a cooperative action research by three researchers on practical cases of `Multicultural Empathy Education.` The specific practical model is a paradigm and description of `Nonviolent Communication` and `Restorative Justice,` which is consisting of narrative curriculum. The research process is categorized into the following three: Telling a story (“We all are different.”), living the story (“It is okay being different.”) and retelling the story (“We are beautiful because we are different.”). When it comes to the outcomes, first, it reminded us of the assignments of multicultural education by vividly describing the reality of schools where various learning subjects are interacting with each other. Second, the two researchers (a teacher and a specialist for each field) worked together to seek and execute the curriculum so that the theories and practice were balanced. Third, it was able to identify the opportunities and details of how practical knowledge was established on multicultural empathy education through the narratives of teachers and children. It is expected to use the research outcomes as basic data for curriculum development for teachers and learners of multiculturalism.

        방사선의 발생독성 검색을 위한 단기 최기형성 시험법의 확립

        The present study was carried out to establish a short-term teratogenicity study for detecting developmental toxic potential induced by gamma radiation in ICR mice. Pregnant mice were exposed at dose levels of 0, 0.5, 1, 2, or 4 Gy on gestational day 8.5. All dams were subjected to caesarean section on gestational day 10.5 and their embryos were examined for growth, differentiation, and morphological abnormalities. An increase in the number of resorption was found at 4 Gy in a dose-dependent manner. Dose-dependent decreases in the developmental score of yolk sac circulation and olfactory system at above 1 Gy, in the number of somite pairs and developmental score of allantois, optic system, and maxillary process at above 2 Gy, and in the all growth and developmental parameters examined at 4 Gy were found. Various types of morphological abnormalities were seen at dose levels of 0.5 Gy or greater. Characteristic malformations induced by gamma radiation were abnormal axial rotation, hematoma, craniofacial hypoplasia, open neuropore, shortened prosencephalon, kinked somites, irregular somites, swelling, hydropericardium, absent branchial bar, and absent limb bud. Morphological alterations such as hematoma, craniofacial hypoplasia, open neuropore, and kinked somites were noted even in the lowest dose (0.5 Gy). These results indicated that the shortterm teratogenicity study established in this study can be a useful tool for not only detecting the developmental toxic potential induced by gamma radiation, but also screening radio-protective agents in ICR mice.
