Outcome of Inhaler Withdrawal in Patients Receiving Triple Therapy for COPD

Background: The purpose of this study was to document outcomes following withdrawal of a single inhaler (step-down) in chronic obstructive pulmonary disease (COPD) patients on triple therapy (long-acting muscarinic antagonist and a combination of long-acting β2-agonists and inhaled corticosteroid), which a common treatment strategy in clinical practice. Methods: Through a retrospective observational study, COPD patients receiving triple therapy over 2 years (triple group; n=109) were compared with those who had undergone triple therapy for at least 1 year and subsequently, over 9 months, initiated inhaler withdrawal (step-down group, n=39). The index time was defined as the time of withdrawal in the stepdown group and as 1 year after the start of triple therapy in the triple group. Results: Lung function at the index time was superior and the previous exacerbation frequency was lower in the stepdown group than in the triple group. Step-down resulted in aggravating disease symptoms, a reduced overall quality of life, decreasing exercise performance, and accelerated forced expiratory volume in 1 second (FEV1) decline (54.7±15.7 mL/yr vs. 10.7±7.1 mL/yr, p=0.007), but there was no observed increase in the frequency of exacerbations. Conclusion: Withdrawal of a single inhaler during triple therapy in COPD patients should be conducted with caution as it may impair the exercise capacity and quality of life while accelerating FEV1 decline.

N/A As a research for developing fine-grained high strength low carbon steels, the effects of austenitization temperature and hot deformation on microstructure was investigated in 0.15 wt% carbon steels with microalloying elements such as Nb and Ti. When the steels were reheated at 1250℃, Nb containing steel showed very coarse austenite grain size of 200㎛ whereas Nb-Ti steel did fine one of 70㎛ because Ti carbonitrides could suppress the austenite grain growth. In case of 50% reduction at 850℃, the austenite grains in the Nb steel partially recrystallized while those in the Nb-Ti steel fully recrystallized probably due to finer prior austenite grains. For the Nb-Ti steel, ferrite grain size was not sensitively changed with austenitization temperature and compression strain and, severe deformation of 80% reduction was not essentially necessary to refine ferrite grains to about 3㎛ which could be obtained through lighter deformation of 40% reduction.
체액세포분석 수기검사: 국내 실태조사 및 임상검사실 지침(2021)

To investigate the current status of body fluid analysis for cellular composition in Korea, an online survey was performed. Responses were received from 76 laboratories of general hospitals in Korea. The questionnaire included questions on pre-analytical, analytical, and post-analytical processes of body fluid cellular analysis (cell count using hemocytometers and cytospin) performed in each laboratory. The survey showed that the body fluid cellular analysis in Korea is not harmonized. Based on this, the “Committee for Standardization in Korean Society for Laboratory Hematology” suggests clinical laboratory guidelines for the pre-analytical (sample collection and handling), analytical (cell count, slide preparation), post-analytical (reporting, turnaround time), and quality control steps. We hope that the results of this survey and guideline will serve as a basis for setting future standards of body fluid cellular analysis in Korea, and ultimately have a positive impact on patient care 국내 체액세포분석 수기검사의 실태조사를 위한 온라인 설문조사에서 종합병원 이상 규모의 76개 기관의 회신을 받았다. 설문 문항은 체액세포분석에 대한 검사 전 단계, 검사 단계, 검사 후 단계 (혈구계수기를 이용한 세포수산정, 세포침전)에 대한 질문을 포함하고 있다. 설문조사 결과 국내 기관마다 체액세포검사의 검사법이 일치하지 않음을 확인하였다. 이를 바탕으로 진단혈액학회 표 준화위원회에서는 검사 전 단계(검체 채취 및 이송), 검사 단계(세 포계수법, 슬라이드 제작법), 검사 후 단계(보고, 검사소요시간), 정도관리에 대한 임상검사실 지침을 제안하였다. 이 설문조사 결 과와 지침이 향후 표준안을 설정하는 바탕이 되어 궁극적으로 환자 진료에 긍정적 영향을 미치게 되길 기대한다.
Endobronchial ALK-Positive Anaplastic Large Cell Lymphoma Presenting Massive Hemoptysis

Primary anaplastic large cell lymphoma (ALCL) of the lung is highly aggressive and quite rare. We report here a case of anaplastic lymphoma kinase–positive endobronchial ALCL, that was initially thought to be primary lung cancer. A 68-year-old woman presented with hemoptysis, dyspnea, and upper respiratory symptoms persisting since 1 month. The hemoptysis and and bronchial obstruction lead to respiratory failure, prompting emergency radiotherapy and steroid treatment based on the probable diagnosis of lung cancer, although a biopsy did not confirm malignancy. Following treatment, her symptoms resolved completely. Chest computed tomography scan performed 8 months later showed increased and enlarged intra-abdominal lymph nodes, suggesting lymphoma. At that time, a lymph node biopsy was recommended, but the patient refused and was lost to follow up. Sixteen months later, the patient revisited the emergency department, complaining of persistent abdominal pain since several months. A laparoscopic intra-abdominal lymph node biopsy confirmed a diagnosis of ALCL.
HVPE법으로 성장시킨 GaN 단결정의 wet etching에 의한 표면 변화

본 연구에서는 HVPE법으로 사파이어 기판(0001) 위에 성장시킨 GaN epilayer의 etching에 따른 표면변화 특성을 조사하였다. 주사전자 현미경(SEM) 관찰 결과, 3가지 형태를 갖는 육각형 모양의 etch pit(edge, screw, mixed) 들이 GaN epilayer의 두께 변화에 따라서 형성되었다. 이러한 관통전위들은(TDs) epilayer의 두께가 얇고, etch pit density가 높을수록 screw and mixed type TDs이 많이 관찰되었고, 두께가 증가할수록 etch pit density가 낮아지면서 edge and mixed type TDs들이 주로 존재하는 것을 관찰 할 수 있었다. In this paper, we investigated characteristics of etching induced surface morphology variation by wet etching of GaN epilayer were grown on sapphire (0001) substrate by hydride vapor phase epitaxy (HVPE). As a results of scanning electron microscope (SEM) observation, three types of hexagonal etch pits (Edge, Screw, Mixed) were formed by the GaN epilayer thickness variations. A lot of etch pits, attributed to screw and mixed type TD, were observed at thinner epilayer, leading to high etch pit density. On the other hand, the thickness of GaN epilayer increased with the number of etch pits corresponding to edge and mixed dislocations, which are the majority of TDs are observed.
Yb<sup>3+</sup>와 Er<sup>3+</sup> 이온이 동시 도핑 된 SrMoO<sub>4</sub>에서 발생되는 업컨버젼 현상에 대한 분석

본 연구에서는 complex citrate-gel method를 이용하여 $Yb3^{3+}$, $Er^{3+}$ 이온이 동시 도핑 된 $SrMoO_4$($SrMoO_4$: $Yb^{3+}/Er^{3+}$)를 성공적으로 합성하였으며, 그 구조적, 광학적 특성을 분석하였다. 980 nm의 근적외선 여기 광 하에서 상기 분말은 530 및 550 nm 부근의 강한 녹색 방출과 670 nm 부근에서의 약한 적색 방출은 $Er^{3+}$ 이온 내의 intra 4f transition $Er^{3+}$($^4F_{9/2}$, $^2H_{11/2}$, $^4S_{3/2}$) ${\rightarrow}$ $Er^{3+}(^4I_{15/2})$에 의한 현상임을 확인하였다. 또한, $Er^{3+}$ 및 $Yb^{3+}$ 이온의 최적 도핑 농도는 각각 2/16 mol%로 조사되었으며 그에 따른 형성 가능한 upconversion 메커니즘에 대하여 논의하였다. $Yb^{3+}$, $Er^{3+}$ co-doped $SrMoO_4$ ($SrMoO_4$ : $Yb^{3+}/Er^{3+}$) specimens have been successfully synthesized via the complex citrate-gel method and their structural and optical properties were investigated in detail. Under 980 nm excitation, $SrMoO_4$ : $Yb^{3+}/Er^{3+}$ UC phosphors have been emitted strong green luminescence at 530 and 550 nm with weak red emission around 670 nm corresponding to the intra 4f transitions of $Er^{3+}$ ($^4F_{9/2}$, $^2H_{11/2}$, $^4S_{3/2}$) ${\rightarrow}$ $Er^{3+}$ ($^4I_{15/2}$). The optimal doping concentrations of $Er^{3+}$ and $Yb^{3+}$ ions were verified to 2/16 mol% and a possible upconversion mechanism depending on pump power dependence is studied in detail.
