        선천성 결손치에 관한 임상적 연구

        The congenital missing of teeth is common, which takes place since the proliferation and differentiation are not allowed in that tooth bud fail to start development. The purpose of this study is to research incidence rate, number, and missing part of congenital missing teeth, and to study whether a person who has missing teeth has other abnormality of teeth or not. For this study, 1,520 subjects(aged 2.9~17) who had visited pediatric dentist department of Chonbuk national university dental hospital within 2 years were examined with an panoramic radiograph ; exempting third molar missing state. The obtained results are as follows. 1. 8.88% among total subjects show missing teeth ; male 9.05%, female 8.64% 2. The most frequently missing permanent teeth were the mandibular second premolars(22.3%). The most frequently missing primary teeth are mandibular lateral incisors(50%). 3. 43.3% patients have one permanent missing tooth, 34.3% have two, and 10.4% have more than six, respectively. In primary teeth, 86.7% patients have one missing tooth, and 13.3% have two missing teeth. 4. 18 patients(13.3%) have missing teeth as well as hyperdontia, while some patients have microdont, ectopic eruption, and fusion teeth. 치아의 선천성 결손은 치배의 발육이 시작되지 못하여 치아의 증식, 분화가 일어나지 못함으로서 발생하는 흔한 치아의 발육 이상이다. 이 연구의 목적은 선천성 결손치의 발생률, 결손치의 수, 발생부위와 결손치 환자에게서 나타난 다른 치아이상의 발생여부를 조사하는 것이다. 본 연구는 2006년 7월부터 2008년 6월까지 전북대학교 소아치과에 내원하여 파노라마를 촬영한 1,520명의 환자(2.9~17세)를 대상으로 제 3 대구치를 제외한 선천성 결손치의 분포를 조사하여 다음과 같은 결과를 얻었다. 1. 전체 1,520명 중 총 8.88%에서 결손치가 관찰되었으며, 남자가 9.05%, 여자가 8.64%에서 결손치가 관찰되었다. 2. 총 350개의 영구 결손치 중 하악 제 2 소구치가 22.3%로 가장 발생률이 높았고, 하악 측절치, 상악 제 2 소구치, 하악 중절치, 상악 측절치 순이었다. 또한 총 18개의 유치 결손치 중 하악 유측절치가 가장 높은 발생률(50%)을 보였다. 3. 1개의 영구치 결손치를 가지는 환자는 43.3%, 2개는 34.3%, 3개는 6,7%, 4개는 1.5%, 5개는 3.7%, 6개 이상은 10.4%이었다. 또한 유치 결손치의 수는 1개는 86.7%, 2개는 13.3%이었다. 4. 결손치가 있는 환자 중 과잉치를 가지는 환자는 18명(13.3%)이 있었으며, 왜소치, 이소맹출, 융합치 등의 치아이상을 지닌 환자도 관찰되었다.

        Korean traditional ogre patterns generally have a meaning of protecting from demons and charming away unfortunates. However, their threatening faces and complexity of shapes have not appealed to the general public as personal accessories. In order to make use of their aesthetic and peculiar beauty of figures, a new concept of ogre accessory having a friendly-butterfly shape pendant is develope This study demonstrates whole processes of a capstone design case study, that is, conceptualization of the butterfly ogre pattern, die design and manufacturing for mass production, real production and quality progress, and commercialization process.

        Function of the pentose phosphate pathway and its key enzyme, transketolase, in the regulation of the meiotic cell cycle in oocytes

        Objective: Previously, we identified that transketolase (Tkt), an important enzyme in the pentose phosphate pathway, is highly expressed at 2 hours of spontaneous maturation in oocytes. Therefore, this study was performed to determine the function of Tkt in meiotic cell cycle regulation, especially at the point of germinal vesicle breakdown (GVBD). Methods: We evaluated the loss-of-function of Tkt by microinjecting Tkt double-stranded RNAs (dsRNAs) into germinal vesicle-stage oocytes, and the oocytes were cultured in vitro to evaluate phenotypic changes during oocyte maturation. In addition to maturation rates, meiotic spindle and chromosome rearrangements, and changes in expression of other enzymes in the pentose phosphate pathway were determined after Tkt RNA interference (RNAi). Results: Despite the complete and specific knockdown of Tkt expression, GVBD occurred and meiosis was arrested at the metaphase I (MI) stage. The arrested oocytes exhibited spindle loss, chromosomal aggregation, and declined maturation promoting factor and mitogen-activated protein kinase activities. The modified expression of two enzymes in the pentose phosphate pathway, Prps1 and Rbks, after Tkt RNAi and decreased maturation rates were amended when ribose-5-phosphate was supplemented in the culture medium, suggesting that the Tkt and pentose phosphate pathway are important for the maturation process. Conclusion: We concluded that Tkt and its associated pentose phosphate pathway play an important role in the MI-MII transition of the oocytes' meiotic cell cycle, but not in the process of GVBD.

        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

        Sonazoid, a second-generation ultrasound contrast agent, was introduced for the diagnosis of hepatic nodules. To clarify the issues with Sonazoid contrast-enhanced ultrasonography for the diagnosis of hepatocellular carcinoma (HCC), the Korean Society of Radiology and Korean Society of Abdominal Radiology collaborated on the guidelines. The guidelines are de novo, evidence-based, and selected using an electronic voting system for consensus. These include imaging protocols, diagnostic criteria for HCC, diagnostic value for lesions that are inconclusive on other imaging results, differentiation from non-HCC malignancies, surveillance of HCC, and treatment response after locoregional and systemic treatment for HCC.

        Lignans from Saururus chinensis exhibit anti-inflammatory activity by influencing the Nrf2/HO-1 activation pathway

        As part of our ongoing program to develop anti-inflammatory agents, an extract derived from Saururus chinensis collected in Korea was found to inhibit nitric oxide (NO) production in RAW264.7 cells. Bioassay-guided fractionation led to the isolation two new (1 and 2) and six known dineolignans (3–8). To the best of our knowledge, manassatin B1 (3) was isolated from S. chinensis for the first time. All structures were elucidated using extensive spectroscopic analysis. Of these compounds, 2 and 8 inhibited lipopolysaccharide (LPS)-induced production of NO and showed IC50 values of 5.80 and 1.52 µM, respectively. LPS-induced expression of inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) was also significantly suppressed by the administration of 2 and 8. In addition, these lignans induced the expression of heme oxygenase-1 (HO-1) in a concentration-dependent manner. Nuclear translocation of nuclear-E2-related factor 2 (Nrf2), a key regulator of HO-1 protein expression, was also induced in RAW264.7 cells treated with 2 and 8. These findings suggested that these lignans exerted anti-inflammatory effects in RAW264.7 cells through modulation of the Nrf2/HO-1 pathway and that they were potential HO-1 inducers for preventing or treating inflammation.

        ANTIMICROBIAL ACTIVITY OFARTEMISIA SPECIES AGAINST CLINICALLY ISOLATED STREPTOCOCCUS MUTANS

        Streptococcus mutans는 Mutans streptococci 그룹에 속하는 치아우식 유발균으로 알려져 있다. 국내 자생쑥 종의 추출물이 이러한 치아우식을 유발하는 Streptococcus mutans균에 대해 활성 억제 효과가 있다는 것이 표준균주를 이용한 실험에서 밝혀져 있으며, 본 연구에서는 환자에서 분리된 S. mutans를 중심으로 쑥의 항균 효과를 알아보았다. 네 가지 임상 분리균주; Ch, L1, L2, P와 네 가지 표준균주: KCTC (Korean Culture for Type Collection) 3298, KCTC 3605 (ATCC 25175), KCTC 3306, Streptococcus mutans GS5를 이용하여 세 가지 종류의 쑥; 참쑥(Artemisia lavandulaefolia DC), 비쑥(Artemisia scoparia), 사철쑥(Artemisia capillaries)의 효능을 검증하였다. 항균활성은 세 가지 종류의 쑥 추출물의 Streptococcus mutans에 대한 최소 억제농도(MIC)와 최소살균농도(MBC), time-kill studies를 통해 알아보았다. 최소억제농도는 표준균주에 대해 각각 참쑥이 0.8 ug/mL, 비쑥이 0.8-1.6 ug/mL, 사철쑥이 0.2-25.6 ug/mL이 었으며, 분리된 균주에 대해서는 참쑥이 0.4-0.8 ug/mL, 비쑥이 0.4-0.8 ug/mL, 사철쑥 1.6-6.4 ug/mL이었다. 최소살균농도는 표준균주에 대해서 참쑥이 1.6-6.4 ug/mL, 비쑥이 6.4-12.8 ug/mL이였다. 분리균주에 대해서는 참쑥이 0.8-3.2 ug/mL, 비쑥이 0.8-3.2 ug/mL, 사철쑥은 6.4-12.8 ug/mL이었다. Time-kill assay에서 KCTC 3298이 4 hrs에 100% 살균되었으며, GS5도 거의 100% 살균되었고 L1과 L2 P는 0.5 hrs에 99.90% 살균되었다. 0.5 hrs에 KCTC 3298와 GS5는 99.99% 이상 살균되었고, 모든 균주들이 2.5 hrs에 99.99% 살균되었다. 전체적인 실험 결과 세 가지 종류의 쑥은 표준 균주 및 네 가지 분리 균주 모두에서 항균 효과가 있음을 확인하였다. Streptococcus mutans plays a major role in the formation of dental plaque and it is considered one of the important pathogens in the development of dental caries. Established dental plaque can be more resistant to antimicrobial agents and offers nutrient rich and relatively stable cariogenic conditions for bacteria. Thus bacteria growing in dental plaque have strong resistance to antimicrobial agents and they are not removed easily by the flow of saliva. Many researchers have been performed using natural materials, especially herbal extracts to prevent dental plaque. However, the strains of mutans streptococci used in the researches were not from Koreans. Therefore, it would be necessary to evaluate the antimicrobial activity of herbal extracts against clinical isolates of Streptococcus mutans isolated from saliva of Koreans living in Jeollabuk-do. For this study four clinical isolates were isolated from saliva samples of seventeen Korean people to investigate whether essential oils of Artemisia spp. have antimicrobial activity against them including four reference strains of Streptococcus mutans. Minimum inhibitory concentrations, Minimum bactericidal concentrations and time kill studies were performed and the results showed that Artemisia lavandulaefolia DC, Artemisia scoparia, and Artemisia capillaries have antimicrobial activity against the Streptococcus mutans clinical isolates and reference strains.

        Reliability and validity of Korean version of modified-Yale preoperative anxiety scale

        Purpose: The modified Yale Preoperative Anxiety Scale (mYPAS) was developed for evaluating the level of preoperative anxiety in children. The purpose of this study was to develop a Korean version of the mYPAS (K-mYPAS) and to establish its validity and reliability based on the Korean preoperative pediatric patients. Methods: K-mYPAS was made through stringent back-translation procedure. Total enrolled 102 patients answered questionnaires of Korean version of State-Trait Anxiety Inventory for Children (K-STAIC), and were videotaped for 2 to 5 minutes before induction of anesthesia. Three observers of experienced psychiatrist, surgeon, and nurse analyzed videotape with K-mYPAS comparing to K-STAIC. The inter- and intraobservers reliability, concurrent and construct validity, sensitivity, specificity, and predictive value were analyzed. Results: The value of Cronbach a for interobservers reliability was 0.939 and intraobserver reliability was statistically significant (P < 0.001). Concurrent and construct validity were also statistically significant (P < 0.001 and P < 0.001, respectively). Sensitivity, specificity, positive predictive value, negative predictive value, and accuracy were 81.3%, 91.4%, 81.3%, 91.4%, and 88.2%, respectively. Conclusion: The K-mYPAS had good psychometric properties and can be used as a reliable and valid instrument for the assessment of preoperative anxiety in children.
