우리나라에서 초등학교 전이는 아동 뿐 아니라 부모에게도 중요한 생애사건이다. 대다수 부모는 자녀의 초등학교 입학을 준비하는 과정에서 개인적인 사회적 관계망을 통해 지원과 정보를 얻는다. 이 연구는 어머니가 실제 받은 사회적 지원과 양육 역량, 학령 전 아동의 학교준비도 간의 관계를 탐색하고 사회적 지원과 학교준비도 간의 관계를 양육 역량이 매개하는지 검증하고자 하였다. 이를 위해 첫 자녀가 초등학교 입학을 앞둔 어머니 368명을 대상으로 설문조사를 실시하였다. 먼저 확인적 요인분석으로 주요 변인의 측정모형을 검증하였으며, 구조방정식 모델링와 붓스트랩 방법으로 연구모형과 매개효과를 검증하였다. 분석 결과 첫째, 어머니가 실제 받은 사회적 지원, 양육 역량, 그리고 유아의 학교준비도 간에는 유의한 정적 관계가 있었다. 높은 수준의 사회적 지원은 어머니의 높은 양육 역량과 관계가 있었으며, 양육 역량은 학령 전 아동의 높은 사회준비도와 관계가 있었다. 둘째, 어머니의 양육 역량은 어머니가 받은 사회적 지원과 아동의 학교준비도 간 관계를 매개하였다. 이 연구의 결과는 자녀의 초등학교 입학을 앞둔 어머니에게 자녀 양육과 관련된 사회적 지원을 제공함으로써 실제 어머니의 역량을 증진하고 결과적으로 유아의 학교준비도를 높일 수 있음을 시사한다. The transition to elementary school is an important but often challenging life event not only for children but also for their parents in Korea. Many Korean parents seek information and support from their personal networks while helping their children prepare for elementary school. In this study, we aimed to examine the relationships among the social support mothers actually receive, mothers’ parenting competency, and children’s school readiness with a special attention given to the mediating role of mothers’ parenting competency between social support and school readiness. We collected data in February 2017 from 368 mothers who were preparing to send their first child to elementary school in March 2017. After verifying the measurement model, we conducted structured equation modeling to test our conceptual model. We found that mothers’ higher levels of social support were related to higher parenting competency, which in turn, was associated with their children’s higher levels of readiness for elementary school. Bootstrapping results indicated that parenting competency significantly mediated the relation between the mothers’ received social support and their children’s school readiness. Our results suggest that efforts to strengthen the social relationships of mothers whose children are entering elementary school facilitate the mothers’ parenting competency, resulting in better school readiness for their children.

Despite the fact that a considerable number of preventable adverse events are managed in primary care settings, medication-related risks have been evaluated mainly through hospital admissions. While interest in high-alert medication management in the community and primary care settings is increasing internationally, Korea does not have accreditation for the same and lacks established standards for high-alert medications. Therefore, this study aimed to develop a nationallevel list of high-alert medications for community and primary care. The candidates for the high-alert medication list included medications recommended by Institute for Safe Medication Practices and those suggested by each guideline of the World Health Organization, Japan and Korea. We analyzed the adverse events related to medication errors reported by the patients' safety reporting system. Seven experts working in the community and primary care settings evaluated the adequacy and priority of the candidate medications. The final list included 20 high-alert medications, including nine essential and eleven optional medications. The national-level list of high-alert medications which we developed may be useful in formulating accreditation guidelines or treatment standards for community and primary care patients.

<P>We demonstrate a widely tunable polymer add/drop reflection filter using double-ring resonators with slightly different radii. The reflector proposed as an alternative to grating-based wavelength-selective reflectors shows a wide tuning range of 42.3 nm using a small amount of current as small as 30 mA. If the proposed reflection filter is hybrid-integrated with a reflective semiconductor optical amplifier or a superluminescent laser diode, a low-cost widely tunable laser could be realized. © 2010 Wiley Periodicals, Inc. <P><B>Abstract</B></P><P>Malabaricone C (Mal‐C), isolated from nutmeg, is known to exert a variety of pharmacological activities. However, the effect of Mal‐C on vascular smooth muscle cells (VSMCs) is unknown. This study examined the effect of Mal‐C on proliferation and migration of primary rat aortic smooth muscle cells (RASMCs) as well as its underlying mechanisms. Treatment of RASMCs with Mal‐C induced both protein and mRNA expression of heme oxygenase‐1 (HO‐1) in a dose‐ and time‐dependent manner. Mal‐C‐mediated HO‐1 induction was inhibited by treatment with actinomycin D or by cycloheximide. SB203580 (a p38 inhibitor), SP600125 (a JNK inhibitor), U0126 (a MEK inhibitor), and N‐acetylcysteine (NAC, an antioxidant) did not suppress Mal‐C‐induced HO‐1 expression. In contrast, LY294002 (a PI3K inhibitor) blocked Mal‐C‐induced HO‐1 expression. Moreover, RASMCs treated with Mal‐C exhibited activation of AKT in a dose‐ and time‐dependent manner. Treatment of RASMCs with Mal‐C increased nuclear translocation of nuclear factor‐E2‐related factor 2 (Nrf2), which is a key regulator of HO‐1 expression, and this translocation was also inhibited by LY294002. Consistent with the notion that HO‐1 has protective effects against VSMCs, Mal‐C remarkably inhibited platelet‐derived growth factor (PDGF)‐induced proliferation and migration of RASMCs. However, inhibition of HO‐1 significantly attenuated the inhibitory effects of Mal‐C on PDGF‐induced proliferation and migration of RASMCs. Taken together, these findings suggest that Mal‐C could suppress PDGF‐induced proliferation and migration of RASMCs through Nrf2 activation and subsequent HO‐1 induction via the PI3K/AKT pathway, and may be a potential HO‐1 inducer for preventing or treating vascular diseases. Here, we report a case of pulmonary paragonimiasis that was improved with initial anti-tuberculosis (TB) therapy but confused with reactivated pulmonary TB. A 53-year-old Chinese female presented with a persistent productive cough with foul smelling phlegm and blood streaked sputum. Radiologic findings showed subpleural cavitary consolidation in the right upper lobe (RUL). Bronchoscopic and cytological examination showed no remarkable medical feature. She was diagnosed with smear-negative TB, and her radiologic findings improved after receiving a 6-month anti-TB therapy. The chest CT scans, however, obtained at 4 months after completion of anti-TB therapy showed a newly developed subpleural consolidation in the RUL. She refused pathologic confirmation and was re-treated with anti-TB medication. Nevertheless, her chest CT scans revealed newly developed cavitary nodules at 5 months after re-treatment. She underwent thoracoscopic wedge resection; the pathological examination reported that granuloma caused by Paragonimus westermani. Paragonimiasis should also be considered in patients assessed with smear-negative pulmonary TB.
