Component mode synthesis which is one of substructure synthesis methods is studied for the determination of vibration characteristics of plate problem. Comparisons are made in terms of accuracy and computational efficiency. As a result, computation time of component mode synthesis is much shorter than that of whole structure analysis. In lower modes natural frequencies obtained from component mode synthesis are almost equal to those obtained from whole structure analysis, but in higher the differences of results obtained from two methods are increased.

심장내로 연장된 정맥내 평활근종증 1예 : 심장내 평활근종증 intracardiac leiomyomatosis-case report and literature review

정맥내 평활근종증은 병리학적으로는 양성 질환이나 진행 양상은 악성으로 모든 종양의 적출술이 시행되어야 하며, 자궁적출술시 병리학적으로 정맥내 평활근종증을 진단받은 경우 정기적인 검사를 통해 종양의 재발이나 하대정맥과 심장내로의 연장을 확인하여야 한다. 우측 심장내 종양이 발견된 경우는 점액종을 포함한 원발성 심내 종양 외에도 다른 원인 질환을 확인하여야 하며, 반드시 정맥내 평활근종증을 감별진단하여야 한다. Intravenous leiomyomatosis is a rare benign vascular tumor defined as the extension into venous channels of a histologically benign smooth muscle tumor arising either from a uterus or from the walls of uterine vessels, and about 10% spread to the heart. The treatment of choice is complete resection of the tumor. Hormonal therapy should be considered in cases of unresectable residual tumor. A 46-year-old woman was admitted for abdominal discomfort and pain. She was found to have intravenous leiomyomatosis of the uterus with extension into inferior vena cava and right atrium. The patient underwent surgery employing simultaneous sternotomy and laparotomy. Radical excision was achieved using cardiopulmonary bypass. We herein describe a patient in whom complete removal of intravenous leiomyomatosis with cardiac extension was successfully performed.

한국 성인에서의 후두개염의 방사선학적 진단기준

Backgrounds: A retrospective study was performed to define objective radiologic parameters in diagnosing epiglottitis on soft-tissue lateral neck radiographic study. Methods: Parameters of soft-tissue structures(epiglottis width, third vertebral body width, ratio of epiglottic width to third vertebral body width) in 30 adult patients compared with those of age and sexmatched controls with foreign body in throat whose radiographic reading was normal. Results: Epiglottis width of more than 11mm, ratio of epiglottis width(EW) to the third vertebral body width(C3W) of more than 0.5 were, respectively, found to be 100% sensitive and specific in differentiating between patients with and without epiglottitis. Conclusion: These preliminary results suggest that EW/C3W ratio of more than 0.5, EW of more than 11mm, respectively, may be useful in the diagnosis of epiglottitis in adult patients.

심혈관 위험요인과 청력역치의 상관성

To investigate the effects of cardiovascular risk factors on hearing loss, hearing threshold and cardiovascular risk factors such as blood pressure, hematologic values, and serum lipid levels of 3,827 persons who visited the Health Center of a University Hospital in Kumi for health examination from January 1, 1995 to July 31, 1997 were analysed. The results were summarized as follows; 1. Hearing threshold was shifted highly in both sex groups with age increased. Smoking did not influence increase of hearing loss, but drinking alcohol amount increased the hearing threshold when sex was adjusted. The most highly increased hearing threshold showed among farmers group, but next was blue collars group. Hearing threshold increasing basis with BMI increased. 2. Systolic and diastolic blood pressure and hematocrit value increased the hearing threshold in both sex groups, but hemoglobin value did not related to the hearing threshold. 3. Total cholesterol, LDL-cholesterol, triglyceride, and atherosclerosis index were significantly increased with the grade of hearing loss increased, when age was adjusted. 4. In the logistic regression analysis, the atherosclerosis index, age, and hematocrit value revealed risk factors which contributed in that orders, though the values were small they showed statistical significaces, but the risks were lowered when hemoglobin values increased, in both sex groups. Consequently, hyperlipidemia was significantly related to the hearing loss when age was adjusted.

벌자상 후에 발생한 급성심근경색증 2례

Acute myocardial infarction after a bee sting is a very rare process. Among proposed mechanisms, vasospasm of the coronary artery ㅑs the most probable mechanism. Many mediators are included in the venom of wasps, and those mediators have vasoconstrictive properties and may constrict the coronary artery. Furthemore, those mediators have thrombogenic properties that lead to thrombogenesis in the coronary artery and aggravate myocardial ischemia. Epinephrine, commonly used in the management of anaphylactic shock, may further aggravate myocardial ischemia, We experienced two cases of acute myocardial infarction after a bee sting. In the first case, we could find no underlying abnormalities of the coronary vessel because the patient died during coronary angiography. In the second case, the electrocardiogram showed inferior wall myocardial infarction, but only mild stenosis of the right coronary artery was seen in coronary angiography.

신체관과 건강관의 관계 규명

In modem society, health means a balanced state of body and mind, and a state of living without being given any threats of accidents in a desirable environment. It is the most important factor in determining the quality of life. Health is defined in various ways in different times and situations. Most of the cases, views on body are directly projected in views on health. The purpose of this study is to investigate different views on health based on the assumption that views on body and health will actually influence the health of contemporary people. For this purpose, relationships between views on body and on health which were closely examined through inquiring various documents and research articles on health. Body can be said to be a crucial factor in determining views on health. Basic conceptions of 'the health' are formed according to views on body. Views on body have been changed from the one that it is merely physical material, to the one that it is related to internal functions, and to the one that body and health are considered as the quality of life. These changes in views on body are connected with views on health: health is simply related to diseases and physical injuries; it is linked with the internal functions of the body; body and mind are one unit closely connected with each other; and body itself is the very quality of life.

비소세포 폐암 환자의 이차 치료로서 Gemcitabine과 Vinorelbine 혼합 요법의 효과

Background : Both gemcitabine and vinorelbine are effective anticancer drugs with mild toxicity on non-small cell lung cancer, and monotherapy of these drugs are effective as a second-line chemotherapy. The aim of this trial was to assess the response and toxicity of a combination of gemcitabine and vinorelbine in patients of previously treated for non-small cell lung cancer. Materials and Methods : 24 patients, initial stage III A/B,IV and previously treated with platinium and taxane based regimens, were enrolled from June 2000 to March 2004. The regimens consisted of vinorelbine 25mg/㎡ followed by an infusion of gemcitabine 1000mg/㎡ on day 1 and day 8 every three weeks. This course was repeated more than twice. Results : Twenty-four patients were analyzed for the response, survival rate, and toxicities. The overall response was 17% with a complete remission rate of 4%. The median time-to progression (TTP) was 3.1 months (95%, CI l-10months), and the survival time was 8.2 months (95%, CI 1-23 months). The grade 3/4 toxicities encountered were neutropenia (12.5%), anemia (0%), thrombocytopenia (0%). Non-hematological 3/4 toxicities were not observed. Conclusion : A combination of gemcitabine and vinorelbine in patients previously treated for non-small cell lung cancer provides a relatively good response rate, and a low toxicity profile. 반복작업에 의해 발생한 척골관 증후군 1례

Objectives : To report cubital tunnel syndrome due to repetitive motions. Methods : A worker complaining muscle weakness and atrophy of the right hand intrinsic muscles admitted to a hospital. We evaluated him with blood tests, neurophysio-logic studies (NCV & EMG), plain X-ray and US at the both elbows. We investigated his occupational history, and videotaped his work motions based on the work cycle at his previous work site. Finally, an ergonomics expert analyzed the motions using rapid upper limb assessment (RULA). Results : NCV & EMG studies reveals slow conduction velocity on both ulnar nerve across the elbow, more severely in the right side. US shows us compatible finding with diffuse neuritis of both ulnar nerves at both elbows. RULA score is 7. Conclusion : We confirmed that the worker's symptoms were related to his previous jobs demanding repetitive motions using the elbow joints. It is necessary that we should prepare appropriate measures to evaluate, prevent, rehabilitate, and help injured workers to return to work.
