        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

      • 내시경적 점막절제술로 치험한 식도 과립상 세포종 1례

        과립상 세포종은 Schwann 세포 기원으로 생각되며 인체에 비교적 드물게 발생한다. 과립상 세포종은 전신 어느 곳에서나 발견될 수 있으나 주로 혀, 구강, 피부 혹은 유방 등에서 호발하며 드물게 위장관에서 발견된다. 위장관에서는 식도에서 가장 호발하며 다음으로 위, 대장 순이다. 과립상 세포종은 대부분, 특히 위장관에서는 양성이며 소수의 악성 병변이 보고되었다. 이러한 이유와 함께 수술 전의 진단이 어렵기 때문에 과립상 세포종에 대한 근본적인 치료는 현재까지 외과적 절제술이다. 최근에 시도되는 치료방법들로는 내시경적 레이저 치료, 용종절제술, 내시경적 점막 절제술 등이 있다. 저자들은 상부 소화관 내시경검사를 시행하여 식도 과립상 세포종을 진단하고 내시경적 점막 절제술을 시행하여 합병증 없이 퇴원하여 현재 재발없이 경과 관찰중인 1례를 경험하였기에 보고하는 바이다. Granular cell tumors, which occur infrequently, are probably of Schwann cell origin. They can occur almost anywhere in the body but usually affect the tongue, oral cavity, skin, or breasts and are rarely found in the gastrointestinal tracts. The esophagus is the most frequent gastrointestinal site, followed by the stomach and the colon. Granular cell tumors are generally benign, especially in the gastrointestinal tract, some malignant lesions have been reported. For this reason, and also because preoperative diagnosis is difficult, the standard treatment for granular cell tumor has until now been surgical excision. In recent years, other therapeutic methods is endoscopic laser therapy (ELT), polypectiomy, endoscopic mucosal resection (EMR). We report a case of esophageal granular cell tumor which was diagnosed by an endoscopy and managed using an endoscopic mucosal resection without complication.

      • 안성시 지역의 농업기계 보관관리 실태조사

        This study was conducted to analyze the storage and management status of major farm machinery, and to present the basic information for the construction of multipurpose storage housing suitable to the individual farmers by identifying current problems. The results of this study are summarized as follows; 1. The average farm area of the surveyed farmers was 4.25㏊, which represents relatively higher than the average of whole country. The mechanization rate for paddy farming was 98% so, the individual storage housing should be concentrated on the farm machinery utilized in paddy farming. 2. The annual operating span of the seasonal farm machinery (such as combine and rice transplanter) showed high difference between farmers and machines. For th rice transplanter, the highest portion of annual operation span was less than 10 days as 70%, and for the combine, that was over 20 days as 36% and the second was 16~20 days as 23%. The average difference in repairing cost for combine between the farmers with and without common storage housing was 100,000won. 3. The percent of storaging farm machinery in the open field for farmers without housing were 44% for power tiller, 16% for farm tractor, 21% rice transplanter, 10% for combine. Each farmer has individual temporary simple housing with various kinds and structure. 4. Most of the farmers, 63.7%, want to use the farm machinery storage housing for multipurpose utilization, 29% only for storaging purpose. Most farmers prefer near the farm house as suitable position of housing, and prefer the area of 132~198㎡ for storage housing. The current problems on common storage housing were insufficient area(44.8%), no problem(29%), improper position, excessive construction cost, inconvenient entrance, etc.

      • 비중을 높인 경량기포콘크리트의 내동해성

        The effect of particle size of Al powder and water repellent agent on frost resistance, absorption ratio and structure of micropore was examined to improve the physical properties and durability of ALC. The compressive strength and the tensile strength are increased and independent of water repellent agent when the specific gravity is increased in the range from 0.4 to 0.7. According as the added amount of water repellent agent was increased, frost resistance was improved and absorption ratio was decreased. ALC produced from fine Al powder has a low volume reduction by scaling and a sound apperance after freezing/thawing test and top surface freezing test. The results from this research can be expected to be utilized for stabilization and growth of ALC industry.

      • PWM을 이용한 트랙터 부착작업기의 자세제어

        This study was conducted to control the posture of implement attached to tractor, which could make important effects on the working performance and accuracy. A tool bar, should be attached to the 3-point hitch, was designed to control the posture of implement by differentiating distances between one upper link and two lower links independently and simultaneously. By using PWM control method, the hydraulic fluid quantity was controlled proportionally according to the change of duty ratio with a high speed electronic solenoid valve. Computer simulation and fundamental test were also conducted for the developed posture control system using AMESim analysis tool The results of this study could be summarized as follows : 1. According to the results of computer simulation, major design parameters such as PWM control frequency, control agin, orifice diameter, and fluid pressure were determined as 30㎐, 1.5, 1.8㎜, and 10MPa, respectively. Time constant to make 100㎜ of stroke in load direction was about 0.55 seconds. 2. The tool bar, which was consisted of hydraulic cylinders and metal guides, was fabricated and tested for the performance of over all system. 1.6 seconds of response time of the control system was needed at inclination of 22°, and the performance of the posture control system was best at 150MPa of relief pressure, 1.5 of control gain, 20㎐ of PWM control frequency, and 2.2㎜ of orifice diameter, which did not represent considerable difference in compare with the results of computer simulation.

      • 가와사끼 질환에서 감마글로불린의 대량 정주 용법의 효과에 관한 연구

        가와사끼병으로 진단받고 고용량의 감마글로불린(2 gm/㎏)을 정맥주사 받은 17명의 환아를 대상으로 임상적 효과와 관상동맥의 변화에 미치는 영향을 연구하여 다음의 결과를 얻었다. 1. 대상 환아의 연령은 평균 2.03세로 3개월에서 5세 사이에 분포하였고, 2세 이하가 58.8%를 차지하고 남여의 비는 1.125 : 1이었다. 2. 가와사끼 진단 기준의 6가지 주증상 중 5가지는 전환아에서 나타났으며, 경부 림프선 종창만 64.7%에서 나타났다. 발생 계절은 봄과 겨울에 76.5%의 환아가 발생하였다. 3. 대량의 감마글로불린 주사 직후 평균 11.4시간에 고열이 없어지고 다른 주증상들도 없어지기 시작하였으며, 대부분(82.3%)이 주사후 20시간 이내에는 소실되었다. 주사중 3례(17.6%)에서 오한 및 일시적인 체온의 상승을 보였으나 주사를 중단할 필요는 없었다. 4. 발병 4주 이내에 관상동맥의 확장이 관찰된 경우는 8례(47.1%)였으나, 이들은 모두 경도의 확장이었다. 발병 2개월 후에 시행한 심초음파도에서는 2명(11.7%)에서만 경도의 확장을 보였고, 나머지는 모두 정상으로 회복되어 있었다. 5. 최저 헤모글로빈치, 최대 C-반응성 단백질, 최대 총백혈구수, 최고 적혈구 혈침속도, 혈소판 최고치는, 발병 4주 이내와 2개월 후에 관상 동맥의 변화가 있는 군과 정상인 군 사이에 의미 있는 차이는 없었다. 이상의 결과로 가와사끼병의 치료에 있어서 단일 고용량의 감마글로불린을 정맥주사하는 방법이 심각한 부작용 없이 임상적 증세를 빨리 호전시키며, 관상동맥의 합병증을 줄이는데 효과적임을 알 수 있었다. 따라서 모든 가와사끼병의 급성기에 가능한 빨리 단일 고용량의 감마글로불린을 정맥주사하는 것을 추천한다. The authors studied the effect of single high-dose intravenous gammaglobulin(2 gm/㎏) on the clinical symptoms and coronary artery abnormalities in seventeen Kawasaki patients and obtained the following results. 1. The patients' ages ranged from 3-month to 5-year, with mean age of 2.03-year. The patients under 2-years comprised 58.8%. The ratio of male to female was 1.125. 2. All of the patients showed 6 major diagnostic criteria with one exception of cervical lymphadenopathy(64.7%). The disease occurred mainly spring and winter season(76.5%). 3. Fever subsided within 20 hours' of intravenous gammaglobulin infusion in most of the patients(82.3%) with mean value of 11.4 hours, Transient side effects such as fever and chilling sense, developed in 3 cases(17.6%), but there was no need to quit infusion. 4. Echocardiographically, mild coronary arterial dilation was observed in 8 cases(47.1%) within 4 weeks' of onset. After 2 months' of onset, only 2 cases(11.7%) showed mild dilation. 5. There were no significant difference between coronary arterial dilation group and normal group before 4wk and after 2 mo of onset, in terms of hemoglobin nadir, maximal CRP, maximal WBC count, peak ESR, and peak platelet count. Conclusively, single high dose intravenous gammaglobulin infusion in Kawasaki disease showed effective improvement of clinical symptoms and decrease of coronary arterial abnormality. There fore all the children with Kawasaki disease should be treated with single high-dose intravenous gammaglobulin infusion as soon as possible.

      • 포토센서를 이용한 풋고추 선별시스템 개발

        The green pepper grading system was developed by recognizing length and the curved segment, Number of grading classification and factors were chosen based on the survey results of farmers. Results were summarized as following. 1. Ninety percent of surveyed farmers grade the green pepper into two classes manually according to the length, curved segment, and color. And about 10% of green peppers are being shipped without being graded, because of its highly labor intensive process. 2. Prototype having two rows of grading system (dimension : 1400×400×700mm) consists of four parts, i.e, pepper supplying device which sends one or two peppers with constant interval, separating device which carries one unit of pepper, grading device which recognizes a pepper according to the length and curved segment by the photo sensor, and discharging and control system which consists of sensors and operating pneumatic cylinder. 3. The bucket elevator type supplying system showed the best performance and it had a 30×60mm size plate mounted with 70˚angle. 4. The performance of the developed grading system showed the success rate of 97.4%. Various shapes of stalk were main error source in photo sensing. 5. The grading rate of the two-row prototype was 34kg/hr, about half of the manual operation. And its grading error was 8.3%, and it was about 20% less than the largest error of manual operation.

      • Systemic Lupus Erythematosus를 동반한 Multiple sclerosis

        Multiple sclerosis and lupus erythematosus are chronic, potentially disabling diseases of unknown cause. It is a rare occasion that the finding of both diseases occurs in one patient. Because both diseases may affect the central nervous system, it may be difficult at times to differentiate symptoms of lupus erythematosus from those of multiple sclerosis. As far as we know, there was no case report of SLE in multilple sclerosis in Korea. We report a case of multiple sclerosis accompanied by SLE.

      • 왕겨의 열분해 특성에 대한 TGA-FTIR 분석

        This study was conducted to investigate the pyrolysis characteristics of rice husk using TGA-FTIR instrument. Rice husk has been considered as a potential biomass energy materials, because of its high energy value. Rice husk was pyrolysed at different heating rates of 10, 20, 30, and 40℃/min in a thermogravimetric analyzer coupled to a Fourier transform infrared spectrometer. The thermal decomposition characteristics of rice husk were analyzed, and the gases that were volatilized during the duration of experiment were identified. The thermal decomposition of irce husk started at approximately 220℃, followed by a major loss of weight, where the main volatilization occurred, and the thermal decomposition was essentially completed by 440℃. The pyrolysis process was found to compose of four stages; moisture evaporation, hemicellulose decomposition, cellulose decomposition and lignin degradation. The peak temperature for hemicellulose (321℃ to 327℃) and cellulose degradation (357℃ to 380℃) increased with heating rates. FTIR analysis showed that the following gases were given off during the pyrolysis of rice husk; CO2, CO, CH4, NH3, COS, C2H4, and some acetic acid. The temperature range at which the production of these gases peaked was from 310℃ to 380℃
