        동종골 이식을 이용한 경골 근위부 절골술

        목적: 내반슬을 동반한 골관절염의 치료방법 중 개방형 경골 근위부 절골술에서 동종골이식의 유용성을 알아보고자 하였다. 대상 및 방법: 1999년 4월부터 2001년 1월까지 내반슬을 동반한 골관절염 환자중 1년 이상 추시가 가능하였던 총 8명(8례)을 대상으로 개방형 경골 근위부 절골술과 함께 동종골로 골이식한후 L-형태의 금속판으로 내고정을 실시하였다. 수술전후 체중부하 전후면 방사선 사진을 통해 내반슬 교정의 정도를 측정하였고 추시관찰중 내고정물 제거술시 골조직을 채취하여 표본을 image analyzer로 분석하여 골유합의 정도를 파악하였다. Lysholm's knee score와 Hopital for Special Surgery(HSS) knee score를 이용하여 환자이 기능적 상태를 평가하였다. 결과: 측정된 수술전 체중부하 전후면 방사선 사진 상 내반각의 평균값은 5.3±4.3˚이었고 수술후 체중부하 전후면 방사선사진상 교정각의 평균값은 16.1±4.1˚ 이었으며 외반각 평균값은 10.7±4.3˚이었다. 추시 관찰에서 시행한 체중부하 전후면 및 측면 방사선사진상 골유합은 모두 이루어 졌으며 교정 손실각온 평균 1.5±1.0˚이었으며 내고정물 제거시 시행한 골조직표본을 image analyzer(ProPlus® system)로 분석한 결과 검체는 모두 골조직으로 이루어져 있으며 이중 생골과 사골의 비율은 3.3:1로 생골의 비율이 전체의 76.7±8.0%로 나타내었다. 최종 추시시인 수술 15.3개월 후의 Lysholm's knee score와 HSS knee score의 평균 값은 각각 66.2±8.3에서 75.7±9.1로, 55.2±9.0에서 79.0±7.5로 증가하였다. 결론: 동종골을 이식한 개방형 경골 근위부 절골술에서 절골부위는 이식된 동종골에 의하여 골유합이 잘 이루어 졌고 만족할 만한 교정각을 얻을 수 있었으며 교정각 소실이 적었고 향상된 슬관절 기능을 나타내었다. Purpose: The purpose was trying to clarify utility of allogenous bone graft on open type high tibial osteotomy, among the method of treatment of osteoarthritis with genu varum deformity. Materials and Methods: Working with eight patients who have osteoarthritis with genu varum deformity, performed the open type high tibial osteotomy, and after transplanting allogenous bone graft, fixed using L-shaped plate. Before and after the operation, measured the degree of correction of genu varum through radiography of anterior and posterior of weigh load, understudied degree of union by testing bone histology during removal of implant, and evaluated functional factor as well as improvement of patients using Lysholm's knee score and HSS knee score. Results: According to radiography of anterior and posterior of weigh load, the average of varus angle was 5.3±3.2˚ before operation, the average of correction angle was 16.1±4.1˚ after operation and the average of valgus angle was 10.7±4.3˚. By radiography of anterior and posterior of weigh load and lateral taken during the continual observation, complete bone union was seen. It got result that bone histology of transplanted part was composed of osteologic tissue and ration of live bone and dead bone was 3.3:1, which live bone was 76.7±8% by image analyzer(Pro Plus® system) during the implant removal. The average of correction loss angle was 1.5±1.0˚ After 15. months, average period of observation, average of Lysholm's score and HSS knee score were increased from 66.2±8.3 to 75.7±9.1 and from 55.2±9.0 to 79.0±7.5 respectively. Conclusion: The method that using a transplantation of allogenous bone graft on open type high tibial osteotomy, can be a fine surgical technique because it gains a perfect union, has an inconsiderable loss of correction angle and precision of correction angle, and can be contributed to the improvement of functional knee joint.

      • 부유대역 용융응고법으로 제조된 YBa_2Cu_3O_x 산화물초전도체의 미세구조와 전기적 특성

        The YBa_2Cu_3O_x-20 wt% Y_2BaCuO_5 composition designed in YBCO system was synthesized by solid state reaction method. The optimum condition of hot zone temperature and growth rate in floating-zone melt were 1080 ℃ and 2.5 mm/hour, respectively. In samples growth as these condition, the low angel boundaries in grain direction and small amount of amorphous phase known as decreasing characteristics of superconducting materials between grain boundaries in microstructure are investigated by polarized light microscopy on the whole. The critical current density(Jc) at 77k of sample using powder fabricated with no CeO_2 and doped 5 wt%CeO_2 were 8,000 A/㎠(Jc : 561A) and 12,000 A/㎠(Jc: 826A) at 0 T by direct current transport method, respectively.

        7차 초등학교 과학 교과서의 삽화에 나타난 성역할 고정관념 분석

        이 연구는 제7차 교육과정기의 과학 교과서와 실험관찰 교과서에 제시된 삽화를 성역할 고정관념 측면에서 분석하고 6차 교육과정기의 과학 교과서와 비교하였다. 교재에 나타난 삽화에서 성별에 따른 등장인물의 빈도를 세고, 활동 유형을 분석하였다. 연구 결과는 다음과 같다. 1) 과학 교과서의 전체 삽화 수는 6차에 비해 7차에서 2배 정도 증가하였다. 2) 아동의 경우 학습활동에서는 성에 관계없이 등장 빈도가 대체적으로 균형을 이루었지만, 학습의 활동에서는 남학생의 등장 빈도가 여학생보다 더 높았다. 3) 행동 특성에서는 남녀 학생 모두 적극적으로 묘사되었고, 소극적으로 묘사되는 여학생의 비율은 6차 과학 교과서에 비해 감소하였다. 4) 성인의 경우에는 남성의 등장 빈도가 여성보다 높게 나타났다. 5) 가사 활동을 하는 모습에서는 여성의 등장 빈도가 40%로 높게 나타난 반면, 남성은 드물게 나타났다. The illustrations in elementary science textbooks and experimental workbooks based on the 7th national curriculum were analyzed for gender-role stereotyping, and compared with those of the 6th curriculum. The frequencies of men and women who appeared on text illustrations were counted and the types of roles assigned to males and females were investigated. The results were as follows. 1) The 7th national curriculum textbooks contained double the number of illustrations in the textbooks based on the 6th curriculum. 2) Boys and girls were illustrated in 'learning activities' with equal frequency, but boys were more frequently represented than girls in illustrations of 'other' activities. 3) Both boys and girls were generally depicted in 'active' processes and the ratio of 'passive girl' illustrations decreased as compared with the 6th national curriculum. 4) Male adults were more frequently represented than female adults in both textbooks and experimental workbooks. 5) 40% of female adults were illustrated doing housework, while male adults were rarely illustrated in such a role.

        소아의 하악 과두 골절의 비외과적 치료 : 증례보고 CASES REPORTS

        Of all facial fractures in children, condylar fractures have the greatest propensity to produce a growth disturbance. The pediatric condylar fracture occurs very frequently in the mandible, but this injury is occasionally ignored due to difficulty of diagnosis and no cooperation of patient. The adequate initial diagnosis and active treatment must be performed because delayed and improper treatment lead to possible severe complications such as facial asymmetry, TMJ pain, limiation of motion and deflection, midline deviation of the dental arch. So, the goal of managing condyle fractures in the growing patient is to restore mandibular symmetry, occlusion, function without interfering with future growth. Commonly, fracture teatment aims at restoring function through repositioning and rigid fixation of the bony fragments. However, the generally preferred management of condylar fractures in growing children is nonsurgical. So, we present the clinical and radiographic follow-up results of 4 condylar fractures in pediatric patients whom we managed in conservative and functional method. We can summarize our protocol in management of pediatric condylar fracture as the following: The IMF using acrylic resin splint with circumferential wiring was performed for 1 or 2 weeks. After releasing IMF, the active physical therapy was done with guiding elastic band. Opening deviation was controlled by mannual pressure of surgeon or parents for 2 weeks. When unstable occlusion or functional problem remained, functional appliance after construction bite registration was used for several months.

        The effects of formaldehyde in anaerobic digestion were : 1. In the cases of batch assay, the methanogenic bacteria were tolerated the toxic when raw and pretreated wastewaters were injected 3 and 5 ml respectively to serum bottle, but shocked when 5 and 10 ml respectively. 2. When raw, pretreated + glucose, and pretreated wastewaters were applied to UASB reators, the results of VLR were obtained 8-10, 3-5, and 2-4 kg cod/㎥ ·d respectived. Result of raw wastewater was the most optima of three. 3. The correlations VLR with bio-gas were obtained 90.5, 46.4 and 34.6% respectively in raw, pretreated + glucose, and pretreated wastewaters. Result of raw wastewater was the most optima of three. 4. In batch assay, pretreatment wastewater is better than raw water, but in continuous assay, raw wastewater is better than pretreatment wastewater. This reason is consideated that unsoluble salt of 373 mg/l is accumulated within UASB reactor, and carries out inhibition to anaerobic bacteria.

        cDNA array 방법을 이용한 망간에 노출된 흰쥐 뇌기저핵의 유전자발현 분석

        Objectives: This study investigated the gene expression profile in basal ganglia of manganese-exposed rats based on cDNA array analysis. Methods: For cDNA array, 25 male Sprague-Dawley rats (250±25 g) were intraperitoneally injected with 25 ㎎/㎏ B. W./day of MnCl2 (0.3 ㎖) for 10 days. For dose-related gene expression analysis, rats were intraperitoneally injected with 0.2, 1.0, and 5.0 ㎎/㎏ B. W/day of MnCl2 for 10 days. Control rats were injected with an equal volume of saline. RNA samples were extracted from brain tissue and reverse-transcribed in the presence of [α^(32)P]-dATP. Membrane sets of the Atlas Rat 1.2 array Ⅱ and Toxicology array 1.2 kit (Clontech, Palo Alto, CA) were hybridized with cDNA probe sets. Northern blot hybridization method was employed to assess the dose-related gene expression. Results: Fifty-two genes showed significant changes in expression of more than two-fold. Twenty-eight were up-regulated and 24 were down-regulated in the manganese-exposed group compared to the control. Among the 52 genes, 28 genes including nuclear factor I-X1 (NF1-X1), neuroligin 2 and 3, mitochondrial stress-70 protein (MTHSP70), neurodegeneration-associated protein 1 (Neurodap 1), multidrug resistance protein (MDR), and endoplasmic reticulum stress protein 72 (ERP72), were reported for the first time related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia. According to the dose-related gene expression analyses, MTHSP70, Neurodap 1 and ERP72 genes were up-regulated compared to the control even in the group exposed to low manganese dose ( 0.2 ㎎/㎏ B.W./day). Conclusions: Twenty-eight genes detected for the first time in this study were closely related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia and further study of these genes can give some more useful information about the manganese metabolism.

      • Antioxidant and Anti-Inflammatory Effects of Rhei Rhizoma and Coptidis Rhizoma Mixture on Reflux Esophagitis in Rats

        <P>The purpose of this study was to investigate the antioxidant and anti-inflammatory effects of the combined extract of Rhei rhizoma and Coptidis rhizoma (RC-mix) in experimental model of acute reflux esophagitis. The antioxidant activity was assessed by<I> in vitro</I> 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2′-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) assays. RC-mix was given at 100, 200, and 400 mg/kg body weight 2 h prior to induction of reflux esophagitis (RE). After 5 h, the effects of RC-mix treated rats were compared with those of normal and control rats. The representative flavonoid contents of RC-mix, such as sennoside A, epiberberine, coptisine, palmatine, and berberine, were detected using HPLC. The elevated esophageal mucosa damage was markedly ameliorated by RC-mix treatment in a dose-dependent manner. Furthermore, the administration of RC-mix reduced the increase of serum reactive oxygen species (ROS) and peroxynitrite (ONOO<SUP>−</SUP>). The improvement of superoxide dismutase (SOD) and heme oxygenase-1 (HO-1) levels were marked in the group given RC-mix. Moreover, the elevation of inflammatory mediators and cytokines by nuclear factor-kappa B (NF-<I>κ</I>B) activation in control rats decreased by RC-mix pretreatment. These results indicate that RC-mix treatment reduces the pathological states of esophagitis<I> via</I> regulating NF-<I>κ</I>B mediated inflammation related to oxidative stress. </P>
