        치위생학에서의 노인연구동향

        The aim of this study was to identify bibliographic characteristics and research trends of articles published on the elderly people in dental hygiene. An analysis was done of 191 articles published from 1999 to 2017. Bibliographic characteristics were nine items such as publication journal, research design, research participants, number of researchers, data analysis method, keywords, research topic, and validity and reliability report. The collected data were analyzed using descriptive statistics of statistical program. First, the research on the elderly, which was published in dental hygiene, started to be published in 1999 and on going. Though major publications have been published in Journal of Korean Society of Dental Hygiene and Journal of Dental Hygiene Science, the kinds of journals are becoming diverse. Second, cross-sectional research was the most. Third, the elderly were the most studied subjects, but the criteria varied. Fourth, the number of authors was largest in order of two and three person. Fifth, data analysis method was in order of descriptive statistics, t-test, and ANOVA. Sixth, there were many duplication of keywords about oral, elderly, and health. Seventh, oral health behavior science was the most studied subject. Eighth, validity and reliability were reported at 27.4% and 51.6%, respectively. Ninth, only 15.7% of study was founded research funding. Based on the results of this study, we consider increase experimental research further. Therefore, it is recommeded that validity and reliability should be reported in research paper, and the keywords used in the field of dental hygiene should be agreed upon. 본 연구는 치위생학 분야에서 노인 관련 연구에 대한 동향을 파악함으로써 연구의 질적 향상과 함께 향후 노인 관련 연구에 대한 방향을 제시하기 위하여 치위생학 분야에서 노인과 관련된 주제로 발표된 학술논문을 검색하고 분석함으로써 다음과 같은 결론을 얻었다. 1999년부터 2017년까지 총 191편의 노인 관련 논문이 치위생학 분야에서 발표되었다. 대부분의 연구가 한국치위생학회지와 치위생과학회지에서 발표되었으며, 횡단연구가 144편으로 가장 많았으며 실험연구는 15편에 그쳤다. 연구대상자는 노인이 152편으로 가장 많았고 돌봄제공자에 대한 연구는 14편이었다. 연구자수는 2인이 65편으로 가장 많았고, 가장 많이 사용된 분석방법은 기술통계, t 검정, 분산분석, 회귀분석 순이었다. 가장 많이 나온 키워드는 구강과 노인, 건강 등이었으며 구강건강행동과학에 대한 주제가 87편, 임상치위생에 대한 주제가 43편이었다. 타당도를 보고한 연구는 42편, 신뢰도를 보고한 연구는 79편이었으며 연구비 수혜를 받은 연구는 30편으로 나타났다. 본 연구의 조사결과에 의하면 실험연구의 비율이 적은 것으로 나타났으며 타당도와 신뢰도를 보고한 연구논문의 편수가 매우 미비한 것으로 나타났다. 또한 같은 의미의 키워드가 매우 다양한 단어로 기재되어 있는 것으로 나타났다. 따라서 실험연구의 수행을 높이기 위한 노력이 필요하며 타당도와 신뢰도가 검증된 도구의 사용과 측정 후 논문에서 이에 대한 보고가 이루어져야 할 것으로 생각된다.

        키워드 네트워크 분석을 활용한 치위생과학회지 연구동향 분석

        This research team extracted keywords from 953 papers published in the Journal of Dental Hygiene Science from 2001 to 2018 for keyword and centrality analyses using the Keyword Network Analysis method. Data were analyzed using Excel 2016 and NetMiner Version 4.4.1. By conducting a deeper analysis between keywords by overall keyword and time frame, we arrived at the following conclusions. For the 17 years considered for this study, the most frequently used words in a dental science paper were “Health,” “Oral,” “Hygiene,” and “Hygienist.” The words that form the center by connecting major words in the Journal of Dental Hygiene through the upper-degree centrality words were “Health,” “Dental,” “Oral,” “Hygiene,” and “Hygienist.” The upper betweenness centrality words were “Dental,” “Health,” “Oral,” “Hygiene,” and “Student.” Analysis results of the degree centrality words per period revealed “Health” (0.227), “Dental” (0.136), and “Hygiene” (0.136) for period 1; “Health” (0.242), “Dental” (0.177), and “Hygiene” (0.113) for period 2; “Health” (0.200), “Dental” (0.176), and “Oral” (0.082) for period 3; and “Dental” (0.235), “Health” (0.206), and “Oral” (0.147) for period 4. Analysis results of the betweenness centrality words per period revealed “Oral” (0.281) and “Health” (0.199) for period 1; “Dental” (0.205) and “Health” (0.169) for period 2, with the weight then dispersing to “Hygiene” (0.112), “Hygienist” (0.054), and “Oral” (0.053); “Health” (0.258) and “Dental” (0.246) for period 3; and “Oral” (0.364), “Health” (0.353), and “Dental” (0.333) for period 4. Based on the above results, we hope that further studies will be conducted in the future with diverse study subjects. 본 연구는 키워드 네트워크 분석 방법을 활용하여 치위생과학회지 2001년부터 2018년 3호까지 게재된 논문 953편의 키워드를 추출하여 연구 구조를 파악하기 위해 키워드분석과 핵심어를 파악하기 위해 중심성 분석을 실시하였다. 그리고 5년 단위로 1기를 2001년부터 2005년까지, 2기를2006년부터 2010년까지, 3기를 2011년부터 2015년까지, 4 기를 2016년부터 2018년까지로 구분하여 연구동향을 분석하여 다음과 같은 결론을 얻었다. 치위생과학회지에서 도출된 총 1,454개의 단어 중에서 17년간 치위생과학회지 투고논문에서 가장 많이 쓰인 단어는 ‘Health’이며 ‘Dental’, ‘Oral’, ‘Hygiene’, ‘Hygienist’에 관한 논의가 가장 활발하였음을 알 수 있었다. 중심성 분석을 통해 치위생과학회지에서 핵심이 되는 단어들과 연결되며 중심을 형성하고 있는단어들은 ‘Health’, ‘Dental’, ‘Oral’, ‘Hygiene’, ‘Hygienist’, ‘Behavior’ 등이며, 매개중심성 상위 단어들은‘Dental’, ‘Health’, ‘Oral’, ‘Hygiene’, ‘Student’ 등으로 나타났다. 시기별 연결중심성 핵심 키워드를 살펴본 결과, 1기에서는 Health (0.227), Dental (0.136), Hygiene (0.136) 등, 2기에서는 Health (0.242), Dental (0.177), Hygiene (0.113) 등, 3기에서는 Health (0.200), Dental (0.176), Oral (0.082) 등, 4기에서는 Dental (0.235), Health (0.206), Oral (0.147) 등의 단어들이 나타났다. 각 시기별로 매개중심성은 1기에서는 Oral (0.281)과 Health (0.199)의 매개중심성이 높게 나타났으며, 2기에서는 Dental (0.205)과 Health (0.169)가 높게 나와 매개역할의 비중이 높다가 Hygiene (0.112), Hygienist (0.054), Oral (0.053) 등으로 매개역할의 비중이 분산되는 것을 알 수 있었다. 3기에서는 Health (0.258)와 Dental (0.246)의 매개중심성이 높게 나타났으며, 4기에서는 Oral (0.364)과 Health (0.353), Dental (0.333)의 매개중심성이 높게 나타났다. 이상의 결과를 바탕으로 향후 치위생학 연구에 있어 학문 주제에 대한 다양성과 다각화를 모색하여 많은 연구가 이루어지길 기대한다.

        국내 치위생(학)과 임상치위생학 교육과정 운영현황 분석

        The purpose of this study was to provide basic data to standardize the clinical dental hygiene curriculum, based on analysis of current clinical dental hygiene curricula in Korea. We emailed questionnaires to 12 schools to investigate clinical dental hygiene curricula, from February to March, 2017. We analyzed the clinical dental hygiene curricula in 5 schools with a 3-year program and in 7 schools with a 4-year program. The questionnaire comprised nine items on topics relating to clinical dental hygiene, and four items relating to the dental hygiene process and oral prophylaxis. The questionnaire included details regarding the subject name, the grade/semester/credit system, course content and class hours, the number of senior professors, and the number of patients available for dental hygiene clinical training purposes. In total, there were 96 topics listed in the curricula relating to clinical dental hygiene training, and topics varied between the schools. There was an average of 20.4 topic credits, and more credits and hours were allocated to the 4-year program than to the 3-year program. On average, the ratio of students to professors was 21.4:1. Course content included infection control, concepts for dental hygiene processes, dental hygiene assessment, intervention and evaluation, case studies, and periodontal instrumentation. An average of 2 hours per patient was spent on dental hygiene practice, with an average of 1.9 visits. On average, student clinical training involved 19 patients and 26.6 patients in the 3-year and 4-year programs, respectively. The average participation time per student per topic was 38.0 hours and 53.1 hours, in the 3-year and 4-year programs, respectively. Standardizing the clinical dental hygiene curricula in Korea will require consensus guidelines on topics, the number of classes required to achieve core competencies as a dental hygienist, and theory and practice time. 국내 치위생(학)과를 대상으로 하여 임상치위생학 교육과정 운영 현황을 조사하고, 심층 분석한 결과 첫째, 임상치위생학 영역의 교과목 명칭은 전체 96개 과목으로 확인되었으며, 학교별로 매우 다양하게 나타났다. 그러나 전체 교과목 중 60.5%가 임상치위생학 또는 임상치위생학 및 실습이라는 명칭으로 사용하고 있었다. 둘째, 임상치위생학 교과목 운영은 졸업시점까지 평균 21.1학점을 이수하고, 15주차수업으로 평균 471.3시간 수업을 운영하는 것으로 나타났다. 또한 교수 1인당 학생 수는 21.4명을 담당하였으며, 3년제에 비해 4년제에서 더 많은 학점과 시간을 배정하여 운영하고 있는 것으로 조사되었다. 셋째, 임상치위생학의 교육주제와 내용에서는 감염관리, 치위생관리를 위한 기초 개념의 이해, 치위생사정, 중재 및 평가, 증례연구, 치주기구조작법 등으로 나타났다. 그러나 치위생관리과정의 개념, 치위생사정, 치아우식증, 치주질환 위험도 평가, 특별환자 관리, 전신질환자의 구강건강관리 등에 대한 수업운영내용은 3, 4년제 학제 간 차이가 컸으며, 사정과 수행 중심의 교육내용으로 운영되며, 학제 내에서도 교육운영내용의 차이가 있는것으로 조사되었다. 넷째, 대상자 실습은 대상자 1인당 평균2시간 소요되었고, 평균 1.9회 내원하도록 하여 실습을 진행하고 있는 것으로 나타났다. 졸업시점까지 필수적으로 진행하는 대상자 실습은 3년제의 경우 평균 총 19개의 케이스를, 4년제의 경우 평균 26.6개 케이스를 실습하고 있었고, 가장적게는 15개의, 많게는 35개 케이스를 실습하였고, 학생 1 인당 대상자 실습에 참여하는 시간은 3년제의 경우 평균38.0시간, 4년제에서는 평균 53.1시간으로 나타났다. 본 연구는 일부 치위생(학)과를 대상으로 조사한 연구라 한계점은 있지만 임상치위생학 교육과정의 전체적인 흐름과 운영방식을 파악할 수 있었다. 학제를 떠나 임상치위생학의 교육과정 표준화를 위해서는 교과목 명칭, 졸업생 수준에서치과위생사로서 달성해야 할 역량에 기반한 최소한의 수업운영내용, 이론과 실습시간, 학생수 대비 지도 교수자의 수등에 대한 치위생계 내 합의 또는 가이드라인이 반드시 필요할 것으로 판단된다. 이러한 과정은 공적인 기관을 통해서 제도적 마련에 근거하여 변화할 수 있도록 노력해야 함을 확인하였다.

        강릉시 자활근로사업 참여자 대상 치위생 과정 사례보고

        This study aimed to provide basic data for establishing the clinical basis for dental hygienist-led dental hygiene process of care by identifying multiple risk factors for self-support program participants in Gangneung city; we also compared oral health status and behavioral changes through customized oral health care. Four dental hygienists who were evaluated for degree of conformity provided dental hygiene process of care to eight self-support program participants who were selected as having an oral health risk among people in the self-support center. The clinical indicators measured during dental hygiene assessment and evaluation and behavioral changes due to dental hygiene intervention were compared and analyzed. With respect to clinical indicators, at the time of probe, the retention rate of patients with gingival bleeding decreased from 61.4% to 14.7% after intervention (p=0.004). Furthermore, the retention rate of patients with a periodontal pocket >4 mm decreased from 15.6% to 5.8% (p=0.001). The average modified O’Leary index of the patients improved from 23 to 40 (p=0.002). Previously, all eight subjects used the vertical or horizontal method of brushing; after dental hygiene care interventions regarding method and frequency of toothbrushing, use of oral care products, and individual interventions, they started using the rolling or Bass method of toothbrushing. Four of eight subjects reported using interdental toothbrushes after intervention. As a result of applying the change model to the transtheoretical behavior change of the subject, the result of strengthening the health behavior was confirmed. For promotion of oral health by the prevention-centered incremental oral health care system, dental hygienist‐led dental hygiene management and maintenance is essential. It is thought that continuous research, such as for feasibility evaluation, cost benefit analysis, and preparation of legal systems, is needed to establish and activate dental hygiene management. 본 연구는 강릉시에 소재한 자활센터 자활사업 참여자를대상으로 하여 대상자의 복합적인 위험요인을 고려한 맞춤형 치위생 과정을 통해 구강건강상태 및 행태의 변화 과정및 결과의 사례를 분석함으로써, 치과위생사 주도의 치위생과정에 대한 임상적 근거를 마련하고 임상 현장 및 지역사회사업에서의 치위생 과정 적용 가능성을 검토하고자 수행하였다. 본 연구에서는 전체 모집단 80명 중 연구참여에 동의한 44명을 대상으로 치과위생사 면허를 소지한 교수 3인이 전신질환 여부(당뇨병, 심혈관계질환 등), 치은 상태 및치석 여부, 구강위생상태 등의 기준으로 구강 내 사정을 실시하였고, 구강건강위험군으로 선발된 21명 중 개인적인 사정과 치과의사 치료 의뢰 등으로 인해 중도 탈락한 13명을제외하고 치위생계획에 따라 모든 과정을 종료한 8명을 최종 연구대상자로 선정하였다. 본 연구에서는 수행자 간 일치도 평가를 거친 치과위생사 4인이 자활센터 자활사업 참여자 8인을 대상으로 치위생 과정을 수행하였다. 대상자별전담 치과위생사를 선정하여 대상자 중심의 맞춤형 관리를제공하였고 모든 치위생 과정 내용은 치위생관리 기록부에기록하여 치위생 과정 전·후에 따라 회차별로 평가한 임상지표 및 치위생 중재에 따른 행동 변화 등을 비교 분석하였다. 복합적인 건강위험요인을 동반한 취약계층인 자활사업참여 대상자의 치위생 과정을 분석한 결과, 인간욕구이론에기반하여 대상자 중심의 목표를 수립하고, 전담 치과위생사와의 지속적인 신뢰관계 형성에 기반하여 행동 변화를 위한중재를 실시하며, 회차별 대상자의 구강건강상태에 따른 맞춤형 전문가구강위생관리를 실시함에 따라 긍정적인 행동변화와 함께 유의미한 임상적 변화를 확인하였다. 범이론모형에 근거한 행동 변화 중재에 따라 칫솔질 방법 및 빈도의 변화가 나타났고, 치간칫솔을 사용한 치간관리의 변화를확인하였다. 개인별 치위생 과정에 따른 구강상태를 비교한결과 치위생 과정을 수행한 후 탐침 시 치은출혈 치아 보유율과 4 mm 이상 치주낭 보유율은 감소하였고, 변형O’Leary index는 향상되었다. 치위생 과정은 치과의료에서환자/대상자 중심의 가치를 실현하고 질병의 예방과 구강건강증진을 지향함에 따라 임상 현장 및 지역사회에서 지속적으로 치위생 과정이 적용될 수 있는 시스템을 구축함으로써구강건강증진을 통한 구강건강 관련 삶의 질 향상과 더불어구강건강의 사회경제적 불평등을 줄이는 데 기여할 수 있을것으로 생각된다.

        치위생학과와 치의학과의 교육과정 비교

        Introduction: All work of dental hygienists in Korea is to be carried out under the supervision of a dentist, but the main tasks of dental hygienists are systematic and sufficiently trained within the college curriculum. The purpose of this study was to compare the curriculum between the Department of Dental Hygiene and the Department of Dentistry to provide a basis for legal revision of the dental hygienist's unique work. Methods: The curriculum was acquired from the homepage of the Department of Dental Hygiene and Dentistry. The curriculum in 23 of 28 dental hygiene schools and 6 of 11 dental schools was identified on the homepage. The swere classified based on the scope of dental hygienist work. Results: The Department of Dentistry has more than twice the number of credits and subjects compared to the Department of Dental Hygiene, and the top subjects belong to the treatment and rehabilitation of dental diseases. However, in the field of preventive dentistry, which is a unique work of dental hygienists, the Department of Dental Hygiene has a score of 9.3 times higher than that of dentistry. In the public oral health field, the oral health education field, and the dental management field, dental hygienists scored twice as high as dentists. Conclusion: Since dental hygienists are receiving more education than dentists in the areas of preventive dentistry, public oral health, oral health education, and dental management, which are unique tasks, it is suggested that the work can be performed independently without supervision of the dentist.

        일부 치위생학과 졸업예정자의 치과위생사 직무에 대한 인식

        Introduction: The legal duties of Korean dental hygienists are different from the actual tasks. These factors cause dental hygienists to experience work stress. Therefore, it is necessary to investigate what tasks the dental hygienist expects to perform. Methods: This study examined the perception of dental hygienists' work on 158 prospective graduates of the 3rd-year and 4th-year course of dental hygiene. The questionnaire about the dental hygienist's duties was prepared based on the job description of dental hygienists by the Korea Health Personnel Licensing Examination Institute. Results: More than 84% said that all of the work in the second job description was possible as dental hygienists. subjects responded that the following items were not dental hygienists' duties: treatment plan based on medical history and dental history, treatment plan based on an intraoral and extraoral examination, treatment plan based on the results of teeth and periodontal examination, analysis of oral health data in community, planning of oral health promotion in community, planing of water fluoridation, reading of radiography, root planing, physical treatment on head and neck, using an ultrasonic device on head and neck, dealing with laser devices, making a temporary crown, suture and stitch-out, and intramuscular injection. Conclusions: Most graduates of dental hygiene departments have recognized that the tasks described in the second job description are dental hygienists' work; therefore, it is necessary to improve the gap between expected work and legal work.

        한국 4년제 치위생학과의 교육과정

        Education should consider the social consensus that changes according to the times and the social environment, and it is also necessary to consider the technically useful subjects. We collected and reported the 4-year course curriculum of dental hygiene when Korean dental hygiene education has continued over 50 years. Each 4-year course curriculum was collected by searching each university website, or requested by email. The curriculum of 23 among 27 schools was collected. According to the classification of dental hygiene in the dental hygienist national examination classification or the Korean society of dental hygiene science, the subjects of the course were divided into clinical dental hygiene, clinical dental treatment support, basic dental hygiene, social and educational dental hygiene, and collected 23 curriculum courses. The average major curriculum was 104.9 credits and the average number of subjects in major courses was 34.3. The average subjects' number of clinical dental hygienic courses was 33.9 (37.5% of the total major credits), clinical dental treatment support was 30.6 (30.2% of the total major credits), basic dental hygiene was 21.8 (20.8% of the total major credits), and social and educational dental hygiene courses was 13.6 (13.0% of total major credits). Integrated subjects' name in clinical dental hygiene was used in all schools of the survey, such as clinical dental hygiene education (and practice), comprehensive dental hygiene education, and integrated dental hygiene. There were 13 schools (56.5%) that use the integrated name in clinical dental treatment support, such as clinical dentistry. There were 14 schools (60.9%) to open dental clinic management, and 22 schools (95.7%) to open national health insurance claims. The basic dental hygiene curriculum maintained the title of each subject and social and educational dental hygiene education was established in most schools, such as community dental health, oral health statistics, and oral health education. Other subjects were English conversation in dental clinic (8 schools) education, clinical dental treatment support, basic science in dental hygiene, social and educational dental hygiene. We knew the clinical dental hygiene and clinical dental treatment support were changing into the integrated subjects, and most schools run dental hygiene research.

        일부 치위생과 학생들의 임상실습 시 교과목 효용에 관한 연구

        Background: The present study aimed to analyze which curriculum is the most relevant to dental hygiene students when they participate in clinical practice in order to provide a useful reference for preparing educational guidance in this field. Method: The survey utilized in the present study consisted of six questions about general characteristics, such as grade, satisfaction with major, amount of clinical practice, period of clinical practice, place of clinical practice, and the most interesting are during clinical practice. When evaluating curriculum relevancy, the following were ranked on a 5-point Likert scale, where 5 = very useful, 4 = comparatively useful, 3 = normal, 2 = comparatively unuseful, 1 = very unuseful: difference in requirements in the field of clinical practice, reason for this difference, and question about the utility of each curriculum. On this scale, higher points implied higher relevance. Result: The highest groups of curricula regarding curriculum utility were as follows: operative dentistry (59.6%), pre-clinical practice (55.2%), dental materials and clinical practice (54.4%), and prosthetic dentistry (49.6%). The lowest groups of curricula regarding curriculum utility were as follows: oral physiology (2.0%), oral histology and embryology (1.6%), and oral microbiology (1.2%). These results imply a lack of connection between the curriculum and tasks in clinical practice. Conclusion: Based on the results of the present study, it appears that both theory and practice courses of the clinical curriculum must be conducted systematically, and that there is a need to conduct education for the fundamental curricula, such as oral physiology, oral histology and embryology, and oral microbiology, regarding the relevance of tasks practiced in clinics.

        치위생과 학생의 결핵에 대한 지식 및 태도가 결핵감염 예방행위에 미치는 영향

        Background: The present study aimed to gather basic data for the development of an educational tuberculosis prevention program for prospective dental hygienists by understanding the level of knowledge, attitudes, and preventive behaviors related to tuberculosis for students in the Department of Dental Hygiene, who need preventive access to tuberculosis infection. Methods: The present study used a self-reported questionnaire on knowledge, attitudes, and preventive behaviors regarding tuberculosis, which was administered to dental hygiene students attending D University located in D city. Results: The average knowledge regarding tuberculosis was 16.17 ± 6.14, out of 30 points. Attitude regarding tuberculosis averaged 48.59 points (± 4.46), out of 60 points, and the degree of preventive behavior was 46.29 points (± 5.0), out of 60 points. Third year students ranked highest for knowledge regarding, attitude toward, and infection prevention behaviors of tuberculosis, compared to those in their first or second year (F = 7.20, p = 0.000). Those who had experienced tuberculosis themselves or with their relatives had higher attitudes toward tuberculosis than those who did not (F = 2.32, p = 0.02). Additionally, the higher the knowledge (β = 0.209, p = 0.004) and attitude (β = 0.425, p = 0.000) about tuberculosis, the higher the level of tuberculosis infection prevention behavior. Conclusion: Based on the results of the present study, it appears to be necessary to provide practical education to ensure that all students in dental hygiene are equipped with knowledge about tuberculosis, and that as a dental hygienist in carrying out dental hygiene management with a changed attitude, the act of preventing tuberculosis infection can be appropriately performed.

        일부 치위생학과 학생들의 MBTI 성격유형에 따른 포괄치위생관리과정 성취도 분석

        Objectives: The purpose of the study is to investigate the academic achievement in comprehensive dental hygiene courses using MBTI personality type. This study will provide the various pedagogical approaches in the dental hygiene education. Methods: A self-reported questionnaire was completed by 58 dental hygiene students in Chungnam from December, 2012 to March, 2014. The questionnaire consisted of academic achievement of comprehensive dental hygiene course and communication skills, After filling out the questionnaire, the students completed MBTI personality type sheet. Results: The students were categorized as extroversion type (58.6%), sensing type (70.7%), feeling type (56.9%), and perceiving type (67.2%). In the academic achievement, extroversion and judging personality type students had higher self-efficacy than the students of introversion and perceiving types. The extroversion personality type students also had the higher assignment level and confidence than the introversion type. Conclusions: In order to enhance the understanding and learning capacity of the students, dental hygiene professors should understand the differences in achievement levels due to different personality types so that they can utilize better pedagogical approaches.
