N/A Background/Aims: Currently, the endoscopic biopsy technique is the most accurate diagnostic modality detecting the ga'tric carcinoma. But, it is thought importantly to evaluate the significance of touch cytology in diagnosing the gastric lesion because the representative area of gastric lesion is possible to be removed during the process making the specimen. Methods. After l,333 cytologic specimens from 1,235 patients were made by touch of the biopsied tissue on the slide glass and were stained by Papanecolaous method, the tissue specimens were obtained by the usual specimen making technique. The results of the cytologic diagnosis were compared to the biopsy results. Results: The accuracy of the cytologic diagnosis was 99.0%(1,108/1,119 cases) in the gastritis or benign ulcer, 20.4%(11/54 cases) in the atypical epithelial lesions, and 75.0%(120/160 cases) in the malignancy. The overall accuracy of the cytologic diagnosis was 92.9%(1,239/1,333 cases). False positivity of the cytologic diagnosis was 0.4%(5/1,177 cases) and false negativity was 16.9%(27/ 160 cases). All of the 4 cases, which were diagnosed class IV or V in the cytologic diagnosis and gastritis or benign ulcer in the biopsy diagnosis in the first study, were found to be benign lesions in the follow up cytologic and biopsy studies. Conclusion: This study indicates that touch cytologic examinations supplementary to biopsies are not so significant enough to performed spending many efforts since biopsied specimen represent the majority of gastric lesion, if they were sampled at correct site of lesion at all. 전신마취시 분시호흡량과 동맥혈 및 호기말 탄산가스 분압의 차이에 관한 연구

The purpose of this study is to evaluate the relationship between arterial and end-tidal CO_2 tension during general anesthesia. Sixty one patients age 21∼40 years old without history of cardiopulmonary disease in American Society of Anesthesiologists physical status 1, were studied. Thoracic or upper abdominal operations were excluded in this study. Anesthesia was maintained by halothane, nitrous oxide and oxygen. Respirations were controlled in all patients with a Dameca ventilator and minute volumes were adjusted to provide F_ECO_2 levels considered appropriate by anesthetist. The concentration of end-tidal carbon dioxide was continuously monitored with ENGS TROM ELIZA DUO CO2 and O2 analyzer. The radial artery was punctured 30 minutes after the surgical incision and both arterial and endtidal carbon dioxide tension were measured simultaneously. The results were as follows : 1) The mean arterial to end-tidal CD_2. tension difference was 2.5±3.9 torr. 2) There was a constant correlation between arterial and d-tidal CO_2 tension regardless of the degree or minute volume. 3) There was no significant correlation between smokers and non-smokers in male patients.
간성 뇌중 환자에서 Flumazenil 의 효과 및 안정성에 관한 전향성 연구

N/A Objectivea: Ther are still many controversies on the pathogenesis of the hepatic encephalopathy. Recently, it has been postulated that endogenous benzodiazepine like substance (s) may be involved in the neuroinhibitory effect of gamma-aminobutyric acid (GABA). For this reason, benzodiazepine antagonist, flumazenil, has been advocated as a new treatment for hepatic encephalopathy. Several studies reported clinical improvement of hepatic encephalopathy in patients treated with flumazenil but these were not controlled studies. Methods: To investigate the effect of flumazenil, we performed a double-blind, prospective study in the 13 patients with hepatic encephalopathy who fulfilled the inclusion criteria of the protocol. The drug was administered intravenously (flumazenil 2.2 mg bolus injection followed by a continuous infusion with a rate of 1 mg/ h or placebo) in coded vial. After the code was open, 6 patients received flumazenil and the other 7 palcebo. The ages were 36 to 61(mean 54) years and 7 were male and 6 female. The causes of liver cirrhosis were HBV related in 7, HCV related in 3, and 3 were alcoholics. Results: In the fumazenil group, 4(67%) demonstrated immediate improvement of mental state with various degrees (from stage III to II in 1 and from stage I & II to 0 in 3). The significant and immediate improvement in the electroencephalographic (EEG) findings were noted in 3(50%), In the placebo group, however, 2(29%) showed an immediate improvement in EEG findings but only one (14%) showed the immediate improvement of mental status. There was no evidence of significant adverse reaction related to the treatment. Conclusion: This study shows that the benzodiazepine antagonist flumazenil may be effective and safe in patients with hepatic encephalopathy in the immediate mangement. Long term beneficial effect may be expected by treating the patients with more convinient oral administration.

Calcium plays a role as a signaling molecule in various cellular events. It has been reported that calcium suppresses adipocyte differentiation only in the early phase of adipogenesis. Herein, we demonstrate that treatment of A23187, a mobilizer of intracellular calcium, on day 4 post adipocyte differentiation could still reduce lipid accumulation in differentiating 3T3-L1 cells for 48 h. In addition, luciferase reporter gene and RT-Q-PCR assays demonstrate that A23187 can selectively inhibit transcriptional activities and expression of PPAR$\gamma$ and LXR$\alpha$, suggesting that A23187 may reduce lipid accumulation in the late phase of adipogenesis via downregulation of PPAR$\gamma$ and LXR$\alpha$ expression and transactivation. Moreover, inhibition of HDAC activity by trichostatin A (TSA) partially blocked A23187-mediated downregulation of transcriptional activities of PPAR$\gamma$ and LXR$\alpha$. Together, our data demonstrate that calcium mobilization inhibits expression and transcriptional activities of PPAR$\gamma$ and LXR$\alpha$, resulting in reduced lipid accumulation in differentiating adipocytes, and thus, mobilization of intracellular calcium in adipocytes may serve as a new preventive and therapeutic approach for obesity.
Resistance of Kanamycin- and Neomycin-Producing Streptomycetes to Aminoglycoside Antibiotics

Streptmyces fradiae NRRL B1195 and Streptomyces kanamyceticus IFO 13414 are highly resistant to the antibiotics they produce. The ribosomes of these organisms are found to be susceptible to the antibiotics, but the cell free extract of S fradiae is found to contain a phosphotransferase and an acetyltransferase which inactivate kanamycin and neomycin, and that of S. kanamyceticus an acetyltransferse which inactivates kanamycin and neomycin. The resistance of these organisms against streptomycin is found to be due to the resistant ribosomes; actually streptomycin activates their ribosomal systems for the synthesis of polyphenylalanine.
첨가물 함량 및 종류에 따른 PAN/DMF 용액의 전기방사에 의한 나노섬유 제조 및 특성

각종 전자통신기기의 소형화, 경량화 및 고성능화 추세에 따라 이들 전자기기의 전원으로 2차전지 및 초고용량 슈퍼캐패시터(Electric Double Layer Capacitor ; EDLC)의 수요가 요구되고 있다. 탄소섬유 전구체인 PAN(polyacrylonitile)/DMF(dimethyl formamide) 용액을 농도 10wt%, 유량 1ml/h, TCD(tip to collector distance) 15cm, 전압 12kV에서 전기방사하여 300㎚의 직경을 갖는 나노섬유를 제조할 수 있었다. 그리고 10wt% 농도의 혼합용액에 SWCNT, NnO₂, Carbon black을 1~20wt% 농도로 분산시켜 전기방사한 결과, PAN을 지지체로 첨가물질이 섬유의 표면에 붙어있는 것을 확인 할 수 있었고, 나노웹의 직경이 감소함을 확인하였다.
