        우리 나라 의학교육 평가의 새 지평

        Over the last two decades, there have been a number of significant changes in the evaluation system in medical education in Korea. One major improvement in this respect has been the listing of learning objectives at medical schools and the construction of a content outline for the Korean Medical Licensing Examination that can be used as a basis of evaluation. Item analysis has become a routine method for obtaining information that often provides valuable feedback concerning test items after the completion of a written test. The use of item response theory in analyzing test items has been spreading in medical schools as a way to evaluate performance tests and computerized adaptive testing. A series of recent studies have documented an upward trend in the adoption of the objective structured clinical examination (OSCE) and clinical practice examination (CPX) for measuring skill and attitude domains, in addition to tests of the knowledge domain. There has been an obvious increase in regional consortiums involving neighboring medical schools that share the planning and administration of the OSCE and CPX; this includes recruiting and training standardized patients. Such consortiums share common activities, such as case development and program evaluation. A short history and the pivotal roles of four organizations that have brought about significant changes in the examination system are discussed briefly.

        Porphyromonas endodontalis 에 대한 Polyphosphate의 항균기전에 관한 연구

        Poly-P has been used to prevent decomposition of foods and has been shown to have inhibitory effect on the growth of gram positive bacteria. The purpose of this study was to evaluate the effect of poly-P on the growth of Porphyromonas endodontalis, a gram negative obligate anaerobic rod, endodontopathic bacterium. P. endodontalis ATCC 35406 was in BHI broth containing hemin and vitamin K with or without poly- P. Inhibitory effect of each poly-P which was added at the beginning(lag phase) or during(exponential phase) the culture, MIC(minimum inhibitory concentration) was determined by measuring the optical density of the bacterial cell at 540nm. Viable cell counts were measured to determined whether poly-P has a bactericidal effect. Leakage of intracellular nucleotides from P. endodontalis was determined at 260nm and morphological change of P. endodontalis was observed under the TEM(transmission electron microscope) . Binding of 32P-labeled poly-P to P. endodontalis was examined, SDS-polyacrylamide gel electrophoresis and zymography were performed to observe the changes in protein and enzyme profiles of P. endodontalis, respectively. The results from this study were as follows : 1.The minimal inhibitory concentration(MIC) of poly-P to P. endodontalis appeared to be 0.04~0.05%. 2.Poly-P added to the P. endodontalis culture during the exponential phase of P. endodontalis was as much effective as poly-P added at the begining of the culture, suggesting that the antibacterial effect of poly-P is not much dependent on the initial inoculum size of P. endodontalis. 3.Poly-P are bactericidal to P. endodontalis, demonstrating the decrease of the viable cell counts. 4.Intracellular nucleotide release from the P. endodontalis, was not increased in the presence of poly-P and was not reversed by the addition of divalent cations like Ca2+ and Mg2+. 5.Under the TEM, it was observed that fine electro-dense materials were prominent in the poly-P grown P. endodontalis, appearing locally in the cell, and the materials were more abundant and more dispersed in the cell as the incubation time with poly-P increased. In addition, highly electron dense granules accumulated in many poly-P grown cells, most of which were atypical in their shape. 6.Binding of 32P-labeled poly-P to P. endodontalis appeared to be 32,8 and 45.5 and 53.4% at 30 minutes, 1 hours and 2 hours, respectively. 7.In the presence of poly-P, the synthesis of proteins with apparent molecular masses of 25, 27, 35, 45 was lost or drastically decreased whereas expression of a protein with an apparent molecular mass of 75 was elevated. 8.Proteolytic activity of P. endodontalis was decreased by poly-P. The overall results suggest that use of poly-P may affect the growth of P. endodontalis, and the anti- bacterial activity of poly-P seems largely bactericidal. Changes in shape, protein expression, and proteolytic activity of P. endodontalis by poly-P may be directly and indirectly attributed to the antibacterial effect of poly-P. Further studies will be needed to confirm the effect of poly-P.

      • Helicobacter pylori 편모 유전자의 클로닝 및 염기서열 분석

        A λgt11 expression libary of H. pylori DNA in E. coli Y1090 was screened with flagellin-specific rabbit antiserum for molecular cloning of the flagellin gene of H. pylori. A positive clone, λHPF4, was obtained and the recombinant antigen expressed from λHPF4 was a fusion protein with the molecular weight of 168kd. Sequence analysis of antigen-encoding DNA showed that an open reading frame composed of 1,536 nucleotides encodes a polypepride with a oredicted molecular size of 54kd. This open reading frame did not show the homology with flaA gene encoding 56kd protein of H. pylori and was confirmed as a unique sequence through homoligy searching. Therefore, the cloned antigen is supposed to be the carboxy-terminal region of the other flagellin protein of H. pylori, flaB, with the molecular weight of 58kd.

        Polyphosphate가 Porphyromonas endodontalis의 성장에 미치는 영향에 관한 연구

The results from this study were as follows : 1. Polyphosphates were shown the growth inhibition of the Porphyromonas endodontalis. 2. The minimal inhibitory concentration(MIC) of polyphosphate was observed to be 0.04%. 3. Polyphosphates with chain lengths of 25 and 75 demonstrated the greatest inhibitory effect on the growth of Porphyromonas endodontalis. 4. Polyphosphates are bactericidal to Porphyromonas endodontalis, demonstrating the growth inhibition of the bacterium. The overall results suggest that use of polyphosphate may affect the growth of Porphyromonas endodontalis. Further studies will be needed to confirm the effect of Polyphosphate.

      • 3차원 비디오 자동 동작분석 시스템 개발

        The purpose of this study was to develope both hardware and software of automatic 3-dimensional video motion analysis system. In this image capture system, the vedeo image was stored as a file in CD-Rom directly, the refore it was possible to analyze motion without VCR. Reflective markers were used to digitize automaticaly. C-language and MS-Visual Basic 5.0 ] anguage were utiliged in this software to analyze motion. The conclusions were as follows; 1. The vedeo information at sixty frames per second could be captured accurately as a image with a resolution photography of 640×480-24 bit. 2. It was possible to digitize both manually and automatically utilizing reflective marks attached the body. 3. The automatic 3-dimensional video motion analysis system, which was developed in this study not only increased accuracy but also reduced digitizing time so much.

      • 한글 워드프로세서의 개선 방안에 관한 연구

        컴퓨터를 이용한 문서의 작성과 관리가 많아짐에 따라 한글 문서의 호환에 대한 문제가 심각하게 대두되었다. 한글 워드프로세서에서 사용하는 한글 코드와 문서 형식의 차이로 인하여 타 워드프로세서의 문서는 사용하는데 많은 어려움이 따른다 본 논문에서는 이러한 한글 문서 호환의 문제점을 해결하는 방안으로 한글 워드프로세서에 대해서 기능과 문서 형식에 대해서 조사 분석하여 한글 문서화일을 관리하기 위해서 보유해야 하는 기능을 정의하고 한글 문서화일을 관리하기 위해서 보유해야 하는 기능을 정의하고 한글 문서화일의 문서 형식에 대해서도 연구하였다. 그리고 기존의 한글 워드 프로세서에서 코드 변환을 자동적으로 하고 문서 형식이 다른 문서화일에 대해서도 사용할 수 있도록 하여 사용자에게 효과적인 한글 문서화일 관리 환경을 제공하는 방안을 설계, 구현하였다.

      • NaBH₄존재 하에서 망간(Ⅲ)아세테이트를 촉매로 이용한 올레핀의 산소화반응

        산소를 산화제로 이용하여 올레핀을 알콜로 산화하는 방법에서 (schiff-base)망간(Ⅲ) 착물 대신에 Mn(OAc)₃·2H₂O 염이 촉매로 사용되었다. 이 반응에서 NaBH₄가 hydride 소스로 사용되었다. 이 반응에서 외부 리간드로 사용된 schiff-base가 매우 중요함을 알 수 있었다. (Schiff-base)Mn(Ⅲ) complex has been used as the catalyst for the olefin oxygenation under O₂/NaBH₄ combination system. In this study, Mn(OAc)₃·2H₂O salt was used as the catalyst. Proper choice of external Schiff-base ligand as critical to achieve the effective oxidation.
