        아세틸 콜린 유발 검사시 허혈성 심전도 변화와 흉통의 발생이 내피세포 기능장애에 미치는 영향

        연구배경: 아세틸 콜린 유발 검사시 발생하는 흉통과 허혈성 심전도 변화가 아세틸 콜린에 의해 발생하는 관상동맥의 내피세포 기능에 어떤 영향을 주는지 알아보고자 했다. 방법: 흉통으로 내원하여 관상동맥 조영술과 아세틸 콜린 유발검사를 시행받은 1,085명의 환자를 대상으로 하였다. 아세틸 콜린 유발 검사를 하는 동안 허혈성 심전도 변화 및 흉통의 발생 유무에 따른 관상동맥 내피세포 기능의 특징을 비교하였다. 결과: 539명의 환자에서 아세틸 콜린 검사시 내피세포 기능 장애가 발생 하였다. 허혈성 심전도 변화가 있었던 그룹이 심전도 변화가 없었던 그룹보다 흉통이 더 많았고(78.1% vs. 60.8%, P=0.007), 관상동맥 조영술상 다발성 관상동맥 경련이 빈번하였고(59.4% vs. 40.6%, P=0.004), 관상동맥의 경련 정도도 더 심하였다(64.1% vs. 46.5%, P=0.006). 흉통이 있었던 그룹이 흉통이 없었던 그룹보다 허혈성 심전도 변화의 빈도가 더 높았다(14.7% vs. 7.5%, P=0.007). 또한 흉통이 있었던 그룹이 관상동맥 경련이 다발성으로 발생하는 경우가 많았고 (50.7% vs. 29.5%, P<0.001), 미만성으로 발생하는 경우가 많았고(87.1% vs. 75%, P<0.001), 관상동맥의 경련의 정도도 더 심하였다(51.6% vs. 43.5%, P=0.041). 결론: 아세틸 콜린 유발검사시 발생하는 허혈성 심전도 변화와 흉통은 관상동맥 경련의 정도와 경련이 발생하는 관상동맥 개수와 연관이 있다. Background: ECG changes and chest pain during acetylcholine (Ach) provocation tests may constitute important clinical parameters for Ach-induced endothelial dysfunction. We investigated the association between ischemic ECG changes and chest pain during the Ach-provocation test and angiographic characteristics of Ach-induced endothelial dysfunction. Methods: A total of 1,085 patients with anginal symptoms underwent diagnostic coronary angiography (CAG) and Ach-provocation tests. We compared angiographic characteristics of Ach-induced endothelial dysfunction according to the presence of ECG change and chest pain. Results: A total of 539 patients experienced Ach-induced endothelial dysfunction. Patients who experienced ECG change group suffered more frequent chest pain (78.1% vs. 60.8%, P=0.007), angiographically more frequent multiple coronary artery spasm (59.4% vs. 40.6%, P=0.004), and more severe coronary artery spasm (64.1% vs. 46.5%, P=0.006) than patients without ECG change. However, there was no difference in the length of endothelial dysfunction between the groups. The incidence of ischemic ECG changes in patients with chest pain was higher than in patients without chest pain (14.7% vs. 7.5%, P=0.007). Patients who experienced chest pain more often experienced multiple (50.7% vs. 29.5%, P<0.001), diffuse (87.1% vs. 75%, P<0.001), and severe coronary artery spasm (51.6% vs. 43.5%, P=0.041) than patients without chest pain. Conclusion: Ischemic ECG changes and chest pain during the Ach-provocation test are associated with multi-vessel involvement and severe coronary artery spasm. Chest pain is associated with the length of endothelial dysfunction, but ischemic ECG change is not.

        Benefits and problems in implementation for integrated medical Information system

        Purpose : Once the decision has been made to adopt an integrated medical information system (IMIS), there are a number of issues to overcome. Users need to be aware of the impact the change will make on end users and be prepared to address issues that arise before they become problems. The purpose of this study is to investigate the benefits and unexpected problems encountered in the implementation of IMIS and to determine a useful framework for IMIS. Materials and Methods : The Yonsei University Dental Hospital is steadily constructing an IMIS. The vendor's PACS software, Piview STAR, supports transactions between workstations that are approved to integrating the healthcare enterprise (IHE) with security function. It is necessary to develop an excellent framework that is good for the patient, healthcare provider and information system vendors, in an expert, efficient, and cost-effective manner. Results : The problems encountered with IMIS implementation were high initial investments, delay of EMR enforcement, underdevelopment of digital radiographic appliances and software and insufficient educational training for users. Conclusions : The clinical environments of dental IMIS is some different from the medical situation. The best way to overcome these differences is to establish a gold standard of dental IMIS integration, which estimates the cost payback. The IHE and its technical framework are good for the patient, the healthcare provider and all information systems vendors.

      • Eurasian Otter (Lutra lutra) Habitat Suitability Modeling Using GIS; A case study on Soraksan National Park

        Eurasian otter (Lutra lutra) is one of endangered wildlife species whose population size is declining in Korea. To manage and conserve habitat for Eurasian otter, it is crucial to understand which habitat components affect otter habitat qualities. The objectives of this study were to develop a habitat suitability model of Eurasian otter in Soraksan National Park, to validate the model in Odaesan National Park. The research methods of this study were as follows. First, trace data and characters of Eurasian otter habitat were collected with Geographic Information System (GIS) data and Global Positioning System (GPS) receivers between 2000 and 2002. Second, the habitat use factors were identified as habitat characteristics of Eurasian otter and classified with habitat use and availability analyses. Third, significant factors of habitat model were extracted by Chi-square test. The last, Eurasian Otter Habitat Suitability Model (EOHSM) was employed by logistic regression method. Otter habitat use was positively associated with the reeds and shrubs areas adjacent to streams, the size of boulders, and low human disturbance in Soraksan National Park by EOHSM. This model had a classification accuracy of 74.4% at cutoff value of 0.5. Model validation showed a classification accuracy of 86.6 % at cut off value of 0.5 for otter habitat in Odaesan National Park.

        보조호르몬요법을 받는 여성 유방암 환자의 모발 변화와 치료 실태에 대한 기초 연구

        Background: Hormone therapy, which includes tamoxifen and aromatase inhibitors, is the most common adjuvant therapy used for breast cancer. However, only a few studies have reported endocrine therapy induced alopecia. Objective: We investigated the effects of long-term adjuvant hormone therapy on hair in patients with breast cancer, in addition to patients’ concerns and current treatment for hair loss. Methods: Patients completed a questionnaire that included information on self-perceived hair changes after each adjuvant therapy session, distress, and current treatment for hair loss. Using a folliscope, we measured hair density and thickness in each patient and in healthy controls. Results: The study included 93 patients with breast cancer (mean age 51.9±9.8 years). The density and hair thickness were 106.36±21.85 hairs/cm2 and 0.07±0.01 mm in the patient group and 147.86±30.67 hairs/cm2 and 0.07±0.01 mm in the control group (n=98, mean age 52.10±8.40 years), respectively. The mean hair density was significantly lower in the patient group than in the control group; however, no statistically significant intergroup difference was observed in hair thickness. Among 76 patients who perceived hair changes after adjuvant therapy, 71.1% (n=54) were distressed with regard to hair changes. However, only 7.8% of the patients, including two who were treated by dermatologists, currently received treatment for hair changes. Conclusion: Dermatologists should be familiar with hair changes in patients with breast cancer and provide appropriate education to encourage patients to consult dermatologists for hair loss and thinning after breast cancer treatment.       • 월악산국립공원의 곤충다양성에 관한 연구 : 노린재목, 딱정벌레목, 나비목

        본 연구는 2007년부터 2009년까지 3년 동안 월악산 모니터링 조사 지점과 동일지구에 대해 정기적인 주간조사를 통해 주간곤충류(노린재목, 딱정벌레목, 나비목)의 종목록을 확보 하여, 월악산국립공원의 생태계에 자연적 혹은 인위적으로 변화된 내용을 측정하여 곤충자원의 체계적인 관리를 위한 자료를 제공하고자 실시되었다. 본 조사기간 동안 채집 확인된 주간곤충류는 65과 404종(노린재목 21과 86종, 딱정벌레목 38과 244종, 나비목 6과 74종)이 조사되었다. 따라서 문헌조사를 포함한 결과 월악산국립공원에 서식하는 것으로 확인된 주간곤충류는 멸종위기야생동물 2종, 고유종 6종, 국외반출승인대상종 10종과 특정종 50종을 포함하여 총 72과 609종(딱정벌레목 44과 368종, 노린재목 22과 126종, 나비목 6과 115종)으로 나타났다. 아울러 조사지점별 분석결과 S3에서 종다양성이 가장 높게 나타났으며, S10은 다른 조사지점과 다르게 종다양성이 현저하게 떨어지는 것으로 나타났다. 각 조사기간별우점종으로는 2007년과 2009년도에 넉점박이송장벌레, 2008년도에는 더듬이긴노린재로 나타났다. This study were conducted for three years from 2007 to 2009 and at the same points as monitoring surveys, through regular surveys insects (Hemiptera, Coleoptera, Lepidoptera) to obtain the list, to measure and assess Woraksan National Park ecosystem’s natural or artificial change, to provide data for systematic management of the contents of insects. As a result of all the collections were examined, 404 species of 65 family under 3 order were identified. Therefore, the species composition of insect over each result from Woraksan National Park’s was a total of 3 orders, 72 families, 609 species including 2 Endangered species, 6 Endermic species, 10 Management of exotic species and 50 Designated species; 368 species of family Coleoptera which is the highest, 126 species of family Hemiptera and 115 species of family Lepidoptera. Site 3 had the highest indices diversity. In contrast, Site 10 indices diversity, different from the other survey points, were significantly low. In each survey period, dominant species included Nicrophorus quadripunctatus in 2007 and 2009. In 2008, dominant species was Pachygrontha antennata.

        일측 폐이식 성견에서 혈역학과 체액조절 호르몬의 상관관계

        폐이식에 의한 체액이나 혈압의 변동 및 신경의 절단은 혈압 및 체액의 조절에 관여하는 호르몬의 분비에 영향을 미치리라 생각되어 좌일측 폐이식 시 이식폐에 분포하는 신경이 절단된 상태에서 폐이식 후 수용견의 혈역학적 변동이 ANP 및 ADH의 분비와 어떠한 상관관계가 있는지를 관찰하였다. 한국산 성견 12마리를 대상으로 좌일측 폐이식을 시행하였다. 공여폐의 보존은 low potassium dextrane glucose용액을 이용하여 10℃에서 20시간동안 보존한 다음 수용견에 이식하였다. 수용견의 좌측 폐를 적출하기 전과 후, 그리고 이식 후 재관류 30분, 2시간 후에 각각 혈역학 및 동맥혈 가스분압을 측정하였고, 동맥혈에서 ANP 및 ADH의 농도를 측정하였다. 동맥혈의 P_(O_2) 및 P_(CO_2)는 유의한 변동은 없었다. 대퇴동맥압을 재관류 후 30분에는 유의하게 감소하였고, 재관류 2시간 후에는 다소 회복세를 보이는 경향이었으며, 맥압은 비교적 유지되었다. 폐동맥압은 좌측폐 절제 시 이완기 폐동맥압은 변동이 없는 반면에 수축기 폐동맥압은 증가하였고, 재관류 시 폐동맥압은 어느 정도 회복되는 양상이었다. 심박출량은 좌측 폐절제 후에는 감소하였으며 재관류후에도 낮게 유지되는 양상이었다. 반면에 심박수는 유의한 변화없이 유지되었다. 전신혈관저항 및 폐혈관저항은 좌측 폐절제 후나 재관류 30분에 증가하였으며, 재관류 2시간 후 폐혈관저항은 회복되는 양상이나 전신혈관저항은 회복되지 못하였다. 혈중 ANP는 일측 폐절제 후 증가하였고, 재관류 후에는 점차 회복되는 양상을 보였고, ADH는 일측 폐절제 후, 30분에 증가한 후 회복되는 양상을 보였다. 동맥혈 ANP의 변동은 동맥혈 P_(O_2)및 P_(CO_2)와 상관관계를 보였고, ADH는 전신 동맥압의 변동과 상관관계가 있었다. 이상의 결과로 좌일측 폐이식 시에도 ANP 및 ADH는 혈역학적 변화와 연관하여 동맥압 및 체액의 조절에 생리적으로 관여하는 것으로 사료되며, ANP의 분비는 동맥혈의 저산소증 그리고 ADH의 분비는 동맥의 압수용기를 통한 신경성 조절의 영향을 받을 수 있는 것으로 생각된다. The purpose of this investigation was to study changes of plasma atrial natriuretic peptide (ANP) and antidiuretic hormone (ADH) in Korean mongrel dogs after a left single lung transplantation and to evaluate the correlation between hemodynamic factors and secretory responses of ANP and ADH. Arterial P_(O_2 and P_(CO_2) did not change significantly. Femoral arterial pressure decreased after 30 min of reperfusion and recovered slightly at 120 min, whereas pulse pressure did not change. After pneumonectomy, systolic pulmonary arterial pressure increased, whereas diastolic pulmonary pressure remained unchanged. Althought cardiac output fell following pneumonectomy and reperfusion, heart rate was unchanged. The secretion of plasma ANP initially increased follwoing pneumonectomy, but subsequently recoverd. In each subject, ANP values showed stronger correlation with arterial P_(O_2) than other hemodynamic parameters. The secretion of plasma ADH also initially increased (p<0.05) after 30 min of reperfusion and thereafter recoverd, showing the strongest correlation with mean femoral arterial pressure. The present results suggest that the secretion of ANP and ADH seem to be well regulated in accordance with hemodynamic changes after a single lung transplantation. Hypoxia and systemic arterial pressure are the prominent factors in the secretion of ANP and ADH, respectively.

        Biotransformation of major ginsenosides in ginsenoside model culture by lactic acid bacteria

        Background: Some differences have been reported in the biotransformation of ginsenosides, probably due to the types of materials used such as ginseng, enzymes, and microorganisms. Moreover, most microorganisms used for transforming ginsenosides do not meet food-grade standards. We investigated the statistical conversion rate of major ginsenosides in ginsenosides model culture during fermentation by lactic acid bacteria (LAB) to estimate possible pathways. Methods: Ginsenosides standard mix was used as a model culture to facilitate clear identification of the metabolic changes. Changes in eight ginsenosides (Rb1, Rb2, Rc, Rd, Re, Rf, Rg1, and Rg2) during fermentation with six strains of LAB were investigated. Results: In most cases, the residual ginsenoside level decreased by 5.9-36.8% compared with the initial ginsenoside level. Ginsenosides Rb1, Rb2, Rc, and Re continuously decreased during fermentation. By contrast, Rd was maintained or slightly increased after 1 d of fermentation. Rg1 and Rg2 reached their lowest values after 1-2 d of fermentation, and then began to increase gradually. The conversion of Rd, Rg1, and Rg2 into smaller deglycosylated forms was more rapid than that of Rd from Rb1, Rb2, and Rc, as well as that of Rg1 and Rg2 from Re during the first 2 d of fermentation with LAB. Conclusion: Ginsenosides Rb1, Rb2, Rc, and Re continuously decreased, whereas ginsenosides Rd, Rg1, and Rg2 increased after 1-2 d of fermentation. This study may provide new insights into the metabolism of ginsenosides and can clarify the metabolic changes in ginsenosides biotransformed by LAB.

      • 중부지역 옥수수 파종기에 따른 생육단계 및 수량성 변화

        Maize hybrids were planted on April 1, April 25, May 20 and June 15 in 2012, 2013 at the experimental paddy and upland fields in Suwon city , respectively to know changes of growth stage and yield according to different planting time. Maize planting on April 1 was not recommended because total growing days at planting on April 1 was not different with that at planting on April 25 due to many days from planting to emergence over 20 days at planting on April 1. Total growing days were not much different among three planting time after April 25 because days from silking to maturity was lengthened though days from planting to silking was shortened at later planting time. Stalk weight was decreased as planting time was delayed, particularly at planting times after May 20 at paddy field by excess water injury during early growth stage. But, grain yield was not low as in May and June. It is needed to introduce early maturity maize variety as Yanganok at late planting for higher grain yield and enough time for soil preparation for next winter crops
