        원시주의를 넘어서 : 애론 더글러스(Aaron Douglas)의 초기 삽화 작업에 나타난 인종적 정체성 문제와 할렘 르네상스

        Aaron Douglas is widely known as a leading Harlem Renaissance artist particularly as a muralist. However, in the latter half of the 1920s and at the height of the Harlem Renaissance, he was deeply involved in print culture and provided graphic designs for covers, illustrations, and posters for the major magazines of the New Negro movement, such as Crisis and Opportunity, as well as several Harlem writer’s publications including The New Negro (1925), Nigger Heaven (1926), Home to Harlem (1928). This paper at first examines Douglas's early life just before and after he arrived in Harlem, focusing on how the questions of racial identity exemplified by the “New Negro” began to be explored in the context of the active support by and cooperation with Harlem Renaissance leaders. It then responds to those questions by analyzing Douglas's works with reference to the specific forms and subject matter. Harlem Renaissance art has been criticized as being other than a form of independent art movement owing to many artists' reliance upon white patronage and often responding to the mainstream expectation for ‘primitive’ cultures from African American artists. Also, the formal and thematic elements embodied in Douglas's works, such as Cubist and Art Deco style and Egyptian and African metaphors, showed the influence of Western modernist primitivism. However, Douglas also created critical differences beyond mere subordination and imitation to such mainstream primitivism. He suggested highly complicated and often paradoxical styles or poses, dialecticizing the relationships of the primitive and the modern, realism and abstraction, fine art and graphic design, Africa and America, nature and city, and the past and the present.

        Sasa quelpaertensis Leaf Extract Improves High Fat Diet-Induced Lipid Abnormalities and Regulation of Lipid Metabolism Genes in Rats

        Sasa quelpaertensis is a bamboo leaf that is only grown on Jeju Island in South Korea. It is used as a bamboo tea that is consumed for therapeutic purposes, particularly for its anti-diabetic, diuretic, and anti-inflammatory effects. This study investigated the effect of S. quelpaertensis leaf extract (SQE) on high fat-induced lipid abnormalities and regulation of lipid metabolism-related gene expressions in rats. SQE supplementation significantly decreased the levels of plasma triglycerides, total cholesterol, and low-density lipoprotein cholesterol as well as the atherogenic index. SQE restored levels of plasma high-density lipoprotein cholesterol, which were lowered by a high fat diet. Plasma and cardiac resistin levels were also significantly decreased by SQE supplementation. In adipose tissue, mRNA levels of CAAT/enhancer-binding protein <TEX>${\beta}$</TEX> (<TEX>$C/EBP{\beta}$</TEX>) were suppressed in the SQE group. SQE supplementation decreased the accumulation of lipid droplets, inflammatory cell infiltrations, levels of triglycerides, and total lipids in the liver and effectively down-regulated expression of sterol regulatory element binding protein-1 (SREBP-1), fatty acid synthetase (FAS), and uncoupling protein 2 (UCP-2). These results suggest that SQE may be a potential treatment for high fat-related disorders by improving lipid profiles and modulating lipid metabolism.

        Dynamic Transcriptome, DNA Methylome, and DNA Hydroxymethylome Networks During T-Cell Lineage Commitment

        The stepwise development of T cells from a multipotent precursor is guided by diverse mechanisms, including interactions among lineage-specific transcription factors (TFs) and epigenetic changes, such as DNA methylation and hydroxymethylation, which play crucial roles in mammalian development and lineage commitment. To elucidate the transcriptional networks and epigenetic mechanisms underlying T-cell lineage commitment, we investigated genome-wide changes in gene expression, DNA methylation and hydroxymethylation among populations representing five successive stages of T-cell development (DN3, DN4, DP, $CD4^+$, and $CD8^+$) by performing RNA-seq, MBD-seq and hMeDIP-seq, respectively. The most significant changes in the transcriptomes and epigenomes occurred during the DN4 to DP transition. During the DP stage, many genes involved in chromatin modification were up-regulated and exhibited dramatic changes in DNA hydroxymethylation. We also observed 436 alternative splicing events, and approximately 57% (252) of these events occurred during the DP stage. Many stage-specific, differentially methylated regions were observed near the stage-specific, differentially expressed genes. The dynamic changes in DNA methylation and hydroxymethylation were associated with the recruitment of stage-specific TFs. We elucidated interactive networks comprising TFs, chromatin modifiers, and DNA methylation and hope that this study provides a framework for the understanding of the molecular networks underlying T-cell lineage commitment.

        Background:?The majority of colonic obstructions result from colorectal cancer. However, malignancies of extra-colonic origin can also disrupt the colorectal tract, and the efficacy of self-expanding metal stents in such cases remains under debate. This study aimed to evaluate the efficacy and survival of self-expanding metal stents in patients with colonic obstruction caused by an extra-colonic malignancy. Methods:?The medical records of patients who underwent SEMS placement for a colorectal obstruction at a single academic tertiary medical center between July 2004 and August 2015 were retrospectively reviewed. Main outcome measures were the success rates and survival of patients who underwent self-expanding metal stent insertion. Results:?Among the 31 patients, excluding two with a history of colorectal surgery, 29 patients who underwent self-expanding metal stent insertion for colorectal obstruction due to extra-colonic origin were included. Of them, 22 underwent self-expanding metal stent insertion for palliation and seven as bridge to surgery. Technical success was achieved in 93.1% (27/29) and obstructive symptoms were resolved within 48 hours in 82.8% (24/29). During the follow-up period, within the palliation group, 14 patients could avoid surgical treatment with successful decompression for a palliative success rate of 63.6% (14/22). Within the bridge to surgery group, five patients underwent elective surgery after successful decompression with the first self-expanding metal stent for a surgical success rate of 71.4% (5/7). The median major adverse colorectal event free survival in the palliation group was 117 days and the median overall survival of the 29 patients was 142 days. Conclusions:?Self-expanding metal stent insertion is a good treatment option for colonic obstructions caused by an extra-colonic malignancy.

        This report includes the recommended immunization schedule table for children and adolescents based on the 8th (2015) and revised 7th (2012) Immunization Guidelines released by the Committee on Infectious Diseases of the Korean Pediatric Society (KPS). Notable revised recommendations include: reorganization of the immunization table with a list of vaccines on the vertical axis and the corresponding age on the horizontal axis; reflecting the inclusion of Haemophilus influenzae type b vaccine, pneumococcal conjugate vaccine, and hepatitis A vaccine into the National Immunization Program since 2012; addition of general recommendations for 2 new Japanese encephalitis (JE) vaccines and their interchangeability with existing JE vaccines; addition of general recommendations for quadrivalent meningococcal conjugate vaccines and scope of the recommended targets for vaccination; and emphasizing catch-up immunization of Tdap vaccine.         The Dilemma of North Korea’s Foreign Policy : 10 years of diplomacy under the Kim Jong-Un regime

        This study examines the dilemma of North Korea’s foreign policy in the 10 years of Kim Jong-un’s regime. To the best of our knowledge, there is currently no study focusing on the diplomatic history of the regime. Therefore, this study aims to provide a meaningful evaluation of North Korea’s foreign policy over the past decade. A particular focus of this research is looking at how North Korea has adapted its foreign policy in light of the secu-rity challenges brought about by strengthening its military capabilities. The study starts with a comprehensive literature review to put the question into context. Relations with countries across the world are analyzed, in particular, foreign policy initiatives as regards with the US, China, and Russia. There are four main options for North Korea in terms of its diplomacy with foreign powers–these are all investigated in a section on the framework of the analysis. The four foreign policy strategies are balancing, conflictual bandwagoning, coercive diplomacy, and offensive revisionist. These are all explained in detail along with a historical analysis to provide examples of the policies in use over the last 10 years. The conclusion brings together the main points of the study and looks to future prospects for North Korea and its relations with the rest of the world.

        본 저자들은 인위적인 인슐린 투여로 인한 인간성 저혈당증 1예에서 보존적인 치료로 의식기능은 회복하였으나, 현저한 인지능력 저하를 하였기에 문헌고찰과 함께 보고하는 바이다. Factitious hypoglycemia is a covert attempt to lower the blood glucose concentration with insulin or a sulfonylurea agent. This attempt is mainly performed by the patients themselves. In a large series of diabetics who had overdosed on drug, less than 5% had used insulin for suicidal or homicidal purposes. Additionally, 4 of 204 hypoglycemic episodes were due to a suicide attempt with insulin. The onset of insulin action, its peak level, and duration, and the degree of hypoglycemia, are determined by insulin's pharmacokinetics. However, the final outcomes are not associated with the amount of insulin used. The complications related to insulin overdose are cerebral damage and hypokalemia. Other complications include, pulmonary edema attributed to congestive heart failure and hypertensive crisis, as well as respiratory insufficiency. We report this case with a review of the literature.

        Radiotherapy for initial clinically positive internal mammary nodes in breast cancer

        Purpose: Internal mammary lymph node (IMN) involvement is associated with poor prognosis in breast cancer. This study investigated the treatment outcomes of initial clinically IMN-positive breast cancer patients who received adjuvant radiotherapy (RT), including IMN irradiation, following primary breast surgery. Materials and Methods: We retrospectively reviewed data of 95 breast cancer patients with clinically detected IMNs at diagnosis treated with surgery and RT between June 2009 and December 2015. Patients received adjuvant RT to the whole breast/ chest wall and regional lymph node (axillary, internal mammary, and supraclavicular) areas. Twelve patients received an additional boost to the IMN area. Results: The median follow-up was 43.2 months (range, 4.5 to 100.5 months). Among 77 patients who received neoadjuvant chemotherapy, 52 (67.5%) showed IMN normalization and 19 (24.6%) showed a partial response to IMN. There were 3 and 24 cases of IMN failure and any recurrence, respectively. The 5-year IMN failure-free survival, disease-free survival (DFS), and overall survival (OS) were 96%, 70%, and 84%, respectively. IMN failure-free survival was significantly affected by resection margin status (97.7% if negative, 87.5% for close or positive margins; p = 0.009). All three patients with IMN failure had initial IMN size ≥1 cm and did not receive IMN boost irradiation. The median age of the three patients was 31 years, and all had hormone receptor-negative tumors. Conclusion: RT provides excellent IMN control without the support of IMN surgery. Intensity-modulated radiotherapy, including IMN boost for breast cancer patients, is a safe and effective technique for regional lymph node irradiation.
