        Backgrounds: Obesity is known to be a risk factor of adult disease. This study is aimed to identify the prevalence rate of obesity, factors related to BM1(Body Mass Index) which is used as a measure of obesity, and the prevalence rates of hypertension and diabetes which are both obesity-related diseases. Methods: We reviewed 1,189 people who underwent National health examination at the Health Examination Center in National Police Hospital from March 1, 2005 through May 31, 2005. And we evaluated their BMIs, ages, whether they exercised regularly, how many pieces of cigarette per day they smoked, and how many days per week they drank alcohol. Hypertension and diabetes mellitus were chosen to be the obesity-related diseases in this study. Results: The subjects of this study were 1,189 people, and they were 561 males(47.2%) and 626 females(52.9%). The average age of the males and the females were 49.9±13.9 and 50.9±11.7, respectively. The BMI of the males was 24.6+2.7㎏/ ㎡, which was higher than the females, with 23.0±3.1 ㎏/㎡. The BMI of the males increased as their ages increased through their forties, and started to decrease from their fifties, In females, the lowest BMI was checked in their 20s, and the BMI kept increasing as the ages increased, In males, the group of regular exercise showed lower BMI than the group of no exercise(P<0.05), and was less than the one of the group who smoke more than 20 pieces, In both males and females, the BMI of the group who drank alcohol more than 3 days per week was higher than the one of the group who drank alcohol less than 3 days per week(males P<0.01, females P<0.05). In female, there was no difference of BMI whether they exercised more or smoked more. Conclusion: The BMI was higher in males than females, In males, there were a increase of BMI through their forties and a decrease of BMI from their fifties, In females, the BMI kept increasing as the ages increased. Smoking and Exercise made significant differences in males only, as the BMI of the male group who smoked less than 20 pieces of cigarette per day was lower than the one of the male group who smoked more than 20 pieces, and also the BMI of the male group of regular exercise was lower than the one of no exercise, In both males and females, the more they drank, the higher BMI they showed. The prevalence rates of hypertension and diabetes increased as the BMI increased.

        Upregulation of heme oxygenase-1 by ginsenoside Ro attenuates lipopolysaccharide-induced inflammation in macrophage cells

        Background: The beneficial effects of ginsenoside species have been well demonstrated in a number of studies. However, the function of ginsenoside Ro (GRo), an oleanane-type saponin, has not been sufficiently investigated. Thus, the aim of the present study was to investigate the anti-inflammatory effects of GRo in vitro using the Raw 264.7 mouse macrophage cell line treated with lipopolysaccharide (LPS), and to clarify the possible mechanism of GRo involving heme oxygenase-1 (HO-1), which itself plays a critical role in self-defense in the presence of inflammatory stress. Methods: Raw 264.7 cells were pretreated with GRo (up to $200{\mu}M$) for 1 h before treatment with 1 mg/mL LPS, and both cell viability and inflammatory markers involving HO-1 were evaluated. Results: GRo significantly increased cell viability in a dose dependent manner following treatment with LPS, and decreased levels of reactive oxygen species and nitric oxide. GRo decreased inflammatory cytokines such as nitric oxide synthase and cyclooxygenase-2 induced by LPS. Moreover, GRo increased the expression of HO-1 in a dose dependent manner. Cotreatment of GRo with tin protoporphyrin IX, a selective inhibitor of HO-1, not only inhibited upregulation of HO-1 induced by GRo, but also reversed the anti-inflammatory effect of GRo in LPS treated Raw 264.7 cells. Conclusion: GRo induces anti-inflammatory effects following treatment with LPS via upregulation of HO-1.

        Bardoxolone Methyl Suppresses Hepatitis B Virus Large Surface Protein Variant W4P-Related Carcinogenesis and Hepatocellular Carcinoma Cell Proliferation Via the Inhibition of Signal Transducer and Activator of Transcription 3 Signaling

        <P>Bardoxolone methyl (CDDO-me) is a synthetic triterpenoid that has been shown to suppress various cancers and inflammation. It has been implicated for the suppression of signal transducer and activator of transcription 3 (STAT3)-mediated signaling, which plays crucial roles in the development and progression of hepatocellular carcinoma (HCC). Previously, we showed that hepatitis B virus (HBV) large surface protein (LHB) variant W4P promotes carcinogenesis and tumor progression through STAT3 activation. Thus, we examined the anti-cancer activity of CDDO-me against HCC using W4P-LHB-expressing NIH3T3 cells and HepG2 and Huh7 HCC cell lines. CDDO-me exerted cytotoxic activity against W4P-LHB-expressing NIH3T3 cells, HepG2 cells, and Huh7 cells, and induced apoptotic cell death in a dose-dependent manner, demonstrating its anti-cancer activity against HCC. Sublethal concentrations of CDDO-me suppressed STAT3 activation by W4P-LHB ectopic expression and interleukin-6 treatment in W4P-LHB-NIH3T3 and Huh7 cells respectively. The suppression of STAT3 activation by CDDO-me in W4P-LHB-NIH3T3 cells was further confirmed by decreased cyclin D1 protein levels and increased p21 and p53 mRNA synthesis. In addition, CDDO-me treatment resulted in decreased cell migration and colony formation in in vitro assays using W4P-LHB-NIH3T3, HepG2, or Huh7 cell lines, supporting its anti-cancer activity through STAT3 inhibition. Furthermore, ­CDDO-me administration significantly suppressed tumor growth induced by W4P-LHB-expressing NIH3T3 cells in nude mice, confirming its anti-cancer activity. Collectively, our findings demonstrated that CDDO-me is capable of suppressing STAT3 activation in HCC cells and cells transformed by the natural variant of HBV protein. The results suggest that CDDO-me can be a potential therapeutic agent against HCC, especially tumors related to HBV mutations.</P>

      • Primary Diffuse Large B Cell Lymphoma of the Lacrimal Gland

        Tumors of the lacrimal gland are uncommon, and most are of epithelial origin. Primary non-Hodgkin’s lymphoma (NHL) of the lacrimal gland is extremely rare. A 45- year-old male was admitted with a complaint of a painless swelling of the right eye superiolateral lesion. We confirmed primary diffuse large B cell lymphoma of the lacrimal gland, and the patient was classified as stage IE. The patient was treated with chemotherapy, followed by radiotherapy. He achieved complete remis-sion, and, after 26 months of follow-up, the patient remained free of disease.

        Screening of Exiguobacterium acetylicum from Soil Samples Showing Enantioselective and Alkalotolerant Esterase Activity

        About 3,000 bacterial colonies with esterase activities were isolated from soil samples by enrichment culture and halo-size on Luria broth-tributyrin (LT) plates. The colonies were assayed for esterase activity in microtiter plates using enantiomerically pure (R)- and (S)-2-phenylbutyric acid resorufin ester (2PB-O-res) as substrates. Two enantioselective strains (JH2 and JH13) were selected by the ratio of initial rate of hydrolysis of enantiomerically pure (R)- and (S)-2-PB-O-res. When cell pellets were used, both strains showed high apparent enantioselectivity ($E_{app}>100$) for (R)-2PB-O-res and were identified as Exiguobacterium acetylicum. The JH13 strain showed high esterase activity on p-nitrophenyl acetate (pNPA), but showed low lipase activity on p-nitrophenyl palmitate (pNPP). The esterase was located in the soluble fraction of the cell extract. The crude intracellular enzyme preparation was stable at a pH range from 6.0 to 11.0.

        임상연구 : 슬관절 전치환술 시 경막외강, 슬관절강 및 슬관절 주위 조직에 병용 투여한 Bupivacaine과 Morphine이 술 후 통증에 미치는 영향

        Background: Epidural opioids and local anesthetics provide excellent postoperative analgesia. In addition, soft tissue and intra-articular injection of bupivacaine and morphine may have a beneficial analgesic effects after total knee arthroplasty (TKA). This study was designed to assess the additional analgesic effect of combined soft tissue, and intra-articular injection of morphine and bupivacaine in patients undergoing TKA under spinal anesthesia with continuous epidural morphine and bupivacaine injection. Methods: Forty patients aged 55 to 75 years for a TKA under spinal anesthesia were randomly divided into 2 groups. The both group of patients received a continuous epidural infusion using a two-day infusor containing morphine 5 mg in 100 ml of 0.125% bupivacaine. The experimental group of patients (n = 20) received soft tissue, and intra-articular injection containing 10 mg morphine in 100 ml bupivacaine 0.125%. The control group of patients (n = 20) received soft tissue, and intra-articular injection of 100 ml normal saline. Postoperative pain was assessed using the visual analog scale (VAS) at 1, 2, 4, 8, 12, 24, and 48 h postoperatively and side effects, such as hypotension, nausea, vomiting, pruritus, and respiratory distress were recorded. Whenever patients demanded supplemental analgesia, diclofenac sodium 75 mg was injected intramuscularly and the need for additional analgesic requirement was recorded. Results: The experimental group of patients had significantly lower pain score and analgesic requirements than the control group for 2 days. There was no difference in the incidence of side effects between groups. Conclusions: Combined soft tissue, and intra-articular injection of morphine and bupivacaine with continuous epidural morphine and bupivacaine infusion reduces postoperative pain and supplementary analgesic requirements than continuous epidural morphine and bupivacaine infusion only in patients undergoing TKA.         한국에서의 궤양성 대장염의 가족 내 발생률

        Background/Aims: Although familial aggregation of inflammatory bowel disease (IBD) has been reported in Western countries, there are no data about familial occurrence of IBD in Korea. The aims of the present study were to determine the rate of family history and relative risk of ulcerative colitis in first-degree relatives of patients with ulcerative colitis in Korea, and to evaluate clinical characteristics of patients with ulcerative colitis having family history of ulcerative colitis in first-degree relatives. Methods: Of 473 ulcerative colitis patients registered at Inflammatory Bowel Disease Clinic of Asan Medical Center, family history of ulcerative colitis and pedigree were taken from 308 patients. Results: Of the 308 patients, five patients (1.6%) had a positive family history of ulcerative colitis in first-degree relatives. Relative risk of ulcerative colitis in first-degree relatives of patients with ulcerative colitis was 25. Of the five ulcerative colitis patients with family history of ulcerative colitis, three cases occurred between siblings, and two cases occurred in parent-child relationship. Conclusions: The rate of family history of ulcerative colitis in Korea is lower than that of Western countries. However, the relative risk of ulcerative colitis in first-degree relatives of patients with ulcerative colitis in Korea is similar to that in Western countries.
