        Clinical Significance of Classification of Graves` Disease According to the Characteristics of TSH receptor Antibodies

        N/A Background:It has been widely accepted that the epitope (s) and/or functional characteristics of thyrotropin receptor antibodies (TSHRAb) from Graves` patients are heterogenous among patients. However, the clinical significance of such heterogeneity has not been systematically evaluatedyet. We were to elucidate and find the clinical significance of heterogeneity for TSH receptor antibodies in Graves` disease. Methods:We measured stimulating TSHRAb (TSAb) activities using CHO-hTSHR cells, FRTL-5 cells and chimeric receptor expressing cells (Mc1+2 and Mc2), specific blocking TSHRAb (TSBAb) activities using Mc2 cells and TBII activities using porcine thyroid membrane in 136 patients with untreated hyperthyroid Graves` disease. Results: Based on various TSHRAb activities from each patient, the patients could be categorized into 7 subgroups by cluster analysis; 1) Group 1 (n=41) was characterized by moderate TSAb activities bothin CHO-hTSHR cells and in FRTL-5 cells, typical TSAb epitope, rare blocking antibodies and high TBII activities. 2) Group 2 (n=16) was characterized by the presence of blocking TSHRAb in most patients, albeit the other characteristics were the same as those in Group 1. 3) Group 3 (n=19) patients had low TSAb activities bothin CHO-hTSHR cells and in FRTL-5 cells, seldom had blocking TSHRAb, but they had high TBII activities. 4) Group 4 (n=30) could be categorized as `mild disease` group, as they had low activities in all kinds of TSHRAb assay and had low antim icrosomal antibody activities. 5) Group 5 (n=14) was characterized by moderate TSAb activities with atypical epitope (s), rare blocking TSHRAb and moderate TBII activities. 6) Group 6 (n=10) patients had veryhigh TSAb activities with typical epitopes, seldom blocking TSHRAb and low TBII activities. 7) Group 7 (n=6) was characterized by very high TSAb activities with atypical epitopes and high TBII activities. Pretreatment serum thyroid hormone level was low only in group 4 patients compared to the other 6 groups (p<0.05). The size of goiter was significantly larger in those in group 1 and group 3 (p<0.05) compared to the other 5 groups. The prevalence of clinically significant ophthalmopathy was higher in group 2 patients than the other 6 groups (50%vs.27.5%,p=0.06). Among 6 kinds of TSHRAb activities, only the blocking TSHRAb activity was significantly associated with the presence of ophthalmopathy in multivariate analysis. Conclusion:These results suggest that the differences inepitopes for TSAb or the presence of blocking TSHRAb is not a major factor in determining the degree of thyrotoxicosis in Graves` disease. Although the pathogenic mechanism is not clearyet, we suggest that patients with ophthalmopathy have different TSHRAb repertoire from those without ophthalmopathy in Graves` disease.

      • 환율, 금리, 주가사이의 동적연관성 연구

        본 연구는 2000년 1월부터 2008년 말까지 콜금리, 3년물 국채금리, 원달러 외환시장 및 KOSPI자료를 이용하여 원달러 환율 및 장단기 금리 변화량, KOSPI수익률사이의 동적연관성을 실증적으로 분석하였다. 실증분석결과 첫째, 콜금리와 원달러 외환시장사이에는 장기적인 균형관계가 존재하고 있으나, 콜금리와 KOSPI, 원달러와 KOSPI사이에는 장기적인 균형관계가 존재하지 않는 것으로 나타났다. 3년물 국채금리는 콜금리와 공적분관계가 있으나 원달러, KOSPI와는 공적분관계가 존재하지 않는 것으로 나타났다. 둘째, 3년물 국채금리는 KOSPI 및 원달러 환율간에는 피드백적인 관계가 존재하고 있으나, 3년물 국채금리의 KOSPI 및 원달러에 대한 영향력이 그 반대의 경우보다 상대적으로 더 강한 것으로 나타났다. 세째, KOSPI와 원달러 환율사이에도 피드백적인 영향력이 존재하고 있으나 KOSPI의 원달러 환율변화에 대한 영향력이 그 반대의 경우 보다 더 강한 것으로 나타났다. 다섯째, 콜금리는 원달러 외환시장에만 영향력을 미치고 있으며, 장단기 금리사이에는 3년물 국채금리가 콜금리에 대한 영향력을 미치고 있으나 그 반대의 경우는 존재하지 않는 것으로 나타났다. 여섯째, 전반적으로 4가지 금융시계열모두 호재보다는 악재에 더 민감하게 반응하는 정보의 비대칭적인 특성이 존재하는 것으로 나타났다. 이러한 실증분석결과는 투자자들의 투자 및 위험관리전략 수립 뿐 아니라 정책당국자들의 통화 및 금융정책수립에 다소 나마 기여를 할 수 있을 것으로 보여 진다. We try to examine on the short term lead-lag relationship between call, KOSPI and won dollar spot markets with a vector error correction model, Granger causality test and impulse response analysis. This paper uses daily return data covering from January 2000 to December 2008. The major results are as follows; First, according to empirical test based on the Granger causality, the returns of won/dollar spot market Granger causes call market but vice versa. Second, we find a bilateral influence between won/dollar and KOSPI spot markets but the impact of KOSPI is relatively more dominant than of that of won/dollar market. Third, we also find that there is no influence between call and KOSPI markets in a statistically significant level. We hope these kinds of empirical results might be helpful for the investors to set up a asset portfolio and risk management system.

        Productivity Loss of Rheumatoid Arthritis Patients according to the Their Stages of the Disease Activity Score

        Objective. Productivity loss was compared by 3-stage of disease activity and associations between higher disease activity and high productivity loss were identified. Methods. Data were extracted from Rheumatoid Arthritis (RA) Patient-reported Outcomes Research, which enrolled 2,000 RA patients (>20-year) on disease-modifying-antirheumatic-drugs (DMARDs) (≥ 6-month) from December 2012 to June 2013. This included 1,457 RA patients with the disease activity score (DAS-28-ESR) in their medical charts. Productivity loss in time and indirect cost was estimated using The World Health Organization Health and Work Performance Questionnaire (HPQ). Baseline characteristics and productivity loss outcomes were compared according to DAS-28-ESR groups. Results. 84.4% were females, 54.2% had low DAS-28-ESR (<3.2), and 38.2% and 7.6% had moderate (3.2∼5.1) and high DAS-28-ESR (>5.1). Patients with moderate to high DAS-28-ESR had higher lost productivity time (LPT) and monthly costs of LPT than those with low DAS-28-ESR (time in hours: 110.0±58.4 vs. 132.4±57.2 vs. 71.5±52.0, p<0.0001; monthly costs of LPT in 1,000 Korean won: 1,097±607 vs. 1,302±554 vs. 741±531, p<0.0001). Multiple regression analyses revealed significant associations with high LPT in high (adjusted odds ratio [OR]=3.87, 95% confidence interval [CI]: 2.18∼6.87) and moderate DAS-28-ESR (adjusted OR=1.88, 95% CI: 1.41∼2.52) compared to low DAS-28-ESR. In addition, positive associations with high monthly costs of LPT were observed in high (adjusted OR=3.45, 95% CI: 1.98∼5.99) and moderate DAS-28-ESR (adjusted OR=1.93, 95% CI: 1.43∼2.54) compared to low DAS-28-ESR. Conclusion. Timely therapeutic strategies should be taken into consideration given that the RA patients with moderate to high DAS-28-ESR showed strong associations with high productivity loss for effective management of RA.         우황청심원(牛黃淸心元)이 저산소증 유발 배양신경세포에 미치는 영향

        Objectives : The purpose of this investigation is to evaluate the effects of Woohwangcheongsim-won on neuronal death of hypoxic E18 cortical neuroblast. Methods : To evaluate the effect of Woohwangcheongsim-won on neuronal death caused by hypoxia, the survival rate of E18 cortical neuroblast was measured with MTT assay and the changes of several synaptic proteins and enzymes were investigated with the immunoblot assays. Results : The E18 cortical neuroblasts were added 50, 100, 500, 1,000, and $5,000{\mu}g/ml$ Woohwangcheongsim-won. They showed neurotoxicity, when the concentration of Woohwangcheongsim-won was above $1,000{\mu}g/ml$. The E18 cortical neuroblasts, which were added 50, 100, and $500{\mu}g/ml$ Woohwangcheongsim-won, were exposed 98% $N_2/5%\;CO_2$ for 3 hours to induce hypoxia, 3 days later, the survival rate of $50{\mu}g/ml$ Woohwangcheongsim-won was 141.5% when compared to the control group. On the immuneblot assays, the expressions of ${\alpha}$CaMKII, NR2A, NR28, PDE2, PSD-95, and eEF-$1{\alpha}$ were increased in normoxia, but those of NR2A, NR2B were decreased in hypoxia when compared to the control group. Conclusions : The data shows that the effects of Woohwangcheongsim-won on neuronal death of hypoxic E18 cortical neuroblast is a significant result.

      • Systematic Investigation into Mg<sup>2+</sup>/Li<sup>+</sup> Dual-Cation Transport in Chevrel Phases Using Computational and Experimental Approaches

        <P>Computational and experimental investigations into the Li+, Mg2+, and Mg2+/Li+ dual-cation transport properties within the Chevrel phase Mo6S8 have been performed. Five representative paths were selected for 3D diffusion, and their corresponding energy barriers were determined. Based on density functional theory calculation results, we reveal,phenomena of the cation trapping, sluggishness of Mg2+ ion transport, and synchronized movement of inserted cations induced by repulsive interactions. The computational results were further validated by cyclic voltammetry carried out at ambient to high temperatures, from which apparent diffusion constants and activation energies for each case were determined. We found broad agreement between the theoretical and experimental results and suggest an optimum scenario for charge-discharge processes within the dual-cation hybrid system.</P>

      • Time course effects of fermentation on fatty acid and volatile compound profiles of Cheonggukjang using new soybean cultivars

        <P>In this study, we investigated the effects of the potential probiotic Bacillus subtilis CSY191 on the fatty acid profiles of Cheonggukjang, a fermented soybean paste, prepared using new Korean brown soybean cultivars, protein-rich cultivar (Saedanbaek), and oil-rich cultivar (Neulchan). Twelve fatty acids were identified in the sample set-myristic, palmitic, palmitoleic, stearic, oleic, vaccenic, linoleic, alpha-linolenic, arachidic, gondoic, behenic, and lignoceric acids-yet, no specific changes driven by fermentation were noted in the fatty acid profiles. To further explore the effects of fermentation of B. subtilis CSY191, complete profiles of volatiles were monitored. In total, 121, 136, and 127 volatile compounds were detected in the Saedanbaek, Daewon (control cultivar), and Neulchan samples, respectively. Interestingly, the content of pyrazines-compounds responsible for pungent and unpleasant Cheonggukjang flavors-was significantly higher in Neulchan compared to that in Saedanbaek. Although the fermentation period was not a strong factor affecting the observed changes in fatty acid profiles, we noted that profiles of volatiles in Cheonggukjang changed significantly over time, and different cultivars represented specific volatile profiles. Thus, further sensory evaluation might be needed to determine if such differences influence consumers' preferences. Furthermore, additional studies to elucidate the associations between B. subtilis CSY191 fermentation and other nutritional components (e.g., amino acids) and their health-promoting potential are warranted.         Topomer-CoMFA Study of Tricyclic Azepine Derivatives-EGFR Inhibitors

        EGFR has been intensively investigated as a target to block the signal transduction pathway which stimulates cancer growth and metastasis. Studies about structure-activity relationship for tricyclic azepine derivatives were performed with topomer-CoMFA. The derived topomer-CoMFA model with steric and electrostatic field parameters based on fragment units gave reasonable statistics ($q^2$=0.561, $r^2$=0.679). The model explains why a halogen atom at the meta position of aniline is important to increases inhibitory activity. This comes from an electrostatically negative groups are favored near this region. The model also shows that there are sterically favored regions around methoxy group extended from oxazepine derivatives. The findings about steric and electrostatic effects can be utilized for designing new inhibitors.

      • Two-Dimensional Atomic-Layered Alloy Junctions for High-Performance Wearable Chemical Sensor

        <P>We first report that two-dimensional (2D) metal (NbSe2) semiconductor (WSe2)-based flexible, wearable, and launderable gas sensors can be prepared through simple one-step chemical vapor deposition of prepatterned WO3 and Nb2O5. Compared to a control device with a Au/ WSe2 junction, gas-sensing performance of the 2D NbSe2/ WSe2 device was significantly enhanced, which might have resulted from the formation of a NbxW1xSe2 transition alloy junction lowering the Schottky barrier height. This would make it easier to collect charges of channels induced by molecule adsorption, improving gas response characteristics toward chemical species including NO2 and NH3. 2D NbSe2/WSe2 devices on a flexible substrate provide gas-sensing properties with excellent durability under harsh bending. Furthermore, the device stitched on a T-shirt still performed well even after conventional cleaning with a laundry machine, enabling wearable and launderable chemical sensors. These results could pave a road toward futuristic gas-sensing platforms based on only 2D materials.</P>
