        우리나라 근대 수질오염문제의 변천분석 -동아일보 1920-1993년 기사를 중심으로-

        This study was to provide fundamental information on the characteristics and the process of water pollution in Korea and to establish proper policy and strategies to establish proper policy and strategies to deal with the future situation. Content analysis method with a daily newspaper was applied to achieve the purpose of the study. Total 447 articles about water pollution were obtained from the DongA daily newspaper during 1920-1993. The results showed like there. The articles were categorized to three aspects: water pollutant, the affect of the water pollution, and management and measures of water quality. Based on the number of articles and the analysis of the content, the transition of water pollution could be categorized to four periods. 1920~1971 (Period 1), 1972~1978(Period 2). 1979~1990(Period 3.). 1991~1993(Period 4).

        상호교환식 메트로놈 훈련이 만성 뇌졸중 환자의 족저압과 낙상효능감에 미치는 영향

        PURPOSE: This study examined the effects of Interactive Metronome training on the plantar pressure and fall efficacy in chronic stroke patients. METHODS: Twenty-two hemiplegic patients were allocated randomly to an experimental group and control group. The experimental group received conventional physical therapy and emphasized weight-bearing interactive metronome training, whereas the control group received conventional physical therapy. The training was performed three times per week, 40 minutes per each session, for a total of seven weeks. The plantar pressure was assessed using the contact area and contact pressure, whereas the fall efficacy was assessed using the FES (Fall Efficacy Scale), ABC (Activities-specific Balance Confidence scale) and FOFQ (Fear of Falling Questionnaire). RESULTS: After training, a significant increase was observed in the paretic side of the contact area and the paretic and non-paretic side of contact pressure in both groups (p<.05). The between-group differences in the changes before and after training were statistically significant in the paretic side of the contact pressure (p<.05). After training, both the FES of the between-group and ABC of the experimental group were increased significantly (p< .05), but the between-group differences in the changes before and after training were not statistically significant in the FES, ABC, and FOFQ (p >.05). CONCLUSION: Interactive Metronome training is considered an effective treatment for improving the contact pressure of the paretic side in chronic stroke patients.

        한국 노인주택에서의 유니버설 디자인 적용을 위한 기초 연구

        본 연구의 목적은 한국에서의 노인주택 내 유니버설 디자인 적용에 앞서 적용방향을 설정할 수 있는 기초자료를 제공하는데 있다. 우선 한국의 상황을 파악하기 위하여 한국과 미국의 유니버설 디자인 관련법과 시행규칙을 비교하고, 다음으로 미국에서의 주택 내 유니버설 디자인 적용내용을 분석하여 세부항목과 가장 중요시되고 있는 점을 살펴보고자 한다. 이를 통하여 앞으로 한국에서 유니버설 디자인 적용내용을 제시하고, 보완해야 할 점을 파악할 수 있는 자료를 제공하고자 한다. 연구방법으로는 문헌조사법을 실시하였다. 조사결과로는 한국과 미국의 유니버설 디자인 관련법과 시행규칙을 비교하였고, 다음으로는 미국의 주택내에서의 유니버설 디자인 적용내용을 사례분석하였다. 마지막으로 미국 주택에서의 유니버설 디자인 적용사례의 분석을 통하여 한국에서 적용할 방향을 분석하였는데, 그 결과 주택전반에 있어서는 기본적 접근성이 가장 많이 나타났으며, 다음으로 작업 접근성에 관한 내용이었다. 기본적 접근성의 내용은 충분한 활동 공간확보, 가정기기의 조절기는 전면에 설치할 것, 레버식 손잡이, 낮은 문지방 등의 내용이었고, 작업 접근성에는 작업대 아래의 무릎에 들어갈 공간 확보, 높이가 조절되는 작업대 등의 적용사례가 나타났다. 전반적인 적용사례에 대한 분석에 이어, 주택내 각 실에서의 적용내용을 보면, 유니버설 디자인적 특성을 가장 많이 적용하고 있는 공간은 부엌이었으며, 다음으로 욕실이었다. 이는 물과 불을 사용하는 공간으로 사고의 위험성이 많은 공간이며, 또한 인간의 기본적인 욕구를 해결하기 위한 공간이기 때문에 노인을 위한 접근성에 대하여 가장 크게 고려해야 하기 때문이라 사료된다. The current research is aimed at providing the preliminary information in applying universal design to the Korean Housing Systems. The report was performed through extensive literature survey. The detailed issues include the relevant laws and administration regulations of Korea and US in universal design. Subsequently, the case studies in US are also included for the report Up to now, the relevant laws and administration regulations in UD were compared and the detailed applications were discussed. First of all, the laws for handicapped people was investigated as a relevant law of UD. In Korea, the eligibility is rather broad but the actual enforcement is somehow weak. In particular, the legal administration of UD in housing is not guaranteed and so, the complementary measure is required. The application direction of UD to the Korean housing was discussed through the case studies of UD in US. The basic accessibility features was found to be stressed most. The accessible work surfaces are found to be the second in the application of UD. The basic accessibility features includes ample clear floor space, lever hardware on doors, and so on. Through the general analyses, the kitchen are stressed most importantly. The bathrooms, entrance halls, bedrooms, laundry rooms, etc are treated significantly in the listed order.

        자가조절 진정을 이용한 기관지 내시경술에서 Midazolam 전투여가 환자 만족도에 미치는 영향

        Background: Patient-controlled sedation (PCS) with propofol is a safe and effective method of attenuating discomfort during fiberoptic bronchoscopy. The purpose was to evaluate the usefulness of midazolam in addition to PCS for fiberoptic bronchoscopy. Methods: We randomly assigned 155 patients undergoing diagnostic bronchoscopy to two groups. Group M (n=79) received 0.03 mg/kg of midazolam for premedication while group P (n=76) received a loading dose (0.05 ml/kg) of PCS solution composed of 200 mg (20 ml) of propofol and 1 mg (2 ml) of alfentanil. Both groups received the PCS solution 0.2 ml/kg/hr with a bolus of 1 ml and a lockout time of 1 min. Vital signs, pulmonologist satisfaction, patient satisfaction and amnesia were evaluated. Results: After the insertion of the bronchoscope, there was a slight decrease of SpO2 and an increase of blood pressure and heart rate in both groups with no significant differences between the two groups. The group P required more bolus injections (1.6±1.7 in the group M vs 2.5±2.2 in group P; P<0.05). There was no difference in the satisfaction of pulmonologists, but the satisfaction of patients was higher in the group M (P<0.05). More patients in the group P (93%) remembered the procedure than in the group M (70%) (P<0.05). Conclusions: PCS is an effective method for sedating patients undergoing fiberoptic bronchoscopy and midazolam provides more patient satisfaction and amnesia. (Korean J Anesthesiol 2009;56:1~5)

        This study was to investigate the tendency of the elderly housing policy in Korea, America, and Japan. The content analysis method was used to achieve the purpose of this study with laws, books and articles related to welfare and housing policy. The result are followed : 1.The elderly housing policy of Korea was started at facility house for homeless elderly in 1960s and then three generation sharing houses have been built in 1980s. 2.The elderly housing policy of U.S.A. was started in 1950s and the housing with fulfillment of elderly needs was provided in 1960s. The elderly housing with housework service was introduced in 1960s, but the elderly housing for low-income was decresed in 1980s. Vauteur program helped meet the various needs of the elderly. 3.The elderly housing policy of Japan was started in 1960s, The pair housing that was near child's house was provided in 1970s and the financial support programs for the elderly was started. Care service was added in the elderly housing with the pursuit of the quantative improvement in the sharing houses in Japan. The elderly housing policy in Japan is under stable stage in 1990s.

        내시경적 밴드 결찰술과 용종 절제술로 치료한 Blue Rubber Bleb Nevus Syndrome 1예

        Blue rubber bleb nevus syndrome (BRBNS) is a rare disorder that is characterized by multiple cutaneous vascular malformations in the skin and gastrointestinal tract and is associated with intestinal hemorrhage and iron deficiency anemia. This syndrome may induce severe or fatal bleeding; however, the causes of this syndrome are unknown. We report here a case of BRBNS that was diagnosed in a 20-year-old male with chronic anemia and a past history of surgery due to cutaneous hemangioma on the shoulder and thigh. One purplish hemangioma on the stomach and multiple bluish hemangiomas on the colon were noted on the endoscopic examination. We successfully treated the GI hemangioma with endoscopic band ligation and endoscopic polypectomy. Blue rubber bleb nevus syndrome (BRBNS)은 피부와 위장관의 다발성 혈관 기형을 특징으로 하는 드문 질환으로, 위장관 출혈 및 만성 빈혈과 관련이 있으며 치명적인 심각한 출혈까지 야기할 수 있다. 저자들은 만성 빈혈과 허벅지와 어깨의 피부에 혈관종으로 수술 받은 병력이 있는 20세 남자의 내시경 검사에서 위에 자주색을 띤 하나의 혈관종과 대장에 푸른색을 띤 수 개의 혈관종을 발견하였으며 BRBNS로 진단 하였다. 저자들은 특별한 합병증 없이 내시경적 밴드 결찰술 및 용종 절제술로 위장관의 혈관종을 치료하였으며 국내에서는 처음으로 내시경적 밴드 결찰술로 치료한 BRBNS에서의 위장관 혈관종을 경험하여 보고한다.

        p38 MAPK Participates in Muscle-Specific RING Finger 1-Mediated Atrophy in Cast-Immobilized Rat Gastrocnemius Muscle

        Skeletal muscle atrophy is a common phenomenon during the prolonged muscle disuse caused by cast immobilization, extended aging states, bed rest, space flight, or other factors. However, the cellular mechanisms of the atrophic process are poorly understood. In this study, we investigated the involvement of mitogen-activated protein kinase (MAPK) in the expression of muscle-specific RING finger 1 (MuRF1) during atrophy of the rat gastrocnemius muscle. Histological analysis revealed that cast immobilization induced the atrophy of the gastrocnemius muscle, with diminution of muscle weight and cross-sectional area after 14 days. Cast immobilization significantly elevated the expression of MuRF1 and the phosphorylation of p38 MAPK. The starvation of L6 rat skeletal myoblasts under serum-free conditions induced the phosphorylation of p38 MAPK and the characteristics typical of cast-immobilized gastrocnemius muscle. The expression of MuRF1 was also elevated in serum-starved L6 myoblasts, but was significantly attenuated by SB203580, an inhibitor of p38 MAPK. Changes in the sizes of L6 myoblasts in response to starvation were also reversed by their transfection with MuRF1 small interfering RNA or treatment with SB203580. From these results, we suggest that the expression of MuRF1 in cast-immobilized atrophy is regulated by p38 MAPK in rat gastrocnemius muscles.
