        Severe dapsone hypersensitivity syndrome in a child

        Dapsone (4,4'-diaminodiphenylsulfone, DDS), a potent anti-inflammatory agent, is widely used in the treatment of leprosy and several chronic inflammatory skin diseases. Dapsone therapy rarely results in development of dapsone hypersensitivity syndrome, which is characterized by fever, hepatitis, generalized exfoliative dermatitis, and lymphadenopathy. Here, we describe the case of an 11-year-old Korean boy who initially presented with high fever, a morbilliform skin rash,generalized lymphadenopathy, hepatosplenomegaly, and leukopenia after 6 weeks of dapsone intake. Subsequently, he exhibited cholecystitis, gingivitis, colitis, sepsis, aseptic meningitis, disseminated intravascular coagulation, syndrome of inappropriate antidiuretic hormone secretion, pneumonia,pleural effusions, peritonitis, bronchiectatic changes, exfoliative dermatitis, and acute renal failure. After 2 months of supportive therapy, and prednisolone and antibiotic administration, most of the systemic symptoms resolved, with the exception of exfoliative dermatitis and erythema, which ameliorated over the following 4 months. Agranulocytosis, atypical lymphocytosis, aseptic meningitis,and bronchiectatic changes along with prolonged systemic symptoms with exfoliative dermatitis were the most peculiar features of the present case.

        해양생물 다양성 연구를 위한 환경유전자(eDNA)의 적용

        eDNA (environmental DNA)란 특정 환경에 서식하는 생물로부터 유래한 DNA를 의미한다. 환경 시료로부터 추출한 eDNA를 활용하면 해당 환경에 서식하는 생물들의 효율적이고 정확한 모니터링을 수행할 수 있다. 해수 시료로부터 얻은 eDNA를 기반으로 해양생물 다양성 연구를 수행할 수 있다. 해수 시료를 채집하고 이로부터 eDNA를 추출한 뒤, metagenome 분석을 통해 서식하는 해양생물의 종 동정과 다양성 분석이 가능하다. 본 리뷰에서는 이처럼 해수의 eDNA를 활용하여 해양 지역의 생물 다양성 연구를 수행하는 전체적인 과정을 제시하고 있다. 아직 국내에는 해양생물 다양성 연구를 위해 eDNA를 적용하는 방법이 보편화 되어있지 않으며, 본 리뷰를 기반으로 이와 같은 eDNA 연구 방법을 정립하는데 도움을 줄 수 있을 것이다. eDNA, an abbreviation for environmental DNA, means DNA derived from organisms inhabiting in a specific environment. The utilization of eDNA extracted from environmental samples allows for efficient and accurate monitoring of organisms inhabiting the respective environment. Specifically, eDNA obtained from seawater samples can be used to analyze marine biodiversity. After collecting seawater samples and extracting eDNA, metagenome analysis enables the taxonomic and diversity analysis among marine organisms inhabiting the sampled area. This review proposed an overall process of marine biodiversity analysis by utilizing eDNA from seawater. Currently, the application of eDNA for analyzing marine biodiversity in domestic setting is not yet widespread. This review can contribute to establishment of marine eDNA research methods in Korea, providing valuable assistance in standardizing the use of eDNA in marine biodiversity studies.

      • 임신 기간 35주 미만인 미숙아에서 광선 치료 횟수에 미치는 인자와 임상적 특징

        Objectives: Clinical features according to the frequency of phototherapy and clinical risk factors on the number of phototherapy were investigated in premature births with gestation <35 weeks and birth weight ≤2,500 g. Methods: The 186 infants with gestation <35 weeks and birth weight ≤2,500 g were admitted to the neonatal intensive care unit of Kosin University Gospel Hospital from March 2009 to August 2010. The 171 infants were alive and had jaundice requiring phototherapy. Phototherapy was usually started to 50-70% of the maximal bilirubin level. They were divided into two groups according to the frequency of phototherapy as single phototherapy group (group I) and multiple phototherapy group (group II). We retrospectively reviewed the medical records of all patients. Results: The mean gestational age and birth weight of group I were 31.0±2.9 weeks and 1,596±485 g and those of group II were 31.1±2.6 weeks and 1,592±430 g. Compared with group I, albumin and Apgar score at 1 minute of group II were significantly higher and the day of peak bilirubin was also late. Duration of phototherapy in group II was statistically longer than that groupⅠ but duration of ventilator and aminophylline use for apnea was significantly shorter. The frequency of antibiotic use, incidence of bronchopulmonary dysplasia (BPD), and intraventricular hemorrhage (IVH) of group II were significantly lower than those of group I. Conclusions: The day of peak bilirubin was late and the frequency of antibiotic use, incidence of BPD, and IVH were low in group II. The aggressive phototherapy may be considered in premature births with jaundice.

        A case of hydrogen peroxide-induced proctocolitis in a child

        Hydrogen peroxide is a strong oxidizing agent widely available as disinfectants. It can be harmful if it enters into the bowels. Within moments after gastrointestinal exposure, the colon becomes distended with reduced blood flow. The reaction with catalase can cause acute colitis due to the release of oxygen. We present a case of a child with hydrogen peroxide-induced proctocolitis.

        ‘사이버 음란물’에서 ‘기술매개 젠더기반 폭력’까지 : 디지털 성범죄 관련 국내 연구동향 분석

        Digital sex crimes are marked by the steady rise in number and severity of devastating effect. However, there is still no uniform definition of the concept, and many similar expressions are used interchangeably, making it difficult to advance a comprehensive discussion on the issue. In recognition of the foregoing, this study analyzed 95 Korean publications related to digital sex crimes in order to review the expressions used, context of such usage, and chronological changes in the naming of the crime. The literature on digital sex crimes began being published in the early 2000’s, but the number increased dramatically in 2017 when the government introduced its plan to fight digital sex crimes. The early expressions used most frequently were ‘cyber sexual violence’ and ‘cyber pornography’, while ‘online sexual violence’ appeared in the 2010’s. Since 2017, ‘digital sex crimes’ and ‘digital sexual violence’ have became the favored expressions in academia; legal terms, phrases that underlie the non-consensual or illicit nature of the crime, as well as expressions translated directly from English started to be used. It is now time to seek an appropriate name for this socio-structural phenomenon to recognize properly the repeated nature of victimization caused by digital sex crimes. 불법촬영, 불법촬영물 유포, 성착취 영상물 제작 등 디지털 성범죄는 나날이 증가하고 있을 뿐만 아니라, 그 피해의 심각성 또한 높다. 그러나 디지털 성범죄에 대한 합의된 정의가 부재하고 다른 의미를 지닌 여러 유사용어들이 혼용되고 있어 디지털 성범죄 대응 및 예방을 위한 종합적이고효과적인 논의 전개를 어렵게 한다. 이러한 문제 인식하에 이 연구는 디지털 성범죄와 관련하여이루어진 국내 선행연구 총 95편을 분석하여, 디지털 성범죄를 가리키는 용어와 사용된 맥락, 그변화과정을 고찰하였다. 디지털 성범죄 관련 국내 선행연구는 2000년대 초반부터 발표되기 시작하였으나, 디지털 성범죄에 대한 사회적 관심이 크게 높아진 2017년을 기점으로 그 수가 크게 증가하였다. 초기 논의에서 자주 사용되었던 용어는 ‘사이버 성폭력’과 ‘사이버 음란물’이었으며, 2010년대에 들어서면서 ‘온라인 성폭력’과 혼용되어 사용되었다. 정부가 「디지털 성범죄(몰래카메라 등) 피해방지 종합대책」 을 발표한 2017년부터는 ‘디지털 성범죄’와 ‘디지털 성폭력’이 학술적으로도 선호되는 경향이 발견되는 가운데, ‘카메라 등 이용 촬영죄’ 등과 같은 법률상 용어를 그대로 사용하거나 ‘비동의 촬영’⋅‘불법도촬’ 등 비동의 내지 불법성과 같은 디지털 성범죄의 특성을 강조한 표현, ‘이미지 기반성폭력’⋅‘기술매개 젠더기반 폭력’ 등 영문표현을 직역한 서술형 방식의 용어들도 등장하고 있다. 디지털 성범죄가 단순히 일회적인 신체 이미지 착취에 그치지 않고 피해상황이 반복, 양산되고 있는사회구조적인 위해임을 정확히 포착하고 그에 걸맞은 명칭을 찾기 위한 노력이 요청되는 시점이다.

      • 8 cm 이상의 대형 위암의 빈도 및 병리학적 분석

        Purpose: Tumor size has been reported to be one of the prognostic factors in the preoperative setting and 8 cm has been confirmed as a cut-off value for large gastric tumors with respect to postoperative complications. The aim of this study was to investigate the clinicopathologic features in patients with tumors larger than 8cm in diameter. Materials and Methods : We studied 443 patients with gastric cancer who underwent a gastrectomy from January to December in 2006 at the Department of Surgery, Kosin University College of Medicine. For a comparative analysis we divided the cases into the large and the small groups according to tumor size. The clinicopathological factors associated with large gastric tumors were analyzed by using cross tab and bivariate correlation analyses. we used p<0.05 as the cutoff value for statistical significance. Result : Thirty-nine(8.8%) of 443 cases were large gastric cancer and four hundred and four cases were small gastric cancer. Cross tab and bivariate correlation analyses disclosed that tumor location(p<0.001), type of operation(p<0.05), depth of invasion(p<0.001), lymph node metastasis(p<0.001), lymphatic invasion(p<0.001), and perineural invasion(p<0.001) were significantly different between the two groups. The incidence of intestinal type of gastric cancer by Lauren was significantly higher in small cancer than that of large cancer(p=0.042). Conclusion : Large gastric tumors showed more aggressive local findings than their smaller counterparts. In patients with gastric tumors, a curative resection was the most important factor for the prognosis. Therefore, we suggest that every effort should be made to do a curative gastrectomy and an accurate preoperative examination.

        Serum Infliximab Cutoff trough Level Values for Maintaining Hematological Remission in Pediatric Inflammatory Bowel Disease

        Background/Aims: Infliximab (IFX) often loses its therapeutic effect in initial responders with inflammatory bowel disease (IBD) over time. Low serum IFX trough levels (TLs) are linked to poor clinical response and outcomes. Maintenance of optimal therapeutic IFX concentrations is important for sustaining response and achieving good clinical outcomes. Measurement of serum IFX TLs is helpful for determining a further proper therapeutic plan. However, adequate therapeutic IFX TLs in pediatric IBD is uncertain. We aimed to identify the cutoff values for IFX TLs associated with laboratory response to IFX maintenance therapy. Methods: Patients with pediatric IBD who had received IFX infusions between December 2008 and March 2015 at Samsung Medical Center were retrospectively investigated. We analyzed 239 blood samples that were collected from 103 pediatric patients. We measured IFX TLs at induction (6 and 14 weeks) and during maintenance therapy (>22 weeks, 8 weeks interval) by fluid-phase radioimmunoassays. Results: A significant association was found between the erythrocyte sedimentation rate (ESR) and IFX TLs during maintenance (correlation coefficient, –0.11; p=0.0005). A cutoff value of 18 mm/hr for ESR was used to define higher levels. Receiver operating characteristic analysis identified optimal cutoff values: IFX TL >1.58 µg/mL (sensitivity 82% and specificity 73%). Conclusions: Cutoff values are considered a prerequisite for further investigating the clinical usefulness of measurements of IFX in patients maintained with IFX treatment.

        남극 로스해 지역의 아델리펭귄과 황제펭귄 분변 유전자를 활용한 남극 펭귄 장내 미생물의 메타지놈 분석

        This study applied a metagenomic analysis of the penguins' gut microbiome from fecal samples of Adélie Penguin (Pygoscelis adeliae) and Emperor Penguin (Aptenodytes forsteri) living along the Ross Sea, Antarctica. As a result of taxonomic analysis, 7 phyla and 18 families were mainly present in the gut microbiome of Adélie and Emperor penguins. To assess microbial diversity, we performed alpha diversity and OTU abundance analyses. It was confirmed that the Adélie Penguin's gut microbial species had a higher diversity than Emperor Penguin's. Based on the Beta diversity analysis using PCoA, differences were observed in the clustering between Adélie and Emperor penguins, respectively. Through the KEGG pathway analysis using PICRUSt, the nucleoside and nucleotide biosynthesis pathway was the most prevalent in Adélie and Emperor penguins. This study enabled a comparison and analysis of the composition and diversity of the gut microbiome in Adélie and Emperor Penguins. It could be utilized for future research related to penguin feeding habits and could serve as a foundation for analyzing the gut microbiomes of various other Antarctic organisms. 본 연구에서는 남극 로스해 연안에 서식하는 아델리펭귄(Pygoscelis adeliae)과 황제펭귄 (Aptenodytes forsteri)의 분변 시료를 기반으로 펭귄 장내 미생물 메타지놈 연구를 수행하였 다. Taxonomy 분석 결과, 아델리펭귄과 황제펭귄의 장내 미생물에는 주로 7개의 문(phylum), 18개의 과(family)가 존재하는 것으로 나타났다. 또한 미생물 다양성을 평가하기 위해 Alpha diversity 및 OTU abundance 분석을 수행한 결과, 전반적으로 아델리펭귄의 장내 미생물 다양 성이 황제펭귄보다 높은 것을 확인하였고, PCoA를 기반으로 한 Beta diversity 분석을 통해 두 개체군 간 장내 미생물 군집에 차이가 존재함을 확인하였다. PICRUSt를 활용한 기능적인 차원의 KEGG pathway 분석을 통해서는 아델리펭귄과 황제펭귄 시료에서 nucleoside and nucleotide biosynthesis pathway가 가장 많이 존재하는 것을 확인하였다. 본 연구를 통해 남극 아델리펭귄과 황제펭귄의 장내미생물 구성과 다양성을 비교분석 할 수 있었다. 본 연구 결과 는 향후 펭귄의 먹이 섭식 관련 연구에 활용될 수 있으며, 더 나아가 다양한 남극 생물의 장내 미생물 메타지놈 분석에 대한 기초가 될 수 있을 것이다.
