        단순소안구증에서 백내장수술의 임상결과

        Purpose: To evaluate the outcomes in patients with nanophthalmos that had phacoemulsification and intraocular lens (IOL) implantation. Methods: This retrospective study included 13 eyes of eight patients with nanophthalmos who had phacoemulsification with IOL implantation: Preoperative and postoperative manifest refraction, best-spectacle corrected visual acuity (BSCVA), preoperative axial length, anterior chamber depth, corneal diameter, manual keratometry, intraocular pressure (IOP), and postoperative complications were analyzed. Results: Eight patients (13 eyes) had phacoemulsification with IOL implantation, 11 eyes by posterior chamber IOL implantation, and two eyes by ciliary sulcus fixation. The mean change in visual acuity from preoperative to postoperative was 2.1±2.0 lines. IOP was well controlled postoperatively, but one eye required a trabeculectomy. No patients required partial sclerotomy during the operation. Choroidal effusions or choroidal detachment was not observed intraoperatively or postoperatively. The mean endothelial cell loss was 12.6±17.7% at two months postoperative. Conclusions: The results of phacoemulsification and IOL implantation through clear corneal incision in nanophthalmic patients are encouraging. Most patients showed improved visual acuity without severe complications.

        조도와 LED 조명의 색온도가 근거리 시작업 능력에 미치는 영향

        Purpose : To evaluate the effect of correlated color temperature of LED light sources and illumination on Visual Performance at Near. Methods : The subjects of this study were 19 male and female college students(23.05±2.25years) with corrected visual acuity 1.0 and they didn't have any eye surgery history and eye disease. The color temperature of LED light sources used in this study were 3,500K, 5,600K, 7,500K. The accuracy and the required time of each light source on error correction work and reading work were measured. The lux of working surface was 400 lux and 700 lux each. Result : The time required for reading work has been reduced significantly from both illumination in 7,500K. The required time of error correction work has been reduced for each 400 lux and 700 lux in 5,600K. The accuracy was lowest in 7,500K and it has been shown a reducing tendency on 400 lux compared with 700 lux in every color temperature. Conclusion : The time and accuracy of error correction work were shown a changing tendency depending on the illumination and color temperature didn’t significantly differ. However the time required for reading work has been reduced significantly from both illumination in 7,500K. Thus, it is necessary to determine the appropriate color temperature of the LED lights for the improvement of visual performance at near. 목 적 : 조도와 LED 조명의 색온도가 근거리 시작업 능력에 미치는 영향에 대해 알아보고자 한다. 방 법 : 안질환 및 안과 수술경력이 없고 양안시 이상이 없는 양안 교정시력 1.0 이상인 대학생 남녀 19명(23.05±2.25세)을 대상으로, 작업면 조도를 400lux와 700lux로 각각 고정시킨 상태에서 LED 조명의 색온도 3,500K, 5,600K, 7,500K 총 3가지로 오류수정작업과 읽기작업을 통해 정확도 및 소요시간을 측정하였다. 결 과 : 오류수정작업 소요시간은 400 lux와 700 lux 모두 5,600K에서 가장 감소하였고, 정확도는 7,500K에서 가장 낮게 나타났으며, 모든 색온도에서 400 lux보다 700 lux에서 더 감소하는 경향을 보였다. 읽기작업의 소요시간은 400 lux와 700 lux 모두 7,500K에서 가장 빨랐다. 결 론 : 오류수정작업의 소요시간과 정확도는 조도와 색온도에 따라 변화하는 경향을 보였으나 유의하진 않았다. 하지만 읽기작업은 두 가지 조도 모두 7,500K에서 소요시간이 유의하게 줄어들었다. 따라서 근거리 시작업 능력향상을 위한 적절한 LED 조명의 색온도의 결정이 필요할 것으로 사료된다.

        Maxillary Sinus Augmentation Using Autogenous Teeth: Preliminary Report

        Purpose: The purpose of this study was to evaluate the effectiveness of autogenous tooth graft materials after maxillary sinus bone grafts. Methods: The study involved 23 implants in 22 patients who visited the Department of Oral and Maxillofacial Surgery and the Department of Periodontics, Chosun University Dental Hospital, in 2008 and received autogenous tooth graft materials for maxillary sinus bone grafts. Results: For eight patients with maxillary bone graft materials prior to implant placement, the healing period averaged five months. For eleven patients with simultaneous maxillary bone graft and implant placement, eight patients received a second surgery, with an average healing time of six months. Three patients had a longer observation period with only a fixture implanted. Three patients who received only a bone graft required more time to implant placement because of the lack of residual bone and also for personal reasons. Only 5 patients had biopsies performed and complications such as infection and dehiscence healed well. The application of autogenous graft materials to the maxillary bone graft sites did not exert any significant effects on the success rates. When a mixture of graft materials was used, the post-surgical bone resorption rate was reduced. Histological analysis showed that new bone formation and remodeling were initiated during the three-to-six month healing period. Bone formation capacity increased continuously up to six months after the maxillary bone graft. Conclusion: According to this analysis, excellent stability and bone-forming capacity were seen in cases where autogenous materials were used alone or mixed with other materials. Autogenous tooth graft materials may be substituted instead of autogenous bones.

        Some physiological changes including complete blood count, plasma hemoglobin, electrolytes with calcium and magnesium, and acid base balance were investigated on hemodilution extracorporeal circulation. As pump-oxygenator Sigmamotor pumps and Rygg-Kyvsgaard bubble type oxygenator were utilized with high flow rate in eighteen acyanotic and fifteen cyanotic cases. The following results were obtained. (1) Significant decrease of hemoglobin and hematocrit was noted after bypass through operation and in the postanesthesia room, but white blood cell count showed no significant deviation from the control value. Free plasma hemoglobin value showed marked increase after cardiopulmonary bypass. (2) Serum sodium and potassium values showed no definite changes during and after extracorporeal circulation and in the immediate postoperative period, though chloride decreased significantly during bypass and returned to the control value at the end of operation in acyanotic group. (3) Serum calcium remained at the normal value during and after total body perfusion and no significant deviation from the control value was noted. (4) Serum magnesium decreased significantly during perfusion but increased to the control value after bypass. (5) Acid base balance study showed the following results. In acyanotic group result before perfusion showed pH of 7.35, at the beginning of perfusion 7.45, after. 30 minutes of perfusion 7.43 with partially compensated respiratory alkalosis, immediately after pump-off 7.36, and 30 minutes after pump-off 7.32 with uncompensated metabolic acidosis. In cyanotic group result before perfusion showed pH 7.26 with uncompensated metabolic acidosis, at the beginning of perfusion 7.47 with partially compensated respiratory alkalosis, after minutes of perfusion 7. 46 with compensated respiratory alkalosis, immediately after pump-off 7.35, nd 30 minutes after pump-off 7.34 with uncompensated metabolic acidosis. In both groups compensated respiratory alkalosis developed during perfusion and 30 minutes after pump-off uncompensated metabolic acidosis are noted.         신속 조형 기술로 제작된 인체모형을 이용한 술전 모의 두 개악안면성형수술

        In plastic and reconstructive craniomaxillofacial surgery, careful preoperative planning is essential to get a successful outcome. Many craniomaxillofacial surgeons have used imaging modalities like conventional radiographs, computed tomography(CT) and magnetic resonance imaging(MRI) for supporting the planning process. But, there are a lot of limitations in the comprehension of the surgical anatomy with these modalities.Medical models made with rapid prototyping (RP) technique represent a new approach for preoperative planning and simulation surgery. With rapid prototyping models, surgical procedures can be simulated and performed interactively so that surgeon can get a realistic impression of complex structures before surgical intervention.The great advantage of rapid prototyping technique is the precise reproduction of objects from a 3-dimensional reconstruction image as a physical model. Craniomaxillofacial surgeon can establish treatment strategy through preoperative simulation surgery and predict the postoperative result.

        한국인의 특발성 망막앞막에서 투명각막절개 초음파유화술과 유리체절제술의 병합 및 연속수술

        Purpose: To evaluate the surgical outcomes on the combined procedures of phacoemulsification, intraocular lens (IOL) implantation, and vitrectomy, and to compare clinical results with sequential surgery results in Korean patients with idiopathic epiretinal membrane. Methods: The present retrospective study included 20 eyes of 19 patients with idiopathic epiretinal membrane who underwent combined phacoemulsification, IOL implantation, and pars plana vitrectomy (combined surgery), and 18 eyes of 18 patients who underwent phacoemulsification and IOL implantation subsequent to vitrectomy (sequential surgery). Postoperative clinical results and intra- and postoperative complications were compared between the 2 groups. Results: Postoperative best corrected visual acuity (BCVA) (logMAR) was 0.41 ± 0.42 in the combined group, and 0.35 ± 0.37 in the sequential group. There was no significant difference in BCVA between the 2 groups (p = 0.675). The mean refractive prediction error was -0.46 ± 0.88 diopters (D) in the combined group, and -0.06 ± 0.68 D in the sequential group, showing no statistically significant difference between the 2 groups (p = 0.147). The present study showed no major differences between the 2 groups in complications such as intraoperative mild corneal edema, postoperative ocular hypertension, and lens capsular opacity. Conclusions: Combined phacoemulsification, IOL implantation, and pars plana vitrectomy as well as the two-step procedure are safe and effective for treating patients with idiopathic epiretinal membrane.
