        동결건조 모시잎 분말 첨가가 돈육패티의 품질특성에 미치는 영향

        본 연구는 동결 건조한 모시잎 분말을 돈육패티에 첨가하여, 이들 제품에 미치는 품질특성을 평가하고자 하였다. 모시잎 분말의 일반성분은 수분 함량 4.67%(w/w), 조단백질 25.61%(w/w), 조지방 6.665%(w/w), 조회분 16.88%(w/w)로 나타났다. 모시잎 분말을 첨가한 돈육패티의 수분 함량은 분말의 첨가량이 증가될수록 감소하는 경향을 보였다. pH는 6.06~6.18로 유의적 차이를 보이지 않았다. 색도는 생육 패티에서 모시잎 분말의 첨가량이 증가할수록 명도와 적색도는 유의적(p<0.001)으로 감소하고, 황색도는 유의적(p<0.001)으로 증가하였으며, 가열육 패티에서 모시잎 분말의 첨가량이 증가할수록 명도(p<0.001)와 적색도는 감소하고, 황색도는 0.75%(w/w) 첨가군에서 가장 높은 값을 보였다(p<0.05). 물성 특성은 모시잎 분말의 첨가량이 증가할수록 경도, 탄력성, 응집성, 씹힘성에서 유의적으로 증가하였다. 지질산패도(TBARS)는 저장 기간의 경과에 따라 측정 값들이 증가하였으며, 모시잎 분말의 첨가량이 증가할수록 유의적(p<0.001)으로 낮은 TBARS 값을 보여 지질산패에 효과적으로 나타났다. 관능적 특성으로 색감에서는 모시잎 분말의 첨가량이 증가함에 따라 유의적(p<0.05)인 경향을 보였으며, 전반적인 기호도에서는 모시잎 분말 0.5%(w/w) 첨가군에서 유의적(p<0.01)인 경향을 보이며 가장 높은 기호도를 나타내었다. 본 연구를 통해 돈육패티 제조 시 모시잎 분말을 0.5%(w/w) 정도 첨가하는 것이 바람직한 것으로 사료되며, 돈육패티에 모시잎 분말을 첨가함에 따라 돈육 특유의 이취를 감소시키며, 색상과 맛 등의 기호도를 향상시켜 관능적 품질을 향상시킬 수 있을 것으로 보이며, 관능적 평가뿐만 아니라, 저장 중 지방산패를 억제할 수 있을 것으로 보인다. This study was conducted to investigate the effects of ramie leaf powder on the quality characteristics of pork patties. The moisture, crude protein, crude fat, and crude ash in freeze-dried ramie leaf powder were found to be 4.67% (w/w), 25.61% (w/w), 6.66% (w/w), and 16.88% (w/w), respectively. Pork patties were prepared by adding varying amounts of ramie leaf powders up to 1.5% (w/w) of the pork patty formulation. The moisture contents of meat patties containing ramie leaf powder were lower than that of the control patty, but there was no significant difference in pH values between the patties. With increasing amounts of ramie leaf powder, chromaticity decreased in brightness ($L^*$) and redness ($a^*$), but increased in yellowness ($b^*$) in the raw patties. In texture analysis, hardness, springiness, cohesiveness, and chewiness of the pork patties increased as more ramie leaf powder was added to the pork patties. Thiobarbituric acid reactive substances (TBARS) values increased during storage in pork patties without ramie leaf powder, but those formulated with ramie leaf powder showed lower TBARS values than those in the control groups. Sensory panels determined that pork patties containing 0.5% ramie leaf powder had higher scores for overall acceptability.

        퇴원환자의 병원중심 방문영양지도 요구도 조사

        The purpose of this study was to provide a basic data of nutrition services in home health care by analyzing hospital-based home-visit nutrition education needs of patients at discharge. Methods: Data was collected from September 11 to October 12, 2012 by administering questionnaires to 289 chronic disease patients to be discharged from a university hospital in Pusan. The home-visit nutrition education instruments used for collecting data were developed by the researcher. Results: Regarding the demands of home-visit nutrition education, 62.3% of subjects were willing to use home-visit nutrition education and 37.7% weren``t. The main reason for using the home-visit nutrition education was “the effective nutrient management in consultation with an individual’s doctor”, 38.9% and 31.2% of patients who did not wish to use the service gave the reason for their decision as, “Just by managing the nutritional requirements of a family’s diet and, the patient will be able to fully recover”, respectively. As for the demand, classified with the areas of home-visit nutrition education, the demand for the area of basic nutrition (3.75/5.00) was the highest followed by, the area of educational nutrition (3.74/5.00), therapeutic nutrition (3.67/5.00), and dietary nutrition (3.55/5.00). The demand for the area of educational nutrition was high “Considering the state of dietary management, such as disease status and drugs”, 73.7%. As for the relation between the characteristics of the study subjects and analysis of demand home-visit nutrition education, the characteristic of subjects, that is, “regular home-visit nutrition education”, “practice of diet therapy after discharge” had a significant difference statistically (p < 0.01). As for the relation between the needs for fundamental home-visit nutrition education and the demand of home-visit nutrition education, basic nutrition, educational nutrition, therapeutic nutrition, and dietary nutrition had a significant difference statistically (p < 0.01).Conclusions: Hospital-based home-visit nutrition education need the access of home nutrition support team.

        콩 완자전의 항산화 활성 및 물성 특성

        This study was conducted to improve the quality of Wanja-jeon, a Korean traditional food utilizing beef. Antioxidative evaluation of the ingredients in Wanja-jeon showed that soybean powder was highest in DPPH, ABTS radical scavenging activities and in reducing power among the food ingredients tested. Since soybean powder demonstrated high antioxidative activity, beef was replaced with soybean powder in Wanja-jeon to increase the antioxidative activity. All antioxidative activities such as DPPH and ABTS radical scavenging activities as well as reducing power increased with increases in the amount of soybean powder replacing beef. In texture analysis, the hardness, chewiness and brittleness of Wanja-jeon were highest when beef was replaced with 20% soybean powder, whereas the springiness and cohesiveness of the Wanja-jeon were highest with 50% of the beef replaced with soybean powder. The Hunter's color L and b values were highest in Wanja-jeon with 50% soybean powder replacing beef, while the a value of the color was highest in Wanja-jeon with 30% soybean replacing beef.

        암환자의 영양상태 평가와 재원일수 및 사망률과의 관계

        Purpose: The length of a patient’s stay in a hospital is an indication of the patient’s recovery rate. The length of the hospital stay has an important economic factor for hospitals. This study was conducted to determine the relationship of the nutritional status at the time of admission to mortality and the length of the hospital stay (LOHS). Methods: The study subjects were 1,619 patients who suffered with cancer, were admitted to Kosin University Gospel Hospital during 2005 and they met the study criteria. The patients were classified to the not at risk group, the patients having one risk factor for malnutrition were the Ⅰ group, the patients having two risk factors for malnutrition were the Ⅱ group, the patients having three risk factors for malnutrition were the Ⅲ group and the patients having 4 risk factors for malnutrition were the Ⅳ group, based on the established criteria of serum albumin <3.0 g/㎗, a total lympocyte count of <1,500 cells/㎣, a cholesterol level of ≥240 ㎎/㎗ or ≤130 ㎎/㎗, weight for height ≥120% or <90% ideal body weight. Results: 24.3% of the patients were classified into the not at risk group, 37.6% were classified into the at risk group Ⅰ, 24.2% were classified into the at risk group Ⅱ, 10.3% were classified into the at risk group Ⅲ and 3.6% were classified into the at risk group Ⅳ. The at risk group (at risk Ⅲ, at risk Ⅳ) had a significantly higher prevalence of liver disease. The relationship between liver disease and low serum albumin albumin levels may have confounded the data. Although the estimated LOHS was similar in all the groups, the average length of stay was 14.4±16.38 days in the malnourished group (at risk group Ⅳ) compared to approximately 2.8 days in the not at risk group. The more nutritional risk factors the patients had, the longer was the LOHS and the mortality rates were higher. Correlation was not observed between the risk factors and the length of the hospital stay, as well as the lack of correlation with the mortality rate. Conclusion: These results suggest that a patient’s nutritional status upon admission has an effect on the length of the hospital stay for patients with carcinoma.

        Kaempferol 및 Kaempferol Rhamnosides의 항산화 활성 및 항염 효과에 관한 연구

        본 연구에서는 kaempferol 및 그 배당체의 항산화 및 항염증 효능을 평가하기 위해 free radical 소거활성, ROS inhibition assay, TARC 생성 억제 관련 실험을 수행하였다. 또한 kaempferol과 그들의 rhamnosides (α-rhamnoisorobin, afzelin, kaempferitrin)의 구조에 따른 생리적 활성의 상관관계를 조사하였다. Kaempferol과 α-rhamnoisorobin은 free radical (1,1-diphenyl-2-picrylhydrazyl, DPPH) 소거활성(FSC50)에서 각각 62.5, 50.0 μM이 측정되었으며 afzelin과 kaempferitrin은 농도와 상관없이 free radical 소거활성을 나타내지 않았다. ROS inhibition assay에서는 시료 농도를 10, 50, 100 μM을 처리한 결과 kaempferol은 대조군(100 %)에 비하여 ROS 발현 농도를 각각 97.5, 57.8, 47.8 %로 감소하였으며 α-rhamnoisorobin은 93.1, 59.1, 41.4 %의 감소를 나타내었다. TARC (thymus and activation regualted chemokine) 생성 억제능 실험을 통해 시료의 항염증 효능을 평가한 결과 kaempferol은 10, 50, 100 μM 농도에서 대조군(100 %)에 대비하여 각각 48.8, 5.5, 4.4 %로 농도 의존적으로 TARC 생성을 감소시켰으며, α-rhamnoisorobin은 88.1, 19.0, 1.0 %로 TARC 생성의 감소 효능을 나타내었다. 결론적으로 kaempferol과 그들의 rhamnoside 중 kaempferol과 α-rhamnoisorobin은 항산화, 항염증에 대한 우수한 효능을 나타내는 것으로 사료되며 더 나아가서는 항노화 및 항염증에 효과가 있는 기능성 화장품에 응용 가능성이 있음을 시사한다. In this study, to evaluate the antioxidative activities and anti-inflammatory effects of kaempferol and its rhamnosides, we performed the free radical scavenging assay, ROS inhibition assay and TARC (thymus and activation- regulated chemokine) assay. Also, we studied physiological activity of kaempferol and its rhamnosides (α-rhamnoisorobin, afzelin, kaempferitn) by structure-activity relations. The free radical (1,1-diphenyl-2-picrylhydrazyl, DPPH) scavenging activities were determined with kaempferol (62.5 μM) and α-rhamnoisorobin (50.0 μM) but afzelin and kaempferitrin did not show free radical scavenging activities. Kaempferol showed a 97.5, 57.8, 47.8 % inhibition of ROS (reactive oxygen species) generated at concentrations of 10, 50 and 100 μM, compared to control (100 %). α-rhamnoisorobin showed a 93.1, 59.1 and 41.4 % inhibition of ROS at the same concentration. We investigated the inhibitory effects of kaempferol and its rhamnosides on TARC expression. Kaempferol showed a 48.8, 5.5 and 4.4 % inhibition of TARC generated at 10, 50 and 100 μM, compared to control. α-Rhamnoisorobin showed a 88.1, 19.0 and 1.0 % inhibition of TARC generated at the same concentration. In conclusion, these results indicate that kaempferol and α-rhamnoisorobin have good antioxidative activities and anti-inflammatory effects that could be applicable to new functional cosmetics for anti-aging and anti-inflammation.

      • 입원 환자 영양검색 전산 프로그램 개발

        Background : About 30% of patients in the hospital are under nourishment. A large portion of people are undernourished when they are admitted to the hospital and in the majority of these, undernutrition develops further while in hospital. Patient-Generated subjective global assessment(PG-SGA), a method of nutritional assessment based on clinical judgment, has been widely used to assess the nutritional status of adults for both clinical and research purposes. Modified PG-SGA can be used as a nutrition screening tool, and a comprehensive nutrition assessment is desirable for those who are malnourished. The purpose of this study is to develop a simple, reliable and valid nutrition screening program(NSP). Methods : We applied the nutrition screening program(NSP) to adult inpatients who are at risk of malnutrition using the Modified Patient-Generated subjective global assessment(PG-SGA) and objective assessment(electrical medical record data). Results : Therefore simple and beneficial NSP is developed for patients with severe malnutrition. Using NSP. nutritional information of the severe malnutrition patient should be shared with the physicians and they should be taken care of by clinical dietitians to improve their nutritional status. Conclusion : The Modified PG-SGA and nutrition screening tool are simple, reliable and valid nutrition screening program(NSP).

      • 위암환자의 입원시 영양상태

        The purpose of this study is to investigate initial nutritional status of stomach cancer patients. The study subjects were 88 patients with stomach cancer admitted at Kosin University Gospel Hospital in Busan. We assessed the initial nutritional status by anthropometric, biochemical and dietary intake data along with subjective global assessment(SGA). The results are as fallows: 1. The mean age, weight, height, triceps skinfold thickness, mid arm circumference, and mid arm muscle circumference of the subjects were 55.9±11.0years, 60.0±9.8㎏, 162.4±8.5㎝, 10.9±5.7㎜, 26.8±3.8㎝, and 23.4±3.5㎝ respectively. The mean body mass index was within the normal range, with 22.7±2.8㎏/㎡, white 15.4% of the patients was underweight. The result shows that body fat mass and body protein mass of the patients with stomach cancer were decreased. 2. The mean biochemical data of the subjects were 4.0±0.5g/dl for albumin, 174.7±41.9㎎/dl for cholesterol, 107.6±57.2㎎/dl for triglyceride, 92.1㎍/dl for Zn, 297.0±103.1㎎/dl for transferring, 1980.0±0.8㏖ for total lymphocyte count. 3. Daily energy intake was 1997.8±579.3㎉. And the ratio of carbohydrate, protein, and lipid to energy intake was 72:14:14. 4. The patents were divided into three groups according to SGA performed by an observer. Group A(well nourished)was 55.7% with 49patients, Group B(moderately malnourished)was 22.7% with 20patients, and Group C(severely malnourished)was 21.6% with 19patients, The three groups showed a significant difference in body weight(p<0.01), 1 month weight loss %(p<0.001), 6 months weight loss %(p<0.001), body mass index(p<0.01), and mid arm circumference(p<0.05), albumin(p<0.01), energy intake(p<0.05) as well as carbohydrate intake(p(0.05). From these results, it may be concluded that SGA can be used as a nutrition screening tool, and comprehensive nutrition assessment is desirable for those malnourished.

      • 비만을 동반한 위암 환자에서 두 병을 같이 수술 치료한 경험(증례)

        Obesity is growing problem in Korea. We had a case of bariatric surgery during gastric cancer operation. Patient was 29 year old Korean lady with early gastric cancer located in the lesser curvature side of the middle 1/3 of the stomach. Her body weight was 89 kg, height 163 cm, and thus body mass index was 33.5 kg/m2. Preoperative blood pressure was 130/90 mmHg, hemoglobin 12.9 g/dL, total lymphocyte count 3,280/mL, serum albumin 4.3 g/dL, CEA 1.1 ng/mL, CA 19-9 9.1 ng/mL, and alpha fetoprotein 2 ng/mL. Another associated disease was right thyroid follicular neoplasm. The chest X-ray was normal. After IM injection of 2,500 units of heparin 30 minutes before the induction of general anesthesia, she received curative subtotal gastrectomy and Roux en Y gastrojejunostomy when a malabsorption loop of 120 cm jejunum was incorporated between the Treitz ligament and the end to side jejuno-jejunostmy site at May 29th 2006. Her postoperative course was uneventful except a minor wound seroma and the elevations of serum amylase(up to 4 folds) and lipase(up to 2 folds). She lost her body weight 9 kg in 5th, 21 kg in 7th, and 27 kg in 10th postoperative months to became 61.5 kg. On follow up exams in July 2008, she gained 4 kg to overcome her weakness and fatigue. From the above result, the incorporation of a malabsorption loop during reconstruction phase of gastric cancer operation was a good option for obese gastric cancer patients especially in cases of early gastric cancer.
