외상성 기관-기관지 손상의 진단 방법

Background: The aim of this study was to identify the distinguishing clinicoradiologic findings of traumatic tracheobronchial injury. Material and Method: Between January 2003 and December 2009, six patients who underwent surgical repair for traumatic tracheobronchial injury due to blunt trauma were included in this study. We evaluated the mechanism of the injury, the coexisting injuries, the time until the making diagnosis and treatment, the diagnostic methods, the anatomic location of the injury and the surgical outcomes. Result: The mechanisms of injury were traffic accident and crushing forces. The frequent symptoms were subcutaneous emphysema, dyspnea and pain, and the common radiologic findings were pneumothorax, mediastinal emphysema, rib fracture and lung contusion. Only 2 patients were diagnosed by chest CT and the others were not diagnosed preoperatively. The location of injury was the trachea in 2 patients and the bronchial tree in 4 patients. There was no postoperative mortality or anastomotic leak; however, vocal cord palsy occurred in one patient. The most distinguishing sign was persistent lung collapse even though the chest tube was connected with negative pressure. Conclusion: Although it was not easy to diagnose traumatic tracheobronchial injury without a clinical suspicion, the distinguishing clinical symptoms and CT findings could help to make an early diagnosis without performing bronchoscopy. 배경: 이 연구는 외상성 기관-기관지 손상의 특징적인 임상 증상 및 영상학적 소견 등에 대하여 알아보고자 하였다. 대상 및 방법: 2003년 1월부터 2009년 12월까지 본원 응급실을 통해서 내원한 외상환자들 중에서 수술을 통해서 외상성 기관-기관지 손상으로 진단된 6명의 환자를 대상으로 하였다. 외상의 종류, 동반된 손상, 진단 방법 및 수술까지 걸린 시간, 수술 소견 및 수술 방법, 예후 등에 대해서 조사해 보고 후향적으로 진단에 중요한 인자 등을 알아 보았다. 결과: 손상의 원인으로는 교통사고가 1명, 낙상 및 흉부에 강한 압박을 받은 경우가 5명이었다. 주 증상으로 피하기종, 호흡 곤란,통증 등이 있었으며 영상소견으로는 기흉, 종격동 기종, 혈흉, 늑골 골절, 폐좌상 등이 있었다. 기관지내시경을 시행하지 않은 상태에서 2명에서는 흉부 CT 소견에서 기관-기관지 손상이 의심되었지만나머지 4명에서는 의심하지 못했다. 수상 부위는 기관부위가 2예, 기관지가 4예 있었다. 수술은 개흉술을 통한 일차 문합을 시행하였고 수술 후 사망과 문합 부위 유출은 없었으며 1명에서 술 후 성대마비가 있었다. 진단에 도움이 되는 특징적인 소견으로는 흉관 삽입 후 음압의 적용에도 불구하고 지속적인 심한 폐 허탈이 가장 중요한 소견이었으며, 흉부 CT에서의 기관-기관지의 주행 경로의 단절이 진단에 중요하였다. 결론: 외상성 기관-기관지 손상은 의심하지 않으면 진단이 쉽지 않으나, 특징적인 흉관 삽입 후의 임상 증상과 영상 소견은 진단에 큰 도움을 주었다.

Hybrid Approach of Ruptured Type B Aortic Dissection with an Aberrant Subclavian Artery in a Single Patient with Turner Syndrome: A Case Report

Turner syndrome, also described as 45, X, may present with most serious cardiovascular anomalies including risk of aortic dissection and rupture. In emergency situation, management for aortic dissection with complicated anatomy accompanying vascular anomaly is challenging. Here, we report a rare case of ruptured type B aortic dissection with aberrant subclavian artery and partial anomalous pulmonary venous connection in a Turner syndrome. Through right carotid-subclavian artery bypass and thoracic endovascular aortic repair, successful hybrid endovascular management correlated with a favorable result in this emergency situation.

Placement of Endovascular Stent Graft in Acute Malperfusion Syndrome After Acute Type II Aortic Dissection

Acute malperfusion syndrome is a serious complication of acute aortic dissection. A 76-year-old female patient was admitted with acute type B aortic dissection and developed renal malperfusion during medical therapy. We are reporting a clinically successful result from the thoracic endovascular aortic repair used for malperfusion syndrome that occurred by acute type B aortic dissection.

Traumatic Right Pulmonary Artery Rupture after Accidentally Being Stepped on the Chest

Traumatic pulmonary artery rupture is a rare, life-threatening injury. Currently, no strict guidelines for its management exist. Herein, we report a successful surgical repair of a right pulmonary artery rupture caused by being stepped on.

Operative Management in a Patient with Scapulothoracic Bursitis

Scapulothoracic bursitis, an uncommon lesion, has been reported to be a painful disorder of scapulothoracic articulation. The articulation may become inflamed secondary to trauma when overused because of sports or work that requires repetitive or constant movement of the scapula against the posterior chest wall. The bursitis usually appears as a growing mass at the scapulothoracic interface and is often confused with a soft tissue tumor. We report on a patient with scapulothoracic bursitis who underwent surgical excision.

A Study of the Impact of COVID-19 Perceptions on the Relationship between Work-family Conflict, Depression, and Job Satisfaction among Married Women Worker

This study identified the causal relationships and pathways between changes in perceptions, work-family conflict, depression, and job satisfaction due to COVID-19 among married working women. The study used a secondary data analysis method to extract 1,973 wageearning married women employed in the workplace from 9,175 households that responded to the 8th Women’s Family Panel Survey (2020), and the statistics were analyzed using SPSS Statistics 26.6 and AMOS 21.0. The results showed that the change in the perception of corona preparedness among married women workers had a significant effect on depression and job satisfaction through work-family conflict, and that work-family conflict had a direct effect on job satisfaction and an indirect effect through depression. Based on these results, it is suggested that it is necessary to develop programs and policy measures to induce conflict mitigation and adaptive capacity improvement through perception change in order to improve the mental health and job satisfaction of married working women in the event of a crisis situation similar to COVID-19.

선교적 공동체로서 신학대학교 사회복지사역의 당위성과 역할 : ACTS의 지역사회 섬김을 위한 과제를 중심으로

When confessing that the Messianic community that has already begun but not yet completed is ACTS, the ACTS people, as members of the Messianic community, want to illuminate their relationships with the community and find alternative ways to contribute to the community and move together. To this end, this paper looks at the results of the corporate operation of the Healthy Family and Multicultural Family Support Center, a representative welfare program that has been conducted in the community for the past eight years. This is based on discussion concerning the identity of ACTS as a missionary church. This paper examines the demographic characteristics of Yangpyeong-gun where our school is located, It suggests the school should play social service role in the community based on regional characteristics and biblical justifications. The smallest groups ACTS should serve are classified as the elderly, the disabled, and multicultural families (especially migrant women, their children, and multicultural youths). These people are weak and their social status is vulnerable in Korean society. In addition, suggestions were made for the program. To prepare for a new era, and to take up the ministry of the church with the coming generations, it is obvious that it will be marked by many challenges, obstacles, and struggles. However, the church of Jesus Christ has a message of hope and life that the world has been waiting for and still waiting for. We should continue to work together for worship, praise, and training. But at any cost, they should be able to devote themselves to the scattering of the church into the world. Only then will the world meet Jesus through us and feel the power of that love. 이미 시작되었지만 아직 완성되지 않은 메시아적 공동체가 ACTS임을 고백할 때 ACTS인들은 메시아적 공동체의 일원으로 지역사회와의 관계를 조명하고, 지역사회에 어떻게 기여하고 함께 나아가야 할 것인가에 대한 대안을 찾고자 하는 것이 본 논문의 주제이다. 이를 위해 선교적 교회로서 ACTS의 정체성에 대한 논의를 바탕으로 지난 8년 동안 지역사회를 위해 수행했던 대표적인 복지프로그램인 건강가정·다문화가족지원센터 법인운영의 성과를 살펴보았다. 또한, 본교가 위치한 양평군의 인구통계학적 특성을 살펴본 후, 이러한 지역적 특성과 성서적 당위성에 근거하여, 앞으로 지역사회에 본교가 어떤 역할을 해야 할 것인지에 대한 제언을 하였다. 구체적으로 ACTS가 함께 해야 할 양평군의 지극히 작은 자들로 노인계층, 장애인계층, 그리고 다문화가정(특히 이주여성과 그들의 자녀인 다문화청소년들)으로 분류 하고 각각의 대상 집단에 대한 섬김의 성서적 근거와 함께 구체적 프로그램에 대한 제언을 하였다. ACTS가 섬겨야 할 지역사회의 작은 자들인 노인, 장애인, 이주민들은 약자들이며, 특히 한국 사회에서 그 사회적 지위가 취약한 계층이다. 새로운시대를 대비하며, 다가올 세대와 더불어 교회의 사역을 감당한다는 것은 참으로 많은 도전과 장애, 그리고 투쟁으로 점철될 것이 자명하다. 하지만, 예수그리스도의 교회는 세상이 기다려온, 그리고 지금도 기다리고 있는 희망과 생명의 메시지를 갖고 있다. 우리는 예배와 찬양과 훈련을 위해 함께 모이기에 계속 힘써야 할 것이다. 하지만 어떤 희생을 감수해서라도 교회가 세상으로 흩어져 나아가도록 스스로를 헌신할 수 있어야 한다. 오직 그러할 때세상은 우리를 통해 예수님을 만나게 되고, 그 사랑의 힘을 느낄 수 있을 것이기 때문이다.

It is this subject- pursuing a common territory between social service values and ideas of ACTS and those of professional social welfare- that the author explores in this article based on author`s review of Dr. Han Chul Ha`s book, "21st Century Mankind Dead or Alive The Only Way to Life." Reading Dr. Han`s book was unique experience to me in several aspects, particularly having a significant impact on my thoughts about professional social welfare, especially in the area of Christian Social Welfare. This article could be a starting point for the profession in order to find a clue to connect ACTS social service idea with that of Christian social welfare as one of meaningful efforts of pursuing ACTS theological synopsis. Today, no one argues that Korean churches have made gigantic numeral growth during the past century. However, trend has been stopped recently and even reverse mainly due to the lack of our passions and commitment as true christians with strong belief about the certainty of Christian truth, namely Theocentricism and Evangelicalism. As is apparent throughout Dr. Han`s book, one of the most imminent task which needs to be accomplished by today`s church members is "life with Bible". Life with the Bible challenges us to live according to what God told us to do. This challenge is not a new experience to our Christians. This is restoring the gospel of salvation in the Bible with sacrificial love until the day Jesus comes again. This challenge leads us to go back to the heart of the Bible, that is "love", specified and translated into two terms of love; love our Father and serve our neighbors. Serving our community has been one of the most important responsibilities which our churches and the people of God have been willing to take. Our churches have now reached a high-level stage of development in which there is more resources than needed for simple maintenance, and therefore more choices are available in extending our focus and devising more services for those in needs. The poor are not basically lazy, shiftless, and even sinful. They are our brothers and sisters who are equally blessed by God. Serving our neighbors is not one of many Christian virtues, but the important symbol of the Cross and love, command from God.
