        직무요구-자원모형을 기반으로 한 요양병원 간호사의 소진에 대한 영향요인 분석

        본 연구는 Xanthopoulou 등(2007)의 확장된 직무요구-자원모형을 기반으로 요양병원 간호사의 소진에 미치는 영향요인을 규명하기 위해 시도되었다. 요양병원 간호사 167명을 대상으로 설문지를 이용하여 2018년 8월 25일부터 2018년 9월 6일까지 자료를 수집하였다. 연구도구는 Maslach Burnout Inventory, Copenhagen Psyco-social Questionnaire version II, Connor-Davidson Resilience Scale 및 Thomas & Ganster (1995)가 개발한 도구를 사용하였다. SPSS WIN 23.0 프로그램을 이용하여 빈도, t-test, ANOVA, 피어슨 상관계수, 위계적 회귀분석으로 분석하였다. 연구결과 요양병원 간호사의 소진은 직무요구(r=.24, p=.002)와는 정적 상관관계를 보이고, 회복탄력성(r=-.43, p<.001)과 사회적지지(r=-.41, p<.001)와는 부적 상관관계를 보였다. 요양병원 간호사의 소진에 회복탄력성(β=-.28, p<.001), 사회적 지지(β=-.20, p=.016), 직무요구(β=.17, p=.016) 순으로 유의한 영향을 미쳤고, 이들 변수의 설명력은 32.0%로 나타났다. 따라서 요양병원 간호사의 소진 예방 및 감소를 위해 직무요구를 줄이고 회복탄력성과 사회적 지지를 향상시킬 수 있는 중재의 수행을 적극적으로 권장한다 This study was conducted to investigate the factors influencing burnout of nurses in long-term care hospitals based on the expanded JD-R model (Xanthopoulou et al, 2007). Participants were 167 nurses who worked in long-term care hospitals. The instruments used in this study were the Maslach Burnout Inventory, Copenhagen Psycho-social Questionnaire version II, Connor-Davidson Resilience Scale and the instrument developed by Thomas and Ganster (1995). Data were collected from August 25 to September 6, 2018 and analyzed based on frequency, t-tests, ANOVA, Pearson"s correlation, and hierarchical regression using the SPSS WIN 23.0 program. The results were as follows: Nurses" burnout in long-term care hospitals was positively correlated with job demands (r=.24, p=.002) and negatively correlated with resilience (r=-.43, p<.001) and social support (r=-.41, p<.001). Factors influencing the level of burnout were resilience (β=-.28, p<.001), social support (β =-.20, p=.016), and job demands (β=.17, p=.016) ), which together explained 32.0% of the total variance in burnout. Therefore, we strongly recommend interventions that reduce job demands and enhance resilience and social support to prevent and reduce the burnout of nurses in long-term care hospitals.

      • 국내 상급종합병원과 종합병원에서의 정맥주입간호실무지침에 대한 태도와 확산정도

        Purpose: This study was conducted to investigate the level of attitudes to and diffusion of Intravenous infusion nursing practice guideline in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 41 nurses who were in charge of guideline diffusion in 41 advanced general hospitals and general hospitals. Data were collected between September 25 and November 2, 2014 by mail (return rates: 68.3 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 18.0. Results: The average attitude score of guideline among nurses in charge of guideline diffusion were 3.98±0.38 (range 1~5). The average guideline diffusion score and levels of diffusion was 2.26±1.39 (level of “persuasion of nursing knowledge”) for staff nurses, 3.05±0.86 (level of “use sometimes”) for nurses in charge of guideline diffusion, 2.87±0.78 (level of “use sometimes”) for nurse directors. 41.4 % of nursing divisions in 41 hospitals reviewed the intravenous infusion nursing practice guideline, educated the guideline to nurses, and implemented the guideline in clinical practice for guideline diffusion. Conclusion: Study findings indicate that nurses in charge of intravenous infusion nursing practice guideline in hospitals had positive attitude for guideline. The levels of guideline diffusion in hospitals was “persuasion of nursing knowledge” to “use sometimes”. Therefore, the strategies for promoting the diffusion and implementation of guideline in clinical practice in the level of nursing division is needed. Further study of outcome evaluation of guideline implementation will be needed.

        Purpose: This study was conducted to investigate the extent of diffusion of Intravenous infusion nursing practice guideline among nurses in advanced general hospitals and general hospitals in Korea. Methods: The subjects were 234 nurses who practice the intravenous infusion in 24 advanced general hospitals and general hospitals. Data were collected between October 5 and November 2, 2015 by mail (return rates: 97.5 %). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/ WIN 21. Results: The average guideline diffusion score and levels of diffusion was 3.39±0.58 (level of “use sometimes”). 44 recommendations (46%) were in level of “use always” and 50 recommendations (53%) were in “use sometimes”. Extent of diffusion were significantly different according to present status (F=2.81, p=.040) and education (F=4.35, p=.014). The facilitating factors to use the guideline were education by department of nursing service, convenient composition of guideline and barrier factors were “no time to use the guideline”, “don’t know the guideline” and “there is no guideline at ward”. Conclusion: Extent of diffusion of Intravenous infusion nursing practice guideline among nurses was moderate level. The strategies for promoting the implementation of guideline in clinical practice is needed. Further study of outcome evaluation of guideline implementation will be needed.

        자기통제의 개념 분석

        Today human needs for health promotion and maintenance are increasing. The nursing profession as a great responsibility to develop the knowledge for helping clients to practice good health behavior under self-control. But there are few studies about the concept of self-control, or the operational definition of self-control. The purpose of this study was to analyze and clarify the meaning of the concept of self-control. This study used Walker and Avant's process of concept analysis. Antecedents of self control consist of 1) perception of the conflict situation to change his or her behavior ; 2) perception of self as the causes of the barrier for a certain behavior ; 3) internal standard or belief such as self-efficacy and learned resourcefulness ; 4) internal motivation for change of behavior. Critical attributes of self-control were 1) goal-orientation ; 2) self-decision ; 3) time sequence ; 4) effort ; 5) possibility of being learned ; 6) individuality ; 7) social desirability. Consequences occurring as a results of self-control consist of 1) achievement of the controlled behavior whose purposes were planned ; 2) enhancement of the internal standard such as self-efficacy and learned resourcefulness ; 3) eventual elevation of physical and emotional wellness. Therefore, this concept is defined as a cognitive behavior which is attempted by his or her decision when one is confronted with the conflict situation, which is characterized by being changed by time, learned by effort and individual characteristics. One is taking such behavior with the perception of self as the causative barrier for a certain behavior, planning a specific objective for a certain behavior which demands self sacrifice, supplies the internal reward, is socially desirable with the internal standard of self-efficacy and learned resourcefulness and environmental factors.

        국내 근거기반 임상간호실무지침의 주제 선정

        Purpose: This study was to identify topics for evidence-based clinical nursing practice guidelines in Korea. Methods: Data were collected from 330 staff nurses from 10 general hospitals and 53 nurses in charge of nursing education in 110 hospitals with over 500 beds. Using open questions, the nurses identified activities which could not be verified, which lacked consistency among nurses, clinical units and/or hospitals, which were not based on the up-to-date knowledge and which needed reform. The data were analysed by content analysis using a qualitative methodology. Results: Collected data consisted of 1882 clinical topics, which were classified into 50 topics, 207 mid-categories, and 456 sub-categories. The most frequent topics in order of frequency were medications, central line management, intravenous injections, urinary catheterization, perioperative nursing care, skin tests, pressure ulcer care, blood transfusions, laboratory examination-culture, respiratory care which were performed routinely in clinical setting by staff nurses. Conclusion: The research findings indicate the urgent need to develop evidence-based clinical nursing practice guidelines related to these research findings. Further research is needed to identify topics related to health promotion, and symptom/management of health problem.

        Today human needs for health promotion and maintenance are increasing. The nursing progression has a great responsibility for helping clients to practice good health behavior. self efficacy was found to be potent predictor in initiating and maintaining the health-related behavior. Therefore, the concept is important in nursig intervention for change of health behavior. The purpose of this paper was to analyze and clarify the meaning of the concept, self efficacy. This study used Walder & Avant's process of concept analysis. Critical attributes of self efficacy were : 1) positive perception of his won capability; 2) intrapersonal strength; 3) possibility of being learned; 4) individuality; 5) specificity; 6) generalizability. Antecedents of self efficacy consist of those facts that 1) new situation occurs; 2) individuals have needs to change the new situation; 3) there are some required actions in new situation. Consequences occurring as a result of self efficacy consist of those facts that 1) individual initiates the of failure; 5) cope with the situation; 6) controls the situation; 7) the level of self efficacy is enhanced; 8) the level of self-esteem is enhanced.
