        대형 국가 R&D 프로젝트의 기술관리 프로세스 : 파이로 프로젝트를 중심으로

        The Pyro project, one of the large national R&D project to construct Korea Advanced Pyroprocessing Facility (KAPF), which has many goals such as development of pyro technology and process equipment, design of equipment and facility, construction, and test operation, is now under research and development. To reduce uncertainty and risk of such complex project, the technical management processes in systems engineering standards and NASA handbook were reviewed, and then the ten common technical management processes were selected for the large national R&D project to meet its goal successfully. And the essential technical management processes were finally suggested for Pyro project in consideration of current situation of the project.

        선진 핵연료주기 기술 개발을 위한 핵연료주기 분석 기술

        핵연료주기 분석 연구는 핵연료주기 단계에서 기술들을 분석하고 요건들을 도출하여 국가적 핵연료주기 정책 설정 및 추진을 체계적으로 수행하기 위한 연구이다. 시스템 분석 기술은 대상 시스템의 비교 분석 평가에 활용되며 핵연료주기를 대상으로 하는 경우 각 국가 또는 관심 범위에 따라 다양한 방법이 사용된다. 본 연구에서는 국내 선진 핵연료주기 개발을 위해 필요한 핵연료주기 분석 전략과 함께 이를 위해 사용될 수 있는 분석 기술들을 제시하였다. 핵연료주기 분석은 전략적으로 기술적 분석, 국내외 이해관계, 국가 에너지 프로그램과 연계되어야 한다. 이를 위해 다양한 핵연료주기를 비교하여 제시된 평가 지표에 따라 분석하는 연구는 트레이드 연구 방법을 적용하여 수행할 수 있다. 본 연구를 통한 조사 분석 결과 핵연료주기 분석 전략과 함께 방법적 측면에서 트레이드 연구가 선진 핵연료주기 도출에 활용될 수 있을 것으로 파악되었다. 트레이드 연구에 필수적인 평가지표를 선정하고 각 지표별 핵연료주기에 대한 정보를 얻기 위해서는 기술성숙도 분석 방법과 핵연료주기 시뮬레이터를 활용할 수 있을 것으로 제시하였다. 이들은 핵연료주기의 기술성, 경제성, 환경영향성 등을 비교 평가하여 기술개발을 위한 방향을 제시하고 체계적인 선진 핵연료주기 도출 및 실현에 기여할 것이다. The nuclear fuel cycle (NFC) analysis is a study to set a NFC policy and to promote systematic researches by analyzing technologies and deriving requirements at each stage of a fuel cycle. System analysis techniques are utilized for comparative analysis and assessment of options on a considered system. In case that NFC is taken into consideration various methods of the system analysis techniques could be applied depending on the range of an interest. This study presented NFC analysis strategies for the development of a domestic advanced NFC and analysis techniques applicable to different phases of the analysis. Strategically, NFC analysis necessitates the linkage with technology analyses, domestic and international interests, and a national energy program. In this respect, a trade-off study is readily applicable since it includes various aspects on NFC as metrics and then analyzes the considered NFC options according to the derived metrics. In this study, the trade-off study was identified as a method for NFC analysis with the derived strategies and it was expected to be used for development of an advanced NFC. A technology readiness level (TRL) method and NFC simulation codes could be utilized to obtain the required metrics and data for assessment in the trade-off study. The methodologies would guide a direction of technology development by comparing and assessing technological, economical, environmental, and other aspects on the alternatives. Consequently, they would contribute for systematic development and deployment of an appropriate advanced NFC.

        파이로 프로젝트에 프로젝트 관리 적용 방안

        Pyro project, one of complex project, is now under research and development. To manage this kind project successfully, it is essential to apply the processes of technical management (TM) based on systems engineering (SE) and project management (PM). In this paper, the project management processes from ISO-21500 standard, PMBOK guide, and PRINCE2 guide were reviewed, and then some common PM for the large national R&D project were selected. An integrated technical management and project management frame work was finally established and suggested for Pyro project.

        A case report of a post-traumatic syringomyelia in a 29-year old male, developed 15 years after an L1 burst fracture, is presented. On preoperative MRI, the syrinx extended cephalad above the fracture site through the whole thoracic and cervical cord. Serial myelo-CT was performed to evaluate the dynamics of CSF. It was managed by lysis of the arachnoid adhesions, syringosubarachnoid shunt, and expansile duraplasty. After surgery, the patient's symptoms improved, and marked decrease of the syrinx was seen on postoperative MRI. The pathophysiology, the role of preoperative diagnostic methods especially serial myelo-CT, and the contmporary management modalities for posttraumatic syringomyelia is reviewed along with the pertinent literature.

        건강보험심사평가원 데이터의 분석을 통한 체성기능부전 환자의 특성 및 M99 진단명의 사용현황 분석

        Objectives : The aim of this study is to assess the usage of diagnosis codes for somatic dysfunctions and the general characteristics of patients diagnosed with the code, by analyzing health insurance data provided by the Health Insurance Review & Assessment Service(HIRA) of Korea. This investigation is intended to outline future and willing to contribute to further use of diagnosis code and the approach of Oriental Medicine to somatic dysfunction. Materials and Methods : By analyzing HIRA data, those diagnosed with M99 codes, a code attributed to somatic dysfunction, were selected for analysis. Patients included were assessed for the relevant general characteristics, and the specific diagnostic criteria. The current usage rates and noteworthy characteristics of diagnostic codes of somatic dysfunctions were assessed. A comparative analysis between clinical departments and subcategories, and a comparative analysis to data of 2014 was conducted. Results : Patients given M99 codes constituted a small minority of all patients diagnosed in 2011 as shown by HIRA data. The codes were more frequently to older patients, females, outpatients, and those who filed for Health Insurance compensation. Medical institutions participating in the diagnosis were mostly primary care facilities, usually specializing in orthopedic(Western medicine sector) and internal medicine (Oriental Medicine sector). The most registered code in 2011 and 2014 was M995. The same trend can be observed in Oriental/Western medicine institutions and Public health center, on the other hand, between them, have some different patterns both 2nd and 3rd. Conclusions : This investigation is that of current usage of diagnostic codes of somatic dysfunction. HIRA insurance claim data was analyzed. Based on the current results, more precise diagnostic standards of somatic dysfunction are warranted. This study will provide a foundation for future Oriental Medicine approach to somatic dysfunctions.

        건선 및 건선관절염에서 손톱 병변에 대한 임상적 고찰

        Background: It has been reported that nail psoriasis is common in psoriatic arthritis (PsA) patients, however, studies on the prevalence of PsA with nail psoriasis and the relationship between the severity of the diseases has not previously been reported in Korea. Objective: The aim of this study was to investigate nail psoriasis and its relation to psoriasis and PsA. Methods: After researching past medical records, 48 patients with nail psoriasis and 247 patients without nail psoriasis were enrolled in this study. Nail psoriasis was classified according to the morphologic types of nails and severity was evaluated using the modified nail psoriasis severity score index (mNAPSI). We assessed the severity of psoriasis and PsA by psoriasis area and severity index (PASI), and swollen and tender joint counts (STJC), respectively. Finally, we analyzed the relationship between mNAPSI, PASI, and STJC. Results: Nail involvement was observed in 48 (16.3%) of 295 patients. Prevalence of PsA was higher in nail psoriasis (31.3%) than in uncomplicated psoriasis (9.3%, p<0.001). mNAPSI showed a positive correlation with STJC (p=0.002, Pearson correlation=0.427), but not with PASI (p=0.094, Pearson correlation=0.202). Conclusion: The results show that PsA is more common and severe in psoriatic patients with nail involvement than in uncomplicated psoriasis. Clinicians should be alert to nail involvement in psoriatic patients, as it could be related to the disease course and prognosis of PsA. (Korean J Dermatol 2016;54(3):178~183)

        보관조건에 따른 당귀수산의 성분 변화

        Objectives Danggwisu-san (Dangguixu-san) is a herbal prescription frequently used to treat pain or swelling caused by contusion. To determine the expiration period through scientific methodology, stability of Danggwisu-san (Dangguixu-san) water extract, a herbal medicine, was examined under various storage conditions and periods. Methods Danggwisu-san (Dangguixu-san) was stored either at room temperature (23±2℃), under a refrigerating condition (4℃) or under a freezing condition (-18±2℃) for 0, 1, 2, 3 or 4 weeks and then freeze-dried. Total phenol and total flavonoid amounts were investigated; contents of amygdalin (Prunus persica), paeoniflorin (Paeonia lactiflora), and glycyrrhizin (Glycyrrhiza uralensis) - the marker compounds of Danggwisu-san (Dangguixu-san) - were also analyzed through high performance liquid chromatography (HPLC). Results No significant change in total phenol and total flavonoid amounts was observed under the indicated storage conditions. Moreover, the contents of marker compounds, i.e. amygdalin, paeoniflorin, and glycyrrhizin, did not alter significantly under the indicated conditions, as well. Conclusions Danggwisu-san (Dangguixu-san) was found to be stable up until 4 weeks under the indicated conditions. Further studies on efficacy and long-term stability are warranted to establish the expiration period of Danggwisu-san (Dangguixu-san). (J Korean Med Rehabil 2016;26(4):1-8)
