교통사고로 한방병원에 내원한 근골격계 환자 2,048명에 대한 임상적 분석

Objectives : The purpose of this study was to investigate the characteristics and visiting patterns of traffic accident outpatients Methods : In this study, we reviewed the medical charts of 2,048 traffic accident patients who visited Jaseng Hospital of Korean Medicine from January 1st, 2012 to December 31st, 2012. Results : In the distribution of gender and of age patients, the male percentage was 52.3% and the majority of patients were in their thirties(47.2%). In the distribution of the patient's initial visit, most patients visited our clinic from Monday to Wednesday. By monthly distribution, the more patients visited the clinic at the latter half of the year. In the duration of treatment, 1,389 patients(67.8%) finished treatment within four weeks. The most frequently cited sites of pain were neck(82.0%), followed closely by low back(74.0%). In the access route, 746 patients(36.4%) visited our traffic accident clinic as a first choice for primary treatment. We referred patients for radiologic examination in 159 patients(7.9%), of which the exams were mainly lumbar spine MRIs(3.6%) and cervical spine MRIs(2.8%). The most frequent diagnosis were herniated nucleus pulposus. Conclusions : This study shows that most of the patients who visited the traffic accident clinic of a Korean Medical Hospital presented neck and low back pain, and the majority showed improvement without surgical treatment. Following the increasing minor injury rate caused by traffic accidents, we expect the role of Korean Medicine Hospital to become more prominent.

Objectives Danggwisu-san (Dangguixu-san) is a herbal prescription frequently used to treat pain or swelling caused by contusion. To determine the expiration period through scientific methodology, stability of Danggwisu-san (Dangguixu-san) water extract, a herbal medicine, was examined under various storage conditions and periods. Methods Danggwisu-san (Dangguixu-san) was stored either at room temperature (23±2℃), under a refrigerating condition (4℃) or under a freezing condition (-18±2℃) for 0, 1, 2, 3 or 4 weeks and then freeze-dried. Total phenol and total flavonoid amounts were investigated; contents of amygdalin (Prunus persica), paeoniflorin (Paeonia lactiflora), and glycyrrhizin (Glycyrrhiza uralensis) - the marker compounds of Danggwisu-san (Dangguixu-san) - were also analyzed through high performance liquid chromatography (HPLC). Results No significant change in total phenol and total flavonoid amounts was observed under the indicated storage conditions. Moreover, the contents of marker compounds, i.e. amygdalin, paeoniflorin, and glycyrrhizin, did not alter significantly under the indicated conditions, as well. Conclusions Danggwisu-san (Dangguixu-san) was found to be stable up until 4 weeks under the indicated conditions. Further studies on efficacy and long-term stability are warranted to establish the expiration period of Danggwisu-san (Dangguixu-san). (J Korean Med Rehabil 2016;26(4):1-8)
건강보험심사평가원 데이터의 분석을 통한 체성기능부전 환자의 특성 및 M99 진단명의 사용현황 분석

Objectives : The aim of this study is to assess the usage of diagnosis codes for somatic dysfunctions and the general characteristics of patients diagnosed with the code, by analyzing health insurance data provided by the Health Insurance Review & Assessment Service(HIRA) of Korea. This investigation is intended to outline future and willing to contribute to further use of diagnosis code and the approach of Oriental Medicine to somatic dysfunction. Materials and Methods : By analyzing HIRA data, those diagnosed with M99 codes, a code attributed to somatic dysfunction, were selected for analysis. Patients included were assessed for the relevant general characteristics, and the specific diagnostic criteria. The current usage rates and noteworthy characteristics of diagnostic codes of somatic dysfunctions were assessed. A comparative analysis between clinical departments and subcategories, and a comparative analysis to data of 2014 was conducted. Results : Patients given M99 codes constituted a small minority of all patients diagnosed in 2011 as shown by HIRA data. The codes were more frequently to older patients, females, outpatients, and those who filed for Health Insurance compensation. Medical institutions participating in the diagnosis were mostly primary care facilities, usually specializing in orthopedic(Western medicine sector) and internal medicine (Oriental Medicine sector). The most registered code in 2011 and 2014 was M995. The same trend can be observed in Oriental/Western medicine institutions and Public health center, on the other hand, between them, have some different patterns both 2nd and 3rd. Conclusions : This investigation is that of current usage of diagnostic codes of somatic dysfunction. HIRA insurance claim data was analyzed. Based on the current results, more precise diagnostic standards of somatic dysfunction are warranted. This study will provide a foundation for future Oriental Medicine approach to somatic dysfunctions.

Objective : To report a case of cervical block vertebra with neck pain successfully treated with conservative Korean Traditional Medicine treatment with Thoracic Chuna Manipulation. Methods : A patient diagnosed with block vertebra was treated with Thoracic Chuna manipulation, acupuncture, pharmacoacupuncture, and herbal medicine. Numeric Rating Scale(NRS) and Neck Disability Index(NDI) scores were collected before and after treatment for comparison. Results : There was a significant decrease in NRS and NDI scores. The mean NRS score decreased from 8 to 1, and NDI from 50 to 8. Conclusion : Conservative Korean Traditional Medicine treatment appears to be effective for treatment of cervical block vertebra.

Objectives This study is a report on the treatment effects of myofascial release pharmacopuncture in for patients with lumbar herniated intervertebral disc. Methods Four patients diagnosed with lumbar herniated intervertebral disc were treated with myofascial release pharmacopuncture. Treatment effects were measured using the numeric rating scale(NRS) and Oswestry disability index(ODI). Results All patients who received myofascial release pharmacopuncture treatment reported an improvement in their symptoms. There was a significant change in the NRS and ODI scores before and after treatment. Conclusions The results suggest that myofascial release pharmacopuncture can be an effective treatment for lumbar herniated intervertebral disc. However, further research is needed.
