      • Dermatophagoides farinae 자극에 대한 알레르기 천식 환아에서의 Th 및 Tc 림프구 반응

        연구목적: 본 연구에서는 Dermatophagoided farninae(Df)에 알레르기를 나타내는 소아 천식 환아들의 말초혈액 단핵세포를 여러 농도의 Df 존재 하에서 배향한 후 T helper(Th) 및 T cytotoxic(Tc) 림프구 반응을 관찰하여 Df 농도 변화에 따른 Th 및 Tc 림프구 반응의 변화를 관찰하고 면역요법 시행 유무에 따른 Th alc Tc 림프구 반응의 차이를 단일 세포 수준에서 관찰하고자 하였다. 대상 및 방법: 천식 증상으로 충북대학교 소아과 알레르기 클리닉을 방문하여 피부 단자 검사상 Df에 강양성(4±)을 나타낸 10 명의 소아 알레르기 천식 환자를 대상으로 하였다. 8세에서 11세 사이의 소아만을 연구에 포함시켰으며, 이 중 소아 5명은 최소 2년 동안 면역요법을 시행 받은 환자들이었다. 환아들로부터 말초혈액 단핵세로를 분리하여 각각 0, 0.1, 1, 10 ㎍/㎖의 Df 존재 하에서 5일 동안 배양한 후 유세포 분석을 실시하여 Th 및 Tc 림프구의 빈도를 관찰하였으며, 이를 이용하여 Th2/Th1 및 Tc2/Tcl 비율을 계산하였다. 결과: 모든 환아에서 Df 자극에 의한 우월한 Th2 림프구의 빈도를 관찰하였으며, 이를 이용하여 Th2/Th1 및 Tc2/Tcl 비율의 증가가 관찰되지 않았으나 10㎍/㎖의 Df 농도에서는 Tc2/Tcl 비율 증가에 비해 Df 자극에 의한 Tc2/Tc1 비율의 증가가 상대적으로 적었으며, 모든 환아들에서 0.1 및 1 ㎍/㎖의 Df농도에서는 현저한 Tc2/Tc1 비율의 증가가 관찰되었다. 면역치료를 시행받지 환아들에 비해 면역치료를 시행 받은 환아들에서 Th2/Tc1 비율의 증가가 관찰되지 않았으나 10㎍/㎖의 Df농도에서는 현저한 Tc2/Tc1 비율의 증가가 관찰되었다. 면역치료를 시행받지 환아들에 비해 면역치료를 시행 받지 환아들에 비해 면역치료를 시행 받은 환아들에서 Th2/Th1 비율 및 Tc2/Tc1 비율이 상대적으로 낮았다. 결론: Tc 림프구는 Th 림프구에 비해 IL-4 생성을 유도하는 Df 자극에 저항성을 나타냈으며, 알레르기 면역치료를 시행받지 않은 환아들에 비해 면역치료를 시행 받은 환아들의 Th 및 Tc 림프구는 IL-4 생성을 유도하는 Df 자극에 저항성을 나타낸다. Purpose : The objectives of the present' study were to demonstrate Th and Tc lymphocyte responses to graded doses of house dust mite (HDM) in HDM allergic child patients and to determine differences in the HDM-induced Th and Tc lymphocyte responses between untreated and treated patients. Materials and Methods : This study included ten Dermatophagoides farinae (Df)-sensitive asthmatics of which five children received immunotherapy to Df over a 2-year period. Peripheral blood mononuclear cells ( PBMC) were isolated from peripheral blood of each patient and cultured in the presence of graded doses of Df extract (0, 0.1, 1, 10 ㎍/㎖) 5 days. After harvest of cultured cells, Th and Tc cell frequencies were determined by a multiparameter cytosine flow cytometric assay that allows simultaneous determination of intracellular IFN-γ and IL-4 in CD4+ or CD8+ cells. Results : Predominant Th2 responses to 8f were observed in all of the patients, especially at ㎍/㎖ of .Df Dose-dependent pattern in 8f-induced increases of Th2/Thl ratio was observed in three of untreated and one of treated patients. Increases in Tc2/Tcl ratio were less prominent than those in Th2/Thl ratio. There were no significant increases of Tc2/Tcl ratio at low concentration of Df, which were observed only at 10 ㎍/㎖ of Df. Df-induced increases of Th2/Th1 and Tc2/Tc1 ratios were less remarkable in treated than untreated patients. Conclusion : Tc Iymphocytes were more resistant to Df-induced stimulation to produce IL-4 than Th lymphocytes. Th and Tc lymphocytes of immunotherapy treated patients were more tolerant to the effect of Df to produce IL-4 than untreated patients.

        한국어 판 구조화 임상면담도구 개발 : 신뢰도 연구

        연구목적: 정신장애의 정확한 진단과 평가는 임상에서뿐만 아니라 신뢰성 있는 연구를 위해서도 매우 중요하다. Structured Clinical Interview for DSM-IV(SCID)는 임상전문가들이 사용하는 진단도구로서, 비교적 짧은 시간에 정확한 진단을 내릴 수 있다. 본 연구는 한국어판SCID를 개발하고 그 신뢰도를 평가하기 위해 수행되었다. 방법: 국문학자가 포함된 번역위원회를 통하여 연구용판 SCID를 번역한 후, 정신과 의사 2인과 임상심리학자 1인에게 한국어판SCID 실시방법을 교육시킨 뒤 한국어판SCID를 이용한 면담의 평가자간 신뢰도 (interrater reliability)를 검증하였다. 면담 대상은 1999년 2월에서 3월까지 2개 병원 정신과에 치료중인 환자 90명(남:41명, 여:29명)이었다. 결과: 현재 장애(current disorder) 평가에서 주요 우울장애, 기분부전장애, 정신분열병, 알코올 남용 및 의존, 기타 물질 남용 또는 의존, 여러 불안장애들 및 섭식장애 등과 같은 대부분 장애의 kappa값은 .70이상으로 매우 높았다. 그 이외의 양극성 장애, 망상장애, 광장공포증, 감별 불능 신체화 장애 및 건강염려증의 kappa값도 .69에서 .40사이로 수용할 수 있는 정도였다. 평생 장애(lifetime disorder)에서는 양극성 장애(k=.69)와 감별 불능 신체화장애(k=.59)를 제외한 다른 모든 장애의 kappa값이 .70이상이었다. K-SCID 면담시 Ⅰ축 질환에 대하여 흔하게 다중 진단이 내려졌으며, 평균 진단 수는 1.5∼1.7개로 나타났다. 결론: 한국어판 SCID는 신뢰도가 높은 진단도구로 생각되며, 향후 정신질환의 정확한 진단과 임상연구에 유용하게 사용될 수 있다. Objectives: Accurate diagnosis and assessment for psychiatric disorders is crucial for research, as well as for clinical practice. Structured Clinical Interview for DSM-Ⅳ(SCID-RV) is a less time-consumimg and more accurate structured diagnostic interview form. It can be used by clinical professions and is known for a reliable diagnostic tool. Present study was conducted to develop Korean version of SCID-RV and to test the inter-rater reliability. Methods: The authors have translated original SCID-RV into Korean, and revised in parallel with sociocultural background of Korea. Ninety patients from two psychiatric hospitals, both outpatient and inpatient, were interviewed and rated independently by three raters. Results: The kappa coefficients for most of illnesses, such as major depressive disorder, dysthymia, schizophrenia, alcohol abuse and dependency, anxiety disorder and eating disorder were excellent(>0.70) in the evaluation of current disorders. And the kappa coefficients for bipolar disorder, delusional disorder, agoraphobia, undifferentiated somatoform disorder, and hypochondriasis were acceptable(>0.40) in the evaluation of current disorders. In the evalua-tion of lifetime disorders, the concordant rates of all the diagnoses except bipolar disorder and undifferentiated somatoform disorder were excellent. Lack of hierarchy in DSM-Ⅳ allows for multiple Axis I diagnoses. Mean numbers of Axis I diagnoses per subject assigned by the three raters were 1.5-1.7. Conclusions: Our findings confirm that SCID-RV yields highly reliable diagnoses. SCID-RV is recommended for accurate diagnosis in clinical practice and research on psychiatric disorders.

        Intentional Replantation에 의한 상악 제 1 소구치 치료의 증례보고

        Intentional replantation of the posterior teeth has been performed as a last resort except extraction when it is impossible to perform the conventional endodontic or surgical endodontic treatment. Many studies have done with the prognosis, and root resorption, of which the responsibility might have been injury of periodontal ligament and remaining periodontal lesion, proved to be major cause of failure. Intentional replantation, however, can exclude anatomic difficulty, risk of nerve injury, and maxillary sinus trauma that surgical access can raise, especially in posterior teeth, and it is reported that success rate is not less than surgical endodontics, independent of replantation of traumatically avulsed teeth. The success of intentional replantation can be evaluated with, clinically, ruduction of pocket depth, pain and mobility(physical mobility), and, radiographically, hard tissue formation, reduction of periapical radiolucency. In this case report, for the maxillary left first bicuspid with dens evaginatus that had periapical lesion, painfulness, mobility and showed no reduction of mobility after even extirpation and a number of irrigation, extraction followed by extraoral root filling with calcium hydroxide and replantation was performed. Based on many studies, it is concluded that hard tissue formation capacity of calcium hydroxide, preceded exterpation before extraction, and careful extraction without injuring periodontal ligament affected successful results of this case.

        Li-ion Conducting Li0.35La0.55TiO3 Electrolyte Thick Films Fabricated by Aerosol Deposition

        Lithium lanthanum titanium oxide (Li0.35La0.55TiO3, LLT) films with thickness of 5 30 μm were fabricated on a stainless-steel substrates by using aerosol deposition with a micron-sized powder at room temperature, and their Li-ion conductivity values were analyzed and compared with bulk ceramics sintered using the same powder. The crystalline size of the film was controlled by controlling the initial particle size of the LLT powder. The phase formation and the microstructural evolution of the films for different deposition conditions were observed by X-ray diffraction and electron microscopy. The lithium-ion conductivity of the film at room temperature was analyzed by the impedance measurement technique. The LLT bulk ceramic sintered at 1200 C and LLT film deposited at room-temperature showed total lithium ion conductivity of 8.37 × 10−6 and 6.38 × 10−7 S/cm, respectively.

        ACTG2 Variants in Pediatric Chronic Intestinal Pseudo-obstruction With Megacystis

        Background/Aims Chronic intestinal pseudo-obstruction (CIPO) is a clinically heterogeneous syndrome characterized by compromised peristalsis and intestinal obstruction. Variants of actin gamma 2 (ACTG2), a protein crucial for correct enteric muscle contraction, have been found in CIPO patients. The aim of this study is to examine the clinical features and ACTG2 variants in Korean patients with CIPO. Methods From January 1995 to August 2020, 12 patients diagnosed with CIPO were included and genetic analysis testing of ACTG2 was performed. Results Heterozygous ACTG2 missense variants were found in 6 patients (50.0%). The p.Arg257Cys variant was found in 3 patients, and p.Arg63Gln and p.Arg178His variants were found in 1 patient each. A novel variant, p.Ile193Phe, was found in 1 patient. Three patients were diagnosed at birth, 2 at the age of 1 year, and 1 at 3 years of age. Abnormal prenatal genitourinary ultrasonographic findings were found in all 6 patients; microcolon was found in 4 patients (66.7%), and megacystis in all 6 patients. The pathology showed abnormal ganglion cells as well as myopathic findings. All patients are dependent on total parenteral nutrition and are to date alive. Conclusions ACTG2 variants are commonly found in Korean patients with CIPO. In CIPO patients with megacystis and abnormal prenatal ultrasonography, genetic testing of ACTG2 should be considered. Molecular diagnosis of CIPO is more important than pathologic diagnosis. (J Neurogastroenterol Motil 2022;28:104-110)

      • Fabrication and characterization of aluminum nitride thick film coated on aluminum substrate for heat dissipation

        <P><B>Abstract</B></P> <P>For effective heat dissipation in high-power LED applications, aluminum nitride (AlN) thick films as thermally conductive dielectric layers were developed, which were deposited on an Al substrate by aerosol deposition (AD). The aerosol-deposited AlN thick films on Al substrates have advantages over conventional polymer-based dielectric substrates or ceramic substrate mounted heatsink systems including an epoxy adhesive, such as excellent heat dissipation capacity and low thermal resistance. AD is an effective method to fabricate high-quality AlN thick film without the Al<SUB>2</SUB>O<SUB>3</SUB> phase because the film is formed at room temperature. Highly dense and well-adhered, pure AlN thick films with thicknesses up to 30µm were deposited on an Al substrate. AlN-Al<SUB>2</SUB>O<SUB>3</SUB> and AlN-polyvinylidene fluoride (PVDF) composite films were also deposited on an Al substrate in order to investigate the effect of Al<SUB>2</SUB>O<SUB>3</SUB> and polymer on the microstructure and thermal properties. Among the films, pure AlN thick film exhibited the highest dielectric strength, the highest thermal conductivity, and the lowest thermal resistance. Therefore, it can be expected that the aerosol-deposited AlN thick film on Al substrate could be used as a powerful heatsink.</P>

      • Effect of fluorine addition on the biological performance of hydroxyapatite coatings on Ti by aerosol deposition.

        <P>Dense and well-adherent fluoridated hydroxyapatite [Ca(10)(PO(4))(6)(OH)(2-x )F( x ), FHA] coatings with various amounts of fluorine contents (x?=?0, 0.5, 1.0, 1.5, and 2.0) were deposited on commercially available pure titanium by aerosol deposition using FHA powders in order to investigate the effect of fluorine content on the properties of the coatings. FHA powders with different compositions were synthesized by solid-state reactions of hydroxyapatite (HA) and fluorapatite (FA) powders at various ratios. X-ray diffraction and Fourier transform infrared spectroscopy results showed that fluoride ions were successfully incorporated into the HA lattice for both the FHA powders and the FHA coatings. Scanning electron microscopy analysis revealed dense microstructures and good substrate adhesion of the coatings with high adhesion strengths of more than 33.1?MPa. The dissolution behavior in a tris-buffered saline solution indicated that the dissolution rate of the FHA coatings decreased as a result of increasing the fluorine content in the coatings. In addition, in vitro cellular tests, including cell attachment, proliferation, and alkaline phosphatase activity of MC3T3-E1 preosteoblast cells grown on the coatings, demonstrated that an FHA coating with a moderate degree of F(-) substitution, x?=?1.0, had a stronger stimulating effect on cell proliferation and differentiation. These results suggested that there exists an optimum fluorine content level in the FHA coatings for the best long-term stability and cellular responses.</P>

        Evaluation of Cluster-Based System for the OLTP Application

        In this paper, we have modeled and evaluated a new parallel processing system called Scalable Parallel computer Architecture based on Xbar (SPAX) for commercial applications. SMP systems are widely used as servers for commercial applications; however, they have very limited scalability. SPAX cost-effectively overcomes the SMP limitation by providing both scalability and application portability. To investigate whether the new architecture satisfies the requirements of commercial applications, we have built a system model and a workload model. The results of the simulation study show that the I/O subsystem becomes the major bottleneck. We found that SPAX can still meet the I/O requirement of the OLTP workload as it supports flexible I/O subsystem. We also investigated what will be the next most important bottleneck in SPAX and how to remove it. We found that the newly developed system network called Xcent-Net will not be a bottleneck in the I/O data path. We also show the optimal configuration that is to be considered for system tuning.

        Intraventricular Vancomycin Therapy for Intractable Bacillus cereus Ventriculitis

        Bacillus cereus causes serious central nervous system infections, especially in immunocompromised patients. Successful treatment requires adequate antimicrobial concentrations in the cerebrospinal fluid; however, in some cases, achieving this with systemic treatment alone is difficult. We treated intractable B. cereus ventriculitis with intraventricular vancomycin, with no major adverse events.
