        姜英豪,金仁達 서울大學校 保健大學院 1968 公衆保健雜誌 Vol.5 No.2

A study was conducted by the author in September 1968 towards the infants and young children in some underprivileged rural villages of Korea by employing such methods as clinical assessment, nutritional biometry, and biochemical tests, in an attempt to evaluate their nutritional status with possible reference to protein calorie malnutrition (PCM). The infants and young children examined totaled 232 of whom 111 were boys and 121 were girls. The results are as follows: Ⅰ. Clinical Assessment The prevalence rates of the signs suggestive of PCM turned out to be as below : 1) Cases with dyspigmented hair were observed with 4.5% of the 45 infants examined. and 18.8% of the 187 young children examined. Cases with thin and sparse hair were seen with 22.8% of the infants, 14.0% of the young children, and l5.2% of both the infants and young children. 2) Prevalence of the pale conjunctiva rated 28.6% of the infants. 32.3% of the young children, and 30.4% of both the infants and young children. 3) Keratomalacia cases were found in 0.5% of the young children only and they corresponded, in turn, to 0.4% of the whole examined. 4) Cases with angulostomatitis rated 14.0% of the young children only, who constituted 10.0% of the total examined. 5) Those with moon-face were observed only with 2.3% of the infants, rating 0.4% of the total. 6) Cases with flaky-paint dermatosis were observed with 2.3% of the infants, 0.5% of the young children, and 0.9% of both the infants and young children. 7) Those who were found to have such skin lesions as fissure of the ear lobe, tender sores, or moist groin rash rated 15.9% of the infants, 20.4% of the young children, and 19.6% of both the infants and children. 8) Rate of those with muscles wasted was 11.4% of the infants, 25.8% of the young children, and 23.0% of both the infants and young children. 9) Those with edema were observed only with 0.5% of the young children. rating 0.4% of the total. 10) Psychomotor changes (apathetic, miserable, inert, withdrawn, or anorexic) rated 7.0% of the young children only, and 5.7% of the whole examined. 11) Prevalence rate of diarrhea precipitating dehydration and such other conditioning infections as thrush was found to be 20.5% of the infants, 7.5% of the young children, and 10.0% of both the infants and young children. 12) The results of clinical assessment led the author to the following findings: (1) Those corresponding to 29.1% of the whole examined had one or two combined signs suggestive of PCM. (2) Those with three or more signs combined rated 1.3% of the total children examined. (3) No clinically obvious kwashiorkor was found, but each of the component signs which, in constant combination, characterize it (edema, muscle-wasting, and psychomotor changes) was separately observed mainly with the young children as presented in table 2. (4) Those who impressed the author with clinically obvious nutritional marasmus were 6.7% of the infants, 3.2% of the young children, and 3.9% of both the infants and young children. Ⅱ. Nutritional Biometry 1) The mean heights. weights, chest circumference, and head circumference obtained were as shown in table 3. 2) The mean triceps skin-fold thickness, upper mid-arm-circumference. and upper-mid-arm-muscle circumference were as presented in table 4. 3) Distribution of the heights by four levels of 10% intervals indicated underweights in the light of Korean general standard, as shown in table 10. 4) Distribution of the weights contrasted likewise with the Korean general standard were as shown in table 11. 5) The underweights demonstrated by cases of mild-moderate PCM computed at four levels of 10% intervals as against the Korean general standard, rated 42.2% of the infants, 42.8% of the young children, and 42.7% of both the infants and young children. by the criteria Dr. Jelliffe suggested ??. The severe PCM cases were comprised 31.1% of the infants, 7.5% of the young children. and 12.1% of both groups of them. In the light of the above rates, more than a half of the children examined were found to be affected with PCM ascribable to deficiency of both the protein and calorie. 6) The cases with "Chest circumference/Head circumference Ratio being below 1 after the first 6 months of life" suggestive of the wasting of pectoral muscles which is attributable to PCM corresponded to 57.1% of the infants, 25.1% of the young children, and 29.3% of both the infants and young children as shown in table 13. Ⅲ. Biochemical Tests 1) Hemoglobin levels were found to be anemic (level less than 10.8gm%) with 13.8% of the 80 children examined as presented in table 5. 2) Serum total protein and serum albumin levels were as shown in tables 6 and 7, respectively. The serum albumin levels turned out to be normal with 75.9%, and "low"(2.80gm%∼3.51gm%) with 24.1% of the examined. No deficiency level (less than 2.80gm%) was found among the 29 children. 3) Serum iron content and total serum iron-bindin capacity (TIBC) were as shown in table 8. The deficiency levels of serum iron content less than 50 microgram % comprised 33.3% of the 21 children examined. Those corresponding to 25% and 75% of the 16 children examined had values of TIBC within normal range (300 microgram %∼400 microgram %) and values below the normal range (less than 300 microgram %), respectively, and no case exceeded over the range of normality inspite of the low serum iron content, suggesting the possible ascription to chronic protein deficiency ??. 4) "Urinary Urea-nitrogen / Creatinie-nitrogen excretion Ratios" regarded as an approximte index of dietary adequacy related to muscle storage of protein were obtained as presented in table 9. The cases with the ratios "30 or less" (an indication of malnutrition related to dietary adequacy of protein) rated 58.8% of the 34 children examined.

Background/Aims: Inflammatory diseasas of the pancreas are not uncommon. The advanced age is one of the significant factors in assessing the severity of acute pancreatitis. It has been shown that the biliary tract disease is a more frequent cause of acute pancreatitis in the elderly than in the young. With the increased life span, the elderly are comprising a large proportion of the population. As a result, physicians are more often being faced with acute pancreatitis in the elderly. The purpose af this study was to evaluate clinical features of acute panereatitis in the elderly. Methods: A total of 188 patients with acute pancreatitis were reviewed. We investigated the differences of clinical characteristics between the elderly patients aged over 60 and the young patients aged under 60. Results: The sex distribution showed male preponderance in the young patients (M:F=2.4:1), but was nearly equal in the elderly patients (M:F=1.1:1). The most common cause of acute pancreatitis in the elderly was biliary tract disease (52.5%), while alcohol abuse was the most common cause in the young patients (38.8%), Local complications of acute pancreatitis, such as psuedocyst, necrosis, abscesses were not differently shown between the elderly (16.9%) and the young (20.2%) patients. Systemic complications including acute puhnonary failure occurred more frequently in the elderly patients than the young patients. The mortality rate of the elderly patients (5.1%) was not significantly different from that of the young patients (3.1%). Conclusions: To detect correctable underlying bihary tract diseases the elderly patients with acute pancreatitis should be completely investigated using endoscopic retrograde cholangiopancreatography (ERCP). In addition, the patient should be closely monitored for the development of the systemic complications during the disease process.

In order to investigate the reinforcemental Effects of Acanthopanax Radicis Cortex about immunity and hematogenic action. The various immune responses were studied. The results were obtained as follows: 1. Positive cell was significantly increased at the 0.25 d r n P degree of density in the effect of CD4 about the spleen cell. 2. In the effect of IgG positive cell was significantly increased as comrared with the data of control only at the 0.25 mg/ml degree of density. 3. In the effect of Mac-1 positive cell was significantly increased in all treated degree of density. 4. In result of observing the change of WBC after Cyclophosphamide-processing the number of WBC was significantly increased in treated Group. 5. Measurement of RT-PCR in Hematopoietic stem cell TPO PCR products was increased as compared with medium at the 1 ㎛/ml degree of density. 6. In reinforcemental effect about Stem cell factor SCF PCR product was increased as compared with medium at the low degree of density. 7. In reinforcemental effect about IL-3 PCR product was increased as compared with medium at the low degree of density. 8. In reinforcemental effect about IL-6 PCR product was significantly increased as compared with all treated group. 9. Assay about positive cell in the spleen cell, the positive cell was significantly increased in CD4, CD19. 10. Created amount NO was treated group significantly increased all degree of density. 11. In promotional dissolution-effect, treated group was increased one and half times or twice all degree of density as compared with negative control. According to the above results, Acanthopanax Radicis Cortex increase coherence with T-cell by dint of activating CD of T-cell, presentate an antigen, activate IL-3, IL-6 which stimulate B-cell mading an antibody, activate IFN- y which appeal anti-viral activity by dint of activating macrophage, activate IgG which being made in B-cell neutralize Virus directly, increase immunity by means of promoting dissolution of Spleen cell which involve immune cell and strengthen hematogenic action by dint of activating TPO, SCF. Although Acanthopanax Radicis Cortex was used in retrogressional joint disease of deficiency syndrome by reinforcing Liver and Kidney, strengthening bones and sinews, because IFN- 7 which appeal anti-viral activity and IL-6 which almost appear in arthritis increased significantly, in future we should make an effort in looking into action of Acanthopanax Radicis Cortex about common cold by infection and arthritis of excess syndrome.

In this paper, authors tried primarily to categorically identify and generalize some fundamental causal factors considered responsible for impinge upon the agricultural and food crisis in many of the latecomer developing countries in the Black African Continent. As a fellow-up to the effort, authors also presented a proposition that agricultural and rural development should be tole ultimate answer to the badly unfortunate state of crisis affairs in agriculture and food in the Sub-Saharan African countries. Specifically, authors took an analytical perspective from the three categories of viewpoints concluded as the most basic and important in an effort of clarification and determination of the crisis causes in agriculture and food, leading so often to the famine : (a) heterogenous, precarious and unfavorable climatic and social environments, (b) chronic and wide-spread economic crisis, and (c) seriously stagnant agricultural crisis. To render the analytical effort, the authors made use of available statistics and empirical evidences derived in general terms, for which, however, no specific sources of information were given right in the text. Instead, references were presented in a backage from at the end of the paper text under the three main categoies : (a) Africa in general, (b) agricultural, rural and economic development of Africa, and (c) statistical source publications on agricultural, economic, social and physical situations in Africa. In view of the fact that not many studios of the African agriculture and rural development have been done in Korea, authors emphatically pointed out the need for Korean academicians to pay their attention to Africa where our national interests within the prevailing international relations have been significantly found, manifestly and potentially. The faculties of our college could have their overseas academic interests by having Africa as a geographical area of interest in their own specialized disciplines in that agricultural and rural development is the national development itself in most of the African countries.

      • In6S7 nanoparticle-embedded and sulfur and nitrogen co-doped microporous carbons derived from In(tdc)2 metal-organic framework

Choi, In-Hwan,Jang, So-Young,Kim, Hyun-Chul,Huh, Seong The Royal Society of Chemistry 2018 Dalton Transactions Vol.47 No.4

<P>Indium sulfide nanoparticle (NP)-embedded microporous carbons co-doped with S- and N-dopants are easily prepared by a direct carbonization of the as-prepared In(iii)-based metal-organic framework (In-MOF), [Et2NH2][In(tdc)2]·DEF, containing ditopic S-containing 2,5-thiophenedicarboxylate (tdc2−) bridging linkers as a potential source of S-dopant. The charge on the anionic framework of [In(tdc)2]− is balanced by Et2NH2+, which is also a potential N-dopant. Simultaneous embedding of In-based NPs, S-, and N-co-doping is achieved in a simple single step carbonization of In-MOF. Three porous carbon materials (PCMs), PCM-700, PCM-800, and PCM-900, are obtained from the carbonization of In-MOF at 700, 800, and 900 °C, respectively. The gas sorption analysis indicates them as good CO2 sorbents. The photocatalytic degradation of methyl orange by PCMs under visible light irradiation is also effectively operable owing to the photocatalytically active semiconducting indium sulfide NP with a small bandgap. The main component of indium sulfide NPs is revealed as In6S7 based on the powder X-ray diffraction pattern. Small amounts of metallic In and In2S3 are also observed. The specific capacitances of PCMs are also estimated from the galvanostatic charge/discharge curves. PCM-900 exhibits the highest gravimetric specific capacitance of 99.0 F g−1 at a current density of 0.05 A g−1.</P>

Conflicting results exist on the outcome of off hours PCI in ST elevation myocardial infarction (STEMI). However, there were only a few studies that have focused on the clinical characteristics and outcomes of off hours PCI in STEMI. So, we studied the clinical characteristics and hospital mortality in STEMI patients treated with primary PCI during regular hours (weekdays 9:00 AM to 6:00 PM) versus off hours Weekdays 6:01 PM to 8:59 AM, weekends, and holidays) in Korea Acute Myocardial Infarction Registry. We analyzed in hospital and one year mortality among 5,665 consecutive ST segment elevation myocardial infarction patients treated with primary PCI between November 2005 to January 2008. Total 2,848 (50.2%) patients were treated during off hours. Baseline finding were similar, although regular hours patients were older. Median symptom to balloon time (304 min, IQR 175 to 750 vs. 270 min, IQR 145 to 551, p=0.001) were longer for regular hours primary PCI. Median door to balloon time (71min, IQR: 48 to 132 vs. 59 min, IQR 39 to 110 min, p=0.001) were longer for off hours pPCI. Also, Cardiac enzyme such as Max CK-MB (212.1±299.3 vs 194.7±303.4, p=0.031) and max TnI (72.6±239.5 vs. 58.9±94.4, p=0.013) were increased in off hours pPCI. However, unadjusted in hospital (6.0% off hours vs. 6.0% regular hours, p=0.946) and one year cardiac mortality (11.3% off hours vs. 11.7% regular hours, p=0.661) were comparable. In multivariate analysis, off hours primary PCI did not predict an adverse outcome. In conclusion, when primary PCI was performed within an appropriate reperfusion strategy, the clinical effectiveness of either off hours or regular hours pPCI is comparable.

In order to elucidate the polycyclic aromatic hydrocarbon concentration and its origin in arctic area, four arctic brown algae (Laminaria saccharina, L. digita, Alaria esculenta, Desmarestia aculeata), one marine invertebrate (Echinoidea) and sediments were collected from Kongsfjorden in Spitsbergen from the late July to early August. 2003. In case of macroalgae, the young blade part above growth point and the old stipes and blades beneath growth point were separated and analyzed for polycyclic aromatic hydrocarbons (PAHs) in an attempt to check the mechanism of uptake in macroalgae to accumulate PAN. There was no difference in PAH concentrations between sampling sites (Stations B and C), species. and blades beneath and above growth point. PAH concentrations in all samples collected in this study were relatively higher than those reported in other areas of arctic. Especially, station C, which is known as an unpolluted area, showed 10 times higher PAH concentration (8,765 ng/g) in sediment than station A (694 ng/g) around harbor. In addition high PAH concentration, station C had very higher proportion of methylated PAH to parent PAH in sediment than station A. Source analysis using PAH isomer pair ratios as indicators showed that Kongsfjorden area seemed to be relatively contaminated with PAH derived from direct petroleum input.

Association of HLA-DRB1 and -DQB1 Alleles with Susceptibility to IgA Nephropathy in Korean Patients

        In Ji Won,Jung Kiwook,Shin Sue,Park Kyoung Un,Lee Hajeong,Song Eun Young 대한진단검사의학회 2022 Annals of Laboratory Medicine Vol.42 No.1

Background: Associations between IgA nephropathy (IgAN) and HLA-DRB1 and -DQB1 alleles have been reported in several ethnic groups. We investigated the association of HLA-DRB1 and -DQB1 alleles with the predisposition for IgAN and disease progression to end-stage kidney disease (ESKD) in Korean patients. Methods: We analyzed HLA-DRB1 and -DQB1 genotypes in 399 IgAN patients between January 2000 and January 2019 using a LIFECODES sequence-specific oligonucleotide (SSO) typing kit (Immucor, Stamford, CT, USA) or a LABType SSO Typing Test (One Lambda, Canoga Park, CA, USA). Alleles with a significant difference in two-digit resolution were further analyzed using in-house sequence-based typing and sequence-specific primer PCR. As controls, 613 healthy hematopoietic stem cell donors were included. Kidney survival was analyzed in 281 IgAN patients with available clinical and laboratory data using Cox regression analysis. Where needed, P-values were adjusted using Bonferroni correction. Results: The allele frequencies of HLA-DRB1*04:05 (corrected P [Pc]<0.001), -DQB1 *04:01 (Pc=0.048), and -DQB1*03:02 (Pc=0.021) were significantly higher in IgAN patients than in controls, whereas those of HLA-DRB1*07:01, -DRB1*15:01, -DQB1*02:02, and -DQB1*06:02 (Pc<0.001 for all) were significantly lower in IgAN patients than in controls. The allele frequency of HLA-DQB1*05:03 (Pc=0.016) was significantly lower in the ESKD group than in the non-ESKD group; however, there was no significant difference for ESKD progression between these groups. Conclusions: We report novel associations of HLA-DRB1*15:01, DQB1*02:02, -DQB1*03:02, and -DQB1*04:01 with IgAN. Further studies of HLA alleles associated with IgAN progression in a larger cohort and in various ethnic groups are needed.
