      • 알콜리즘에서 Buspirone에 대한 Cortisol과 Prolactin 반응

        연구목적: 본 연구는 buspirone에 대한 cortisol과 prolactin 반응을 통해 알콜리즘에서 5-HT1A 수용체기능을 조사하고자 하였다. 방법: 알콜리즘 환자군은 DSM-IV의 알콜리즘 진단기준에 부합되고 금주기간이 최소 3개월 이상이며 우울 및 불안증상이 없는, 한 정신병원 알콜 센터에 입원한 22명의 남자 환자였다. 대조군은 정상성인 남자 15명이었다. 이들에게 오전 9시에 5-HT□ 효현제인 buspirone 30mg을 경구 투여한 후 0, 30 , 60, 90, 120, 150분의 혈청 cortisol과 prolatine 농도를 측정한다. 결과: 환자군과 대조군간에 cortisol 기저치에는 유의한 차이가 없었다. Buspirone 투여 후 혈청 cortisol 농도는 정상대조군에서는 기저치에 비해 유의한 증가를 보였으나(p<0.01) 환자군에서는 유의환 증가가 없었다. 혈중 cortisol 농도는 buspiron 투여 60분 이후부터 환자군에서 정상 대조군에 비해 유의하게 저하되어 있었다(p<0.05). Buspirone에 대한 혈청 prolactin 반응은 환자군과 정상대조군간에 유의한 차이가 없었다. 결론: Buspirone 투여 후 둔마된 혈청 cortisol 반응을 통해 알콜리즘에 5-HT1A 수용체 반응성이 둔마되어 있음을 확인할 수 있었다. Objectives: The purpose of this study was to evaluate the prolactin and cortisol responses to 5-HT1A receptor activation by buspirone in alcoholics. Methods: The subjects were twenty two male alcoholic patients meeting the DSM-IV criteria for alcohol dependency and abstaining for more than 3 months. Patients were free from overt anxiety and depressive symptoms. Controls were fifteen male normal volunteers, with no psychiatric and medical illness. Blood samples for the measurement of serum cortisol and prolactin levels were drawn 0, 30, 60, 90, 120, 150 minutes after oral administration of 30mg buspirone hydrochloride at 9:00a.m. Results: The baseline cortisol levels were not significantly different between alcoholics and controls. Serum cortisol levels of controls after buspirone administration were significantly increased over time(p<0.01), but those of alcoholics did not increased. After 60 minutes following buspirone administration, cortisol levels were significantly lower in alcoholics than in contrlos(p<0.05). Prolactin responses to buspirone were not significantly different between the two groups. Conclusions: Our results suggested that 5-HT1A receptor function is decreased in alcoholic patients.

        By means of the self consistent calculation of the related Poisson's equation and the Wigner distribution function, the quantum transport of electrons in a double barrier tunneling diode(DBRTD) has been studied. The computer simulation has been accomplished by assuming the contacts/terminals of a DBRTD are open with regard to the surrounding. The eigenvalues of the Liouville superoperator of the equation of motion were also calculated. The derived Ⅰ-Ⅴcharacteristic of a DBRTD shows a similar trend with the results obtained by the conventional tunneling models.

        대사성 산증을 동반한 알코올성 간질환 환자의 단기 예후

        목적: 알코올성 간질환에서의 대사성 산증은 지속적인 과량의 알코올 섭취로 인한 당 신생 합성의 억제와 BOHB/AcAc 비율의 증가, 단기간의 금식으로 인한 저혈당, 인슐린 분비 저하, 유리 지방산과 케톤체 형성의 증가, 구토와 수분 섭취 부족에 의한 심한 탈수에 의해 발생하는 것으로 알려져 있다. 알코올성 케톤산증과 감별할 질환으로는 당뇨병성 케톤산증, 젖산증, 살리실레이트, 메탄올, 에틸렌글리콜 중독 등이 있다. 본 연구는 대사성 산증을 동반한 알코올성 간질환으로 내원하여 입원한 환자들을 대상으로 생존군과 사망군에서 임상적 양상 및 검사 소견들의 차이를 비교하여 단기 예후와 연관 있는 인자들을 알아보고자 하였다. 대상과 방법: 만성 알코올 남용자이면서 대사성 산증을 동반하여 입원한 알코올성 간질환 환자 29명을 생존군(18명)과 사망군(11명)으로 나누어 임상 소견과 검사실 소견을 비교하였다. 결과: 1) 대상 환자군은 연령은 36-65세였으며, 성별은 남자가 26명이었고 여자가 3명이었다. 2) 임상 증상은 의식저하, 복통, 구토, 호흡 곤란, 토혈, 흉통 등 다양하게 나타났으며, 신체검사 소견은 탈수, 황달, 빈혈, 간비대, 복수, 부종 등이었고 두 집단을 비교했을 때 사망군에서 복수가 유의하게 더 많은 빈도로 관찰되었다. 일일 평균 음주량은 사망군에서 유의하게 더 많았고(198 vs. 123, P=0.034) 음주 기간과 총 음주량은 두 군 간에 유의한 차이를 보이지 않았다. 4) 내원시 혈액검사 소견을 비교하였을 때 사망군에서 동맥혈 산염기 차이가 유의하게 낮았고 헤모글로빈, 혈소판도 유의하게 낮았다. 사망군에서 총 비리루빈, 크레아티닌, 무기인, 암모니아가 유의하게 높았고 알부민, 클로라이드가 유의하게 낮았다. 사망군에서 프로트롬빈 시간이 유의하게 연장되었으며 섬유소원은 유의하게 낮았고 D dimer는 유의하게 높았다. 결론: 대사성 산증을 동반한 알코올성 간질환 환자에서 일일 알코올 섭취량을 파악하고 복수의 유무, 동맥혈 산염기 차이, 헤모글로빈, 혈소판, 총 빌리루빈, 알부민, 크레아티닌, 무기인, 클로라이드, 암모니아, 프로트롬빈 시간, 섬유소원, D dimer 등을 검사하는 것이 단기 예후를 예측하는 데 도움이 될 것으로 사료된다. 향후 많은 환자들을 대상으로 하여 예후 관련 인자들에 대한 전향적이고 체계적인 연구가 요망된다. Background/Aims: Alcoholic liver disease with metabolic acidosis may have possible causes such as alcoholic ketoacidosis, diabetic ketoacidosis, lactic acidosis. Salicylate, methanol, and ethylene glycol intoxication should also be considered. The aim of this study was to investigate the short-term prognostic factors in patients with alcoholic liver disease with metabolic acidosis. Methods: Clinical data related to twenty-nine patients with alcoholic liver disease and metabolic acidosis was analysed retrospectively. Patients were divided into two groups according to the outcome (survival or death). Past medical history, and physical, laboratory and radiologic data at admission were compared. Results: The amount of daily alcohol intake differed significantly between the two groups (P=0.034), but duration and total amount of alcohol intake did not differ significantly between the two groups (P=0.128; P=0.360). The presence of ascites differed significantly between two the groups (P=0.019). On laboratory testing, the following differed significantly: base excess (P=0.038), hemoglobin (P=0.019), platelet (P=0.040), total bilirubin (P=0.007), albumin (P=0.012), creatinine (P=0.014), phosphorus (P=0.021), chloride (P=0.010), ammonia (P=0.003), prothrombin time (P=0.033), fibrinogen (P=0.011) and D-dimer (P=0.024). Review of the medical history of the patients showed diabetes (10/29), cirrhosis (10/29), and hepatocellular carcinoma (1/29). Combined conditions at admission were sepsis (8/29), pneumonia (7/29), acute renal failure (6/29), rhabdomyolysis (5/29), gastrointestinal hemorrhage (4/29), acute pancreatitis (3/29), acute respiratory distress syndrome (2/29), and acute myocardial infarction (1/29). Conclusions: The amount of daily alcohol intake, base excess, hemoglobin, platelet, total bilirubin, albumin, creatinine, phosphorus, chloride, ammonia, prothrombin time, fibrinogen and D-dimer seemed to be useful parameters in predicting short-term prognosis of patients with alcoholic liver disease with metabolic acidosis.       • 아동의 사회적 문제해결능력과 어머니의 대화형태와의 관계연구

        The present study investigated the relationship between the social problem solving ability of children and the language patterns of their mothers. The subjects included 193 nine year­olds (185 girls, 108 boys), and 230 twelve year­olds (115 girls, 115 boys). The instruments used were a language patterns test for the mother and IPA (Interpersonal Problem Solving Ability Test) for children. Major finding from this study were: ⑴ the correlation between the social problem solving ability of children and the language patterns of their mothers was not statistically significant, ⑵ the language patterns of the mothers were significantly different by child's sex; for boys, mother used more ordered language patterns and for girls, mothers used more ordered language patterns and for girls, mothers used more human­oriented language patterns, ⑶ the social problem solving ability of children were significantly different by child's age; 12­year­olds exhibited higher ability than 9­year­olds.

      • Simultaneous Determination of Free Capsorubin and Capsanthin in Red Pepper Powder Using u-HPLC

        <B>Abstract</B><P>A simultaneous ultra-HPLC (u-HPLC) method for the determination of free capsorubin and capsanthin in red pepper powder was validated in terms of its precision, accuracy, and linearity. The u-HPLC separation was performed on an RP C18 column (particle size 2 μm, id 2 mm length 100 mm) and with a photodiode-array detector. The recoveries of capsorubin were greater than 83.8 ± 1.7%; the LOD and LOQ of the u-HPLC analyses were 0.043 and 0.129 mg/kg, respectively. The intraday and interday precisions for capsorubin were less than 9.01%. The recoveries of capsanthin were greater than 87.7 ± 1.5%, and the LOD and LOQ were 0.101 and 0.306 mg/kg, respectively. The intraday and interday precisions for capsanthin were less than 12.66%. All calibration curves for capsorubin and capsanthin exhibited good linearity (r2 = 0.99) within the tested ranges.</P>

        Risk Factors of Postpartum Depression Among Korean Women: An Analysis Based on the Korean Pregnancy Outcome Study (KPOS)

        Background: Postpartum depression (PPD) can negatively affect infant well-being and child development. Although the frequency and risk factors of PPD symptoms might vary depending on the country and culture, there is limited research on these risk factors among Korean women. This study aimed to elucidate the potential risk factors of PPD throughout pregnancy to help improve PPD screening and prevention in Korean women. Methods: The pregnant women at 12 gestational weeks (GW) were enrolled from two obstetric specialized hospitals from March 2013 to November 2017. A questionnaire survey was administered at 12 GW, 24 GW, 36 GW, and 4 weeks postpartum. Depressive symptoms were assessed using the Edinburgh Postnatal Depression Scale, and PPD was defined as a score of ≥ 10. Results: PPD was prevalent in 16.3% (410/2,512) of the participants. Depressive feeling at 12 GW and postpartum factors of stress, relationship with children, depressive feeling, fear, sadness, and neonatal intensive care unit admission of baby were significantly associated with a higher risk of PPD. Meanwhile, high postpartum quality of life and marital satisfaction at postpartum period were significantly associated with a lower risk of PPD. We developed a model for predicting PPD using factors as mentioned above and it had an area under the curve of 0.871. Conclusion: Depressive feeling at 12 GW and postpartum stress, fear, sadness, relationship with children, low quality of life, and low marital satisfaction increased the risk of PPD. A risk model that comprises significant factors can effectively predict PPD and can be helpful for its prevention and appropriate treatment.

        Sestrin2 Mediates IL-4-induced IgE Class Switching by Enhancing Germline ε Transcription in B Cells

        Sestrin2 (Sesn2), a metabolic regulator, accumulates in response to a diverse array of cellular stresses. Sesn2 regulates cellular metabolism by inhibiting the mammalian target of rapamycin complex 1 through the AMP-activated protein kinase (AMPK) signaling pathway. Recently, researchers reported that Sesn2 regulates the differentiation and function of innate immune cells and T cells; however, the role of Sesn2 in B cells is largely unknown. In this study, we investigated the role of Sesn2 in Ig class switching and Ig production in mouse B cells. We observed that mouse B cells express Sesn2 mRNA. Interestingly, the expression of germline ε transcripts (GLTε) was selectively decreased in lipopolysaccharide-stimulated Sesn2−/− splenocytes. Overexpression of Sesn2 increased GLTε promoter activity in B cells. In addition, AICAR (an activator of AMPK) selectively increased IL-4-induced GLTε expression and surface IgE (sIgE) expression in splenocytes. Furthermore, AICAR selectively enhanced IL-4-induced GLTε expression, sIgE expression, and IgE production by anti-CD40-stimulated B cells. We observed that ovalbumin (OVA)-specific IgE concentration was reduced in OVA-challenged Sesn2−/− mice. Taken together, these results indicate that Sesn2-AMPK signaling selectively enhances IL-4-induced IgE class switching and IgE production by B cells, suggesting that this could be a therapeutic strategy targeting Sesn2 in IgE-mediated allergic diseases.

      • Rapid Method for the Determination of Vitamins A and E in Foods Using Ultra-High-Performance Liquid Chromatography

        <B>Abstract</B><P>A rapid and novel ultra-HPLC (u-HPLC) method for the determination of vitamins A (retinol) and E (α-, γ-, and δ-tocopherol) in foods was validated in terms of its precision, accuracy, and linearity. The u-HPLC separation was performed on an RP C18 column (particle size 2 μm, id 2 mm, and length 75 mm), followed by fluorescence detection. The recovery of retinol was more than 84.58%; the LOD and LOQ of the u-HPLC analysis were 0.015 and 0.045 mg/kg, respectively. The intraday and interday precision was less than 9.12%. The recoveries of α-, γ-, and δ-tocopherol were more than 81.37%; the LOD and the LOQ were 0.014, 0.002, and 0.001 mg/kg and 0.042, 0.005, and 0.004 mg/kg, respectively. All calibration curves had good linearity (r2 = 0.99) within the test ranges. The novel, rapid method coupled to u-HPLC can provide significant improvements in the speed, sensitivity, and resolution compared with a conventional HPLC method.</P>

      • Rapid Method for the Simultaneous Determination of Flavonol Aglycones in Food Using u-HPLC Coupled with Heating Block Acidic Hydrolysis

        <B>Abstract</B><P>A rapid method for the simultaneous determination of flavonol aglycones in food using ultra-high-performance LC (u-HPLC) coupled with a heating-block acidic hydrolysis method was validated in terms of precision, accuracy, and linearity. The u-HPLC separation was performed on an RP C18 column (particle size 2 μm id, 2 mm, length 100 mm) with a photodiode array detector. The LOD and LOQ of the u-HPLC analyses were 0.15 and 0.47 mg/kg for myricetin, 0.09 and 0.28 mg/kg for quercetin, 0.16 and 0.49 mg/kg for kaempferol, and 0.08 and 0.25 mg/kg for isorhamnetin. The intraday and interday precisions of the individual flavonol aglycones were less than 9.31%. All calibration curves exhibited good linearity (r2 = 0.99) within the tested ranges. Total run time of u-HPLC was 13 min. The rapid u-HPLC method presented herein significantly improved the speed, sensitivity, and resolution of the analyses of myricetin, quercetin, kaempferol, and isorhamnetin in food.</P>
