진성 복압성 요실금 환자에서 복강경하 방광경부현수술의 유용성: 타술식과의 비교 연구

연구목적: 진성 복압성 요실금의 치료법으로서 복강경을 이용한 Burch 수술을 타수술법과 비교하여 그 유용성을 알아보고자 하였다. 대상 및 방법: 진성 복압성 요실금을 주소로 본원 비뇨기과에서 수술을 시행받은 44명의 환자를 수술법별로 분류하여 수술법에 따른 성공률, 수술 시간, 입원 기간 및 합병증의 빈도를 비교 분석 하였다. 결과: 전체 환자에서 수술 성공률은 86.4% 였으며 이들 중 복강경을 이용한 Burch 수술군에서는 성공률이 100% 이었다. 복강경을 이용한 Burch 수술군은 타수술군에 비하여 입원 기간이 짧았다. 복강경을 이용한 Burch 수술군은 합병증이 매우 경미하였으며 전체 환자의 31.8%에서 단기간의 배뇨곤란을 호소하였으나 장기간의 배뇨곤란은 없었으며 슬링 수술군에서는 3례(16.7%)에서 심한 절박성 요실금을 호소하였다. 결론: 복강경을 이용한 Burch 수술은 높은 치료 성공률과 낮은 합병증의 빈도를 나타내어 진성 복압성 요실금의 치료에서 매우 우수한 수술법으로 사료된다. Purpose: We performed this study to investigate the usefulness of laparoscopic Burch colposuspension as a treatment of genuine stress incontinence compare to other anti-incontinence operation. Materials and Methods : We analyzed success rate, operation time, admission day, and complication rate of various anti-incontinence operations in 44 genuine stress incontinence patients who have been received operation in our hospital. Results : Overall success rate was 86.4%, but laparoscopic Burch coposuspension showed 100% success rate. Laparoscopic Burch colposuspension required shorter admission day compared to other anti-incontinence oparations. Complication rate was lower in laparoscopic Burch colposuspension group. Short-term voiding problems were identified in 31.8% of total patients but 3 patients(16.7%) who have received sling operation complained severe urge incontinence. Conclusion : Laparoscopic Burch colposuspension is considered as an effective surgical method for genuine stress incontinence due to high success rate and low complication rate.

Reinforced concrete walls subjected to cyclic loading show complicated inelastic behavior varying with the shear span-to-depth ratio, re-bar detail, and loading condition. In the present study, a macro-model was developed to perform nonlinear numerical analysis of multi-story wall structures. To conveniently evaluate the coupled flexure-compression and shear responses, a reinforced concrete wall is idealized with simplified longitudinal and diagonal uniaxial elements. The uniaxial elements consist of concrete and re-bars. Simplified cyclic material models were used to describe the cyclic behavior of concrete and re-bars. For verification, the proposed method was applied to various existing test specimens of isolated and coupled walls. The results showed that the predictions of the proposed method agreed with the test results including the load-carrying capacity, deformation capacity, energy dissipation, and failure mode.

<P>We propose a label-free depth-resolved tomographic scheme for imaging a single live cell in fluid. This approach utilizes a modified time-domain full-field optical coherence tomography (FF-OCT) system combined with an optical tweezer technique. The optical trap for holding a moving specimen is made by tightly focusing a 1064 nm Q-switching pulsed laser beam with a 1.0 NA microscope objective in the sample arm of the FF-OCT part. By cosharing the probe for both systems, the optical actions of trapping and cellular resolution tomographic imaging could be achieved simultaneously. Feasibility of the combined system is demonstrated by imaging micron-sized polystyrene beads and a living suspension cell in medium.</P>
Nondestructive and Rapid Estimation of Chlorophyll Content in Rye Leaf Using Digital Camera

We have developed and tested a new method for nondestructive estimation of chlorophyll- and nitrogen-contents in rye leaf. It was found that the relation-ships among nitrogen, chlorophyll content and fresh weight were significantly positive correlated. Nitrogen and chlorophyll content were positively correlated whereas correlation coefficients among R, G, R-B and G-B on the basis of photo-numerical values were negative. We have found that R/(R-B) obtained from data of digital camera is the best criterion to estimate the chlorophyll content of leaves. The regression curves of the relation between R/(R-B) and chlorophyll content were also calculated from the data collected on cloudy days. The coefficients of determination (~textrmr2 ) were ranged from 0.33 to 0.99. In this study, the accuracy in estimating chlorophyll content from the color data of digital camera image could be improved by correcting with R, G, and B values. It is suggested that, for practical purposes, the image values estimated with sufficient accuracy using a portable digital camera can be applied for determining chlorophyll content and nitrogen status in plant leaves.
Characterization and in vivo evaluation of calcium phosphatecoated cp-titanium by dip-spin method

Calcium phosphate ceramics like hydroxyapatite (HA) and tricalcium phosphate (TCP) are good candidates for bone substitutesdue to their chemical similarity to bone minerals. Additionally, the biodegradation property of bone substitutes may allow theorganism to replace the foreign material by fully functional new bone in a balanced time schedule. Thus, the coating of biodegrad-able calcium phosphate ceramics on dental implants was conducted in this study to enhance the biocompatibility and eliminate thelong-term adverse eects of HA coated implants. First, biodegradable calcium metaphosphate(CMP) sol was prepared and thencoated on cp-Ti substrates by a dip-spin coating technique. The coated specimens were heat-treated at 600.C for 6 h. CMP coatedlayer on cp-Ti was smooth and uniform with ne grains. It is observed that bone-to-implant contact area of CMP coated specimenat both 2 and 6 weeks was signicantly higher than that of non-coated group. The biocompatibility and cytotoxicity of the CMPcoated specimen were better than those of the non-coated specimen. Therefore, the CMP coating on cp-Ti is a good candidate for. improving bone-to-implant osseointegration, although it is difficult to extrapolate in vitro results directly to in vivo situation.
The effects of newly formed synthetic peptide on bone regeneration in rat calvarial defects

Purpose: Significant interest has emerged in the design of cell scaffolds that incorporate peptide sequences that correspond to known signaling domains in extracellular matrix and bone morphogenetic protein. The purpose of this study was to evaluate the bone regenerative effects of the synthetic peptide in a critical-size rat calvarial defect model. Methods: Eight millimeter diameter standardized, circular, transosseus defects created on the cranium of forty rats were implanted with synthetic peptide, collagen, or both synthetic peptide and collagen. No material was was implanted the control group. The healing of each group was evaluated histologically and histomorphometrically after 2- and 8-week healing intervals. Results: Surgical implantation of the synthetic peptide and collagen resulted in enhanced local bone formation at both 2 and 8 weeks compared to the control group. When the experimental groups were compared to each other, they showed a similar pattern of bone formation. The defect closure and new bone area were significantly different in synthetic peptide and collagen group at 8 weeks. Conclusions: Concerning the advantages of biomaterials, synthetic peptide can be an effective biomaterial for damaged periodontal regeneration.

Al alloy has been widely used in the structure of aircraft and passenger car because of its lightweight. It is still interested in the lightweight industry to improve the structural safety and the performance. In this research, forging design of Al rotating arm holder has been studied by FE analysis. Therefore, structural analysis of the initial three dimensional model of the product was performed at first. From the result of the analysis, the load and the stress distribution acting on the component were obtained. Three dimensional measurements have been done to evaluate the validity of the FE analysis to the forging process design of the Al rotating arm holder.
