‘병동 중심의 처치수가 산정지침 개발’ 등을 통한 간호업무 표준화 및 그 효과

문제: 병동 처치수가 산정 부정확 및 누락으로 인한 부적절한 재고관리로 진료차질, 불필요한 업무발생 및 수익이 감소한다. 목적: 병동 처치수가 산정 정확화를 위한 업무 표준화 도구 및 시스템을 개발하여 적정 재고관리를 통한 수익증대 및 직무만족도를 향상시킨다. 의료기관: 서울시 종로구에 소재한 대학병원 질 향상 활동: 병동중심의 처치 산정지침 개발 및 수가물품의 적정재고 관리방안을 모색하였다. 개선효과: 병동중심의 처치수가 산정 지침서를 제작하여 업무표준화를 기하였음. 응급청구 품목 및 수량이 ’07년 대비 71%감소하고 타 병동 차용품목이 활동 전에 비해 61% 감소, 수량은 77% 감소함. 응급청구 총소요시간이 활동 전에 비해 ’07년 대비 77% 감소하고 타 병동 차용 총 소요시간이 61% 감소함. 전년 동기간 대비 수익이 4% 증가하였으며 또한 54병동과 보험 심사팀에서 직원들의 직무만족도가 향상되었다.

Background: Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease showing recurrent painful nodule, abscess, sinus and scar. Many treatment modalities are used, but there is no single effective treatment. Objectives: The aim of this study is to compare the clinical outcomes of treatment between systemic antibiotics and systemic retinoids. Methods: We analyzed the clinical features, treatment modalities and clinical outcomes of the patients with HS and we compared the clinical outcomes among treatment modalities. Clinical outcomes were assessed according to treatment result estimated by dermatologist, treatment duration and the presence of recurrence. Results: A total 77 patients were enrolled. There was male predominance and mean age was 28.1 years old. Buttock (42.0%) was the most common affected site and 63 patients (81.8%) showed Hurley staging I. Systemic antibiotics showed relatively high improving rate, short mean treatment duration and low recurrence rate compared with systemic isotretinoin, but there were no statistical significant differences. Among systemic antibiotics, both tetracycline and macrolide showed good therapeutic effects on HS and there were no statistical significant differences. Conclusion: In most of patients showing mild lesion, systemic retinoid had good effect as much as systemic antibiotics, so systemic retinoid seem to be a useful treatment modality especially in patient showing mild lesions.
Rare minisatellite alleles of MUC2-MS8 influence susceptibility to rectal carcinoma

Background Previously, we identifed eight novel minisatellites in the MUC2, of which allelic variants in MUC2-MS6 were examined to infuence susceptibility to gastric cancer. However, studies on the susceptibility to gastrointestinal cancer of other minisatellites in the MUC2 region still remain unprogressive. Objective In this study, we investigated whether polymorphic variations in the MUC2-MS8 region are related to susceptibility to gastrointestinal cancer. Methods We assessed the association between MUC2-MS8 and gastrointestinal cancers by a case–control study with 1229 controls, 486 gastric cancer cases, 220 colon cancer cases and 278 rectal cancer cases. To investigate whether intronic minisatellites afect gene expression, various minisatellites were inserted into the luciferase-reporter vector and their expression levels were examined. We also examined the length of MUC2-MS8 alleles in blood and cancer tissue matching samples of 107 gastric cancer patients, 125 colon cancer patients, and 85 rectal cancer patients, and investigated whether the repeat sequence afects genome instability. Results A statistically signifcant association was identifed between rare MUC2-MS8 alleles and the occurrence of rectal cancer: odds ratio (OR), 6.66; 95% confdence interval (CI), 1.11–39.96; and P=0.0165. In the younger group (age,<55), rare alleles were signifcant associated with an increased risk of rectal cancer (odds ratio, 24.93 and P=0.0001). Suppression of expression was found in the reporter vector inserted with minisatellites, and loss of heterozygosity (LOH) of the MUC2- MS8 region was confrmed in cancer tissues of gastrointestinal cancer patients (0.8–5.9%). Conclusion Our results suggest that the rare alleles of MUC2-MS8 could be used to identify the risk of rectal cancer and that this repeat region is related to genomic instability.

<P>With the advent of next-generation sequencing (NGS), targeted sequencing is now contributing to decision making for which chemotherapeutics to administer to cancer patients, especially in refractory and metastatic cancer. Given that most patients with refractory cancer develop resistance to chemotherapy and have few treatment options, we performed NGS test to evaluate the efficacy and clinical feasibility of NGS-based targeted anticancer therapy. We used a gene panel for capturing target regions covering 83 cancer-related genes. A total of 25 refractory metastatic solid tumor patients were enrolled in this study. Among the 25 patients, 7 had FDA-approved drug-responsive or -resistant alterations. However, the effectiveness of targeted therapy was assessed by follow-up in three patients (12%). These included crizotinib for ALK-EML4 fusion in a malignancy of undefined origin patient and everolimus for <I>AKT3</I> amplification in a uterine sarcoma patient. In addition, we identified a <I>KRAS</I> codon 146 mutation (A146V), which is associated with resistance to anti-EGFR, in a cetuximab-resistant colon cancer patient with wild-type <I>KRAS</I> exons 2 and 12, and <I>EGFR</I> amplification. He received bevacizumab therapy. All three patients showed partial response after targeted therapy. Furthermore, we characterized <I>KRAS</I> A146V biologically using colon cancer cells. In conclusion, this study suggests that targeted therapy based on NGS test may be a good choice for improving the care of patients with refractory solid tumors.</P>

Since the prevalence of allergies is increasing, food allergy is a major concern for consumers, as well as for the food industry. The foods that account for over 90% of all moderate to severe allergic reactions to food are milk, eggs, peanuts, soybeans, fish, shellfish, wheat, and tree nuts. Of these food allergens, milk is one of the major animal food allergens in infants and young children. Milk is the first food that an infant is exposed to; therefore, the sensitization rate of milk in sensitive individuals is understandably higher. The mechanisms involved in allergic reactions caused by this hypersensitivity are similar to those of other immune-mediated allergic reactions. The reactions occur in the gastrointestinal tract, skin, and respiratory tract, with headaches and psychological disorders occurring in some instances. The major allergenic proteins in milk are casein, β-lactoglobulin, and α-lactalbumin, while some of the minor allergenic proteins are lactoferrin, bovine serum albumin, and immunoglobulin. Reliable allergen detection and quantification are essential for compliance with food allergen-labeling regulations, which protect the consumer and facilitate international trade.
Effects of dynamic taping on shoulder joint proprioception

Objective: There are many cases of applying various taping methods to prevent muscle damage and to assist with movement. The purpose of this study was to investigate and to compare the effects of dynamic taping on joint position sense and to find out the difference in error values during various degrees of shoulder flexion. Design: Cross-sectional study. Methods: A total of 20 subjects participated in this study with a randomized cross-over design. The order of taping was adjusted by randomly proceeding with dynamic taping, sham taping, and no taping. After the taping, the proprioception of the shoulder joint was evaluated. The evaluation of proprioceptive sensation was performed by evaluating joint position sensation. The sequence was adjusted by randomly performing joint position tests at each shoulder flexion of 50, 90, and 110 degrees. All angles were repeated 3 times. Results: There was a significant difference between dynamic taping and no taping in 50 degrees of shoulder flexion. There was a significant difference between sham taping and no taping in 90 degrees of shoulder flexion (p<0.05). No significant difference was found in 110 degrees of shoulder flexion. Conclusions: In this study, it was confirmed that dynamic taping is effective in improving the joint position sense in 50 degrees of shoulder flexion. In the future, it is expected that further studies will be conducted on patients with shoulder dysfunction with decreased proprioception.
