저자들은 신부전이 동반된 당뇨병 환자에서 발생된 부비동형 모균증에 대하여 수술과 함께 liposomal amphotericin B 및 GM-CSF의 복합치료를 하여 성공적으로 치료한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Mucormycosis (zygomycosis) primarily affects diabetic or immunocompromised patients and typically progresses rapidly, necessitating surgical excision and antifungal therapy with amphotericin B. Large doses of amphotericin B are needed for cure, but it has the risk of causing significant renal toxicity. The recent development of liposomal amphotericin B allows antifungal therapy to be administered with potentially improved efficacy and reduced nephrotoxicity. We have experienced a case of paranasal mucormycosis successfully treated with surgery, liposomal amphotericin B and GM-CSF. A 59-year-old male suffering from diabetes mellitus for 6 years was admitted with pain at left maxillary area. He was diagnosed as mucormycosis after cytologic exam on the necrotic nasal mucosa, which showed typical hyphae. He have had diabetic nephropathy with macroproteinuria and had rapidly rising serum creatinine levels with the amphotericin B treatment: creatinine levels reverted to basal level with the use of liposomal amphotericin B. Despite surgical excision and continued antifungal therapy, his infection was not effectively controlled. Therefore, GM-CSF was administered additionally to improve phagocytic activity of leukocytes. He was finally cured after receiving a combination of aggressive surgery, liposomal amphotericin B and GM-CSF. To our knowledge, this is the first detailed clinical description of the treatment of mucormycosis with liposomal amphotericin B in Korea.
Geometric Modeling and Radioactive Modeling of RVI Using MAVRIC Code

In 2017, the permanent shutdown of Kori Unit 1 was decided, marking the initiation of preparations for the decontamination and decommissioning of Kori Unit 1. The dismantling of radiologically contaminated equipment and concrete structures such as the Reactor Vessel (RV), Reactor Vessel Internals (RVI), and the Bio shield is crucial in the nuclear decommissioning process. These components became radiologically contaminated due to nuclear fission reactions occurring in the reactor during its operational period. The RVI dismantling at Spain’s Jose Cabrera Nuclear Power Plant involved the use of mechanical saws and disk cutters to divide it into approximately 430 pieces, taking 16 months to complete. Germany’s Stade Nuclear Power Plant employed mechanical circular saws to segment their RVI into about 170 pieces, which took 30 months to accomplish. Meanwhile, the RVI at Germany’s Wurgassen Nuclear Power Plant was subdivided into approximately 1,200 pieces using a combination of mechanical saws and abrasive water jets, requiring 61 months for completion. Due to the radioactivity in Kori Unit 1’s Reactor Vessel (RV) and Reactor Vessel Internals (RVI), remote-controlled systems were developed for cutting within the cavity to reduce radiation exposure. Specialized equipment was developed for underwater cutting operations. This paper focuses on modeling related to RVI operations using the MAVRIC code. The upper and lower parts of the RVI are classified as low-level radioactive waste, while the sides of the RVI that come into contact with fuel are classified as intermediate-level radioactive waste. Therefore, the modeling presented in this paper only considers the RVI sides since the upper and lower parts have a minimal impact on radiation exposure. Accurate calculations were performed through geometric modeling and radiation dose modeling. These research findings are anticipated to contribute to enhancing the efficiency and safety of nuclear reactor decommissioning operations
Gamma-aminobutyric acid-salt attenuated high cholesterol/high salt diet induced hypertension in mice

Excessive salt intake induces hypertension, but several gamma-aminobutyric acid (GABA) supplements have been shown to reduce blood pressure. GABAsalt, a fermented salt by L. brevis BJ20 containing GABA was prepared through the post-fermentation with refined salt and the fermented GABA extract. We evaluated the effect of GABA-salt on hypertension in a high salt, high cholesterol diet induced mouse model. We analyzed type 1 macrophage (M1) polarization, the expression of M1 related cytokines, GABA receptor expression, endothelial cell (EC) dysfunction, vascular smooth muscle cell (VSMC) proliferation, and medial thicknesses in mice model. GABA-salt attenuated diet-induced blood pressure increases, M1 polarization, and TNF-α and inducible nitric oxide synthase (NOS) levels in mouse aortas, and in salt treated macrophages in vitro. Furthermore, GABA-salt induced higher GABAB receptor and endothelial NOS (eNOS) and eNOS phosphorylation levels than those observed in salt treated ECs. In addition, GABA-salt attenuated EC dysfunction by decreasing the levels of adhesion molecules (E-selectin, Intercellular Adhesion Molecule-1 [ICAM-1], vascular cell adhesion molecule-1 [VCAM-1]) and of von Willebrand Factor and reduced EC death. GABA-salt also reduced diet-induced reductions in the levels of eNOS, phosphorylated eNOS, VSMC proliferation and medial thickening in mouse aortic tissues, and attenuated Endothelin-1 levels in salt treated VSMCs. In summary, GABA-salt reduced high salt, high cholesterol diet induced hypertension in our mouse model by reducing M1 polarization, EC dysfunction, and VSMC proliferation.
Anti-Oxidant Activities of Acanthopanax senticosus Stems and Their Lignan Components

The antioxidant activities of Acanthopanax senticosus stems were evaluated in $CCl_4$-intoxicated rats. The n-butanol fraction from the water extract of the stems, when pretreated orally at 200 mg/kg/day for 7 consecutive days in rats, was demonstrated to exhibit significant increases in antioxidant enzyme activities such as hepatic cytosolic superoxide dismutase, catalase and glutathione peroxidase by 30.31, 19.82 and 155%, respectively. The n-butanol fraction whereas showed a significant inhibition of serum GPT activity (65.79% inhibition) elevated with hepatic damage induced by $CCl_4$-intoxication. Eleutheroside B, a lignan component, isolated from the n-butanol fraction was found to cause a moderate free radical scavenging effection DPPH, its scavenging potency as indicated in $IC_{50}$ value, being 58.5$\mu$ M. These results suggested that the stems of A. senticosus possess not only antioxidant but also hepatoprotective activities.

<P>This paper presents a new buck-boost converter. Unlike the single-switch buck-boost converter, the proposed converter has low-voltage stresses on semiconductors. Moreover, although both the conventional twoswitch buck-boost (TSBB) and the proposed converters have the same number of passive and active components, and the proposed converter can reduce the conduction loss as a result of having fewer conducting components. Therefore, the proposed converter obtained a higher efficiency than the TSBB converter. A 48-V output voltage and 150-W output power prototype was fabricated to verify the effectiveness of the proposed converter.</P>

PURPOSE: Forward head posture (FHP) is a head-on-trunk malalignment that results in musculoskeletal dysfunction and neck pain. To improve forward head posture, both the craniocervical flexion exercise (CCFE) and the visual guide (VG) technique have been used. This study compared the immediate effects of CCFE and VG combined with CCFE on craniovertebral angle (CVA), as well as on the activity of the sternocleidomastoid (SCM) and anterior scalene (AS) muscles during CCFE in subjects with FHP. METHODS: In total, 16 subjects (nine males, seven females) with FHP were recruited using the G-power software. Each subject conducted CCFE and CCFE combined with VG in random order. The CVA was recorded using a digital camera and the ImageJ image analysis software. The EMG data of SCM and AS were measured by surface electromyography. A paired T-test was used to assess differences between the effects of the CCFE and VG combined with CCFE interventions in the same group. RESULTS: The CVA was significantly greater for CCFE combined with the VG than for CCFE alone (p<.05). The activity of the SCM and AS muscles was also significantly greater when the VG was combined with CCFE than during CCFE alone across all craniocervical flexion exercise phases (p<.05). CONCLUSION: Use of the VG technique combined with CCFE improved FHP in subjects with FHP compared to CCFE alone.

Meningococcal infection is a life threatening disease that leaves serious sequelae in spite of appropriate treatment, thus vaccination for high risk groups are Important for the prevention of meningococcal diseases. However, the vaccine for Neisseria meningitidis has not been available in Korea until we introduced bivalent (serogroup A and C) polysaccharide vaccine for the first time for relief works in our university hospital. The vaccine was administered from January 2005 to March 2007 to 317 persons. of the groups administered, the largest group among them were 133 (133/317, 42%) students who planned to study abroad and needed the vaccination for secure entrance to school dormitories. This group was followed by health care workers, travellers to the regions of the world with high risks of meningococcal diseases, and splenectomised patients. To ratioalize the domestic use of meningococcal vaccine, the availability of vaccines first needs to be simplified by introducing them to the domestic market; for this to be possible, the approval system for vaccines should be reformed and epidemiogical studies need to be carried out.
