Rifampicin에 의한 것으로 추정되는 위막성 대장염1예

Pseudomembranous colitis, caused by altering the normal colonic flora and allowing the multiplication of Clostridium difficile, is an deleterious adverse effect of antibiotics. But it is rarely reported by rifampicin. Rifampicin is one of the first line drug in the treatment of tuberculosis and many patients are exposed to its potential adverse effects. We experienced a patient that had abdominal discomfort and hematochezia due to pseudomembranous colitis after receiving antituberculous medication, and which was probably caused by rifampicin. A 82 years old man was admitted with abdominal discomfort and hematochezia for one week. On the past history he had been diagnosed as endobronchial tuberculosis about 4 months ago. Colonoscopy revealed multiple discrete whitish mucosal lesion on rectosigmoid colon, and histologic findings were consistent with pseudomembranous colitis. The antituberculous agents were discontinued and vancomycin was administered. The patient's symptoms were resolved within several days. There was no recurrence after reinstitution of the antituberculous agents excluding rifampicin. We report here on a case of pseudomembranous colitis probably due to rifampicin.

Tracheobronchopathia osteoplastica is a rare disorder of unknown cause characterized by the projection of numerous bony or cartilaginous submucosal nodules into the tracheobronchial lumen with sparing of the posterior membranous portion of tracheobronchial tree. In the past, most of the cases were diagnosed as secondary finding at autopsy. Nowaday, the key component of the evaluation is flexible fiberoptic laryngoscopy and bronchoscopy, which typically demonstrates irregular spicules of subrnucosal bone and cartilage projecting into the tracheobronchial lumen and causing various degrees of airway obstruction. A 45 years old man was visited Sunchon St. Carollo hospital due to discomfort on throat. We saw several nodules just below vocal cord via laryngoscope. The computed tomography revealed multiple small calcified nqdules on trachea and both main bronchi. Pathologic finding of bronchoscopic biopsy showed abnormal proliferation of bony and cartilagious nodules in the tracheal submucosa. These fingdings were consistent with tracheobronchopathia osteoplastica. We report here on a case of tracheobronchopathia osteoplastica with review of literature.

Relation between the microstucture and mechanical property of Al-xFe-yCr alloy, with various types, volume fractions and sizes of dispersoids, has been studied. 'The alloys contain high concentrations of transition elements(Cr, Fe) which form thermally stable precipitates in the aluminium matrix. High temperature tensile tests were performed to investigate the possibility of elevated temperature applications. Oxidation of powder surface during atomizing strengthens the bulk extrudates by providing the obstacle layer. The oxide layer is very chemically and thermally stable and prevent the coarsening of the intermediate compounds. X-ray diffraction shows that the volume fraction of the α-Al matrix and the intermetallic compounds in the extrudates are changed depending on the composition of alloying elements. The extrudates exhibit a fine microstructures.

<P>Mitochondrial dysfunction reflects a lifelong cumulative burden of cellular damage, and a decrease in mitochondrial DNA (mtDNA) copy number is associated with oxidative stress and chronic inflammation. The goal of this study was to assess whether mitochondrial dysfunction and a decrease in mtDNA copy number are common features of patients with obstructive sleep apnea syndrome (OSA). We compared mtDNA copy number between 20 healthy volunteers and 20 patients with OSA and investigated whether a significant attenuation of mtDNA copy number was observed in genomic DNA isolated from whole blood of OSA patients. Our observations lead to the hypothesis that mtDNA copy number is lower in whole blood DNA of OSA subjects and might be related to OSA severity, reflecting excessive oxidative stress in patients with OSA.</P>
위선종과 조기위암에서 투명캡을 이용한 흡인점막절제술의 임상적 유용성 평가

Background/Aims: Endoscopic aspiration mucosectomy (EAM) can always lift up the lesion by suction regardless of its location and does not injure the mucosal surface. The clinical usefulness of this method was evaluated as follows. Methods: Thirty two patients with gastric adenoma (35 lesions) and 6 patients with early gastric carcinoma were treated by EAM from March 1997 to February 1999. Their characteristics (diameter, macroscopic appearance, and histologic diagnosis) were verified at endoscopy with a biopsy specimen. The complete resection was defined as the presence of normal mucosa at the resected margin. Results: The average size of the resected specimens was 17.8 mm in antrum and 16.5 mm in body. The ratio of complete resection by location was 67% in anterior wall, 88% in posterior wall, 86% in lesser curvature and 92% in greater curvature. The complete resection rate for smaller lesions (≤15 mm) was highly distinguished from that cf larger lesions (>15 mm). No serious complication was encountered. Coclusions: EAM is suitable for the treatment of gastric tumors. Lesions 15 mm or less in diameter can be resected easily by single procedure regardless of its location.
한국 유·초·중등 미술표현 발달 측정을 위한 검사지 및 검사지 분석 도구 개발

본 논문에서는 국내 유·초·중등 학생들의 미술표현발달 과정에 활용된 검사지와 검사지 분석 도구가 제시 되었다. 지금까지 미술표현발달연구들의 문제점이었던 주관적 분석방법을 극복하고 한국형 미술표현 발달과정을 제시하기 위한 자료 수집과 분석에서 활용 될 수 있는 검사 도구를 개발함으로서 보다 객관성과 신뢰도가 높은 한국 유·초·중등 미술표현발달과정에 대한 연구가 진행 되도록 했다. 특히 검사도구 개발과 더불어 검사 도구의 활용 방법과 검사 결과에 대한 해석 방법을 제시함으로서 교육현장에서 학생들의 개인적 미술표현발달 정도를 파악 하는데 활용 되도록 했다. 예비검사와 본 검사 과정을 통해 개발된 검사지와 검사지 분석 도구는 미술표현발달에 대한 연구가 보다 과학적으로 진행되는데 기여 할 것이며 한국 학생들의 표현 능력 발달 정도를 측정하는데, 학생 개인의 미술표현발달 정도 측정하는데 크게 기여할 것으로 사료된다. This study is to make out objective criteria for the developmental process of artistic expression in Korean students. In the process, researchers felt the need objective methodology reliable. Because with the subjective methodology, which has been used in the discipline of art education so far, the result has been faced the lack of reliability. Therefore researchers made up mind to develop the objective test, and we made Development Test of Artistic Expression(DTAT). DTAT has been executed from the age of 4 through 15, nationwide, except Jeju Province. Its total population was 6,036 and rater reliability is .835, and the Cronbach alpha is .816. It means the test is reliable. As a result, researchers put out the criteria to rate the items and norm table, which can give information about the characteristic and grade with regard to one’s points. When looking back upon the past, when there has been no ways or methods to give out objective perspective in regard to artistic development. DTAT has posibility to be used for assessing one’s developmental stage in the discipline of art education and other related discipline, such as special education, etc.

본 연구에서는 알루미늄 폼 코어 복합재료의 기계적 성질을 시뮬레이션 해석을 통하여 고찰하였다. 시험편 모델에는 50J, 70J, 100J의 충격에너지를 가하였다. 50J, 70J 및 100J의 충격에너지에 대해서는 각각 3.4 ms, 3.2 ms 및 3.2 ms의 시점에서 최대하중이 발생하였다. 50J의 충격에너지를 가한 경우는 스트라이커가 복합재료의 상부 면재 를 뚫고, 심재까지 손상을 입혔다. 그러나 하부 면재까지는 손상이 없이 그대로이었고 52 J의 에너지가 발생하였다. 70J의 충격에너지를 가한 경우는 스트라이커가 상부 면재를 관통한 후 하부 면재까지 손상을 주었고 65 J의 에너지 가 발생하였다. 100J의 충격에너지를 가한 경우는 스트라이커가 면재와 심재를 완전히 관통하여 손상을 주었다. 스트 라이커가 상부 면재를 관통하는 시점에서는 최대하중이 되고 그 후로는 하중이 급격히 감소하다가 하부 면재에 도달 할 때에 다시 급격히 증가하고 있다가 감소하였고 95 J의 에너지가 발생하였다. In this study, mechanical property on the composite material of aluminum foam core is investigated by simulation analysis. Impact energies such as 50J, 70J, and 100J are applied to the specimen model. The maximum load occurs at 3.4ms for 50J, 3.2ms for 70J, and 3.2ms for 100J respectively. The striker penetrates the upper face sheet, causing the core to be damaged at 50J test but the lower face sheet remains intact with no damage. It results in occurring with the energy of 52 J. At 70J test, it penetrates the upper face sheet and penetrated the core. And the striker causes the lower face sheet with damage. And it results in occurring with the energy of 65 J. Finally at 100J test, the striker penetrated both the upper face sheet and core and even the lower face sheet. The load becomes maximum at the time when striker penetrates through the upper plate and it rapidly reduced. And then the load increases rapidly when reaching the lower face sheet. And it decreases again. It results in occurring with the energy of 95 J.
