      • High-Resolution Capacitive Microinclinometer With Oblique Comb Electrodes Using (110) Silicon

        <P>We propose a new capacitive microinclinometer where oblique comb electrodes and double-folded suspension springs are aligned parallel to the vertical (111) plane of (110) silicon. The oblique comb utilizes both the overlapped area and the gap between movable and stationary electrodes, resulting in a considerable increase in sensitivity (capacitance change/angle). The resolution becomes even higher by taking advantage of the smooth (111) sidewalls of the oblique comb electrodes, which are fabricated using silicon deep reactive ion etching followed by crystalline wet etching. The surface roughness was reduced from 200 (R<SUB>rms</SUB>) to 20 nm (R<SUB>rms</SUB>), and the verticality was improved from 88.7° to 89.7°. The capacitance change of the fabricated inclinometer experimentally ranges from -0.793 to 0.783 pF for the full range of inclination angle (from -90° to 90°). The estimated worst resolution, which is obtainable at an inclination angle of ±85°, was as low as 0.25°.</P>

      • Large infantile hemangioma with subfascial location: a case with difficulty of clinical diagnosis

        The unexpected finding of newly growing lump in an infant induces anxiety to parents and results in an urgent visit to hospital. Before performing invasive interventions for this infant patient, benign conditions like infantile hemangioma (IH) should be excluded as it could show variable clinical characteristics. A 6-month-old girl was presented with protruding skin-colored tumor on right upper back, with approximately 10cm in diameter and without surface change. The tumor abruptly enlarged over the period of 2 weeks without any previous episode. MRI showed a lobulated hypervascular mass in the subfascial layer with neovascularization inside. Through this findings, thought by radiologist and orthopedic surgeon to be malignant tumor, exploratory surgery was planned. Before surgical excision, this patient was consulted to our department. As the lesion was softly palpable and movable, incisional biopsy was performed to rule out the benign tumor like deep type IH. Histopathologic examination showed numerous vessels lined by plump endothelial cells. Immunohistochemical staining revealed the positivity of endothelial cells for CD-31, GLUT-1. Under the diagnosis of IH, the patient received oral propranolol medication for 1 year. Following the patient, we found the tumor diminished dramatically in volume. Although IHs are common benign tumor in infancy, Subfascial location is unfamiliar and rare. So, we herein report a case of subfascial lH.

      • A Study on the Correlation of Left Atrial Volume Change according to Left Ventricular Contraction in Atrial Fibrillation

        The purpose of this study was to increase accuracy and success rate of radiofrequency catheter ablation by analysis of left atrial volume change relative to left ventricular volume change and left atrial diameter change relative to left atrial volume change. 20 patients with normal heart rhythm were set as the control group, and 20 patients with diagnosed atrial fibrillation were set as the experimental group. The cardiac cycle was divided into 10 stages ranging from 0% to 90%, and left ventricular volume, left atrial volume, left atrial diameter, pulmonary vein diameter were measured for each group. the correlation between the left ventricular volume and the left atrial volume was analyzed, and the correlation between the left atrial volume and left atrial diameter was analyzed. The diameter of the pulmonary veins was measured. As a result of analyzing the correlation between left ventricular volume and left atrial volume through Spearman correlation, the control group showed a negative correlation.(P<0.001) The experimental group were not correlated.(P=0.075) As a result of analyzing the correlation between the left atrial volume and the left atrial diameter, both groups showed a positive correlation.(P<0.001) As a result of analyzing the diameter of the pulmonary vein by Wilcoxon signed-rank test, both groups showed a significant difference in the maximum diameter and the minimum diameter.(P<0.001) Because the volume and diameter of the left atrium in patients with atrial fibrillation change irregularly, the radiologist check all cardiac cycle and reconstruct to increase the accuracy and success rate of radiofrequency catheter ablation.

        Low-level Green and Red Laser Treatment of Shaochong (HT9)·Dadun (LR1) and Shaohai (HT3)·Yingu (KI10) Acupoints in a Rat Model of Focal Cerebral Ischemia

        Low level laser therapy (LLLT) has facilitated an improvement in acupuncture treatment. In this study, we stimulated Shaochong (HT9), Dadun (LR1), Shaohai (HT3), and Yingu (KI10) acupoints with pulsed laser diodes 532 nm [green laser] and 658 nm [red laser] in rats with induced middle cerebral artery occlusion(MCAO). The animals were divided into 6 groups: intact control; MCAO control without LLLT; LLLT with red laser at HT9·LR1 and HT3·KI10 (RR); LLLT with green laser at HT9·LR1 and HT3·KI10 (GG); LLLT with green laser at HT9·LR1 and red laser at HT3·KI10 (GR); and LLLT with red laser at HT9·LR1 and green laser at HT3·KI10 (RG). We evaluated the immunohistochemical changes in the hippocampal CA1 region, and complete blood count changes. Compared to the MCAO control group, the RG group showed a significant decrease in Bax and cytochrome c levels in the hippocampus, and a significant increase in hemoglobin level, hematocrit, total white blood cell, neutrophil, lymphocyte, monocyte, and erythrocyte counts.

        모반세포성 모반에서 발생한 악성 흑자 1 예

        Lentigo maligna(LM) is a pigmented lesion that occurs on the sun-exposed skin of an older patients and may become lentigo maligna melanoma(LMM). Currently the percentage of LM type melanoma appears to be increasing, and this rise has been attributed to increasing UV radiation exposure. In addition to UV radiation, estrogen and progesterone, and nonpermanent hair dyes have been proposed as risk factors. A case is a 66-year-old female with a 3 years history of pigmented patch. She had a gradually enlarging irregular bordered brown to black mottled patch on the right temple area. Histologic examination revealed atypical melanocytes along the basal layer of atrophic epidermis, arranged in solitary units and small nests. Perilesional skin showed many scattered melanophages and regressing nevus cell nests which suggested preexisting nevus underwent regressive change. On the basis of clinicopathologic examination we diagnosed this case as lenigo maligna arising from nevocytic nevus and then we removed the lesion by wide surgical excision and could not find any recurrence until now.

        소매틱스 프로그램이 고등학생의 자세인식과 신체적 · 심리적 요인에 미치는 영향

        본 연구의 목적은 소매틱스(Somatic) 프로그램을 적용하여 고등학생들의 자세인식과 통증의 변화를 분석하고 신체적, 심리적 변화과정을 탐색하는데 있었다. 연구대상은 서울시 소재 1개 고교의 학생 25명을 목적적으로 선정하였으며 학생들의 자세인식과 통증정도를 파악하였다. 이후 소매틱스 프로그램을 주 2회 1시간씩 총 4주에 걸쳐 8차시를 실행한 후 자료수집이 완료되었다. 자료분석 도구는 학생들의 자세 인식 분석을 위해 개방 설문지와 심층면담, 차시가 거듭됨에 따라 나타나는 심리, 신체적 변화를 파악하기 위해 학생일지가 이용되었다. 사전-사후검사에 따른 변화를 검증하기 위한 자료처리는 SPSS 18.0 통계 패키지가 이용되었으며 통계적 유의수준은 .05에서 설정되었다. 자료분석 결과는 아래와 같이 나타났다. 첫째, 소매틱스 프로그램은 학생들의 자세인식 변화와 통증개선에 긍정적 효과를 나타냈다. 둘째, 신체적 변화과정에서 호흡, 긴장, 통증 요인은 감소하였고 신체인지 능력이 높아졌다. 셋째, 심리적 변화과정에서는 심리적 안정요인은 감소하였고 자세인식, 집중력, 흥미요인이 증가하였으며, 그 중 자세인식 요인이 가장 많은 증가율을 나타냈다. 본 연구는 소매틱스 프로그램이 학생들의 자세인식과 통증개선에 효과적이며, 학생들에게 신체인지능력을 향상시킨다는 도움이 된다는 사실을 보여주었다. 따라서 소매틱스 프로그램이 학교 체육교육의 한 프로그램으로서 제공될만한 가치가 충분함을 나타내고 있다. 특히 다양한 자세문제와 기능적 불편함이나 장애를 갖고 있는 학생들에게 더욱 유용할 것임을 강조하고 있다. 아울러 본 기술연구는 자세교육의 중요성과 소매틱스를 활용한 실천적 자세교육의 중요성에 대한 인식제고와 실천의 장이 시급하다는 사실을 나타내고 있다. Purpose: The purpose of this study was to explore the high school students’ postural perspectives and pain. Methods: The subjects for this study were 25 high school students who purposively selected at A high school in Seoul metropolitan area. Before conducting somatics programs, pretest was applied to determine the students’ postural perspectives and pain. Somatics program was conducted two times a week through 4 weeks. Posttest was conducted after total 8 classes were completed. Somatics programs included total 9 items. PASW(Predictive Analytics Software) Statistics 18.0 program was utilized and statistical alpha level was set at .05. Chi-square analysis was used to determine students’ posture perspectives and t-test was conducted to determine the rate of pain through pre and post-test. Students’ diary and participation record were used to analyze students’ physical and psychological changes. Results: The results of this study shows that somatics program was positively effective to enhance students’ postural perspectives. Somatics also contribute to students’ pain reliefs. Students’ physical change was significantly showed in breathing, tension, and pain reliefs. physical awareness also improved. In psychological change, concentration, interest, and postural perspectives were notably improved. Conclusion: The study strongly suggests that somatics can be applied as a one of school physical education programs. It clearly will be benefited for the students who suffers various postural problems and functional ineffectiveness.

        Prevention of Adjacent Segmental Disease after Fusion in Degenerative Spinal Disorder: Correlation between Segmental Lumbar Lordosis Ratio and Pelvic Incidence–Lumbar Lordosis Mismatch for a Minimum 5-Year Follow-up

        Study Design: Retrospective study. Purpose: Associations among risk factors related to adjacent segmental disease (ASD) remain unclear. We evaluated the risk factors and segmental lordosis ratio to prevent ASD developing after lumbar spinal fusion. Overview of Literature: Risk factors related to ASD development are age, sex, obesity, pre-existing degeneration, number of fusion segments, and decreased postoperative lumbar lordosis (LL). However, the associations among these factors are still unclear and should be clearly identified. Methods: We retrospectively reviewed data on 274 patients who underwent lumbar spinal fusion of three segments or below for lumbar degenerative disease from January 2010 to December 2012, with over 5 years of follow-up. Patients with preoperative sagittal vertical axis (SVA) >5 cm were excluded due to sagittal imbalance. A total of 37 patients with ASD and 40 control patients (CTRL) were randomly selected in a similar distribution of matching variables: age, sex, and preoperative degenerative changes. Sex, age, number of fusion segments, radiologic measurements, L4–5–S1/L1–S1 LL ratio, and spinopelvic parameters (pelvic incidence [PI], pelvic tilt [PT], sacral slope [SS], and SVA) were analyzed. Logistic regression was used to analyze the correlation between PI–LL mismatch and L4–5–S1 segmental lordosis rate. Results: No significant difference was found between ASDs and CTRL groups regarding age, sex, number of fusion segments, fusion method, and preoperative and postoperative spinopelvic parameters (PI, SS, PT, and LL). However, regarding the L4–5–S1/L1–S1 lordosis ratio, 50% (p=0.045), 60% (p=0.031), 70% (p=0.042), 80% (p=0.023), and 90% (p=0.023) were statistically significant; <20% (p=0.478), 30% (p=0.223), and 40% (p=0.089) were not statistically significant. In the postoperative PI–LL <10 group, ASD occurred less frequently than in the PI–LL >10 group, and the difference was statistically significant (p=0.048). Conclusions: Patients with a postoperative L4–5–S1/L1–S1 lordosis ratio >50% had less occurrence of ASD. Correcting LL according to PI and physiologic segmental lordosis ratio is important in preventing ASD.

        PVS 유도체에 대한 L-Cysteine 의 친핵성 첨가반응에 관한 연구

        Phenylvinylsulfone derivatives were synthesized by Kirners condition. The structure of these compounds were ascertained by means of ultraviolet, melting point, IR and ^1H-NMR spectra. The nucleophilic addtion reaction kinetics of L-cysteiene for phenylvinylsulfone was investigated by ultraviolet spectrophotlmetery in 40% EtOH-H_2O at 25℃. The rate equations which were applied over a wide pH 1.0~13.0 range. On the basis of general base catalysis and confirmation of addtion reaction product, the nucleophilic addtion reaction kinetics of L-cysteiene for phenylvinylsulfone were measured by the pH change. From the result of the above caption, a plausible nucleophilic addtion reaction mechanism of L-cysteiene for phenylvinylsulfone was proposed. These compounds may by used ad the starting materials for the preparation of the engineering plastics or the germicide.

        부인과 암수술에 있어서 외과의사의 역할

        Purpose: Resection of the bowel or solid organs may be required for pelvic tumor surgery. The present study was performed to assess combined surgical procedures and determine the role of the general surgeon in gynecologic oncology surgery Methods: We performed a retrospective study of 135 patients with gynecologic malignancy who underwent a combined operation with a general surgeon between January 2000 and December 2005 at Asan Medical Center. The purpose of the combined operation was categorized into 3 groups: a tumor debulking operation, a resolution of intraoperative complications by a gynecologic surgeon, or an intraoperative diagnostic change. Results: The incidence of combined operations for debulking, resolution of intraoperative complications, and intraoperative diagnostic changes were 103 cases (76.3%), 22 cases (16.3%), and 10 cases (7.4%), respectively. Ovarian cancer was the most common gynecologic malignancy (74.1%) and the rate of a combined operation with a general surgeon in ovarian cancer was 18.5% during the time period. Colorectal resection was the most common procedure by a colorectal surgeon (61.5%). Twenty four patients (17.8%) experienced postoperative complications, including ileus, wound infection, pancreas leakage, and stomal necrosis, with no significant differences according to the purpose of combined operation, pathologic diagnosis, or bowel preparation. The rate of a preoperative consultation to a general surgeon by gynecologic surgeons in cases with colorectal or solid organ invasion preoperatively was 53.2%. Conclusion: A general surgeon is an important consultant in a debulking operation of gynecologic malignancies, especially ovarian cancer, or treatment of complications. Thorough preoperative evaluation, bowel preparation, and preoperative consultation to a general surgeon is important in gynecologic oncology surgery.

        N/A Objectives : Oxidized LDL is thought to play a key role in atherogenesis. Among their wide variety of biological properties, oxidized LDL injures the endothelium as an early event of atherogenesis. However, the mechanisms by which oxidized LDL injures endothelial cells are not definitely known. In order to evaluate the involvement of GTP-binding protein on the mechanism of apoptosis, we studied the effects of pertussis and cholera toxin on oxidized LDL-induced apoptosis in bovine aortic endothelial cells(BAECs). Methods : Oxidized LDL elicited apoptosis in cultured BAECs as shown by characteristic morphological and biochemical changes. Chromatin condensation and nucleus fragmentation were visualized by using fluorescence microscopy of intact cells staining by acridine orange/ ethidium bromide. DNA fragmentation was quantified by an ELISA with specific antibody for bromodeoxyuridine- labelled DNA fragments and confirmed with DNA ladder formation. Results : Studies using a combination of bacterial toxins which change the function of GTP-binding protein suggest that oxidized LDL-induced apoptosis was regulated by GTP-binding protein. Oxidized LDL-induced apoptosis was not changed by pretreatment of BAECs with pertussis toxin. In contrast, pretreatment with cholera toxin completely prevented the oxidized LDL- induced apoptosis. Conclusion : These results show that oxidized LDL induces apoptosis of BAECs and suggest that cholera toxin-sensitive G-proteins are involved in signal transduction of oxidized LDL-induced apoptosis of BAEC.
