Cisplatin에 의해 유발된 급성신부전에 단삼추출액이 미치는 영향

한의학에서 강장약, 통경약, 진통약, 항염약, 지혈약으로 월경불순, 복통, 폐경, 산후통 등에 효과가 있다고 알려진 단삼(Salvia mulitorrhiza Bunge)이 cisplatin에 의해 유발된 신장의 세포손상에 대하여 항산화작용과 신장의 기능에 미치는 영향에 대하여 조사한 결과 다음과 같은 결과를 얻었다. 1. 단삼 추출액이 cisplatin에 의하여 유발된 신피질 절편의 독성효과에 대한 효과를 알아 보기위한 실험으로 300mM cisplatin에 노출시켰을 때 단삼 추출액 0.05%, 0.1%를 함께 첨가한 군이 LDH 유출 및 지질과산화를 유의성 있게 감소시켰다. 2. 단삼 추출액 0.3g/kg을 7일간 전처리 한 다음 cisplatin 5㎎/kg을 복강투여한 후 24시간 뒤에 혈액을 채취하여 혈청 BUN과 creatinine 농도를 측정한 결과, 단삼 추출액을 전처리 한 군에서 cisplatin 단독처리한 군보다 신장의 BUN 및 creatinine 제거율이 유의성 있게 높게 나타났다. 이상의 결과로 볼 때 단삼 추출액은 cisplatin에 의하여 유발된 신장의 독성효과에 대하여 항산화작용과 세포보호 작용을 나타내며, 신장의 기능에도 영향을 미쳐 BUN과 creatinine 제거율도 높게 나타내었다. 따라서 단삼이 신장의 세포손상이나 질환이 있을 경우 보호효과를 나타낼 것으로 생각되며, 이러한 계기로 단삼을 대량 재배하여 신장보호를 위한 기능성 식품을 개발할 수 있다면 신장기능부전환자나 단삼재배 농가에 큰 도움을 줄 수 있으리라 생각한다. Salvia mulitorrhiza Bunge is known to be effective on healthiness drug, pain-killing drug, anti-inflammation, hemostatic agent, irregular menstruaction, abdominal pain, menopause, after parturition pain activity in oriental medicine. In addition of the effects, I further investigated the effect Salvia mulitorrhiza Bunge extract(SMBE) on the antioxidant effect on a renal cortical slices cell and kidney protecting effects. The results were as follows. 1. When renal cortical slices separated from a rabbit's kidney were treated with oxidant 300μM cisplatin in the presence of 0.05% or 0.1% SMBE at 37℃ for 60min. Cisplatin caused an increase a LDH release and lipid peroxidation. Cisplatin also induced a increase in LDH release and lipid peroxidation. SMBE significant prevented cisplatin-induced increase in LDH release and lipid peroxidation. 2. Pretreatment with 0.3g/kg SMBE for seven day and treatment with 5mg/kg cisplatin by the intraperitoneal injection. After 24hours, BUN and creatinine in serum were measured. The results were that the pretreatment group with SMBE showed a significant clearance rate of BUN and creatinine in the kidney than the administering single agent group of cisplatin. These results indicate that lipid peroxidation plays a critical in cisplatin-induced acute renal failure. SMBE exerts the protective effect against acute renal failure induced by cisplain, and its effect may be attributed to an antioxidant action. Furthermore I suggest the development of functional food with SMBE for the kidney tissue damage or renal incompletion in human.

<P>We isolated a highly serine protease-producing <I>Bacillus subtilis</I> strain (PRY) from a clinical sample and identified it through biochemical testing and ribosomal DNA sequencing. The PRY strain exhibited a robust swarming behavior and was able to digest human transferrin efficiently, concomitantly with the production of catechol-siderophore in the exponential growth phase. The growth of PRY was in proportion to increased iron availability resulting from transferrin destruction. These results suggest that proteases of the <I>B. subtilis</I> PRY strain may play a significant role in the pathogenesis of human infections by facilitating siderophore-mediated iron uptake from transferrin and swarming motility.</P>
표준 전자선 cone의 확장된 SSD에서의 선량평가 및 자체제작한 전자선 cone의 특성

In general, the patients of the head and neck cancer are treated with 4MV photon beam up to prescribed dose, but spinal cord should be excluded in the treatment field. When its absorbed dose is limited at the tolerance dose. In case of the patients who has the positive posterior neck nodes need a boost electron beam treatment to the prescribed dose. In that case, the anatomical structure of the neck and the physical structure of the standard electron cone interrupt to allow proper access to the disease site. Therefore, we extended treatment SSD for the remove of the those hindrances. In this study, we evaluated the dosimetric variation of the standard electron cone for the extended SSD, from 100cm to 120cm, 5 cm increment, and compare to the custom-made electron cone. As a result, the $\%$ depth dose, the point of maximum dose and the range of maximum were changed within the $2\%$. The penumbra width was increased from 1.0cm to 2.0cm. However, the dosimetric characteristics of the custom-made electron cone was very similar to that of the 100cm SSD standard electron cone and due to its characteristic of physical structure, patients didn't need re-positioning after photon beam treatment, therefore accurate treatment was possible, we conclude that the custom-made electron cone was very useful for the clinical practice.

Purpose: To compare the efficacy and safety of conventional scleral fixation (C-SF), retropupillary iris-claw intraocular lens (RP-IOL) implantation, and intrascleral fixation (ISF). Methods: This retrospective observational study included 58 patients (58 eyes) who underwent C-SF (23 eyes), RP-IOL (23 eyes), and ISF (12 eyes) by a single surgeon at Samsung Medical Center from October 2017 to July 2020 and were followed up for at least 6 months. This study analyzed various clinical outcomes before surgery, and 1 day, 1 week, 1 month, 3 months, and 6 months after surgery. Results: Six months after surgery, best-corrected visual acuity in logarithm of minimum angle of resolution was 0.08 ± 0.10, 0.08 ± 0.16, and 0.03 ± 0.04 in C-SF group, RP-IOL group, and ISF group, respectively, and there was a significant improvement in each group compared to preoperative best-corrected visual acuity. All groups showed a significant increase in astigmatism postoperatively, but no between-group differences were observed. The prediction error was -0.15 ± 0.77, 0.56 ± 0.62, and 0.44 ± 1.00 diopters in the three groups, respectively, indicating RP-IOL group and ISF group for hyperopic shift. The three groups did not differ in terms of absolute prediction error. Six months after surgery, the corneal endothelial cell counts were 2,073 ± 691, 2,014 ± 692, and 1,712 ± 891 cells/mm2, respectively, which were lower than before surgery. IOL dislocation occurred in five eyes only in RP-IOL group, two of which underwent two reoperations, and reenclavation was performed smoothly without complications in all cases. Conclusions: Although the frequency of IOL dislocation in RP-IOL group was higher than that in the other groups, it can be reenclavated relatively easily. As a method of secondary IOL fixation, both RP-IOL implantation and ISF were as effective as conventional scleral fixation.
