목적: 버찌씨 수술은 복압성 요실금 수술에 효과적인 수술로 여겨져 왔다. 요실금의 원인이 내인성 요도 괄약근의 결손소견 없이 요도의 과운동성으로 인한 경우 버찌씨 수술이 일반적으로 추천되어진다. 본 연구에서는 복강경 버찌씨 수술에서 최씨 훅의 유용성에 대해 알아보고자 하였다. 연구방법: 2000년 10월부터 2005년 3월까지 본원에서 복강경 버찌씨 수술을 시행받은 47명의 환자를 대상으로 하였다. 27명은 고전적 복강경 버찌씨 수술을 시행하였고 20명은 내골반 근막을 Cooper 인대를 부착하는데 유용한 본원에서 고안한 최씨 훅을 이용하여 복강경 버찌씨 수술을 시행하였다. 결과: 고전적 복강경 버찌씨 수술을 시행한 군과 최씨 훅을 이용한 복강경 버찌씨 수술을 시행한 군의 평균 나이, 분만력, 폐경률, BMI는 두 군간에 통계학적으로 유의한 차이를 보이지 않았다 (p>0.05). 수술 시간은 고전적 복강경 버찌씨 수술은 90.52±15.32분, 최씨 훅을 이용한 복강경 버찌씨 수술은 38.50±10.14분으로 최씨 훅을 이용한 경우 수술시간이 통계적으로 유의하게 짧았다 (p<0.05). 두 군간에 재발률 및 합병증 발생은 통계적 유의성을 보이지 않았다 (p>0.05). 결론: 본 연구에서 본원에서 고안한 최씨 훅을 이용하여 복강경 버찌씨 수술을 시행한 결과 수술 시간을 단축시킬 수 있었으며 수술 효과는 기존 방법과 동일하게 관찰되었다. 이에 최씨 훅은 복강경 버찌씨 수술에서 사용할 때 집도자의 봉합 과정을 단순화시켜 수술 시간을 단축시키므로 복강경 버찌씨 수술에 용이한 기구이다. Objective: Burch operation has been effective method for the treatment of stress incontinence. Burch operation is a well-accepted procedure for treating stress urinary incontinence secondary to urethral hypermobility without intrinsic sphincter deficiency and is the reference standard with which other procedures are compared. This study is to evaluate the effectiveness of the Choi`s hook in laparoscopic Burch operation. Methods: This study included 47 patients who had undergone laparoscopic Burch operation from October 2000 through March 2005. Of these 47 patients, 27 patients underwent traditional laparoscopic Burch operation (Group 1) and 20 patients underwent laparoscopic Burch operation with Choi`s Hook on anchoring the endopelvic fascia to the Cooper`s ligament (Group 2). Results: The mean age of patients of Group 1 was 55.6±9.37 years of age and that of Group 2 was 56.0±5.93 years of age. There is no difference in the mean age of patients (P>0.05). The mean operating time was 90.5±15.32 minutes for Group 1 and 38.5±10.14 minute for Group 2. The mean operating time was shorter in Group 2 than Group 1 (P<0.05). There was no difference in recurrence rate and complication rate between Group 1 and Group 2 (P>0.05). Conclusion: We performed laparoscopic Burch operation with the Choi`s Hook on anchoring the endopelvic fascia to the Cooper`s ligament. When we compared traditional laparoscopic Burch operation with laparoscopic Burch operation with Choi`s Hook, we could shorten the operation time using the Choi`s Hook.

<P><B>Objectives—</B></P><P>The purpose of this study was to investigate the quality of ultrasound (US) imaging for hepatocellular carcinoma screening.</P><P><B>Methods—</B></P><P>The investigation was performed at all medical institutes participating in the National Cancer Screening Program in Korea. For assessment of personnel, we inquired who was performing the US screenings. For phantom image evaluation, the dead zone, vertical and horizontal measurements, axial and lateral resolution, sensitivity, and gray scale/dynamic range were evaluated. For clinical image evaluation, US images of patients were evaluated in terms of the standard images, technical information, overall image quality, appropriateness of depth, foci, annotations, and the presence of any artifacts.</P><P><B>Results—</B></P><P>Failure rates for phantom and clinical image evaluations at general hospitals, smaller hospitals, and private clinics were 20.9%, 24.5%, 24.1% and 5.5%, and 14.8% and 9.5%, respectively. No statistically significant difference was observed in the failure rates for the phantom images among groups of different years of manufacture. For the clinical image evaluation, the results of radiologists were significantly better than those of other professional groups (<I>P</I> = .0001 and .0004 versus nonradiology physicians and nonphysicians, respectively). The failure rate was also higher when the storage format was analog versus digital (<I>P</I> < .001).</P><P><B>Conclusions—</B></P><P>Approximately 20% of US scanners failed the phantom image evaluation. The year of scanner manufacture was not significantly associated with the results of the phantom image evaluation. The quality of the clinical images obtained by radiologists was the best.</P>
Most uterine fibroids significantly decrease in size spontaneously after delivery

Objective: Uterine fibroid is most common benign tumor in childbearing age women and is increasing with age. As the maternal age of childbearing is getting higher in Korea, pregnancies with uterine fibroid are more common. Because of superfluous concern about the effect of fibroid on pregnancy, myomectomy beyond recommended guideline is sometimes performed and it can increase the risk of fatal complications during pregnancy like uterine rupture. The aim of this study was to reveal how much the size of uterine fibroid has decreased spontaneously in postpartum period. Methods: This study included woman who had fibroids during pregnancy and underwent transvaginal sonography within two months after delivery in our institution (study period: Jan 2010 - May 2020). The maximal diameter from uterine fibroids during all trimesters and in postpartum period was collected and compared by paired t-test. The changes were also assessed by the ratio of diameter of uterine fibroid during postpartum period divided by the diameter during pregnancy as a denominator. Results: A total 199 patient was included in this study. Median maternal age was 35 years old [range, 28-45]. Maximal diameter of uterine fibroid during pregnancy and after delivery was 6.6 cm [range, 1.4-17.3 cm], and 4.1 cm [range, 1.0-14.8 cm], respectively. Uterine fibroids significantly decrease in size after delivery, compared to during pregnancy (p<0.001). The median ratio of fibroid in postpartum period was 0.70 [range, 0.03-1.60]. In one-third of patients, the maximal diameter of fibroid was reduced by more than 40%. We also analyzed the difference of decreasing ratio between single and multiple uterine fibroids, but there was no significant difference. Conclusion: Most uterine fibroid significantly decrease in size after delivery, compared to the period of during pregnancy. Our results will help to reduce unnecessary worries for the pregnant women with fibroids and provide useful information to doctors for their counseling.

An Epstein-Barr virus (EBV)-based plasmid contains the EBV nuclear antigen 1 (EBNA1) gene and EBV replication origin (oriP) sequence. Since EBNA1 (the only EBV-encoded protein) is combined with oriP, it is replicated simultaneously with chromosomal DNA in human, primate, and canine cells and is faithfully segregated at a stable copy number upon cell division. Consequently, it can be used to stably express gene inserts over a prolonged time in target cells. We have previously shown that the polyamidoamine (PAMAM) dendrimer can be surface-modified with L-arginine. Arginine is present at a high frequency in the transactivator of transcription (Tat) sequences of human immunodeficiency virus (HIV). It presents high membrane permeability and permits effective transfer of DNA inside the cells. In this study, we constructed two kinds of recombinant DNA by inserting the luciferase gene and enhanced green fluorescence protein (eGFP) gene as reporter genes into the pCEP4 plasmid vector. We measured dynamic light scattering (DLS) and zeta potential after preparing PAMAM-based cationic polymer/EBV-based plasmid complexes. We performed transfection of HEK 293 cell lines with the polyplexes, and monitored luciferase activity and green fluorescence protein (GFP) expression. Our results show that PAMAM-based cationic polymer/EBV plasmid complexes provide enhanced and sustained gene expression.

<P>To enhance high power characteristics for piezoelectric transformer, an alloy design approach in PZT base ceramic system was considered. Various Zr/Ti ratio compositions in 0.03Pb(Sb0.5Nb0.5)O3- 0.03Pb(Mn1/3Nb2/3)O3-(0.94-x)PbTiO3-xPbZrO3 (0.445x0.475 mol)[PSN - PMN - PZT] system were synthesized by the conventional bulk ceramic processing technique. To improve high power characteristics at their morphotropic phase boundary (MPB) composition of PSN-PMN-PZT system, various spectra of Zr/Ti ratio were systematically experimented, and their effects on the subsequent piezoelectric properties and dielectric properties for high power piezoelectric transformer application were investigated using an impedance analyzer and a laser vibrometer. Microstructure and phase information were characterized using X-ray diffractometer (XRD) and a scanning electron microscope (SEM). When the Zr/Ti ratios were 0.475/0.465, the mechanical quality factor and electromechanical coupling factor were shown to reach the maximum, indicating that this alloy design can be a feasible composition for high power transformer.</P>

<P>Cyclic AMP-cAMP receptor protein (CRP) complex plays an essential role in the global regulation of Vibrio vulnificus virulence. We found that growth retardation of V. vulnificus caused by mutation of the crp gene encoding CRP was exacerbated under iron-limited conditions. Accordingly, we investigated the effect of crp mutation on the expression of the vulnibactin-mediated iron-uptake system and the ability of V. vulnificus to utilize transferrin-bound iron, and thus to grow in cirrhotic ascites, a human ex vivo system. The production of vulnibactin was suppressed, and the transcription of the vis and vuuA genes, which encode an enzyme required for vulnibactin synthesis and vulnibactin receptor protein, was also suppressed in the crp mutant. Moreover, the crp mutant could not utilize transferrin-bound iron, and its growth was severely suppressed both on transferrin-bound iron and in cirrhotic ascites. All the defects in the crp mutant were recovered by the in trans complementation of the wild-type crp gene. Putative CRP-binding sequences were found in the regulatory regions of the fur, vis and vuuA genes. These results indicate that crp mutation attenuates the ability to grow on transferrin-bound iron and in a human body fluid by down-regulating the vulnibactin-mediated iron-uptake system.</P>
