The effectiveness of a peripheral nerve block with 7% phenol solution in children with spastic cerebral palsy was evaluated in 37children quantitatively. Kinematic and kinetic data as well as the clinical data were obtained in the computerized motion analysis laboratory before and after the nerve block. Dynamic electromyographic data was also collected from the muscles of the legs with the multichannel equipment before and after the blocking procedure. Obturator and tibial nerves were most frequently blocked. All the measurement data obtained showed significant improvement of function after the nerve block with phenol until 3 months after the block. After 6 months. satisfactory outcome was maintained in 42.9% of the children. The peripheral nerve block with 7% phenol solution is a effective and affordable method in the management of spasticity in the children with spastic paralysis in the carefully selected cases.

Many studies revealed the neuroprotective, cardioprotective, and chemopreventive actions of dietary flavonoids. The plausible mechanistic interpretation of the various effects of flavonoids was concentrated on the anti-oxidant or free radical-scavenging properties of these phytochernicals, both in model systems and under in vivo conditions. While there has been a major focus on the anti-oxidant properties. there is an emerging view that flavonoids and their in vivo metabolites. do not act as conventional hydrogen-donating anti-oxidants. but they may exert regulatory functions in cells through actions at protein kinase or lipid kinase signaling pathways. Flavonoids and more recently their metabolites. have been reported to act at phosphoinositide 3-kinase(PI 3-kinase). Akt/protein kinase B(Akt/PKB), protein kinase C (PKC), mitogen activated protein kinase(MAP kinase), and various tyrosine kinases signaling cascades. Inhibitory or stimulatory actions at these pathways are likely to affect cellular function profoundly by altering the phosphorylation state of target molecules and by modulating gene expression. A clear understanding of the mechanisms of action of flavonoids, either as anti-oxidants or modulators of cellular signaling pathways, and the influence of their metabolism on these properties are key to the evaluation of these potent biomolecules as anti-cancer agents, cardio-protectants, and inhibitors of neurodegeneration.
Multiphoton microscopy: an introduction to gastroenterologists.

<P>Multiphoton microscopy, relying on the simultaneous absorption of two or more photons by a fluorophore, has come to occupy a prominent place in modern biomedical research with its ability to allow real-time observation of a single cell and molecules in intact tissues. Multiphoton microscopy exhibits nonlinear optical contrast properties, which can make it possible to provide an exceptionally large depth penetration with less phototoxicity. This system becomes more and more an inspiring tool for a non-invasive imaging system to realize 'optical biopsy' and to examine the functions of living cells. In this review, we briefly present the physical principles and properties of multiphoton microscopy as well as the current applications in biological fields. In addition, we address what we see as the future potential of multiphoton microscopy for gastroenterologic research.</P>

<P>Lysophosphatidylcholine (lysoPC) induces vascular smooth muscle cell (VSMC) proliferation and migration, which has been proposed to initiate the intimal thickening in coronary atherosclerotic lesions. Berberine is an alkaloid in Berberis aquifolium and many other plants. Recently, it has been shown to have beneficial effects on the cardiovascular system, such as anti-hyperglycemic and cholesterol-lowering activity. In this study, we investigated its effects on lysoPC-induced VSMC proliferation and migration. Berberine inhibited lysoPC-induced DNA synthesis and cell proliferation in VSMCs, as well as migration of the lysoPC-stimulated VSMCs. It also inhibited the activation of extracellular signal-regulated kinases (ERKs) and reduced transcription factor AP-1 activity and the lysoPC-induced increases in intracellular reactive oxygen species (ROS). These results indicate that the inhibitory effects of berberine on lysoPC-stimulated VSMC proliferation and migration are attributable to inhibition of ROS generation and hence of activation of the ERK1/2 pathway. This suggests that berberine has potential in the prevention of atherosclerosis and restenosis.</P>

Background: Anticoagulation is a key component in running extracorporeal membrane oxygenation (ECMO). Anticoagulation, however, is related with serious bleeding complications. Previous studies suggested that bleeding complications are more commonly related to serious morbidity and mortality than thromboembolic ones. In this study, we hypothesized that lower dose of anticoagulation may be safe and effective and assessed the clinical outcomes according to different anticoagulation strategies. Methods: We reviewed retrospective data of ECMO database from December 2008 to March 2014. In the conventional group, anticoagulation targeted to maintain an activated clotting time (ACT) in between 180 and 220 seconds by a continuous infusion of unfractionated heparin. In the conservative group, anticoagulation targeted to activated partial thromboplastin time (aPTT) in between 50 and 70 seconds. We assessed the incidence of bleeding events, transfusion requirements, thromboembolic complications, and other clinical outcomes in each strategy. Results: A total of 626 days on ECMO in 58 patients were reviewed. The 59 percent of the patients receiving ECMO therapy were successfully weaned and the overall mortality was 50 percent. Overall, 40 percent of the patients (n=23) were treated with conventional anticoagulation strategy (S group) and 60 percent of the patients (n=35) were treated with conservative anticoagulation strategy (C group). Incidences of major bleeding and bleeding related death were significantly lower in the C group (57% versus 29%, p=0.033 and 13% versus 0%, p=0.028). The average daily transfusion doses of RBC unit and FFP were higher in the S group (3.4 versus 1.4, p=0.001, 2 versus 0.6, p=0.000). Conclusions: In this study, conservative anticoagulation strategy showed acceptable results in terms of major bleeding event, transfusion dose and thromboembolic complications. Such results make conservative anticoagulation strategy capable of reducing bleeding complications without serious thrombotic complication.

<P><B>Abstract</B></P> <P>Despite large economic losses attributable to white spot syndrome virus (WSSV), an infectious pathogen of penaeid shrimp and other crustaceans worldwide, no efficient vaccines or antiviral agents to control the virus are available at present. Here, we designed and constructed baculovirus-based vaccines delivering genes encoding the WSSV envelope proteins, VP28 and VP19. To enhance the immunogenicity of the baculovirus-based vaccine, we fused a Salmonella typhimurium flagellin 2 (FL2) gene with VP28 or VP19 gene. Both vaccine constructs elicited similar high titlers of anti-WSSV IgG after oral immunization in mice. The protective effect of oral vaccines upon WSSV challenge was observed in Macrobrachium nipponense. Bivalent vaccine displaying WSSV envelope proteins, VP19 and VP28, led to enhanced more than 10% survival protection against WSSV infection, compared to monovalent vaccine containing WSSV envelope protein, VP19 or VP28. Furthermore, a baculovirus-based WSSV vaccine fused with FL2 gene, Ac-VP28-ie1VP19FL2, efficiently protected mice against WSSV challenge (89.5% survival rate). In support of the efficacy of FL2 in our vaccine, we verified FL2 enhanced survival rate and induced the NF-κB gene in <I>Palaemon paucidens</I>. The collective results strongly suggest that our recombinant baculoviral system displaying WSSV envelope protein and delivering FL2-fused WSSV envelope gene effectively induced protective responses, supporting the utility of a potential new oral DNA vaccine against WSSV.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Here, baculovirus-based WSSV vaccine fused with Salmonella typhimurium flagellin 2 (FL2) was constructed. </LI> <LI> Oral administration of <I>Macrobrachium nipponense</I> with the resulting vaccine (Ac-VP28-ie1VP19FL2) efficiently protected against WSSV challenge, and FL2-fused WSSV envelope gene effectively induced protective responses. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
Video Content-Based Bit Rate Estimation Scheme for Transcoding in IPTV Services

In this paper, a new bit rate estimation scheme is proposed to determine the bit rate for each subclass in an MPEG-2 TS to H.264/AVC transcoder after dividing an input MPEG-2 TS sequence into several subclasses. Video format transcoding in conventional IPTV and Smart TV services is a time-consuming process since the input sequence should be fully transcoded several times with different bit-rates to decide the bit-rate suitable for a service. The proposed scheme can automatically decide the bit-rate for the transcoded video sequence in those services which can be stored on a video streaming server as small as possible without losing any subject quality loss. In the proposed scheme, an input sequence to the transcoder is sub-classified by hierarchical clustering using a parameter value extracted from each frame. The candidate frames of each subclass are used to estimate the bit rate using a statistical analysis and a mathematical model. Experimental results show that the proposed scheme reduces the bit rate by, on an average approximately 52% in low-complexity video and 6% in high-complexity video with negligible degradation in subjective quality.
