        Productivity Loss of Rheumatoid Arthritis Patients according to the Their Stages of the Disease Activity Score

        Objective. Productivity loss was compared by 3-stage of disease activity and associations between higher disease activity and high productivity loss were identified. Methods. Data were extracted from Rheumatoid Arthritis (RA) Patient-reported Outcomes Research, which enrolled 2,000 RA patients (>20-year) on disease-modifying-antirheumatic-drugs (DMARDs) (≥ 6-month) from December 2012 to June 2013. This included 1,457 RA patients with the disease activity score (DAS-28-ESR) in their medical charts. Productivity loss in time and indirect cost was estimated using The World Health Organization Health and Work Performance Questionnaire (HPQ). Baseline characteristics and productivity loss outcomes were compared according to DAS-28-ESR groups. Results. 84.4% were females, 54.2% had low DAS-28-ESR (<3.2), and 38.2% and 7.6% had moderate (3.2∼5.1) and high DAS-28-ESR (>5.1). Patients with moderate to high DAS-28-ESR had higher lost productivity time (LPT) and monthly costs of LPT than those with low DAS-28-ESR (time in hours: 110.0±58.4 vs. 132.4±57.2 vs. 71.5±52.0, p<0.0001; monthly costs of LPT in 1,000 Korean won: 1,097±607 vs. 1,302±554 vs. 741±531, p<0.0001). Multiple regression analyses revealed significant associations with high LPT in high (adjusted odds ratio [OR]=3.87, 95% confidence interval [CI]: 2.18∼6.87) and moderate DAS-28-ESR (adjusted OR=1.88, 95% CI: 1.41∼2.52) compared to low DAS-28-ESR. In addition, positive associations with high monthly costs of LPT were observed in high (adjusted OR=3.45, 95% CI: 1.98∼5.99) and moderate DAS-28-ESR (adjusted OR=1.93, 95% CI: 1.43∼2.54) compared to low DAS-28-ESR. Conclusion. Timely therapeutic strategies should be taken into consideration given that the RA patients with moderate to high DAS-28-ESR showed strong associations with high productivity loss for effective management of RA.         사람 적혈구 조혈인자 DA-3585 의 토끼에 대한 단회 및 4주 반복투여 독성시험

        DA-3585, a biosynthetic recombinant human erythropoietin has been developed as a treatment for anemia associated with chronic renal failure in Dong-A pharmaceutical Co. Ltd. This study was carried out to assess its acute and subacute toxicities in rabbits. DA-3585 was intravenously administered to rabbits at dose levels of 6250, 12500 or 25000 IU/kg for single dose toxicity study and at dose levels of 100, 500 or 2500 IU/kg/day for 4-week repeated dose toxicity study. In the acute toxicity study, dose up to 25000 IU/kg had no adverse effect on the behavior or body weight gain. Pathological examinations revealed no abnormal gross lesions related to DA-3585. In the subacute toxicity study, all animals survived until termination of treatment. DA-3585 had no influence on clinical signs, food and water intake or on body weight changes. Hematological examination showed increases in the number of RBC, hemoglobin contents and hematocrit values with a dose dependent manner in the animals treated with DA-3585. Histopathological examination revealed erythroid hyperplasia in the bone marrow and extramedullary hematopoiesis in the liver. The changes detected in the hematological and histopathological examination presumably represent exaggerated pharmacological effects of erythropoietin. The NOAEL (no-observed-adverse-effect-level) of DA-3585 was estimated to be 100 IU/kg/ day under this study condition.

        한국인과 일본인의 환자용 사상체질 설문지 문항 응답 비교 연구

        Objectives This study was performed to compare responses of Korean to the Sasang Constitution questionnaire with those of Japanese and to learn difference in characteristic according to the Sasang Constitution between two countries. Methods 301 Korean visiting the department of the Sasang Constitution, Dong-Eui Medical Center in Busan, Korea from November 2006 to September 2010 responded to the SSCQ-P(Sasang Constitution Questionnaire for Patients). Sasang Constitution specialist interviewed subjects and diagnosed their Sasang Constitution. 361 Japanese visiting the center for Kampo Medicine, Keio University in Tokyo, Japan from January 2010 to February 2011 responded the SSCQ-J(Sasang Constitution Questionnaire for Japanese). The Sasang Constitution was diagnosed in the same way as Korean. We compare responses to the SSCQ-P in Korean with those to the SSCQ-J in Japanese. Results 1. Among Soyangin related 58 items of Sasang Constitution questionnaire, 26, 46.36% items had statistically significant response results in both Korean and Japanese and response disposition of all these items was same. Among Taeeumin related 68items, 36, 52.94% items had statistically significant response results in both Korean and Japanese. Of these, response disposition of 35 items was same and that of 1 item was different. Among Soeumin related 71 items, 31, 43.66% items had statistically significant response results in both Korean and Japanese. Of these, response disposition of 28 items was same and that of 3 items was different. 2. The proportion of items having statistical significance and same disposition in both Korean and Japanese by Sasang Constitutional characteristic category[Features and Way of Speaking, Physical Appearance, Temperament and Talent, Pathological Syndromes] was as follows; In Soyangin, the proportion in Pathological Syndromes was 27.8% and that in the others was more than 41.7%. In Taeeumin, the proportion in Pathological Syndromes was 33.3% and that in the others was more than 57.9%. In Soeumin, the proportion in Features and Way of Speaking was 70.6%, that in Physical Appearance was 8.3% and that in the others was 30~40%. Conclusions The response disposition of many of items having statistical significance between Korean and Japanese was same and that of a few was different. From this, there are many common Sasang Constitutional characteristics between two countries, and possibility of applying the Sasang Constitutional Medicine of Korea to Japan.

        Rh-Mn/tungsten carbides for direct synthesis of mixed alcohols from syngas: Effects of tungsten carbide phases

        <P><B>Abstract</B></P> <P>Effects of the crystalline tungsten carbide (WxC) phases on an ordered mesoporous bimetallic Rh-Mn/WxC, which were prepared by changing carbon source to tungsten (C/W) ratios of the WxC support using a hard-template of an ordered mesoporous SBA-15, were investigated for a direct synthesis of mixed alcohols by CO hydrogenation from syngas. The C/W ratios on the mesoporous Rh-Mn/WxC showed a significantly different catalytic activity, especially on the C<SUB>1</SUB> - C<SUB>3</SUB> alcohol productivity. The Rh-Mn/WxC prepared at C/W molar ratio of 10 having a metastable W<SUB>2</SUB>C main phase (Rh-Mn/WxC(10)) revealed a higher CO conversion of 8.1% and selectivity to higher alcohols of 54.4% compared to other catalysts having a main crystalline phases of WO<SUB>3</SUB> or WC. The enhanced catalytic activity and selectivity to mixed alcohols on the Rh-Mn/WxC(10) were attributed to the largely exposed smaller active Rh nanoparticles with its stronger interactions with the metastable W<SUB>2</SUB>C phases. The superior activity was originated from the intimate interactions of Rh nanoparticles with Mn promoter by maintaining proper oxidation states confirmed by surface ratios of the metallic Rh to oxidized Rh<SUP> <B>n+</B> </SUP> species. The stable preservation of the ordered mesoporous structures of the W<SUB>2</SUB>C phase in the amorphous carbon matrixes significantly altered the chemical states of the small Rh nanoparticles below 2 nm in size by preferentially existing on the outer surfaces of the W<SUB>2</SUB>C support, which resulted in showing an enhanced productivity of higher C<SUB>1</SUB> –C<SUB>3</SUB> alcohols with 171.8 g/(kg<SUB>cat</SUB>·h).</P> <P><B>Highlights</B></P> <P> <UL> <LI> WxC phases on mesoporous Rh-Mn/WxC changed C<SUB>1</SUB>-C<SUB>3</SUB> alcohol selectivity from direct CO hydrogenation. </LI> <LI> Metastable W<SUB>2</SUB>C phase is beneficial for a higher productivity of mixed alcohols. </LI> <LI> Different oxidation states of strongly interacted small Rh nanoparticles are well correlated with catalytic activity. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Effects of multi-walled carbon nanotubes on rheological and physical properties of polyamide-based thermoplastic elastomers

        The polyamide-based thermoplastic elastomers (Pebax$^{(R)}$) were melt compounded with multi-walled carbon nanotubes (MWNTs: 0.25~5 wt%) and the variation of rheological and physical properties with MWNT contents was investigated. The crystallization temperature ($T_c$) of the nanocomposites with 0.5 wt% MWNTs was most increased by ca. $8^{\circ}C$, but it was decreased by further addition. In addition, the presence of MWNTs broadened the $T_c$ peak with increasing nanotube contents. In contrast, the melting behavior was little influenced by the presence of MWNTs for all compositions. The incorporation of MWNTs increased the complex viscosity with MWNT contents and the abrupt increase was observed from 1 wt%. In addition, lower Newtonian flow region became disappearing with increasing MWNT contents, exhibiting notable shear thinning behavior from 1 wt% loading. Storage modulus was increased with MWNT contents in a similar manner to viscosity. Casson plot demonstrated a non-zero positive intercept for all the samples. In particular, the abrupt increase of yield stress was observed from 1 wt% loading. In the Cole-Cole plot, the nanocomposites gave a deviated curve from pure Pebax and the slope was decreased with increasing MWNT contents. The relaxation time calculated from viscoelastic parameters was increased with nanotube contents, but the increasing extents were reduced with increasing frequency. From 2 wt% MWNTs, the electrical conductivity was observed, indicating that the electrical percolation existed between 1.5 and 2 wt%. At 0.25 wt% loading the tensile strength was slightly increased, but it was gradually decreased by further addition. The introduction of MWNTs increased the tensile modulus with nanotube contents. In addition, ductile properties were reduced with increasing MWNT contents, resulting in low toughness.

        <P>We report on mode locking of a bulk Cr:YAG laser by using a transmission-type single-walled carbon nanotube saturable absorber. Stable and self-starting laser operation in the picosecond and femtosecond regimes is obtained at wavelengths around 1.5mum. Tunable transform-limited sub-100fs pulses are generated at a repetition rate of 85MHz with an output power up to 110mW.</P>

      • Hydrogen Peroxide Activates p70^S6k Signaling Pathway

        We investigated a possible role of reactive oxygen species (ROS) in p70^S6k activation, which plays an important role in the progression of cells from G_0/G_1 to S phase of the cell cycle by translational up-regulation of a family of mRNA transcripts that encode for components of the protein synthetic machinery. Treatment of mouse epidermal cell JB6 with H_2O_2 generated extracellularly by glucose/glucose oxidase led to the activation of p70^S6k and p90^Rsk and to phosphorylation of p42^MAPK/p44^MAPK. The activation of p70^S6k and p90^Rsk was dose-dependent and transient, maximal activities being in extracts treated for 15 and 30 min, respectively. Further characterization of ROS-induced activation of p70^S6k using specific inhibitors for p70^S6k signaling pathway, rapamycin, and wortmannin revealed that ROS acted up-stream of the rapamycin-sensitive component FRAP/RAFT and wortmannin-sensitive component phosphatidylinositol 3-kinase, because both inhibitors caused the inhibition of ROS-induced p70^S6k activity. In addition, Ca^2+ chelation also inhibited ROS-induced activation of p70^S6k, indicating that Ca^2+ is a mediator of p70^S6k activation by ROS. However, down-regulation of 12-O-tetradecanoylphorbol-13-acetate (TPA)-responsive protein kinase C (PKC) by chronic pretreatment with TPA or a specific PKC inhibitor Ro-31-8220 did not block the activation of p70^S6k by ROS, indicating that the activation of TPA-responsive PKC was not required for stimulation of p70^S6k activity by H_2O_2 in JB6 cells. Exposure of JB6 cells to platelet-derived growth factor or epidermal growth factor led to a rapid increase in H_2O_2, phosphorylation, and activation of p70^S6k, which were antagonized by the pretreatment of catalase. Taken together, the results suggest that ROS act as a messenger in growth factor-induced p70^S6k signaling pathway.

        <P><B>Abstract</B></P><P>Acrylic pressure sensitive adhesives (PSAs) have a range of applications in industry, such as medical products, aircraft, space shuttles, electrical devices, optical products, and automobiles, etc. In this study, acrylic PSAs with fluorinated groups were synthesized using 2,2,2‐trifluoroethyl methacrylate (TFMA) under UV radiation. The surface properties and adhesion strength were measured. The results showed that the addition of TFMA reduced the surface energy of the PSAs and improved the adhesion strength. POLYM. ENG.         The role of surgical clips in the evaluation of interfractional uncertainty for treatment of hepatobiliary and pancreatic cancer with postoperative radiotherapy

        Purpose: To evaluate the utility of implanted surgical clips for detecting interfractional errors in the treatment of hepatobiliary and pancreatic cancer with postoperative radiotherapy (PORT). Methods and Materials: Twenty patients had been treated with PORT for locally advanced hepatobiliary or pancreatic cancer, from November 2014 to April 2016. Patients underwent computed tomography simulation and were treated in expiratory breathing phase. During treatment, orthogonal kilovoltage (kV) imaging was taken twice a week, and isocenter shifts were made to match bony anatomy. The difference in position of clips between kV images and digitally reconstructed radiographs was determined. Clips were consist of 3 proximal clips (clip_p, ${\leq}2cm$) and 3 distal clips (clip_d, >2 cm), which were classified according to distance from treatment center. The interfractional displacements of clips were measured in the superior-inferior (SI), anterior-posterior (AP), and right-left (RL) directions. Results: The translocation of clip was well correlated with diaphragm movement in 90.4% (190/210) of all images. The clip position errors greater than 5 mm were observed in 26.0% in SI, 1.8% in AP, and 5.4% in RL directions, respectively. Moreover, the clip position errors greater than 10 mm were observed in 1.9% in SI, 0.2% in AP, and 0.2% in RL directions, despite respiratory control. Conclusion: Quantitative analysis of surgical clip displacement reflect respiratory motion, setup errors and postoperative change of intraabdominal organ position. Furthermore, position of clips is distinguished easily in verification images. The identification of the surgical clip position may lead to a significant improvement in the accuracy of upper abdominal radiation therapy.
