        1차 의료기관에서 상급종합병원으로 진료의뢰 시 발생하는 예약불이행 환자들의 이탈요인에 관한 연구

        [Purpose]In this study, we verify the existence of a factor which causes reservation nonfulfillment during the medical treatments transfer. The factor we found is ‘Medical Treatment Delay’, which is the waiting period between a day of treatment transfer request from a primary medical institution and a reserved day for the patient’s treatment in a general hospital. [Methodology]For the analysis, we used a data set on 296 patients who were transferred from a primary medical institution to a general hospital in the same district. Our data set indicates 98 no-shows which is 33% nonfulfillment upon 7.3 average delay days. [Findings]In terms of the logistic regression analysis, when a day of medical treatment delay is increased, patients’s reservation nonfulfillment is increased 2.519 times more, and it proved medical treatment delay has a correlation with patients’s reservation nonfulfillment. [Implications]We verified that the patients’ reservation nonfulfillment increases in Medical Treatment Delay factor, so we suggests patients management for general hospitals should include effective strategy of managing medical treatment delay to transfer patients from primary medical institutes. [연구목적]본 연구에서는 1차 의료기관에서 상급종합병원으로의 진료의뢰 시 환자들이 예약된 병원에서 진료를 받지 않고 예약을 불이행하게 하는 요인들 중에 진료의뢰일과 진료예약일 사이에 존재하는 진료 지연일수가 있음을 알아내고 이러한 진료 지연일수가 기 예약된 병원에서 환자들을 이탈하게 만드는 직접적인 요인으로 작용한다는 것을 실증하고자 하였다. [연구방법]우리는 일개의 1차 의료기관에서 같은 지역의 일개 상급종합병원으로 진료를 의뢰한 총 296명의 환자 데이터를 입수하였고, 예약불이행 환자 98명, 이탈률 33%, 평균 진료 지연일수 7.3일인 데이터 세트를 가지고 진료 지연일수와 예약환자 이탈률에 대한 가설을 설정하였고, 로지스틱 회귀 분석을 통해 가설을 검증하였다. [연구결과]로지스틱 회귀 분석 결과, 지연일수가 1일 증가할 때 환자의 예약 이탈은 2.519배 증가하는 것으로 나타났으며, 결론적으로 진료 지연일수와 이탈률은 양의 상관관계가 존재하는 것으로 나타났다. [연구의 시사점]본 연구에서 제시하는 진료 지연일수라는 요인은 예약된 환자들의 예약불이행과 밀접한 관련을 가지고 있으므로, 상급종합병원들은 1차 의료기관으로부터의 환자 전원과정에서 발생되는 진료 지연일수에 대한 효율적 관리시스템의 도입과 이를 통한 신규고객의 이탈을 최소화하는 경영전략의 수립을 고려해야 할 것이다.

        This study was conducted to investigate the effects of water stress(drought) at early growth stage, side dressing and topping stage on the growth pattern and yield components of flue-cured tobacco. The results are summarized as follows. 1.Drought at early growth stage retarded the rate of early growth, but the total amount of growth was recovered because of accelerated later growth. 2.Side dressing on the 30th day after transplanting(N : P2O5 : K2O = 1.5 : 1.5 : 3.0 g/plant) made the growth rate faster during maximum growth stage. Total growth amount showed little difference between side dressing and no side dressing, 3.In the plots of no side dressing, drought at early growth stage had little effects on the leaf area and dry leaf weight of Cutters. But in the plots of side dressing leaf area and dry leaf weight of Cutters were smaller in the plats of 15 day and 20 day drought than in the plots of 10 day drought and no drought. The longer the duration of draught was, the lesser the weight of unit leaf area was. 4.In the plats of no drought, leaf area of Leafs and Tips and dry weight of Leafs were unaffected by side dressing. But in the plots of drought at early growth stage, those were significantly affected by side dressing. The weight of unit leaf area of Tips and Green tips was lesser in the plots of 15 day and 20 day drought with side dressing than the other treatments. 5.Early topping made larger the leaf area of Leafs, Tips and Green Tips, and dry weight of Leafs and Tips than these of recommended stage of topping. Early topping in the plots of drought at early growth stage made little the weight of unit leaf area of Tips and Green Tips. 6.Though the dry leaf weight per a plant showed little differences among the treatments, yield components according to the leaf positions showed significant differences among the treatments.

        소양호 빙어 개체군의 생태적 특성

        To investigate the ecological characteristics that affect the growth of the Hypomesus nipponensis population in Lake Soyang, 615 H. nipponensis specimens were collected from the lake and examined 10 times between January and April 2019 and between January and March 2020. In 2019, the total length of Lake Soyang’s H. nipponensis was 73.3 ± 4.7 mm, and the weight was 1.9 ± 0.5 g. In 2020, the total length was 64.4 ± 12.3 mm, and the weight was 1.4 ± 1.3 g. The individuals collected in 2020 were smaller than those collected in 2019. During the survey period, the growth b values were highest in January and lowest in February, at 3.4 and 2.9, respectively. The obesity K value was negative in three out of four surveys conducted in February. The sex ratio of Lake Soyang’s H. nipponensis was 1 : 1.93, with males being more numerous, and the average number of females was 5457.9 ± 2152.9. As previous studies have shown, Bosmina longirostris, a tectonic species, accounted for 73% of H. nipponensis’ diet. The geographical distribution of H. nipponensis differed depending on its growth stage. The total length and weight of H. nipponensis collected in Bupyeong-ri, upstream of Lake Soyang, were 74.9 ± 10.9 mm and 2.4 ± 2.8 g, respectively. In contrast, specimens collected from Susanna-ri, located downstream, were much smaller, measuring only 48.1 ± 7.7 mm in length and 0.54 ± 0.3 g in weight. This study revealed that the distribution of H. nipponensis living in Lake Soyang, which has a long resin-type topography, varies depending on the growth stage. H. nipponensis is a cold-water fish species highly sensitive to water temperature changes. Its stocks are distributed throughout the country, including in Gangwon-do, and serve as effective environmental bioindicators. As such, these findings contribute to the understanding of H. nipponensis populations and water environments in distribution areas, particularly with regard to the impacts of climate change.

        어류군집 분석을 통한 송천 수계 내 도암호의 생태학적 역할

        This study evaluated the ecological role of Lake Doam (in six sites: two upper reach points, two lake points, and two lower reach points) using the fish community distribution and eight matrices of fish assessment index (FAI) from June 2021 to October 2022. This research on fish community distribution identified three legally protected species at the lower reach points and two at the upper reach points. Concerning Korean endemic species, ten were identified at the lower reach points, whereas eight were identified at the upper reach points. The lower reaches attained approximately 16 points higher FAI than the upper reaches. The water quality analysis revealed that the quality of water of lower reaches was better than that of the upper reaches, particularly in terms of electrical conductivity and suspended matter. In conclusion, the purification actions such as sedimentation and dilution in Lake Doam had a positive effect on the lower reach points of the aquatic ecosystem.
