Klippel-Trenaunay syndrome (KTS) is a rare congenital malformation syndrome involving blood and lymph vessels, which is characterized by triad of cutaneous hemangioma, venous varicosities, and overgrowth of the affected limbs. Because vascular malformation in KTS can be located anywhere except the face and brain, the clinical presentation could be extremely variable. But there are only rare case reports that KTS is associated with spinal cord lesion. We report a case of recurrent myelopathy in a patient with KTS.

Electrical injury can damage any structure in the peripheral and central nervous system through which it passes. Spinal atrophic paralysis is a syndrome of focal muscular atrophy occurring after a shock from man-made electricity. A previously healthy 29-year-old male has suffered from non-progressive atrophy of both upper extremities, especially hands, which insidiously progressed and became static for a few years, after electrical injury to right hand by household, low-tension current (110 V, a.c.) when he was 17-year-old. There was a noticeable muscle wasting of both hands with mildly diminished muscle power. Sensation was normal. Nerve conduction studies showed low CMAP amplitude of right ulnar nerve and absence or prolongation of F-wave of right ulnar and median nerves. Needle EMG of the muscles of right upper extremity demonstrated giant, polyphasic motor unit potentials of long duration without active denervation potentials. There was mid- to lower-cervical cord atrophy on the spinal MRI.
세균뇨 검출을 위한 Firefly Luciferase ATP 측정법에 관한연구

The extreme sensitivity of the firefly luciferase ATP assay could be applied in detection of bacteriuria. The methods available to determine the presence and quantity of microorganisms in a urine specimen can be divided into those that are growth dependent and those that are non-growth dependent. Growth dependent methods require dilution, inoculation and incubation on to a suitable medium and an incubation period of 18-24 hours. This will result in an enumeration as well as the identification of the microbes present. Nongrowth dependent methods, in contrast, do not require cultivation of the organism, but rather provide a direct enumeration of the bacterial population present. In a typical microbiology laboratory as many as 70% or more of urine specimens may not contain significant populations of bacteria. In such a setteing, bacteriuria screening tests are very useful in eliminating these samples from further analysis. This study was carried out to investigate the application of a firefly luciferase ATP assay intended for rapid detection of bacteriuria in clinical specimen. The results were as follows: 1. The somatic ATP was effectively eliminated by pretreatment of NRS/Apyrase (p=0.001<0.05). 2. The limit of sensitivity of the luciferase ATP assay was 10?? M ATP, the lineality and correlation were y=33.46±0.93x and r=0.997 (p<0.05), respectively, 3. The linearity was seen at 10?? CFU/ml of urine. The lineality and the correlation were y=-49.06+2.32x and r=0.980 (p<0.05) in 10??-10?? CFU/ml, and y= -3.30+0.73x, and r=0.976 (p<0.05) in 10??∼1.5×10?? CFU/ml, respectively. 4. In luciferase ATP assay, sensitivity, specificity and positive predictive value were 90.9%, 61.3%, 34.5%, respectively, otherwise negative predictive value was 95.0%. In conclusion, luciferase ATP assay provided a useful method for rapid detection of bacteriuria.

The cerebral compression and edema were successfully produced by the expansion of the stalk of laminaria in the right epidural space of the rabbits. By weighting the wet and dry brain tissues, and by calculating the percentage of water content and that of swelling, with the rate of per cent water content to that of control group, the studies of the occurence and changes of experimental brain edema have been observed. In the normal rabbits, the average per cent water content of cerebral gray and white matters were 79.9±0.7% and 73.0±1.0%, respectively. The percentage of water content and swelling of cerebral gray and white matters in the compression site showed higher values than those in control group. As compared with normal group, the rate of per cent water content of cerebral gray matter at the site of compression showed gradual increasing tendency as time elapsed by 24 hours, while that of white matter demonstrated rapid increased of it by 6 hours and decreased gradually thereafter. And it appeared that the value of the cerebral white matter showed higher than that of gray matter throughout the period of 24 hour's compression. Cerebral edema induced by laminaria compression was more evient and severe in the white matter than in the gray, which was more striking at the early stage of cerebral compression.

5 cases of Diabetes Insipidus following head injuries were reported. he conclusions are as follow: 1) 2 patients of the 5 cases sustained basal skull fracture. Others, I subdural hematoma, 1 subdural hygroma and another was cerebral laceration. Diabetes insipidus was recovered in 2 basal skull fracture spontaneously and others all expired with gradual deterioration. 2) Appearance of the diabetes insipidus was 10 days to 6 weeks following the head, injuries. 3) Spontaneous recovery could be expected in less severely traumatized occasions and the diabetes insipidus would be permanent in cases of severe head injuries. 4) Antidiuretic hormone was the only effective medicine for the post-traumatic diabetes insipidus.

Graves disease occur in association with myasthenia gravis is rare. We report a case of Graves disease and myasthenia gravis treated by bilateral subtotal thyroidectomy and total thymectomy simultaneously. A 37 year old woman was admitted with anterior neck mass and ptosis. Various examinations were compatible with combined Graves disease and myasthenia gravis. The bilateral subtotal thyroidectomy and total thymectomy were done simultaneously. The pathologic diagnosis was Graves disease and thymic hyperplasia. The patients postoperative course was uneventful. The thyroid function of patient became euthyroid and the clinical symptoms related with myastenia gravis resolved during follow up period.
