Objectives: Organizational culture has beer important in field of organizational behavior research for the past decade. Although there has been a growing interest in the organizational culture and organizational effectiveness, there is few research in health care field. This study was carried out to investigate the relationship of organizational culture and organizational effectiveness at general hospital. Methods: Data was collected by self-administrated questionares. Organizational cultures were measured by using Likert scale. A general hospital in Kyunggi-Do was selected and survey was conducted to 675 workers. Data was analyzed with computer package, PC-SPSS. Results: There were four types of organizational culture in this hospital consensual culture, developmental culture, hierarchical culture, rational culture. Many workers recognized their culture as rational culture and developmental culture. This finding showed that the hospital had both human related and task related climate. There were some differences in recognition of sub-organizational culture by occupational group, but perceived organizational culture was in accordance with sub-organizational culture in general. Multiple regression analysis and multiple logistic regression analysis were conducted to find the relationship of organizational culture and organizational effectiveness. As a result, developmental culture showed a strong relationship with organizational commitment and job-satisfaction. Conclusions: These results showed that types of organizational culture were significantly related to organizational effectiveness and understanding the existing culture is essential to develope their organizational effectiveness.

The study has its purpose on providing basic resource to enforce the capability of the middle managers by examining the level of performance and the level of awareness about the capabilities of the managers and by understanding the significance of the difference and the reasons for the differences. The source of the study was 195 survey questionnaires that were carried out to the managers of the 9 general hospitals and the method of the analysis was the frequency analysis, analysis of the credibility, matching to sample T-test, independent sample T-test, dispersion analysis, correlation analysis, and multiple linear regression analysis using accumulated variables. The followings are the main result of the study. First, the difference between the level of awareness about the capabilities and the level of performance of the mediator managers in general hospitals had high capability in change management. The following orders were: competence in achievement and behavior, competence in management, competence in recognition, competence in influence, competence in individual effectiveness, and competence in personal relationship service. Second, as the result of the relation analysis in order to understand the correlation between awareness and performance of the mediator managers, everything had significant positive correlation. In the study about the level of importance, the cognitive capability and the management capability had the highest correlation with the correlation number of 0.88. In the study about the level of performance, the cognitive capability, individual capability, and the management capability had the highest correlation with the correlation variable number of 0.79. Third, as the result of studying the reason for the difference between the level of the awareness capability and the level of the performance, lack of the support·recognition·compensation in the organization level, inappropriate work environment, limit in the regulation were found as the highest reason in the order. As the result of the study, it was concluded that the creation of the efficient capability estimation model and the securement of the system that estimate the capability of the managers should be carried out in order to enforce the capability of the mediator managers in general hospitals.

Vancomycin frequently induces hypersensitivity reactions including red man syndrome (RMS) and anaphylaxis. Lowering infusion rate with antihistamine premedication is usually effective to reduce RMS, however, desensitization should be considered for severe reactions not responding to usual measures. Here, we report a case of a patient with pyogenic spondylitis who had developed hypersensitivity reaction to vancomycin, got a full recovery with vancomycin desensitization. A 63-year-old man was transferred to our hospital for back pain, proved to pyogenic spondylitis. As methicillin-resistant Staphylococci aureus infection was suspected, vancomycin was administrated. But, he showed hypersensitivity reactions such as hypotension, dyspnea and severe flushing after vancomycin administration at previous hospital. Readministration of vancomycin at a lower infusion rate with premedication was tried. Three hours after vancomycin infusion, he developed fever, chills, rash and hypotension. Thrombocytopenia was occurred after administration of other antibiotics including cefazolin and teicoplanin. Vancomycin administration was attempted according to a rapid desensitization protocol. The infusion rate of vancomycin was increased to the standard rate. After the desensitization, he successfully completed the full course of treatment with vancomycin. Vancomycin desensitization could be the option for the vancomycin hypersensitivity when other antibiotics are not feasible. (Allergy Asthma Respir Dis 2013;1:168-171)

Heungdukwang's(興德王) costume ordinance(834A.D.) is a very important written historical record because it reveals Tongil Shilla's(統一新羅) costume. And among the clothing pieces that appeared in Heungdukwang's(興德王) costume ordinance, from Pyosang(表裳), Naesang(內裳), we know they wore one skirt over another. But skirts appearing in visual records are worn in waist line or breast line, and when they wore skirts in breast line, there were shoulder belts or not. Also among the clothing pieces that appeared in Heungdukwang's(興德王) costume ordinance, Yo(요) and Ban(반) seems to be connected with skirtt but it is not clear. So this study focuses on the history of wearing skirt in breast line and one skirt over another, shoulder belts of skirt, and relation among Yo(요) Ban(반) and skirt. The results are as follows. First, wearing skirt in breast line is already appeared in Ancient Shilla's(新羅) clay figure, and wearing one skirt over another is also appeared in Sunhung tomb's mural painting. so it is not completely under the influence of Tang(唐). Second, the shoulder belts of skirt is for convenience of action, But, at that time, there is no shoulder belts of skirt in Tang(唐). Third, between Yo(요) and Ban(반), If Yo(요) is connected with skirt, it is a shoulder belts of skirt or waist of skirt. And Ban(반) is a girdle of skirt
